In today's VETgirl online veterinary CE podcast, we're going to discuss subaortic stenosis and the effects of medical management with two different beta-blockers: sotalol versus atenolol. This is based off a study by Tjostheim et al out of University of Wisconsin-Madison entitled “Association of sotalol versus atenolol therapy with survival in dogs with severe subaortic stenosis.”

Lamotrigine is an antiseizure medication and also may be used for bipolar disorder. A rash is a major side effect to remember with this medication. Valganciclovir is an antiviral medication that can be used to prevent cytomegalovirus (CMV) in patients with a suppressed immune system. Fluconazole is an azole antifungal that can be used to treat candidiasis, blastomycosis, and tinea infections. Drug interactions, QTc prolongation, and hepatotoxicity are potential risks. Atenolol is a beta-blocker used to treat atrial fibrillation and hypertension. It is relatively selective for beta-1 receptors meaning that it doesn't affect the lungs as much as non-selective agents. Montelukast is a medication that blocks the actions of leukotrienes. This can be beneficial for the management of allergies and asthma.

In this VETgirl online veterinary CE podcast, we discuss feline HCM and whether the use of beta-blockers are beneficial. So, Coleman and al wanted to evaluate this in a study entitled "Atenolol in cats with subclinical hypertrophic cardiomyopathy: a double-blind, placebo-controlled, randomized clinical trial of effect on quality of life, activity, and cardiac biomarkers." We likely should be dispensing beta blockers - specifically atenolol - less frequently - if at all, as this study didn't show any benefit in quality of life, activity, or treatment benefits for cats with subclinical HCM. When in doubt, pick a different cardiac medication and consult with your local cardiologist!

On this episode, I discuss atenolol pharmacology, adverse effects, pharmacokinetics, and drug interactions. Atenolol is primarily cleared by the kidney which should tell you that we need to pay attention to dose adjustments as renal function declines. Atenolol is a beta-1 selective agent that is NOT a preferred beta-blocker in HFrEF. Pulse and blood pressure monitoring is essential with any beta-blocker like atenolol.

Atenolol is a Beta-1 Selective Beta Blocker Antihypertensive Agent that is also known by the brand name Tenormin. The common indications are for Hypertension (HTN), Angina, and Post-MI Cardiovascular Event Prevention. It also has an off label use for Migraine Prophylaxis. Dosing range is between 25-200 mg PO qd depending on the indication. Should the patient ever need to discontinue the medication they should be tapered off gradually due to risks of exacerbation of treated indication. Common monitoring parameters include but are not limited to Cr and BP at baseline and periodically thereafter, ECG, and 10-year ASCVD risk assessment. Apple juice and orange juice should be avoided when taking this medication because they may prevent atenolol from being absorbed properly. The most common side effects are bradycardia, drowsiness, hypotension, fatigue, and cold extremities. Our Visit to the DoctorFor various reasons, but mostly because of travel, I have not had a medical check-up for twenty-four years - since I was forty. My wife is fourteen years younger than me, and she has never had one. So, being back in the UK 'for good' now, I thought that it was about time that I got that sorted out.We registered with Ravenscourt, a doctor's surgery in Barry, my home town, and were told that it would take six weeks. That was quite a surprise, shock even, but I had heard about the cut-backs, and since we were 'non-essential' and there was nothing obviously wrong with us, we let that go, even though the appointment fell inconveniently on my birthday.So, we went along, at the also inconvenient time of nine a.m. not knowing what to expect. Friends had suggested that a full medical check-up would take 'about an hour'.We arrived punctually at the surgery, awaited our turn, which was also on time, and went up together, although only I was actually called. The doctor was surprised, but was all right about it when I explained that I sometimes needed to clarify some terminology for my Thai wife.That wasn't a problem and the young man took our blood pressure and listened to our breathing. We were both 'A OK'.'How are you feeling?' he asked.'OK', we replied. 'However, my back is pretty bad', I said. 'I can't walk a hundred metres without having to sit down'. He gave us forms to go to two separate hospitals for blood tests and heart traces.'I take these blood pressure tablets', I offered. 'I've been taking them for twenty-four years, are there better ones available now?' I wanted to say that they were effecting me in other ways, but didn't.'Oh, they are working for you, and they are readily available, so I recommend that you stick with them'.'I disagree', I countered, 'The last prescription I had for them in Barry, could not be fulfilled by Boots in Barry. I was looking for sixty and they only had twenty-eight...'His attitude changed visibly - he obviously didn't like being contradicted.Our interview was over, but my back problem hadn't been mentioned, so I asked him to validate my application for a bus pass (I am sixty-four, and one qualifies at sixty).'That isn't my job', he replied. I took a deep breath, could see that we weren't going to get anywhere and left with the strange feeling that there was something he wasn't telling me.
