Melissa Barnett, OD, FAAO, was involved early in the development of scleral lenses and is now considered a legend in specialty contact lens fitting. In fact, she was one of the authors to publish the first contemporary scleral lens textbook in 2017. She also spent four years in an ophthalmology practice, where she became certified in California to treat glaucoma. Currently, she serves as the Director of Optometry at UC Davis in Northern California. She believes every optometrist should get involved with their local and state optometry associations. Today, she is clearly passionate about discussing keratoconus with colleagues and patients on her Clearly KC podcast. In the tenth and final instalment in the BCLA CLEAR podcast series Dr Melissa Barnett – a Fellow of the BCLA and Global Ambassador for the BCLA – interviews Dr Laura Downie about the BCLA CLEAR paper on anatomy and physiology of the anterior eye. A key element of contact lens practice involves clinical evaluation of anterior eye health, including the cornea and limbus, conjunctiva and sclera, eyelids and eyelashes, lacrimal system and tear film. Of course there is a lot to discuss in a short space of time but these highlights will give you an insight into some of the findings in the paper. Please take the time to read the full paper. This report reviews the fundamental anatomy and physiology of these structures, including the vascular supply, venous drainage, lymphatic drainage, sensory innervation, physiology and function. This is the foundation for considering the potential interactions with, and effects of, contact lens wear on the anterior eye. This information is not consistently published as academic research and this report provides a synthesis from all available sources. With respect to terminology, the report aims to promote the consistent use of nomenclature in the field, and generally adopts anatomical terms recommended by the Federative Committee for Anatomical Terminology. Techniques for the examination of the ocular surface are also discussed. Dr Downie is an Associate Professor in the Department of Optometry and Vision Sciences at the University of Melbourne, where she heads the ‘Anterior Eye, Clinical Trials and Research Translation Unit' comprising 13 members. Laura's research combines laboratory, clinical and implementation science as a foundation for improving patient outcomes, with a focus on the anterior eye. In the ninth instalment in the BCLA CLEAR podcast series Dr Melissa Barnett – a Fellow of the BCLA and Global Ambassador for the BCLA – interviews Dr Daddi Fadel about the BCLA CLEAR paper on orthokeratology. Orthokeratology (ortho-k) is the process of deliberately reshaping the anterior cornea by utilising specialty contact lenses to temporarily and reversibly reduce refractive error after lens removal. Modern ortho-k utilises reverse geometry lens designs, made with highly oxygen permeable rigid materials, worn overnight to reshape the anterior cornea and provide temporary correction of refractive error. More recently, ortho-k has been extensively used to slow the progression of myopia in children. Melissa and Daddi discuss just some of this in the episode. The full report reviews the practice of ortho-k, including its history, mechanisms of refractive and ocular changes, current use in the correction of myopia, astigmatism, hyperopia, and presbyopia, and standard of care. Suitable candidates for ortho-k are described, along with the fitting process, factors impacting success, and the potential options for using newer lens designs. Ocular changes associated with ortho-k, such as alterations in corneal thickness, development of microcysts, pigmented arcs, and fibrillary lines are reviewed. The safety of ortho-k is extensively reviewed, along with an overview of non-compliant behaviours and appropriate disinfection regimens. Finally, the role of ortho-k in myopia management for children is discussed in terms of efficacy, safety, and potential mechanisms of myopia control, including the impact of factors such as initial fitting age, baseline refractive error, the role of peripheral defocus, higher order aberrations, pupil size, and treatment zone size. Daddi Fadel DOptom, FSLS, FBCLA, FAAO, is a contact lens designer, a Pioneer of modern lens designs, and a specialist in contact lenses for irregular cornea, scleral lenses, myopia control, and orthokeratology. She has 20 years' experience in optometry and specialty contact lenses. Daddi runs an optometric practice specializing in contact lenses in Italy where she personally designs and fits special customized contact lenses. She is internationally recognized as a key opinion leader, who has several peer-reviewed publications in the area of specialty contact lenses and is a speaker in national and international symposia, and also holds fellowships with numerous institutions. Dr Zeri is a Professor at the University of Milano-Bicocca and Visiting Research Fellow at Aston University in Birmingham. Contact lens materials have undergone significant changes over the past 20 years, particularly with respect to the introduction of silicone hydrogel materials. Whilst this development addressed hypoxic issues, other important areas relating to contact lens success, notably comfort, require further research. Contact lens wettability remains a crucially important part of biocompatibility. Dr Barnet and Dr Zeri cover some of this and more in this episode. Contact lenses can be made more wettable by incorporation of surfactants into blister packs, internal wetting agents, surface treatments or care solutions. However, there remains no clear association between contact lens wettability and comfort, making it challenging to determine the potential for these approaches to be of significant clinical benefit. Most contact lenses are used on a daily wear, reusable basis, which requires them to be disinfected when not worn. The ideal disinfecting solution would also improve comfort during wear. However, balancing these requirements with other factors, including biocompatibility, remains a challenge. Soft lens materials invariably take up and subsequently release certain components of disinfecting solutions onto the ocular surface. This may affect tear film stability and the normal ocular microbiome, and further research is needed in this area to determine whether this has any affect on comfort. Finally, contact lens materials sorb components of the tear film, and these interactions are complex and may change the biochemistry of the tear film, which in turn may affect their comfort. The development of highly oxygen permeable soft and rigid materials has extended the suitability of contact lenses for such applications. There is consistent evidence that bandage soft contact lenses, particularly silicone hydrogel lenses, improve epithelial healing and reduce pain in persistent epithelial defects, after trauma or surgery, and in corneal dystrophies. Drug delivery applications of contact lens hold promise for improving topical therapy. Modern scleral lens practice has achieved great success for both visual rehabilitation and therapeutic applications, including those requiring retention of a tear reservoir or protection from an adverse environment. This report offers a practical and relevant summary of the current evidence for the medical use of contact lenses for all eye care professionals including optometrists, ophthalmologists, opticians, and orthoptists. Topics covered include indications for use in both acute and chronic conditions, lens selection, patient selection, wear and care regimens, and recommended aftercare schedules. This is the fifth episode in the BCLA CLEAR series. Scleral lenses were the first type of contact lens to restore vision and protect the ocular surface. With the advent of rigid corneal lenses in the middle of the twentieth century and soft lenses in the 1970's, the use of scleral lenses diminished; in recent times there has been a resurgence in their use driven by advances in manufacturing and ocular imaging technology. Scleral lenses are often the only viable form of contact lens wear across a range of clinical indications and can potentially delay the need for corneal surgery. This report provides a brief historical review of scleral lenses and a detailed account of contemporary scleral lens practice including common indications and recommended terminology. Recent research on ocular surface shape is presented, in addition to a comprehensive account of modern scleral lens fitting and on-eye evaluation. A range of optical and physiological challenges associated with scleral lenses are presented, including options for the clinical management of a range of ocular conditions. Future applications which take advantage of the stability of scleral lenses are also discussed. In summary, this report presents evidence-based recommendations to optimise patient outcomes in modern scleral lens practice. Dr Eef van der Worp discusses some of the key findings of this paper with Dr Melissa Barnett. Eef van der Worp is an educator and researcher. This is the fourth episode in the BCLA CLEAR series. By reviewing the physiological and anatomical changes which may occur while wearing contact lenses this paper provides eye care practitioners with a better understanding of the key clinical learnings for modern contact lens practice. This report consolidates clinical research information about the various physiological and anatomical changes which may result from using contact lenses. It takes an anatomy-centric approach covering changes to the lids and adnexa, the conjunctival regions and the various layers of the cornea and presents changes which are sub-clinical rather than those which are symptomatic or generally considered as 'adverse events'. Professor Phil Morgan discusses some of the key findings of this paper with Dr Melissa Barnett. Professor Morgan is Professor of Optometry, Head of Optometry, Deputy Head of the Division of Pharmacy and Optometry, and Director of Eurolens Research at The University of Manchester. His main research interests relate to the clinical performance of contact lenses and he teaches on the same subject area at undergraduate and postgraduate level. He has managed an international survey of contact lens prescribing trends since 1996. He is also President of the International Society for Contact Lens Research and President of the International Association of Contact Lens Educators. The most fundamental aspect of a contact lens is its optics; the manner in which the refraction of light is managed to optimise vision to the clinical benefit of the lens wearer. This report presents contemporary information on the optical structure of the eye and the optical models employed to understand the correction of refractive error. The design, measurement and clinical assessment of spherical, aspheric, toric, multifocal and myopia control contact lenses are described. The complexity and variety of multifocal lenses is recognised and detailed information is provided for alternating, simultaneous, diffractive, annular, aspheric and extended depth of field lens designs. In terms of clinical assessment, a contemporary review is provided for the measurement of: visual acuity, contrast sensitivity, through focus curves, reading performance, peripheral refraction, toric displacement realignment and patient reported outcomes. Overall, the paper aims to serve as a resource for the prescribing clinician, who can optimise contact lens corrections for patients by building on the optical rationale of these devices; and also highlights future opportunities for research innovation. Dr Kathryn Richdale is interviewed by Dr Melissa Barnett, who leads the BCLA CLEAR podcast series. Dr. Richdale is currently an attending in the Cornea and Contact Lens Service and co-director of the Myopia Management Service, and teaches in both the optometric and graduate programs at UHCO. She conducts research in the areas of cornea, contact lenses and refractive error. This episode features, Professor James Wolffsohn, who will discuss the work in the report on evidence based contact lens practice.
