(2021). Family presence during resuscitation in paediatric and neonatal cardiac arrest: A systematic review. Resuscitation, 162, 20-34. https://doi.org/10.1016/j.resuscitation.2021.01.017.Presenter: Mariez Gorgi (emergency medicine advanced trainee).Summary: This systematic review set out to determine the impact of family presence during resuscitation (specifically in cardiac arrest) on patient outcomes, family-centered outcomes and healthcare-provider outcomes. A total of 38 papers were included in the review; consisting of observational studies, qualitative interviews or surveys, spanning from 1999 to 2019 in 11 different countries. No papers explored the impact of family presence on patient outcome. Most parents wished to be offered the opportunity to be present in the resuscitation in order to provide comfort to their child, as well as assisting in accepting the outcome and knowing that everything that could be done had been done for their child. The opinion of healthcare providers was more widely varied, citing the concern of psychological trauma to the parents, interference with management of the patient and stress on the treating team (performance-related anxiety). However, the more experienced a healthcare worker with having family presence in a resuscitation, the more agreeable they were to this. However, the evidence provided in all papers was of very low quality, and there is an urgent need for better quality data surrounding this topic. Important things to consider with family presence is to flag that you may need them to leave the room during the resuscitation, and having a dedicated support person for the family present. Hot and cold debriefs involving all relevant staff (paramedics, nursing staff, allied health, doctors) are also incredibly important following a resuscitation such as this. Take-Home Points: It would be great to have better studies looking into family presence in resuscitation (looking into outcomes for the patients themselves as well as better qualitative data looking into what happens to families and healthcare workers afterwards). If there is family around, they should be asked whether they want to be present in the resuscitation. There should be a resource allocated to families to prevent their presence taking away from the resuscitation itself. If you are struggling following a resuscitation, seek support. So much was going on, so much was uncertain and uneven as Louise mulled over the decision for Archer to have permanent pacemaker surgery and experienced collaboration with the medical team for the first time. There were tough decisions to make as the Senft family's community surrounded them with love and support in tangible ways. Self-advocacy looks different for everyone. That's what Louise Phipps Senft realized when her husband Billy asked for time alone with Archer at the hospital, and what she has continually been reminded of throughout her marriage. Some people need space and time as a result of trauma, and some people need immediate, warm connection. In Season 2: Episode 7: Don't Settle For Less, Louise recounts her advocacy for a meeting with 3 of Archer's doctors and Billy's advocacy for his own needs, like time with his son and a solid plan to pick Dutch, their other son, up from camp. She also reflects on how past childhood trauma and generational trauma impacts a marriage, and she shares with us an approach she calls the The Positive Frame based in neuroscience findings that helped her navigate to help others see what was possible. When Louise's sister Lillian Johnston arrived in Atlantic City with her tiny backpack, Louise felt a pang of relief, of release. Her sister had arrived on a whim but helped create a healing space around Archer and Louise just by her presence at a time when the family was feeling powerless as the medical staff was running out of treatment options. In Season 2: Episode 6: Healing Sanctuary, Louise scans Archer's hospital room and realizes it could be transformed into a healing sanctuary, a spa-like experience for Archer. She reflects on power and looks back at how the healing sanctuary was created and the ways it rippled out and positively impacted her own psyche and the work of the medical team around Archer. 50 hours after Archer's cardiac arrest, Louise turned to journaling to make sense of the confusing medical world around her and her family. Das Jahresende ist so eine Zeit mit viel Nebel, sehr kurzen Tagen und wenig Sonne. Da möchte man gerne weit weg sein, an einem sonnigen warmen Ort und sich wohlfühlen. Deshalb bringt uns das Babebibobu, das Zauberflugzeug, diesmal zu einer Trauminsel. Wie ihr in der Geschichte merken werdet, passt es dabei sehr gut etwas genauer auf die Atemübung einzugehen. Das Atmen geschieht im Körper ja ganz automatisch, ohne dass wir etwas machen müssen, zum Beispiel auch im Schlaf. Durch das Atmen kommt der Sauerstoff in unser Blut und wird so im ganzen Körper verteilt. Beim Einatmen füllt sich die Lunge und der Brustkorb mit Luft und drückt die inneren Organe nach unten Richtung Bauch. Alles ist angespannt. Beim Ausatmen senkt sich der Brustkorb wieder und alles entspannt sich. Wenn du das mal beobachtest, merkst du, wie ein Rhythmus entsteht von ein und aus, auf und ab, hin und her. Und es geschieht ganz von selbst. Allein das Beobachten des regelmäßigen Atmens ist schon sehr beruhigend.
