Diabetes-related mortality is on the rise in South Africa, and the management of the condition remains suboptimal, even within the private healthcare sector – yet despite the high burden on healthcare resources, numerous cases go undiagnosed. Against this backdrop, the 2023 Diabetes Summit Report and Recommendations were recently presented to the National Department of Health following the 2023 Diabetes Summit held at the University of Pretoria's (UP) Future Africa Campus late last year. The summit brought together people living with diabetes, diabetes advocates, healthcare professionals, health authorities, government officials, policymakers, academics, public health specialists, private medical scheme and pharmaceutical company representatives, as well as nonprofit organisations, to highlight the challenges faced by people living with diabetes and contribute to improving their well-being. Dr Patrick Ngassa Piotie, Chairperson of the South African Diabetes Alliance and Co-Founder of the University of Pretoria Diabetes Research Centre explains more.See omnystudio.com/listener for privacy information.

“A decrease in insulin sensitivity and an increase in insulin resistance is often seen in aging and is the main feature of type 2 diabetes, but it doesn’t have to be this way. An attention to diet and exercise can maintain youthful levels of insulin sensitivity.” Those are the words of Dr. Mowll’s guest today — scientist, author, and blogger, P.D. Mangan. With his scientific background, P.D. writes about health, fitness, and anti-aging through lifting weights, maintaining a paleo diet, and intermittent fasting. He’s written six books on health and fitness, including Muscle Up and Dumping Iron. In their interview today, Dennis discusses how he uses his background in clinical biochemistry to understand diabetes and how to regulate blood sugar; why he gave up being vegetarian in favor of a low-carb, high fat diet; and how he cured his chronic fatigue. He also talks about why meat and saturated fats have very little — if anything — to do with our current health problems, the big problem with seed oil (most typically called vegetable oil) and what they do to our health, how high iron levels can be dangerous to our health, the importance of calorie restriction, how to slow the aging process, and prevent and reverse chronic disease. He also shares his journey of switching from being a marathon runner to giving up running in favor of resistance and strength training, as well as “vegetarian dark secrets,” and how the main contributors of the diabetes and obesity epidemic are actually plant foods. “Weight and food have kept you trapped in a life you don’t love. It’s time to finally change your habits and behaviors, and break all patterns and conditioning around food and eating. It’s time to love yourself and love your life!” Those are the words of Dr. Mowll’s guest today — Nutritionist Lisa Goldberg. Lisa received a Master’s degree in Clinical Nutrition from New York University. She believes in eating real — not processed — foods. She also believes in ‘everything in moderation’ and not being deprived or counting points and calories. She says, “When you begin to create an awareness of what your habits and behaviors are around food you can ‘change your brain’ and your relationship with food. You will be able to stop that negative and self-sabotaging voice in your head that keeps you from reaching your goals and maintaining your ideal weight.” Her goal is to help people make long-term changes and not just give them quick fixes or crash diets. She teaches you how to make the right food choices to help you lose the weight for good. In today’s interview, Lisa and Dr. Mowll discuss how mindset and behavior change are essential to sustainable weight loss, how to deal with night-time eating habits in a healthy way, the importance of enjoying the food you’re eating and finding recipes that excite you, and ideas for rewarding yourself for positive behaviour and reaching your goals. She also reveals the main reason why people gain weight back after they lose it on a diet (and how to solve that problem), the importance of keeping a food journal, and describes the best ways to address negative self-talk and creating awareness around the messaging you’re sharing with yourself. Hanna says she loves helping people with diabetes to be healthier and more confident with figuring out a few simple lifestyle changes to drastically improve their quality of life. “While carbs and protein raise insulin in the short time, the fat on our body and the fat in our diet have the most significant influence on our insulin levels.” Those are the words of Dr. Mowll’s guest today, engineer and creator or the website Optimising Nutrition, Marty Kendall. Marty Kendall is an engineer with an interest in nutrition who seeks to understand things numerically. His wife is a type 1 diabetic and he has a family history of types 2 diabetes and obesity. He has created a system to prioritize low insulinogenic nutrient-dense foods that he and his wife, Monica, have been using to improve low blood sugar, increase energy and to reduce depression and anxiety (that so often comes with blood sugar dysregulation). Marty says he’s been able to step up to a new level of vitality, reach his ideal weight, and reverse his pre-diabetes with this system. In today’s episode, Marty talks about how he uses his engineering background to understand and optimize blood sugar health. He also discusses the personal fat threshold and how it relates to insulin resistance, diabetes, and blood sugar dysregulation. Dr. Mowll and Marty Kendall talk all about calories and how they fit in — and how overeating even the right foods can still prevent you from burning fat and losing weight. Marty shares information on how to minimize cravings by improving insulin and leptin signaling and increasing the nutrient density of the foods you eat. Key Takeaways: [:58] About Dr. Mowll’s guest today, Marty Kendall. [3:37] Be sure to head over to iTunes and leave a review. Welcome to the Mastering Blood Sugar podcast! This is episode three, with Dr. Jason Fung. Dr. Jason Fung is the Medical Director and co-founder of Intensive Dietary Management. He’s a Toronto-based kidney specialist, having graduated from the University of Toronto and finishing his medical specialty at the University of California, LA, in 2001. He’s the author of the best-seller’s The Obesity Code, The Complete Guide to Fasting, and his new book, The Diabetes Code. He’s pioneered the use of therapeutic fasting for weight loss and type 2 diabetes reversal in his IDM Clinic. In today’s interview, Dr. Fung discusses his new book, The Diabetes Code; how he defines diabetes and why that makes such an important difference in treatment; how diabetes can be reversed; and how conventional treatment only masks symptoms and does nothing to actually treat the issue. He also talks about how high blood sugar is like overflowing garbage in your kitchen and reveals the two most important steps to lower your blood sugar and reverse type 2 diabetes. Dr. Fung discusses how overconsumption of carbohydrates (like fructose) raises insulin, causes insulin resistance, and contributes to the storage of fat. He talks about why eating fat does not cause insulin resistance and fat storage (contrary to what other experts are saying). He also dives into the topic of fasting and goes over both advanced and beginner strategies for getting started to help improve blood sugar health. Key Takeaways: [:58] About Dr. Mowll’s guest and topics for today. [3:10] Be sure to head over to iTunes and leave a review. Dr. Mowll will be reading reviews and sending you the best of the Diabetes Summit on a flash drive if he reads your review at the beginning of the podcast! [4:11] Welcoming Dr. Jason Fung to the podcast. [4:19] Dr. Jason Fung speaks about his new book, The Diabetes Code. Dr. Fung also addresses the two big misunderstandings about diabetes. [8:39] How Dr. Fung sees diabetes and explains why insulin is only a treatment of the symptom, not the disease. [13:30] About Dr. Fung’s Intensive Dietary Management program. [14:34] How do insulin resistance and hyperinsulinemia fit into the picture? [19:04] Too much glucose in the cell, fat around the muscles and liver, high insulin levels, insulin resistance — How do all of these pieces fit together? [22:08] Dr. Fung’s recommendations to those who want to begin fasting. [26:58] If there was one message, idea, or quote — professionally — that Dr. Fung would like to be remembered by, what would it be? [30:53] If Dr. Fung had a friend or family member that was recently diagnosed with type 2 diabetes, what advice would he give them? Welcome to the Mastering Blood Sugar podcast! This is episode two, with Dr. Benjamin Bikman — a Ph.D. scientist who specializes in insulin resistance. Dr. Bikman earned his Ph.D. in bio-energetics and was a postdoctoral fellow at the Duke National University of Singapore in metabolic disorders. Currently, his professional focus as a scientist and professor at BYU is to better understand chronic, modern-day diseases with an emphasis on the origins and consequences of metabolic disorders (including obesity and diabetes). He frequently publishes his research in peer-reviewed journals and presents at international science meetings. During this interview, Dr. Bikman discusses why diabetes is more of an insulin disease and less of a glucose disease. He describes the glucagon to insulin ratio and what that actually means for you — and how you can use his findings to improve your blood sugar control. He also speaks about the important role of glucagon in metabolism and blood sugar regulation and gives his formula to optimize protein consumption. Dr. Bikman also talks all about why we should not be afraid of protein in regards to gluconeogenesis in the liver, and explains why someone eventually moves from prediabetes to type 2 diabetes and what the triggering event might be. He explains why insulin is the worst treatment for type 2 diabetes, and gives his insight and recommendations for a low-carb, low-fat diet — and goes into detail about why it makes the most sense from a scientific perspective. Key Takeaways: [:58] About Dr. Mowll’s guest today, Dr. Bikman. [3:20] Head on over to iTunes and leave a review and subscribe to the Mastering Blood Sugar podcast. Dr. Mowll will be reading a review and sending you the best of the Diabetes Summit on a flash drive as thanks. [4:24] Welcoming Dr. Benjamin Bikman to the podcast! [4:51] Dr. Bikman speaks about his recent talk at the Low Carb Breckenridge event, as well as the glucagon to insulin ratio, and people’s fear of protein because of gluconeogenesis. [7:02] What glucagon does in contrast to insulin. [8:56] How does the body balance the glucagon hormone for type 1 and type 2 diabetics? [13:00] What is insulin’s main role? [15:29] Is the insulin’glucagon ratio something that’s clinically useful or it more of a research tool to learn about how these hormones interact with the body? [17:49] Dr. Bikman summarizes some of his findings and key takeaways that he presented at the Low Carb Breckenridge event — especially in regards to gluconeogenesis. [20:51] Should insulin-resistant individuals (prediabetic or early type 2) with hyperinsulinemia be careful about the proteins they consume? [25:59] Why someone with extra weight may not need extra protein to fuel their metabolism. [28:51] Dr. Bikman’s definition of type 2 diabetes. [33:00] Dr. Bikman’s story of how a talk he did made a physician realize he was treating his patients wrong. [35:54] How does Dr. Bikman feel about the other model to reverse insulin resistance (that is being pushed by the vegan community)? The model says insulin resistance is caused by ectopic fat, and by cutting fat down to nearly 0% in the diet, the fat can be burned and reverse the insulin resistance. [41:22] What else promotes ceramide accumulation other than keeping your insulin low? [44:25] What Dr. Bikman believes to be the central message of his work. [47:30] If Dr. Bikman had a good friend of family member that was diagnosed with type 2 diabetes and had three minutes to give them advice, what would it be? [52:59] If Dr. Bikman was sitting around with the leaders of the world to solve the diabetes epidemic, what would be his advice?
