When a young Catholic priest saw many of his parishioners being sent to war in Vietnam, he felt it was his duty to serve as well. It took Father Richard Shannon years of lobbying, but eventually, he was granted permission to enlist. During his tour of duty, Father Shannon said mass and ministered to countless troops, including at LZs deep in the jungles of Vietnam.
In this episode of the Habitual Excellence Podcast, Ken Segel interviews Dr. Richard Shannon, Senior Vice President and Chief Medical Officer of Duke University Health System, about the evolution of healthcare quality and safety over the past 25 years. Dr. Shannon discusses his journey, which shifted focus after encountering Paul O'Neill and the Pittsburgh Regional Healthcare Initiative, emphasizing that the current healthcare environment has fallen into a "tyranny of measurement" that often overlooks real improvements. He highlights how Duke has implemented a management system that emphasizes people development, standard operating procedures, and continuous improvement to reduce variability in care, leading to improved patient and staff outcomes. They explore the importance of servant leadership, lean management, and addressing social determinants of health. Dr. Shannon calls for a shift at the national level—from focusing solely on metrics to embracing comprehensive improvement methods, incentivized by organizations like CMS. He also expresses concerns about the upcoming generational shift in healthcare leadership and the importance of developing new leaders who can continue advancing these quality initiatives. Dr. Shannon ends by discussing his legacy, emphasizing that true transformation requires spending all political capital and leaving an organization better than when one arrived. He provides an example of addressing racial disparities at Duke, where maternal morbidity for African American women was significantly reduced through improvements in patient access and care coordination. This case illustrates how a robust quality system can lead to impactful and equitable healthcare outcomes, reinforcing that healthcare transformation is both achievable and necessary.

Episode page with video clip, transcript, and more Welcome to Episode #71 of Habitual Excellence, presented by Value Capture. Joining us today as our guests are Leah Binder and Dr. Richard Shannon. Leah Binder is President & CEO of The Leapfrog Group, representing employers and other purchasers of health care calling for improved safety and quality in hospitals. Under her leadership, The Leapfrog Group launched the Leapfrog Hospital Safety Grade, which assigns letter grades assessing the safety of general hospitals across the country. Richard P. Shannon, MD serves as the Chief Quality Officer for Duke Health. He is responsible for the overall direction, leadership and operational management of the quality and safety programs of Duke Health, and provides leadership in strengthening a quality culture where everyone is engaged and respected. Leah and Rick are both amongst the great lineup of presenters at an executive seminar that's being hosted by Duke Health in Durham, NC — on September 15th and 16th — titled “Leading with Safety.” Today we're going to be talking about the urgent need to improve safety and quality in healthcare — and what leading organizations are doing to make progress toward ideal care and zero harm. In today's episode, Leah and Rick talk with host Mark Graban, about topics and questions including: How would you describe the landscape of patient safety today? 20 years since the IOM Report - accelerating in the past decade? Getting worse during the pandemic? Headline: “U.S. Hospitals Are Getting Safer for Patients, Study Finds” - Certain adverse events down from about 20% of patients to 10%… thoughts? What's the difference between the Leapfrog Hospital Safety Grade and the Leapfrog Hospital Survey / ASC Survey? Beyond the grades, what do you see happening in the A hospitals vs. the others with lower grades?? Rick — Tell us about Duke Health's language around having a “commitment to zero harm” and how that's not just a slogan? How do you make practical and meaningful progress toward zero harm? Leah — How do employers look at the issue and what are they asking for or demanding now? Why are we doing this seminar for CEOs and the C-suite instead of quality leaders?? Rick, why host the seminar at Duke Health? The Consolidated Appropriations Act of 2020

Today brings us a special episode, where we have an opportunity to revisit Mark Graban's interview of our original non-executive chairman, Paul H. O'Neill, Sr. This interview originally appeared in the Lean Blog Interviews podcast back in July 2011. In the discussion, Mr. O'Neill shared his thoughts on patient safety and healthcare, including his time spent as the Chair of the Pittsburgh Regional Health Initiative and his work with Dr. Richard Shannon in dramatically reducing hospital-acquired infections (HAIs) to their “theoretical limit” of zero harm. Mr. O'Neill talks about the leadership required to have such an impact on safety and quality, drawing on lessons from his years as Alcoa's CEO. In this episode we discuss a question posed by Richard Shannon- how do we ensure that horticulture is more resilient in the face of extreme weather events? To set the scene, what conditions are Australian horticulture growers operating under and how are they ensuring they remain financially viable when dealing with natural disasters? Central Queensland's Sandi Groves has ensured multiple disaster events and has emerged with a stronger business- listen to how she did it. Ed Cowie is in partnership on a farm with his father and was severely impacted by the same 2019 fires. But in terms of disaster fatigue, Ed has unique insights, stemming from his roles in emergency services as a police officer, fire officer and SES controller, with an in-depth knowledge of farming systems. So how bad can it get… and what can growers do to be resilient to natural disasters? Richard Eckard is Professor of Sustainable Agriculture at the University of Melbourne and Director of the Primary Industries Climate Challenges Centre (www.piccc.org.au), a research centre addressing the impacts of climate change on agriculture. He says proactive farmers are already counteracting the effects of climate change. This episode is brought to you by Growcom and the Rural Financial Counselling Service NQ and the Rural Financial Counselling Service Network.
Concerns from within horticulture that the Queensland Government announcement of a new Wellcamp Airport quarantine hub won't do anything to ease the growing agriculture worker crisis, with Growcom's Richard Shannon suggesting a third of usual workers are missing and Whitsunday farmer Tony Jeppersen says he has been left high and dry after the Queensland Government's snap decision to cut off access to irrigation water from the O'Connell River, meaning his recently planted cover crops are left without water and at risk of causing environmental damage.

With millions of dollars’ worth of produce being left to rot, farmers are desperate for hands to pick bumper crops while they face a shortage of people interested in a potentially high-paying job while a million Australians remain out of work. Thursday: Four Eastern, Three Central: Two Mt and One Pacific: Fran Lewis the host of Book Discussion welcomes Best selling author Larry Thompson: Insanity Plea and the author of Sentinel's Choice: Mike Witzgall. As a member of the elite U.S. Marine Corps Force Reconnaissance teams, Renee LaFleet learned hard lessons about survival. This is where he met the brother of his heart, Navy SEAL commander, Richard Shannon, under the harsh conditions of hand to hand combat. But nothing he experienced in Iraq prepared him for the evil he would face as a homicide detective in Dallas, Texas. At least in battle you know the enemy. When bodies start turning up all murdered in the same manner, Ren is confronted with the unthinkable possibility that his best friend, Father Richard Shannon might be involved. Ren must decide where the bonds of brotherhood end and his duty as a Texas Peace officer starts. Learn more when Mike and I discuss Sentine's Choice. A young nurse is savagely killed during a pre-dawn run on Galveston's seawall. The murderer slices her running shorts from her body as his trophy and tosses the body over the wall to the rocks below. As dawn breaks, a bedraggled street person, wearing four layers of old, tattered clothes, emerges from the end of the jetty, waving his arms and talking to people only he hears. He trips over the body, checks for a pulse and, instead, finds a diamond bracelet which he puts in his pocket. He hurries across the street, heading for breakfast at the Salvation Army two blocks away, leaving his footprints in blood as he goes. Just who is accused and who will defend him you will learn when I interview Larry Thompson. Join me in the chat room when I interview both mystery/thriller authors. Insanity Plea will keep you guessing until the very end. Sentinel's Choice is a fast paced novel with twists and turns you won't see coming. One show you want to hear! .
