Today and next week, we will take a deep dive into how we understand and think about chronic pain. And to do that, we have a special guest. Dr. Heather Martarella is an expert on helping clients reclaim their lives and find relief from chronic pain and insomnia/sleep issues. She also provides treatment for those coping with chronic illness, most recently including Post-Acute COVID-19 Syndrome. Dr. Martarella became passionate about specializing in treating chronic pain after experiencing her own physical injury in 2012, which left her with chronic pain for years and significantly changed her lifestyle. Years after her injury, she was able to begin recovering and now manages the occasional pain flare-up well and enjoys a good quality of life, and is committed to helping others find relief, too. These clients' physical situations often manifest into anxiety, depression, insomnia, relationship conflicts, fatigue, irritability, or overwhelm. Life as you knew it may have stopped, but there is hope, and Dr. Martarella can help you find it again. Dr. Martarella uses many evidence-based therapy modalities and will develop the best combination for each client based on their needs and goals. She provides Cognitive Behavioral Therapy for Insomnia (CBT-I) for those suffering from sleep difficulties to help clients improve their sleep. For those with a trauma history, whether from childhood, medical, or other traumas, she may provide EMDR or Trauma-Informed CBT, to help clients heal from trauma. Disruptive thoughts are commonly and somewhat lightly referred to as the “monkey mind.” It is not a joke and has a serious impact on our capacity to enjoy life. Since we can't escape our thoughts, what do we do? There are three core aspects to calming down your racing thoughts – 1) separating/ diverting from them 2) lowering threat physiology 3) moving on. This episode will address ways to lower the threat physiology. There are many ways to calm down the body's activated threat response by stimulating the calming effects of the 10th cranial nerve called the vagus nerve. Some of them include: · Breath work – many techniques · Music – unique to each individual · Rhythm/ dance · Humming – stimulates the 7th cranial nerve close to the vagus nerve · Rubbing forehead – stimulates the 5th cranial nerve and then the vagus nerve · SSP (safe and sound protocol) provides auditory cues of safety (8th cranial nerve) that powerfully stimulates the vagus nerve. These tools just last for a short time but are intended to be used enough that they become somewhat automatic. They are necessary to get your brain back “online” in order not to act out. “Sticks and stones will break my bones, but words will never hurt me.” Nothing could be further from the truth and holding onto this concept is damaging. Research has shown that emotional pain is processed in overlapping circuits in the brain, and the physiological response is the same. Thoughts and concepts are sensory input and emotions are the sensations generated by your body's physiological makeup. Anxiety is an activated threat response and relaxed is what you feel when you are safe. There are many strategies to process threats in a manner to decrease your levels of stress chemicals and move on.   Emotional wounds are not visible to the body but expressed by the nervous system by either thinking about what was done to us and/or when the nervous system senses danger or threat that resembles the original occurrence from the smallest hurts to the small traumas to the large trauma experience. The symptoms are diffuse and can affect any part of your body.  Understanding the nature of your body's unconscious reactions to real or perceived threats is the first step in dealing with them. For Pete's Sake, Kassira and I are meeting this week on Saturday night instead of Sunday night and we will be joined by Teresa Cook and the topic will be The Evolution of Change. Evolutionary change is incremental and takes place gradually, over time. Slow, gradual change often takes place to ensure the survival of the organization. It's incremental in that it happens step by step, little by little. Teresa Cook is a clinical psychologist at the Memphis VA Medical Center, specializing in medical psychology. She is the Health Behavior Coordinator, Lead Tobacco Cessation Clinician, Pain Psychologist, and the LGBTQ+ Veteran Care Coordinator. She was a professional actress for over twenty years, performing in musicals, plays, television, and commercials. After moving to Los Angeles, she began a journey of self-actualization that started with psychotherapy and included energy work, Rolfing, sacral-cranial therapy, and acupuncture. In 1998, Teresa was diagnosed with Stage 3 breast cancer and underwent surgery, two different chemotherapy protocols, and 28 treatments of radiation therapy. She was prescribed 5 years of adjuvant therapy with medication. On the day she was diagnosed, she made a decision to change her life if she lived through cancer. While performing was a lifelong passion, the need to give back and help others became the next chapter of her life. She returned to Waco, TX to do two years of post baccalaureate work to prepare to apply to graduate school. She applied to the Baylor University doctoral program and was accepted for three years of difficult, life changing education. She graduated with a Psy.D. in 2006.
