A new digital health solution will help Irish women receive help for a previously taboo subject of intimate pain associated with Genito-Pelvic Pain Disorders, GPPPDs. Norma, an Irish company, tackles this widespread but often overlooked and underdiagnosed health issue. The figure for those suffering in silence is staggering. "It is estimated that 10-28% of reproductive-aged women in the general population complain of persistent pain during sexual intercourse". What are GPPPDs? These are conditions that cause chronic pain and discomfort during activities like intercourse, tampon use, and intimate medical exams and generally, there are no universally accepted treatment approaches. As a GPPD sufferer, a female entrepreneur, and an Irish innovator, Norma's founder, Gráinne Byrne suffered from this painful condition. Like most patient innovators she sought to find a solution as she set about her innovation journey to develop a technology for herself and others. Grainne's response was to create a new Irish FemTech digital solution which is an expert-led intimacy education platform. The condition can be isolating so it is important to know that other people in the community are also suffering in silence. Gráinne reassured those with GPPPDs that there is help out there: "My personal experience with vaginismus, a form of genito-pelvic pain, was deeply isolating. I struggled with basic things like inserting a tampon, undergoing an intimate medical exam, or having penetrative intercourse. Each attempted insertion was painful and distressing. Over eight GP appointments, I was repeatedly dismissed or told my pain was 'normal,'" said Byrne. "As the years passed, my mental health and sexual well-being severely declined due to the lack of accessible care and proper guidance," Byrne said, "I founded Norma with Clinical Psychologist & Psychosexual Therapist Dr. Natasha Langan to fill this gap. Our mission is to connect people with accredited psychosexual therapists to provide the expert support and resources they truly deserve, at every stage of life." The unique digital psychosexual health platform offers its expert-led intimacy education through online sexual wellbeing workshops. The overall mission is to increase access to accredited sex therapists. Norma's key goals set out to address treatment for a range of GPPPDs, such as vaginismus and dyspareunia, by delivering evidence-based educational tools. Norma's online workshop programme combines insights from psychology, cognitive therapies, psychosexual therapy, breathwork, and pelvic floor physical therapy to support individuals in managing their conditions and improving sexual well-being. This innovative digital education solution provides accessible support for those with the condition. The programme is a lifesaver for women who may not previously have come forward and sought treatment. It helps participants understand their condition, manage pain, and improve sexual health. A recently completed pilot study conducted by Norma with Health Innovation Hub Ireland showed promising results. HIHI provided the clinical research and facilitated patient feedback necessary to complete this successful pilot. Participants felt that the digital health solution positively impacted their sexual well-being. Confidence was increased by having the access and the tools to manage the symptoms more effectively and consequently strong user satisfaction was reported. Persistent genital arousal disorder (PGAD), now called Genito-Pelvic Dysesthesia, was first described by Leiblum and Nathan in 2001 in a five-patient case series. This is now a well-recognized pelvic floor/genital abnormality with a consensus statement being released in 2019 by the International Society for the Study of Women's Sexual Health. Both tamoxifen and aromatase inhibitors (AIs) are hormonal therapies used in the treatment of oestrogen-positive (ER-positive) breast cancers to stop tumour growth and recurrence and to treat cancer that has come back after initial treatment or that has spread to other parts of the body.These treatments come with the benefits of reducing the risks of cancer recurrence and they also come with a host of unwanted side effects. Menopausa é a parada da produção dos hormônios sexuais da mulher: estrógeno e progesterona; ocorre em média no Brasil com 51 anos. Porém duas coisas são importantes: 1) a menopausa não é só restrita a redução desses hormônios: ocorrem alterações em outros hormônios do corpo como tiroideanos, cortisol, melatonina, vitamina D, DHEA, testosterona, entre outros e 2) ocorrem transformações importantes no corpo da mulher em decorrência das reduções hormonais. Importante ressaltar que antecedendo a menopausa há o período transição menopausal que acontece em torno de 10 anos antes da menopausa; a detecção dessa fase é fundamental para tratamento para reposição hormonal se indicada e alivio dos sintomas com objetivo de reduzir as chances de doenças orgânicas e evitar o declínio da qualidade de vida. A idade menopausal pode ser prevista através do exame para dosagem de hormônio anti-milleriano em fases mais precoces da vida a fim de planejamento do tratamento da gestação, uma vez que a mulher contemporânea engravida mais tarde, e desta forma, pode optar em congelar óvulos, por exemplo. A gestação na transição menopausal é de risco para ocorrência de diabetes, hipertensão e malformações. Portanto duas coisas são importantes: 1) planejamento da gestação e 2) acompanhamento adequado dessa gestação com especialista. Ao redor de 35% dos ciclos menstruais na transição menopausal são ovulatórios na transição e desta forma, mulheres nesta fase podem engravidar espontaneamente. Porém, essa gestação não altera a idade da menopausa e não influencia as transformações do corpo da mulher durante a menopausa. Dra Paula Palacio indica por ordem quais são os hormônios mais importantes para dosagem, acompanhamento e reposição durante a transição menopausal. Primeiro é a melatonina pois melhorar o sono é fundamental para regulagem dos outros hormônios. Além disso, a menopausa é hormônio anti-oxidante que possivelmente inibe ação de substancias oxidantes no corpo que podem causar danos às células. Depois hormônios tiroidianos que são fundamentais no metabolismo do corpo. O cortisol, hormônio de defesa, mantem o paciente alerta e concentrado durante o dia e é produzido durante o sono: o pico sérico ocorre pela manhã, portanto o sono é fundamental para reposição desse hormônio. Em por último os hormônios sexuais (estrógenos, progesterona). Quais os principais sintomas? Aparelho Genito urinário: secura vaginal, irritação urinária por alteração do PH da urina e da vagina e dor na relação sexual. Aparelho ósteo muscular: perda óssea e da massa magra (músculos). Cérebro: irritabilidade, falta de concentração e depressão. Vasomotor: fogachos. Além disso a obesidade, principalmente visceral Quanto ao tratamento há dois tipos: o hormonal e o não hormonal. Importante que para ambos tratamentos a paciente tem que estar preparada e o principal preparo é: estar desinflamada. Mas o que isso significa? A paciente tem que procurar ter um bom estilo de vida com boas práticas alimentares, físicas e psíquica. Isso ajuda muito na reposição dos hormônios, especialmente dos sexuais que poderão ser feitos de forma mais fisiológica, ou seja, com doses mais baixas. Quais contraindicações do tratamento hormonal? A principio, antecedente de câncer de mama, hipertensão e diabetes refratária. Ter parentes de primeiro grau com câncer de mama não contraindica reposição dos hormônios sexuais. O sono regular, alimentação adequada e preparo físico tem papel importante na regulação hormonal e desta forma a reposição hormonal será com doses mais baixas e fisiológicas. Dra Paula Palacio citou as principais formas de reposição, como subcutânea, oral e tópica, sendo a tópica no sistema Genito urinário muito usada nas pacientes com contraindicação. #menopausa #mulher #transição menopausal #hormonios sexuais #exercício fisico #sono

Dr. Brotto holds a Canada Research Chair in Women's Sexual Health. She is the director of the UBC Sexual Health Laboratory where research primarily focuses on developing and testing psychological and mindfulness-based interventions for women with sexual dysfunction and those who experience chronic genital pain. Dr. Brotto is an Associate Editor for the Archives of Sexual Behavior, has >200 peer-reviewed publications, and is frequently featured in the media on topics related to sexuality. Her book, Better Sex Through Mindfulness: How Women Can Cultivate Desire (2018) is a knowledge translation product aimed at sharing the science of sexual desire with a broad audience, and the accompanying Workbook will be published by Greystone in October 2022. She is a strong advocate for empowering women to take on leadership roles.Natalie is a PhD student in the UBC Clinical Psychology program, working under the supervision of Drs. Lori Brotto and Alan Kingstone. She completed her BA in Psychology at Queen's University in 2017, and her MA in Clinical Psychology at UBC in 2020.
