Meet Dr. Benjamin Hardy Dr. Benjamin Hardy is an organizational psychologist and bestselling author of Willpower Doesn't Work and Personality Isn't Permanent. His new book, The Gap and the Gain, comes out in October with Hay House. Ok, ready to dive into the Gap and the Gain? Let's do this, my friend! What is the Gap? Dr. Benjamin Hardy explains that the gap is all about how you measure yourself and specifically measuring yourself against an ideal or something you wish were different. This kind of measuring always leads to unhappiness and, in psychology terms, is known as the hedonic treadmill. The gap is always about operating from a place of need, lack, or needing something else than what is in the moment. By default, when you're in the gap, you are out of the present moment. What is the Gain? With the gain, you no longer measure yourself against anything else. You measure yourself from your past by looking at the progress. You make yourself the measuring stick, not someone or something else. This is quite profound and powerful. Intrinsic motivation comes from this kind of frame. The gain is twofold: You can see your past from a positive lens and recognize the difference between your former and current self. You learn to see everything (you read that right) as a gain. We can train ourselves to see the gains through selective attention, and what you train yourself to see is what you'll want to start measuring. What you measure grows. The gap and the gain are a frame or perspective. You always have an opportunity to take ownership over your life, experiences, and how you view them. This will help you to become better, not bitter, as Dr. Benjamin Hardy said. How to stay in the Gain Here are some great ways to train your brain to stay in the gain: Pull out your journal and list 3 gains at the end of the day. You can also chart 3 wins you're expecting to get the next day. Find wins and gains in all things. Look out for the gains, even when you're in the gap. Write out all the ways you've grown, learned, and progressed over the past year. Do this as often as possible. Get ready to be wowed by and with yourself! Rather than measuring yourself against others, be inspired by them. I've personally found this to be very helpful when dealing with any kind of comparison or 'gap' issues. How to be Happy Happiness is truly about being in the here and now; otherwise, it's contingent on something else. Happiness is dependent on what and how you measure yourself! As Dr. Benjamin Hardy said, happiness is a one-player game where you're not worried about what others think. Final Thoughts As part of The Gap and Gains, we learn to start measuring ourselves against our own achievements. Where we are today versus where we've been. No one and nothing else matters when it comes to defining yourself. This retraining of your mind and habits can be practiced in the exercise above. Meet Ainslie MacLeod Ainslie MacLeod is an internationally acclaimed past-life psychic, spiritual teacher, and award-winning author of The Instruction, The Transformation, and most recently, The Old Soul's Guidebook. If you're tuning in, we already established that you are most likely a pretty old soul. Anyone listening to this podcast would be, by definition, in the old soul category, as so much of my interviews are about connecting to your higher self and spiritual connection. This is a conversation most old souls are having...with one another and with oneself. Welcome, old soul, to this powerful convo that I hope will expand your thinking and way of being, especially right now. Are You An Old Soul In The Instruction, Ainslie MacLeod reviews the ten soul ages in great detail. In this talk, he explains that the first five soul levels are what he'd consider the 'young soul years', which is really about learning, lessons in building community, exploring, and typically are connected more to fear. After you cross over soul age 5, the soul levels of 6 - 10 are considered old souls and are all about learning how to live in love. By definition of old soul, they are tuned to introspection, spiritual connection, and working on self and creativity. The younger your soul age is, the more fear-based your life will be. The more lives you've lived, the more comfortable you are with soul-based living and experiencing uncertainty. Ainslie explains that we chose our personalities and life plan of who we are and why we're here, on the astral plane, before coming to earth. You might ask, what is the astral plane? He describes it as the nearest non-physical plane and dimension where the soul leaves the body or before it comes into a body. We live now on the physical plane, but we choose our life plan on the astral plane. Our spirit guardians also live in this astral plane. See below to learn more about the spirit world. Connecting to the Spirit World Think of the spirit world as your team of support, in the non-physical world, ready and eager to guide you 24/7. All you have to do is ask and, as Ainslie says, use them to support you on your journey. If you have a question for your spirit guide, it's helpful to verbalize it by writing it out to them. I've been doing this for years, and it is beyond powerful as it has helped me personally access greater wisdom beyond what my mind can come up with. We live lives that are so noisy, so you will need to bring in tranquility and a ritual of stillness to start bringing in the ability to hear and commune with the spirit world. The veil becomes thinner as you experience more lives, as the soul and conscious mind are fused. The Soul of our Country I found this answer to be fascinating and frankly rang true with my own soul and inner knowing. Ainslie MacLeod described our country (the US) as being the 5 soul age and level of consciousness, meaning we're still in a fear-based consciousness but moving to level 6, which is characteristically changing to an older soul experience of life. You could think of Fox News as the quintessential level 5. Typically younger souls are more conservative, and older souls tend to be more progressive. Countries that are considered older soul-based are the Scandanavian countries. They are the definition of old soul, where their focus is mainly on the good of the community and people, focusing on humanitarian service. The more fundamental-based countries would be younger soul-based. Be an Ambassador of Love In simple terms, we are all doing the best we can in our varying soul levels, wanting to experience and learn the fundamental lessons of peace, love, and truth. I use this affirmation every day, and Ainslie confirmed it for me when he said, in simple terms, declare that you are an ambassador of love. That is my hope for you and all of us, to embody this soul-based living, this ambassadorship of love, and peace, with ourselves and our soul brothers and sisters. Final Thoughts As part of The Old Soul's Guidebook, Ainslie MacLeod tells us how tapping into our old soul's experiences will lead to not only a more fulfilling life for ourselves but our country and planet. Soul-based living is, in essence, the definition of old soul. How can we deal with unchecked sin in the church? And how do we ensure that we have safe people to confess to and receive accountability from? What happens when leaders do not create this structure in their lives? In this episode, Holly and I talk with wife, mom, author, and Facebook ads consultant Beth Nyhart. Beth shares her story of pornography addiction and how this has been redeemed in her life and work. She walks us through why it’s important to be accountable to others in the context of the church and how the church can encourage and equip those struggling with chronic sin. Beth’s Leadership Journey: Growing up in a Christian home, Beth was very engaged with her church and youth group. She had a reputation as one of the “good kids.” As a student, she led the youth drama team, started the first live worship band, and was a student representative on the church board. Beth believes people felt that she was a leader because she was available and responsible. Behind-the-scenes, though, Beth was struggling with a pornography addiction, something she’d been battling for years. In college, she tried all different majors, but didn’t find the right fit. Then she got married, had a baby, and “dropped off the map.” She didn’t hold a leadership position for a while, which was hard for her extroverted, productive personality type. Recently, God has been slowly putting her back into leadership. She first found “her people” online when she started a mastermind, which encouraged her to write a book of her story. Because of her consulting work and her mastermind group, she knows that being a leader has done a lot for her confidence. Through the book-writing process, Beth has wanted to share how she lives out what she preaches by posting on Facebook and her website. Why We Need Accountability As Leaders Throughout her life, Beth has had many different mentors hold her accountable, and she’s always seeking out who can disciple her. Currently, her mastermind group, accountability partner, and therapist mentor her. Beth believes strongly that leaders need to have accountability in order to stay healthy and avoid falling into sin. As we have seen recently, more and more leaders’ “secret lives” are being exposed, and it shows us the need for having a structure in place for this. It’s valuable to have several people holding you accountable so that you are not relying on just one person to keep you on track. How Beth Embraced her Wiring as a Leader: Beth hasn’t found personality tests to be practical to her, but her mastermind group has taught her so much about herself. With a group that holds her accountable to change and is committed to communication, she knows they will call her out when necessary. Her mastermind makes space for her and she makes space for them. Beth’s Advice for Younger Leaders: Most importantly, let go of your expectations of what life and leadership will look like. Give God access to everything in your life. Remember that effective leadership may look like being vulnerable. Obey what God is calling you to do or share. If you’re unsure, sit on your idea for a few months. Beth’s Struggle with Pornography Addiction: Beth was introduced to pornography by age eight and this introduction became a full-blown addiction when her family got the internet at home. Because she was viewed so highly both those in her church youth group, she didn’t feel like she could talk to anyone about her struggle because they had a different perception of her. Her struggle went unchecked from ages eight to 18. Beth attended a small discipleship school with the purpose of dealing with her pornography addiction. Her advisors there helped rebuild her understanding Christianity, who God was, who she was, and how she needed to interact with the truths of the Bible as well as the Holy Spirit’s prompting. This process took another 10 years. It took time for her to feel comfortable talking about her long struggle, but she felt God’s prompting to write about it. This idea annoyed Beth, but God impressed on her that He’d redeem the worst of her story. Beth explains that when we come into relationships with our strengths, we neglect what unites all of our: our sin nature. Beth encourages us to get to a place where we can share our struggle so we can actively deal with our sin. How the church can help those struggling with pornography If your church offers resources, make sure that people know about them through testimonials and sharing what’s available. Start looking into other ministries that help such as Pure Desire Ministry and Bravehearts. Follow authors on social media who write about Christianity and pornography addiction If your church feels unequipped or too busy to help those struggling, consider these steps: Remember to open up conversations so that shame doesn’t prevent people from getting help. Beth’s Everyday Life: Beth works from home, has a four year-old, and is in the midst of launching a book so her life feels really full. Here are two habits that are helping her right now: Develop a daily routine. Swap kid-watching time. Session 59 Looking at the Psychiatry Match data, it's easy to see that it is becoming a more popular field. I discuss all the data in today's Specialty Stories podcast. Today, we cover Psychiatry match data based on the 2017 NRMP Main Residency Match Data. [03:20] General Summary There are 236 programs in psychiatry. Comparing it with other specialties, pediatrics has 204 programs. So there are 32 more psychiatry programs than there are pediatric programs. The total number of positions offered for Psychiatry is 1,495 spots. This means a little over 6 spots per program. Whereas pediatrics is much bigger with twice as many spots of 2,738 in 204 program. So it's almost 13 1/2 spots per program - almost double the size of psychiatry programs. There were 3 unfilled programs in 2017 and the total number of U.S. Senior applicants for those 1,495 spots was only 1,067. These are the students at MD medical schools who are still in school, and not those that have graduated. So there were less students applying to those spots than there were spots available. There were 2,614 applicants. It's almost 1,200 more applicants than there were spots available. This is still pretty competitive to apply. Looking at the number of those that matched, only 923 of those 1,067 U.S. Seniors did match while over a hundred of those did not match into Psychiatry. It was only about 61.7% of those that matched are U.S. Seniors. Looking at other fields that matched, Anesthesiology was 66.8%, Dermatology at 92.3%, and PGY-2 positions for dermatology is 81.8%. [07:05] Types of Applicants and Growth Trends Table 2 of the NRMP Match Data for 2017 shows the matches by specialty and applicant types. For Psychiatry, there were four spots that went unfilled, 923 were U.S. Seniors and that's about almost 62%. 49 were U.S. Graduates. These were students who went to anMD school but had graduated already. Maybe they didn't get in the first time or they were just taking a gap year doing some research or travel, whatever. "A good percentage of osteopathic students are getting into Psychiatry." There were 216 osteopathic students, 166 U.S.-Citizen International Medical Graduates, and 137 non-U.S.citizen International Medical Graduates. There were four unfilled positions. Table 3 shows the growth trends covering 2013 to 2017. Psychiatry is growing a bunch, about 5% every year. In 2017, it grew 5.2%. Table 7 shows positions offered and number filled by U.S. Seniors and all applicants from 2013 to 2017. There's an interesting trend in Psychiatric that it's becoming more popular among U.S. graduates. When you go back to 2013, it was only about 52% of the class who were U.S. graduates. "If you're interested in Psychiatry, it seems to be growing. Hopefully there'll be spots for you as you continue down your training path." In 2017, U.S. Seniors comprised 61.7% while in 2013, it was only around 52% and been going up year over year. Table 9 shows you how big a specialty is int he grand scheme of specialties. Ophthalmology is a separate match so that's not included. For Psychiatry, 5.4% of all students who matched, matched into Psychiatry. Just to give you a scale, Anesthesiology was only 4.1%, Emergency Medicine is 7.4%, Internal Medicine is 25.6%, Family Medicine is 11.6%. OB/GYN is 4.7%. [11:40] Osteopathic Students And if you're an osteopathic student interested to know your chances, Table 11 will show it to you. 7.4% of all DO students who matched into an MD program, matched into Psychiatry. Compared to the rest, Family Medicine and Internal Medicine are huge, Anesthesiology is 5.6%, Emergency Medicine is 9.6%. So the ratios are very similar. Figure 6 shows the percentages of unmatched U.S. Seniors and Independent Applicants who ranked each specialty as their only choice. If you are only ranking Psychiatry, there's a total unmatched percentage of 30.8%. But don't let that scare you if you're a U.S. Senior. Because their unmatched percentage is only at 7.4%. 52.9% of those who are independent applicants (ex. DO students, U.S. citizen and non-U.S. citizen international medical graduates, Canadian students) did not match [13:13] SOAP Let's go to the SOAP (Supplemental Offer and Acceptance Program). There were four unfilled programs, when you look at the data. But interestingly, when you look at the SOAP for 2017, there were only three available positions. One of those positions was either filled or taken off of the board. So there were two programs with three positions available. And all three of those spots were filled through the SOAP. [13:45] NRMP Charting the Outcomes 2017 - Contiguous Ranks Based on the 2016 NRMP Charting the Outcomes, Table 1 shows the number of applicants and positions in the 2016 match by preferred specialty. Psychiatry has 1,586 spots, 2,134 applicants, number of all applicants per position was 1.54, as one of the highest on this list. There are a lot of International Medical Grads and DO students are applying for these spots. Specialties like vascular surgery only has 56 positions and the total number of applicants per position is 1.91. The only one here above Psychiatry other than that is Neurosurgery which is 1.58. So there are a lot of applicants for those spots. Chart 4 tells the median number of contiguous ranks of U.S. Allopathic Seniors. Those who matched in Psychiatry ranked 9 programs in a row; while those that did not match only ranked 5. "You need to apply to a lot of programs and rank a lot of programs. That is the same across the board for every specialty." Further down the report is Table P-1 is the summary stats for Psychiatry. The mean number of contiguous ranks for those who matched is 9.6 while those who didn't is 5.6. Mean number of distinct specialties ranked is 1.1 for those who matched and 1.3 for those who went unmatched. For those who did not match were a little bit wishy washy with their specialty choice. [16:30] Step 1 and Step 2 Scores, AOA Members, and PhD Degree Step 1 scores for Psychiatry is not as high as some of the other programs. Psychiatry has 224 for mean Step 1 score and 214 for those that did not match. For Mean Step 2 Scores, 238 for those that matched, and 226 for those that did not. For AOA (the honor society for medical students) members, only 6.2% of the students who matched were AOA members while none of those that did not match were AOA members. 4.4% had a PhD degree and 0 unmatched had a PhD degree. You could look at that saying that you have to have a PhD degree to match into Psychiatry or you have to be an AOA member to match. But you have to look at the data yourself. [18:00] 2018 Medscape Lifestyle and Compensation Reports The 2018 Medscape Lifestyle Report shows the highest outside of work is 61% for Allergy Immunology and the lowest is Cardiology at 40%. Psychiatry is right in the middle at 51%. But let's also look at the 2017 NRMP Main Residency Match Data. Psychiatry and mental health are at the bottom for burnout at 42% versus Emergency Medicine at 59%. How severe is the burnout, they're much lower at 4 on the scale. "Not surprisingly, the question which physicians are most burnt out, Psychiatry and Mental Health are at the very bottom. They probably have some coping skills." Which physicians are happiest outside of work and at work? Psychiatry is lower on the list with 66% happy outside, and 37% are happy at work. And they're found on the lower end of the scale. Looking at the 2017 Medscape Compensation Report, Psychiatry is on the lower end. That's understandable because usually, the more procedure-heavy specialties are compensated more. Their average annual compensation is at $235K while Orthopedics is up at $489K. So that's a big difference. Pediatrics is the lowest at $202K. However, they're getting 4% salary increase year over year. Slide 18 shows which physicians feel fairly compensated and Psychiatry is top 3 at 64%, next to Dermatology and Emergency Medicine (first). Slide 38 is a question about whether they'd choose medicine again and Psychiatry is top 2 at 82%, Rheumatology is at 83%. 87% of Psychiatry also say that they would choose the same specialty again. The highest is Dermatologist and Orthopedics is next. [21:38] Final Thoughts As mentioned earlier, we are in need of more physicians to interview. Today's question is about whether an applicant's medical history would affect their application to medical school, which is a common question we get. Do you need to mention it in your personal statement? If so, how will this affect your application? [04:00] OldPreMeds Question of the Week: “I am currently back in school working on completing course prereqs for medical school. I'm just wondering, is a medical history taken into account when applying to medical school especially mental health? In 2016, undiagnosed OCD led me to develop an eating disorder for which I'm currently in recovering as well as chronic depression. I'm wondering if these life events could affect my medical school application. Aside from my mental health history, I am working towards a strong application (knock on wood). My undergrad GPA is 3.9. My current science and math GPA is 4.0. I'm about to begin volunteering at a hospital and I work 24+ hours a week in an advanced stage dementia nursing home, basically, as a CNA. I serve as a peace corps volunteer and I'm nearly fluent in Spanish.” Here are my thoughts: [05:00] Medical School Application Luckily, for this student, their mental health has obviously not affected their grades. Ultimately, what the question comes down to is, what do I have to tell the medical schools? The plain answer is nothing. There is nothing on the application that asks about your health. What could come up is if you had any significant gaps in education and you have to explain those. And even if you do have those gaps in your education, you don't have to explain those by saying you've had chronic depression, OCD, or an eating disorder. Instead, be very generic and say you've been dealing with some health issues that have gotten under control and since coming back to school, you've been fantastic. And it shows! [06:34] Medical License Application When you go for your medical license, you will be asked a question about any health issues that are going to prevent you from taking care of patients. It doesn't sound like this poster has anything that would prevent them from taking care of patients so this is not something to worry about. However, this comes into play if you have brain tumor and it's affecting your cognition and your ability to think and control impulses. Then you're starting to deal with some questionable issues. If you have a vision problem or severe carpal tunnel, for instance, and you're a surgeon then maybe that's a problem. You're going to have to disclose that on your applications for your medical license as well as your credentials at the hospital. [07:33] A Red Flag Whatever you have is none of the medical school's business. I highly recommend that even if this has motivated you to go into medicine, be very careful about talking about those things on your application and in your personal statement. This could pose a big red flag. Medical schools are looking at thousands of medical schools so why would they take a chance on you when they can just put you aside and look at somebody else, just as qualified as you are, that doesn't have any red flags? Back in Episode 194 of The Premed Years podcast, I talked to a student who is legally blind from a condition and he applied to a medical school. His personal statement was all about his blindness and how that's motivating to continue. However, he didn't get into medical school and thought maybe he shouldn't have done that. The next time he applied, he didn't mention anything about his blindness. He was able to hide it to a certain extent, like if it was bright out, he was able to see well enough that he didn't need a walking stick. Then he got accepted to medical school and the packages that came after his acceptance asked him about whether he needed accommodations and that's when he said he's legally blind. Currently, he is residency and is doing very well. [09:46] Final Thoughts As you are struggling with mental health issues or whatever you're dealing with, guess what? We are all dealing with something. One of the reasons I left the Air Force and I'm doing the podcasts full time now and I don't practice medicine anymore is I was diagnosed with MS (which was also one of the reasons I got into the Air Force instead of doing Orthopedic Surgery which was what I really loved to do but it could have made my disease worse but it wasn't worth it). Hopefully, this discussion has helped you figure out whether this is something that's going to affect you. Usually, it doesn't. We have our own concerns like how the admissions committee members are going to look at you. But for the most part, your business is not the medical school's. Keep it to yourself and keep taking care of yourself on your journey. Hopefully, you will do very well.
