Tonix Pharmaceuticals CEO Dr. Seth Lederman joined Steve Darling from Proactive to discuss promising topline results from the company's Phase 1, single ascending dose trial of TNX-1500 in healthy participants. The trial evaluated safety, tolerability, pharmacokinetics, and pharmacodynamics of intravenous TNX-1500 and was designed to support dosing for an upcoming Phase 2 trial in kidney transplant recipients. Two published studies in the American Journal of Transplantation demonstrated that TNX-1500 can prevent rejection, prolong survival, and preserve graft function in non-human primate renal and heart allografts, either as a single agent or in combination with other therapies. Dr. Lederman shared that Phase 1 results showed TNX-1500 at 10 mg/kg and 30 mg/kg doses effectively blocked both primary and secondary antibody responses to a test antigen. Pharmacokinetic data suggest that monthly dosing at 10 mg/kg or higher could be effective. These findings reinforce the company's confidence in TNX-1500's potential to prevent organ transplant rejection and improve graft survival with a lower long-term toxicity burden compared to existing immunosuppressive therapies. Tonix plans to present these results to the U.S. Food and Drug Administration (FDA) in an End-of-Phase 1 meeting. Dr. Becca Bruning, Infectious Diseases Pharmacy Specialist at the Moffitt Cancer and Research Institute in Tampa, FL, takes a deep dive into the subject of resistance mechanisms for specific gram negative bacteria. Dr. Bruning begins her talk by discussing the primary resistance mechanisms, including porin channels, efflux pumps, and other target sites. Extended spectrum beta-lactamases (ESBLs) are next covered, including typical bacterial species, identification techniques, ESBL testing, and the use of Piperacillin-tazobactam and carbapenems. Next, Dr. Bruning mentions AmpC-type resistance, and identifies the “HECKYES” bacteria. Pharmacokinetic and phamacdynamic considerations are explored, and specific antimicrobial agents are discussed. Lastly, Dr. Bruning covers the use of carbapenemase testing including the modified hodge test and the Carba-R test.

Xylazine Pharmacokinetics in Patients Testing Positive for Fentanyl and Xylazine  Clinical Chemistry This study of xylazine pharmacokinetics used plasma samples from 28 patients who had urine screens positive for xylazine and fentanyl. The patients were being treated for skin lesions, most commonly, then shortness of breath or opioid overdose. At least two subsequent plasma samples were analyzed for xylazine and xylazine metabolites by LC-MS/MS. The median terminal half-life for xylazine in plasma was 12 hours (range 6-21 hours). Animal studies show xylazine to be extensively metabolized, with little unchanged xylazine eliminated in urine. The two most abundant metabolites were oxo-x and sulfone-x, which did not have a window of detection longer than xylazine. Researchers had no information as to the timing or route of xylazine ingestion or if additional xylazine was consumed during the study period. Episode 59 – Detomidine OTM Gel – The perfect PVP Protocol for Zack!   I really wanted to share Zack's story as I get asked so many questions about how best to handle very fearful and potentially aggressive canine patients in the veterinary setting. The answer is - there is no simple or blanket answer – and every patient must be treated and considered as an individual. It helps, of course, if you have many different tools in your toolbox and you have the knowledge and experience to use these tools wisely and to select the right tool for the right patient.   In Zack's case this was detomidine oral trans-mucosal gel, an equine sedative that can absolutely be safely used in dogs, if you know how. Zack's caregiver was able to apply the gel while playing with Zack before he came to the clinic, and I was then easily able to administer an intra-muscular sedative to facilitate the examination and veterinary care he needed.   Here are the papers on the use of detomidine OTM gel in dogs originally published by Dr Hopfensperger in 2013 and 2016, but the doses in these have been superseded in a recent abstract, which was presented at the Veterinary Behaviour Symposium in 2023.   1.      Hopfensperger, M. This research was completed during her time as a post-doctoral associate at Iowa State University with support from the USDA National Institute of Food and Agriculture, Agricultural and Food Research Initiative Competitive Program, Mitigating Antimicrobial Resistance Across the Food Chain, Grant/Award Number: 2021-69015-33501. Dr. Buckley is currently an Assistant Professor of Clinical Practice at Texas Tech University: https://www.depts.ttu.edu/vetschool/about/meet-our-team/faculty/michelle-buckley/In this episode we discuss two drugs that can be used in an extra-label manner to address subclinical mastitis in dairy goats during the dry period: Orbenin DC (cloxacillin benzathine) and ToMORROW (cephapirin benzathine). Dr. Buckley's work evaluated how long it took for both drugs to leave the goats' bodies after administration in both meat and milk as well as how effective they were at curing subclinical mastitis due to non-aureus Staph. She also evaluated antimicrobial resistance development due to treatment with these long-acting intramammary products. However, these patients often need DOACs to prevent blood clots due to surgery and obesity.The concern is that bariatric surgery might change how the body absorbs DOACs. This could make the medications less effective. There's limited data on long-term DOAC use after bariatric surgery.Dr. Gunka and Dr. Mithoowani's study investigated DOAC levels in patients after bariatric surgery. In this episode of the Psychedelic Medicine Podcast we discuss the topic of psychedelics and postpartum mood disorders. Melissa Whippo is a licensed psychotherapist who specializes in the intersection of women's health and psychedelics. She founded a non-profit, Deva Collective, which has fiscal sponsorship with MAPS, and is currently conducting research about perimenopause and microdosing San Pedro. In this conversation, Melissa shares the details of existing research into psychedelic treatments for postpartum populations. She discusses the findings from her study on “The Pharmacokinetics of Ketamine in the Breast Milk of Lactating Women,” reporting that ketamine was not detected at all in breast milk twelve hours after dosing. She also shares information on parents' intentional use of microdosing and what motivates them to take up this practice. In closing, Melissa discusses her upcoming research on microdosing San Pedro among perimenopausal populations and why she decided to focus on San Pedro for this study.   In this episode you'll hear: The early research into ketamine for postpartum depression and whether ketamine remains in breast milk The prevalence of mood concerns in the postpartum population Issues of access with ketamine treatments for new mothers Improvements in postpartum mood patients following ketamine treatments that Melissa has seen in her practice Hormone changes after giving birth and how this might relate to mood concerns Ketamine for postpartum pain   Quotes: “The reason why ketamine and other psychedelics appeal to me more [than other treatments for postpartum depression], is it's not a medicine you have to take every day and it's not a medicine that's going to cause a significant separation [with the newborn] like a hospital admission would.” [5:55] “More SSRI—which is still considered safe by the medical profession—passes through the breastmilk than ketamine. Probably nanograms. And ketamine has such a short half-life in the system, particularly with the intramuscular route of administration, that twelve hours after dosing, the breast milk was completely clear.” [14:22] “That's what ketamine does so well—it gives people a different view of some of the things they're really struggling with that they can't quite break through. Some of these more recursive ruminations that seem to befall mothers quite a bit—and they're usually pretty negative. Numerous Population Pharmacokinetic (PopPK) models have been developed for Piperacillin (PIP), most of which are based on small monocentric studies and may not be generalizable to other populations. A recent evaluation of six PIP models in 30 ICU patients receiving CI demonstrated large inter-model variability regarding predictability. The transferability of these results to other populations is uncertain due to the limited number of patients and the monocentric setting. Furthermore, a clinically oriented model assessment in conjunction with TDM (Bayesian forecasting) was lacking. A recent study aimed to evaluate the predictive performance of available PIP PopPK models with and without TDM using an external multicenter dataset to facilitate model selection for MIPD in critically ill patients. The practice of critical care cardiology relies on the use of invasive hemodynamics, mechanical ventilation, mechanical circulatory support, and other advanced techniques to help our patients recover from critical cardiac illnesses. To facilitate these interventions, it is essential to have a broad understanding of how sedation and analgesia keep our patients comfortable and safe throughout their time in the CICU. In this episode, series co-chair, Dr. Yoav Karpenshif, and CardioNerds co-founder, Dr. Daniel Ambinder, are joined by Dr. Natalie Tapaskar, cardiology fellow and CardioNerds FIT Ambassador from Stanford, and faculty expert, Dr. Chris Domenico, to discuss sedation in the cardiac ICU. We dissect three cases of VT storm, heart failure associated cardiogenic shock, and cardiac arrest. Pearls and Quotes - Sedation in the Cardiac ICU with Dr. Christopher Domenico Think about analgesia and sedation as separate entities with management of analgesia first and sedation second. Frequent re-assessment of needs should be performed to reduce ICU delirium and improve long-term outcomes. Fentanyl is generally a good starting point for analgesia in the ICU since it is fast on/fast off, but can stick around for a long time the longer it is used. The choice of bolus or continuous infusion opioids depends on the clinical scenario and personal/institutional preference. Remember to administer bolus doses that are 50-100% of the hourly continuous infusion dose to reach steady state faster. When managing refractory VT storm with sedative agents (propofol, benzodiazepines and/or dexmedetomidine), you should target the deepest level of sedation necessary to suppress sympathetic drive. For cardiogenic shock patients, the choice of sedative agent is a nuanced decision. Think about etomidate first for intubation as it has the least cardiovascular and hemodynamic impact. And remember the propofol trifecta: negative inotropy, direct vasodilation, and bradycardia! Pharmacokinetics are disrupted during targeted temperature management, thus be weary of overly sedating patients due to reduced drug clearance.
