“The concepts of sex surrogates and surrogate partner therapy were originally introduced by pioneering gynecologist Dr. William Masters and sexologist Virginia Johnson to help men overcome sexual dysfunction. Currently, sex surrogates work with both men and women. In surrogate partner therapy, the sex surrogate takes a client through a multi-step process, beginning with very basic tools of communication and touch. Surrogate partner therapy can include mutual nudity and two-way touch, as well as manual, oral sex or sexual intercourse. While other forms of sexual help – like experiential sex coaching or therapy can be accessed directly – surrogate partner therapy can only be accessed through a recommendation and ongoing work with a licensed psychotherapist. The therapist, client, and surrogate are thought of as a three-person therapeutic team. The function of the sex surrogate is to engage with the client in exercises to help them become more comfortable and adept at sexual and emotional intimacy. Who Should See a Sex Surrogate? A sex surrogate or surrogate partner therapy is often recommended by a therapist to clients who have a very high level of sexual dysfunction or interpersonal inhibition. These clients need to practice with a partner in a more structured environment to help them overcome their sexual challenges. These challenges might include sexual dysfunctions such as erectile dysfunction, pain during penetration, early ejaculation, or inability to orgasm. Surrogate partner therapy can also help if you are highly anxious sexually, have disabilities that make it more challenging to have sex, or have little or no experience with sex. What is Sexual Surrogate Therapy? In the initial consultation, you and your surrogate will get to know one another, begin to build rapport and make sure that surrogate partner therapy is right for you. After that, a step-by-step process begins where you and the sex surrogate communicate through each progressive step of intimacy and touch. One main tool of surrogacy is sensate focus, initially developed by Masters and Johnson. Sensate focus is meant to enhance relaxation, lower anxiety, and help you stay in touch with your sensations and arousal. Sex surrogates also help you process emotions around issues with sex and intimacy. As you move through the phases of the therapy, you may practice back-and-forth touch. Your surrogate starts with the hands, then you might move to the face or other parts of the clothed body. In ongoing sessions, your sex surrogate will take time to educate you about sexual comfort, relaxation, touch, and best practices. You will slowly remove more clothing, learning to give and receive touch on all the different parts of your own and your surrogate's body. “Who Needs Surrogate Partner Therapy? The problems that motivate clients to seek Surrogate Partner Therapy range from general anxiety in social situations to specific sexual dysfunctions. Concerns for any gender might result from one of the following: Negative body image or physical disfigurement Medical conditions Sexual, physical, or emotional abuse and/or trauma (rape or incest, for instance) Intimacy issues Phobias and anxieties Unresolved relationship trauma Confusion about sexual orientation Lack of social or sexual self-confidence. Sexual concerns may include: Orgasmic inhibition Genital or pelvic pain Avoidance of physical and/or emotional intimacy Lack of experience due to anxious avoidance. “Selecting a Surrogate Partner It is important to consider who will be most useful to the client's learning and then to interview and select a professional surrogate partner with the necessary expertise for that specific case. Experience level, physical appearance, and age vary among surrogates. Physical appearance and age are usually the least important criteria when selecting a surrogate partner. It is often desirable to avoid meeting the client's “ideal” in order to provide optimal learning for the client about “real” relationships with “real” people. High-quality training and emotional maturity are essential features of a professional surrogate partner. Supervising therapists may contact the IPSA Referrals Coordinator for assistance in finding a compassionate, well-trained, professional surrogate partner IPSA-Certified Surrogate Partners To assist the therapeutic community and consumers in evaluating the qualifications of individuals offering surrogate partner therapy, IPSA established a certification program. To qualify for IPSA certification, surrogate partners must demonstrate that they have had adequate training; understand the principles and practices of surrogate partner therapy; possess personal qualities commensurate with the large responsibilities of a surrogate partner; and commit to honoring the IPSA Code of Ethics. Intern Surrogate Partners Trainees who successfully complete the first phase of training are approved to begin working as Intern Surrogate Partners. Interns work with real clients, closely collaborate with each client's therapist, and prepare for and review every session with an experienced surrogate partner mentor. Interns are providing benefits to clients while continuing their learning through these real-world experiences. While there are some cases that are too complex for a new intern, many cases are appropriate for intern surrogate partners. IPSA Training staff and mentors help evaluate cases to determine whether an intern has sufficient experience and skills to successfully assist each client. Confirmation of a specific surrogate partner's training, certification, and good standing in IPSA can be verified through IPSA Membership Committee or the IPSA Referrals Coordinator. You may also consult our list of IPSA-Certified Surrogate Partners The Character of a Surrogate Partner Although there are no specific academic degrees required of a surrogate partner, there are certain personal qualities and life experiences that seem to provide a valuable foundation for surrogate partner skills. These include intelligence, compassion, warmth, comfort sharing one's own body and sexuality, empathy, patience, knowledge of sex therapy principles and practices, and good communication skills. Non-judgmental attitudes toward sexual orientation, choice of lifestyle, and consensual sexual activities are also highly important. Gender and Sexual Orientation Surrogate partners work with clients of all sexual orientations and genders. Gender and sexual orientation are often a consideration when the client's difficulties present at the intersection of emotional intimacy and sexuality, and the intention of the therapy is to create experiential learning at that intersection. Today, there are approximately 1,200 RCCs in our country, each tasked with serving their local communities by providing survivor support services and community education. These centers offer different resources based on community needs and financial resources, but the overarching goals remain the same. For those impacted by sexual violence, RCCs oftentimes offer crisis intervention hotlines, support groups, counseling, legal and medical aids (to accompany survivors to medical exams or law enforcement interactions), as well as referrals to other local or virtual resources. In terms of community outreach, some RCCs provide a form of prevention education, advocacy, and/or prevention training. Crisis Intervention/24 Hour Hotline Perhaps the most uniform service across RCCs, crisis intervention hotlines are intended to provide immediate support in the wake of a crisis. In the aftermath of harm, these hotlines serve as a resource for survivors to talk or learn about legal, medical, or counseling options. These hotlines are typically staffed by victim advocates who have undergone trauma-informed training and who are prepared to listen and assist the caller in a time of crisis. Centers offer free crisis and advocacy services since they are typically funded with federal and/or state dollars. Referral Services RCCs may not always provide all of the services someone may need, but they often have a plethora of resources they can direct people toward. For instance, not all RCCs offer therapy or legal resources, but they may know the best options for people who are searching for these services. Sexual assault coalitions also provide great referral services. Education: Training Employees, Volunteers, and the Community Although the required hours of training for staff varies across states, RCCs often provide the training necessary for their staff and volunteers to equip them to effectively serve survivors of sexual assault. Furthermore, most RCCs conduct outreach to the community and provide trainings such as workshops to local schools or organizations on topics such as consent, workplace sexual harassment, or stalking. Accompaniment Some RCCs have staff available to accompany victims to medical exams or legal consultations such as police interviews or court proceedings. These staffers sometimes accompany victims as a form of support, and sometimes they can serve as an advocate for the victim in helping them navigate appointments or meetings. Short-Term Counseling/Therapy/Group Therapy Forms of counseling and therapy services vary broadly across RCCs nationally. These services may also vary in price: Some RCCs will offer a certain number of free sessions after which payment may be required, while others may not be able to offer therapy at all.
