Child physical abuse often presents first with injuries that appear minor, but missing these early warning signs can have devastating consequences. Two recent CMAJ papers examine how sentinel injuries in infants may signal escalating risk and how patterns of severe maltreatment shifted during the COVID-19 pandemic. Together, they offer practical guidance on when clinicians should escalate concerns and highlight system factors that shape risk for vulnerable children.Dr. Megan Cooney, a child maltreatment pediatrician at Health Sciences Centre Winnipeg and co-author of “Five things to know about sentinel injuries and indicators of child physical abuse”, explains why medically minor injuries in pre-cruising infants require careful scrutiny. She notes that more than one quarter of children who experience catastrophic abuse had previously been seen for minor injuries. Any unexplained or poorly explained injury in a non-cruising infant should raise concern. She also reviews the validated TEN-4-FACESp clinical decision rule to help clinicians identify bruising patterns that warrant further action.Dr. Matthew Carwana, a pediatrician at BC Children's Hospital and clinical investigator at the BC Children's Hospital Research Institute, discusses findings from “Hospital admissions for maltreatment among children younger than 2 years during the COVID-19 pandemic in Canada”. He describes an early drop in maltreatment hospitalizations during the first 16 weeks of the pandemic, followed by a return to expected levels. However, ICU admissions rose above baseline for roughly the following year, raising concern that children may have been presenting with more serious injuries. Carwana suggests reduced early detection and loss of family and community supports during periods of isolation may have contributed.For physicians, the message is to remain objective and act early. Simms is an Chaplain assigned to the 412 Test Wing, Edwards Air Force Base, CA. He provides spiritual care and ensures the delivery of chaplain support to Airmen, Guardians, and DoD employees across two local area installations. As part of the wing staff at the 412 Test Wing, Chaplain Simms is tasked with advising command regarding the spiritual readiness, morale, ethics, and quality-of-life issues of all Air & Space Forces personnel and authorized DoD personnel.A native of Florida, Chaplain Simms currently resides in Edwards, CA, with his wife and young daughter. He was commissioned as a Chaplain in April of 2018 and is endorsed by the International Council of Community Churches. Prior to his military service, Chaplain Simms spent over a decade in both local parish ministry and as an ICU/ER chaplain at a level one trauma medical center.He has served as a Traditional Reservist, IMA Reservists, & and now on Active Duty. His time in the ICU at an urban level one trauma hospital as well as two of his deployments (Kuwait – Operation Freedom's Sentinel, JBMDL – Operations Allies Welcome/Refuge) occured during the COVID pandemic. He also served as Lead Chaplain on a joint reserve mission in the Appalachian Mountains providing no-cost healthcare to the community.He is a three time graduate of Joint Special Operations University Chaplaincy programs, and is also a graduate of the Air Force Leader Development Course at Maxwell AFB, a course typically reserved for incoming squadron commanders and senior enlisted leaders. INDICATORS OF CONFLICT MOUNTING IN MIDDLE EAST.// -----BEGIN TEARLINE------International Events-Middle East: Indications and warnings are mounting concerning increased conflict in the region. Over the past few days, several nations have re-iterated urgent requests for their citizens to depart Iran, and the White House has continued to issue increasingly more serious rhetoric regarding potential military action.Analyst Comment: As always, what exactly is on the menu is anyone's guess. Timeline wise, if strikes are intended to be carried out by the US as they were last time (as in, a one-and-done strike mission), no large-scale troop movement will be necessary. However, if a more prolonged conflict is being planned, a lot more equipment is going to be needed in theater. Over the past few days, rumors have emerged indicating that the United States is reducing a presence at Al-Udeid Airbase in Qatar, presumably so as to reduce the potential for retaliatory strikes from the Iranians. This is the same base that was struck by Iran for face-saving reasons following the American operation in June of last year. As of this morning, the Sentinel 2 satellite conducted a pass of this airbase, providing fresh imagery of the facility. At 0712Z no abnormal aviation presence has been noted at the airbase, so if an evacuation or aircraft re-location has been ordered it's not discernible from satellite imagery just yet. It's certainly possible that non-essential personnel are being evacuated, but the late-stage indicator last time was the mass exodus of anything with wings from this facility before the strikes. Last year, every single aircraft was flown out of "The 'Deid" and relocated to Saudi Arabia several days before the American strikes (and the expected Iranian counterattack).More broadly, more aircraft have been observed conducting flights around the region over the past few weeks, though as always these are very challenging indicators to monitor during the initial stages of operational planning.-HomeFront-Minneapolis: Overnight the general state of unrest has continued as before, with demonstrators settling in for the long haul. Yesterday evening protesters expanded unrest operations to a new hotel, the Hilton Graduate hotel adjacent to the University of Minnesota. A few hundred demonstrators were observed at this location carrying out the standard tactic of making as much noise as possible to prevent the ICE agents inside from being able to sleep well.To the south, the persistent protest at the Whipple Federal Building has continued, and has remained low-intensity throughout the daytime, with bursts of more kinetic unrest later in the evenings. Around the city, more roving protests are common, as Defend612 members have continued to follow ICE convoys around the city, and these amateur Quick Reaction Force (QRF) efforts often result in pop-up protests at the site of a raid, and due to the active nature of these incidents, a handful of activists are usually arrested at each raid site after they attempt to either conduct a vehicle ramming or otherwise interfere with federal operations.-----END TEARLINE-----Analyst Comments: Concerning the demonstrations in Minneapolis, it's helpful to get a bead on the atmospherics (AKA the general feeling or mood in the air) of how things are going. This helps with understanding the wider context of how the unrest is progressing, and in Minneapolis, demonstrators are trying to emulate the persistent protests of Portland, Oregon. West coast ANTIFA cells have industrialized the practice of protest, and they fully understand the impact of low-intensity, but long-term protests. Read transcriptListen up, mutation enthusiasts and multi-platform adventurers, because this week on Play Comics we're strapping on our Kevlar suits and diving straight into the bewildering, beast-infested, cross-console chaos of X-Men: The Official Game! We're talking about the 2006 game that launched on practically every system known to mankind (GBA, GameCube, Nintendo DS, PS2, Xbox, and Xbox 360. Seriously, did they forget a platform?), which based the story nominally on the third X-Men film from Fox. You know, the one that showed us what happens when Professor Xavier and Magneto finally decided to outsource their beef settlement to a video game developer. This particular romp through Marvel's merry mutant universe was brought to you by the folks who looked at a film featuring Wolverine, Nightcrawler, and Iceman and thought, “What if we made this game SLIGHTLY different on each platform?” It's like they were challenged to see just how elastic the definition of ‘the same game' could be, and frankly, the results are beautifully inconsistent. The story was co-written by Chris Claremont (yes, THAT Chris Claremont) and Zak Penn, and it featured voice acting from the actual film cast, which means you got Hugh Jackman's growl in your living room, your handheld, and probably also your neighbors' living rooms at 2 AM. Joining us to make sense of this portable and stationary pandemonium is none other than Alex Zalben from Comic Book Club, a weekly live talk show about comics that's been running since 2006, performed at every major comic convention you can think of, written up in the New York Times more than once, and hosted literally hundreds of guests with more swagger than most podcasts muster in a lifetime. Alex is a writer, editor, and podcaster who knows his way around both four-color storytelling and video game adaptations, making him the perfect guide to help us determine whether this cross-generational, cross-console adventure managed to capture what makes the X-Men actually work, or if it just made us wish we could teleport away from our screens. So sync up your Danger Room protocols, pick your favorite handheld or home console, and get ready for an episode that's guaranteed to be more chaotic than a Sentinel factory explosion and infinitely more confusing than trying to figure out why THIS game exists on THAT console!
