New court filings in Coomer v. Byrne are raising serious questions after years of denials. Patrick Byrne revealed to Emerald Robinson that the filings now acknowledge Eric Coomer had contact with foreign nationals regarding Dominion voting system source code—including overseas contacts and foreigners working in Denver offices. Byrne says this directly contradicts long-standing public claims that Dominion's software was strictly American and tightly controlled, noting that election code is supposed to be handled only by fully cleared personnel. At the same time, Coomer is reportedly asking the court to bar questions about Venezuela, Serbia, and China, even as these foreign contacts are now on the record. The judge has not yet ruled, and the case is rapidly developing.

By Bram Duffee, PhD, EMT-P For EMS providers who respond to a traumatic cardiac arrest, the instinct to initiate CPR is almost automatic. But could this life-saving intervention inadvertently hinder recovery in cases of severe hemorrhagic shock? A recent study, “Effect of Cardiopulmonary Resuscitation on Perfusion in a Porcine Model of Severe Hemorrhagic Shock,” challenges conventional wisdom and offers new insights that could reshape trauma care protocols. The Study: A Closer Look at CPR in Trauma-Induced PEA The research, led by Dr. Patrick Greiffenstein, professor of clinical surgery at LSU New Orleans and trauma ICU director at the Norman McSwain Spirit of Charity Trauma Center, addresses a critical gap in trauma care. While CPR is a cornerstone of resuscitation in cardiac arrest, its application in trauma-induced pulseless electrical activity (PEA) has not been rigorously validated. Trauma PEA, unlike medical cardiac arrest, is a low-flow state caused by severe blood loss (hypovolemia), where the heart shows electrical activity but fails to generate a palpable pulse. “CPR is a cornerstone of resuscitation and cardiac arrest, but its application in trauma-induced PEA has not really been rigorously evaluated,” explains Dr. Greiffenstein. “Trauma PEA is fundamentally a low-flow state caused by hypovolemia—insufficient blood volume.” The study aimed to determine how CPR affects tissue perfusion—specifically oxygen delivery to the brain and skin—during severe hemorrhagic shock. Using a porcine model, researchers simulated life-threatening blood loss and compared outcomes between two groups: one receiving automated CPR and the other left untreated during the shock phase. Key Findings: When CPR May Do More Harm Than Good The results were both surprising and concerning: No Improvement in Perfusion: CPR did not enhance oxygenation in the brain or skin. In fact, skin perfusion was significantly lower in the CPR group during both the shock and recovery phases. Adverse Hemodynamic Effects: While CPR increased systolic blood pressure (SBP), it significantly reduced diastolic blood pressure (DBP), which is critical for coronary and organ perfusion. Potential Harm: CPR caused a threefold increase in intracranial pressure (ICP), suggesting that chest compressions might disrupt normal blood flow dynamics in the brain. “Knowing now that extreme efforts like lining people up to do CPR can cause turbulence within the system is a significant advancement,” says Dr. Greiffenstein. “It's possible to have perfusion at these unreadable MAP scores, which is a critical insight for trauma care.” Implications for Trauma Care These findings challenge the one-size-fits-all approach to CPR in cardiac arrest scenarios. In cases of hemorrhagic shock, CPR might: Divert attention from more effective interventions, such as rapid blood transfusion or surgical control of bleeding. Worsen perfusion to vital organs, potentially exacerbating the patient's condition. “In military cases, field medics often don't have the opportunity to perform full chest compressions on the battlefield. Sometimes, all they can do is drag a person to a safe position,” notes Dr. Greiffenstein. This study underscores the importance of prioritizing interventions that address the root cause of trauma PEA—severe blood loss—over traditional resuscitation techniques. A Call for Updated Guidelines The American Heart Association's current guidelines broadly recommend CPR for all pulseless patients. However, this study adds to a growing body of evidence suggesting that trauma-induced PEA requires a different approach. By focusing on restoring blood volume and controlling bleeding, paramedics and EMTs can improve outcomes for patients in hemorrhagic shock. As Dr. Greiffenstein puts it, “This research is a step toward more tailored and effective trauma care protocols. It's about understanding the unique physiology of trauma and adapting our interventions accordingly.” For EMS providers on the front lines, this study serves as a reminder to critically evaluate the tools and techniques we rely on in emergency care. While CPR remains a vital intervention in many scenarios, its role in trauma-induced PEA warrants careful reconsideration by physician medical directors. By staying informed about the latest research, we can continue to improve outcomes for the patients who depend on us most. Human factors and translational simulation: misunderstandings and potential opportunities JOHS 2025 by Paul O'Connor, Dara Byrne, Angela O'Dea  This article is a call to “join the conversation with human factors practitioners to realise the significant, yet largely untapped, synergies between human factors and translational simulation” This episode is one of those conversations!  The authors call for closer collaboration between human factors experts and simulation teams to directly improve patient care and health systems. Vic speaks with Dara Byrne, Paul O'Connor and Angela O'Dea to explain definitions, common misunderstandings, and practical examples — from device design and procedural kits to preparing new hospitals.  The key points: Translational simulation acts as a safe "lab" for systems testing, the importance of systems thinking and data, and practical steps to build sustainable multidisciplinary partnerships that produce measurable, realworld improvements.  Paul O'Connor is a human factors psychologist and Personal Professor in the School of Medicine at the University of Galway, Ireland. He is the Research Director of the Irish Centre for Applied Patient Safety and Simulation, and Diploma and Masters in Healthcare Simulation and Patient Safety.  Dara Byrne is the Health Service Executive National Clinical Lead for Simulation in the Republic of Ireland and the Professor of Simulation at the University of Galway. She established the Irish Centre for Applied Patient Safety and Simulation (ICAPSS) in 2017 – accreditation and awards  Angela O'Dea is human factors psychologist with a background in organisational psychology. She is a senior Lecturer in Human factors Patient Safety and Simulation at University of Galway and leads the PG Diploma in Healthcare Simulation and Patient Safety.

Andy, Michael and Michael would like to wish all of our listeners a very happy 2026! Michael M and Andy start off 2026 on a good note – or perhaps better said – a series of good notes. Michael shares some of his vacation reading, beginning with the book, I Heard There Was a Secret Chord by Daniel Levitin. In this book, Levitin highlights the power of music to move and heal, and provides a Linktree to listen to the songs featured in the book, which is included in the show notes below. One particular example from the book was the Ella Fitzgerald recording of Mack the Knife in Berlin, and the magic she created in the moment when she forgot the lyrics. Andy highlights an amazing musical creation moment with Jacob Collier's improvisation with the National Symphony Orchestra. This reminded Michael of Rosamund Stone Zander & Benjamin Zander's book, Art of Possibility, and maestro Zander's TED talk on the power of classical music. Michael also brought up David Byrne's book, How Music Works, and his learning in Puerto Rico on how dancers conduct the musicians as they perform together. Byrne discussed mixtapes in his book, and the modern equivalent of them are the playlist, which is exactly what Levitin's Linktree leads to.  Michael created a mixtape to express musically what he was trying to say in words for his NCSSM convocation speech at the start of the 2025-26 school year.  Andy shares a couple of intriguing ways to create music through retro devices and common household products – all of these are in the links below. Moving to the building part of the episode, Andy and Michael start off with LEGO, and this is about to be a banner year for the company with so many new sets coming on the market.
