In this evidence-packed episode, Dr. Scott Watier and Tommy Welling dissect a December 2025 systematic review and meta-analysis from Frontiers in Nutrition examining how intermittent fasting improves metabolic syndrome outcomes compared to traditional calorie restriction. They reveal that participants with metabolic syndrome—defined as having three or more risk factors including elevated waist circumference, high triglycerides, low HDL, elevated blood pressure, and high fasting glucose—experienced significant improvements in fasting blood glucose, A1C, insulin resistance, LDL cholesterol, and inflammatory markers with high certainty of evidence when using various IF protocols. The hosts explain why smart, experienced people spend years tracking and counting calories yet feel stuck, emphasizing that intermittent fasting addresses the root drivers of metabolic dysfunction rather than just creating a caloric deficit through willpower and restriction. They demonstrate how modified alternate-day fasting showed 50-100% greater reductions in inflammatory markers over 16 weeks, and why time-restricted eating delivers faster blood sugar improvements, providing practical guidance on matching your fasting pattern to your metabolic profile while focusing on beneath-the-surface changes happening long before the scale or the world can see them. Dr. Hoffman continues his conversation with Dr. Bret Scher, medical director of the Coalition for Metabolic Health.
Dr. Bret Scher, medical director of the Coalition for Metabolic Health, discusses making metabolic health the foundation of medicine amid rising obesity and type 2 diabetes and reports that 93% of Americans have suboptimal metabolic health. Scher defines metabolic health using markers including glucose, insulin, triglycerides, HDL, blood pressure, and waist size, and cites evidence linking insulin resistance to heart disease, stroke, cancer, psychiatric illness, and other complications. They discuss simple self- and lab-assessments (waist-to-height ratio, fasting insulin with glucose/HOMA-IR, triglyceride-to-HDL ratio, CGMs). Scher critiques the Eat Lancet report for assuming one optimal diet, reliance on low-quality nutrition epidemiology, potential nutrient shortfalls, and environmental oversimplification, while supporting newer dietary guidelines that allow lower-carb approaches. Part two covers contradictory nutrition studies, distinctions between low-carb and ketogenic diets, emerging “metabolic psychiatry” and ketogenic therapy for mental illness and cognitive decline, limits and rebound risks of GLP-1 drugs, and Coalition efforts to improve school food and influence policy.

Is obesity really just about eating too much — or is something deeper driving it? In this episode, we sit down with Dr. Yasmine Elamir to unpack the concept of “diabesity” — the intersection of insulin resistance, blood sugar regulation, and chronic overeating. We explore how glucose swings influence hunger, why many people struggling with weight may have underlying insulin resistance, and how muscle, sleep, and diet can improve metabolic health over time. We also take a thoughtful look at GLP-1 medications, why some patients stall on them, and the biggest mistakes people make when relying on medication without addressing behavior and lifestyle. Dr. Elamir explains how to interpret glucose patterns, determine carb tolerance, and break emotional eating cycles using practical strategies. If you want a deeper understanding of the biology and behavior behind modern obesity, this episode delivers a clear, evidence-based perspective. You've treated the pain. You've tracked the triggers. You've adjusted food, sleep, and stress and yet migraines keep finding a way in.What if the missing piece isn't in your head… but in your metabolism? In this episode of Migraine Heroes Podcast, host Diane Ducarme explores one of the most overlooked drivers of migraine: the thyroid. Not as a single lab value, but as a system that sets the rhythm for your brain, your nervous system, and your tolerance to pain. The thyroid doesn't just influence weight or energy. It acts as a metabolic pacemaker, shaping blood flow, heat production, neurotransmitter balance, and stress resilience. When that rhythm slows or becomes unstable, the migraine brain becomes far more reactive — even to triggers that once felt manageable.Blending modern neuroscience with an Eastern medicine lens, this episode unpacks why migraines often show up alongside fatigue, coldness, brain fog, pressure headaches, and that persistent feeling of running on empty. In this episode, you'll learn:Why the thyroid functions as the metabolic pacemaker for the brain and how a slowed rhythm lowers your migraine thresholdHow reduced internal “fire” contributes to dampness, heaviness, and pressure in the headFive subtle yet powerful ways a struggling metabolism signals the nervous system to trigger migraineWhy thyroid-linked migraines often feel slower, heavier, and harder to shakeHow restoring rhythm, warmth, and flow can change how your migraine brain respondsThis episode isn't about diagnosing disease or blaming a single gland. It's about understanding the deeper patterns your body is communicating and responding before those whispers become pain.If your migraines come with fatigue, cold sensitivity, brain fog, or a sense that your system just can't keep up anymore, this conversation may finally bring clarity.

Exploring the Benefits and Science of CBD and Canna Mimetics with Expert Maggie Frank, the national educator for CV Sciences, makers of PlusCBD Oil. She has a deep-dive into the latest updates on CBD (cannabidiol) products, their benefits, and distinctions from THC (tetrahydrocannabinol). Frank explains the endocannabinoid system and how phytocannabinoids like CBD can help achieve homeostasis in the body, aiding in stress management, inflammation, and gut health. They discuss the nuances of different cannabis plants, the current landscape of CBD research, and the misconceptions around THC use. Additionally, Maggie introduces some innovative CV Sciences products, including those that do not contain CBD but offer similar benefits through canna mimetics—natural compounds that mimic the effects of cannabinoids. Among these products are a focus-enhancing gummy and a metabolic support formulation called ReShape. They also touch upon upcoming legislation and how it impacts the hemp industry. The discussion highlights the broad-spectrum potential of CBD and related compounds in promoting overall health and addressing various conditions.

Welcome to my podcast. I am Doctor Warrick Bishop, and I want to help you to live as well as possible for as long as possible. I'm a practising cardiologist, best-selling author, keynote speaker, and the creator of The Healthy Heart Network. I have over 20 years as a specialist cardiologist and a private practice of over 10,000 patients. Dr. Warrick Bishop, a cardiologist and author, discusses reversing metabolic syndrome, which he also calls the "modern syndrome." He explains that metabolic syndrome is not a disease itself but a warning sign of increased risk for heart disease, stroke, type 2 diabetes, kidney disease, and dementia. The syndrome is characterized by having at least three of five criteria: elevated waist circumference, high triglycerides, low HDL cholesterol, high blood pressure, and elevated fasting blood sugar. The root cause is often insulin resistance, where the body's cells stop responding properly to insulin, a hormone responsible for energy storage. This resistance is exacerbated by modern diets high in carbohydrates and a sedentary lifestyle. Other contributing factors include sleep deprivation, chronic stress, and genetic predisposition.
A long-term Framingham Heart Study analysis shows that regular physical activity beginning in midlife and continuing into older age significantly lowers the risk of dementia, including Alzheimer disease, with the most active individuals experiencing roughly 40–45% risk reduction. A phase 3 trial of the first oral PCSK9 inhibitor, enlicitide, demonstrated substantial LDL-C and apoB reductions in patients with heterozygous familial hypercholesterolemia already on statins, offering a well-tolerated oral option that helps most patients reach guideline targets. Finally, a large U.S. cohort study revealed persistent under-treatment of chronic hepatitis B—especially among women and non-Asian racial groups—highlighting the need for standardized assessment and timely antiviral therapy to close these gaps.
