In this episode of New Insight, host Veeral Sheth, MD, MBA, director of clinical research at University Retina and Macula Associates, explored the approval and impending launch of Qlosi (pilocarpine HCL ophthalmic solution) 0.4%, a groundbreaking pharmacologic option for managing presbyopia, with Elad Kedar, MBA, chief executive officer of Orasis Pharmaceuticals. A condition that affects nearly all adults as they age, presbyopia is characterized by the gradual loss of near vision due to reduced lens elasticity. Sheth and Kedar highlighted the significance of finding non-invasive treatment options for this pervasive issue, given the limitations of traditional solutions like reading glasses or surgical interventions. In October 2023, the US Food and Drug Administration (FDA) approved Qlosi, a preservative-free, low-dose eye drop for presbyopia, based on strong efficacy, safety, and tolerability data reported in the pivotal Phase 3 NEAR-1 and NEAR-2 clinical trials. On Day 8, patients treated with Qlosi achieved statistically significant ≥3-line gain in distance-corrected near visual acuity (DCNVA) and no loss of ≥1-line in distance visual acuity. Sheth and Kedar delved into Qlosi's innovative mechanism of action, emphasizing how it leverages a unique pharmacologic approach to improve near vision. By modulating pupil dynamics, Qlosi enhances the depth of focus, effectively allowing patients to perform near-vision tasks without additional aids. Together, Sheth and Kedar walked through the drug's clinical development journey, highlighting his journey through the founding of Orasis and the company's name in Greek, meaning “good vision or sight.” The discussion included insights into the populations most likely to benefit from Qlosi and the practical considerations for prescribing. Sheth offered a nuanced take on how this therapy could fit into the broader landscape of presbyopia management, addressing questions about patient education, adherence, and expectations. Dr. Marc Odrich is Chief Medical Officer at LENZ therapeutics and has decades of experience in private practice, teaching, and medical research that contributes to LENZ Therapeutics success by driving the clinical trial and approval of our novel products. His expertise in authoring and directing over 25 clinical trials throughout his career makes him uniquely qualified to execute the approval process. In addition to his duties as Chief Medical Officer, Dr. Odrich is an Associate Professor of Ophthalmology at the University of Virginia, where he practices and teaches Clinical Ophthalmology specializing in Refractive Surgery and Ocular Surface Disease. He is also the Chief Medical Officer for TearSolutions, Inc., a startup in clinical trials for a new drug developed for dry eye disease. In addition, he is a consultant and member of the Board of Directors for CXLO, Inc., a developer of an innovative treatment for keratoconus and other corneal ectasias, as well as a consultant and medical monitor for Johnson & Johnson's refractive products. Previously, he played pivotal roles in the development and commercialization of the Excimer and Femto lasers as Medical Director at Visx. Dr. Odrich is a graduate of Columbia College and received his medical degree from Columbia University College of Physicians and Surgeons. He completed an internship in internal medicine at Yale's Danbury Hospital before moving to pursue his residency at Columbia's Harkness Eye Institute. Odrich then completed a two-year fellowship focused on cornea and external disease at Harvard's Massachusetts Eye and Ear Infirmary.

Dr Michael Lange hosts "ask the dr" today with his guest Michael Walach. Michael is an optical lens industry veteran and expert. The evolution of presbyopia spectacle lens correction is here today with the all new Eyefocal Omni NAL. NAL stands for natural accommodation lens and was invented by Michael Walach. NAL is a true multifocal lens that provides correction from infinity to near in one continuous individually adjusted power curve. Eyefocal Omni NAL is the next generation in lens concept for the over 40 crowd with presbyopia. The Lange Eye Institute in The Villages Florida is one of the first in the country to offer this new Eyefocal Omni NAL concept. Progressive lenses have been around for over 60 years and have not changed a significant amount in this time period. The NAL is a revolutionary new lens concept that allows people to see clearly in the distance and up close with no peripheral distortion or "swim" sensations. This new lens concept also reduces human error in the Measurment's for the progressive lenses since no seg height measurement is necessary with this lens. Dr Lange feels this new NAL will be a much better option for patients with macular degeneration or patients with visual field loss from a stroke or traumatic brain injury. Both Michael Walach and Dr Lange feel this NAL will be a big advantage for anyone playing sports over a progressive lens. Dr. Barnett most recently chaired the BCLA CLEAR report on scleral lenses. She is currently serving on the Tear Film & Ocular Surface Society (TFOS): A Lifestyle Epidemic Ocular Surface Disease Workshop. Dr. Barnett was awarded the inaugural Theia Award for Excellence for Mentoring by Women in Optometry. She was granted the Most Influential Women in Optical from Vision Monday in 2019. Dr. Barnett and Dr. Tom Arnold are co-hosts of the popular podcast GlobalEyes. In her spare time, she enjoys cooking, yoga, hiking and spending time with family.

The changes in our metabolism and body systems that we associate with aging  are variable, and manifest at different times in different individuals. Presbyopia, the difficulty in close up focussing that requires us to get reading glasses, may become problematic in the early forties for some, while others may not experience it until much later in life. Other aging associated changes like wrinkles from  loss of elasticity in our skin, thinning, baldness or greying of our hair, or increasing middle-age spread, also become evident in individuals at  different ages. As women, we become accustomed from puberty, to cyclical fluctuations in our physiology associated with  hormonal changes. These effects are magnified as we transition through menopause and peri-menopause too is variable in timing and duration. Today I speak with award-winning Clinician-Scientist, and 40+ year University of British Columbia Endocrinology Professor. Dr. Jerilynn Prior, about the Paradox of Perimenopause. Dr. Prior's research and innovative concepts are changing women's reproduction. She shows that progesterone must be in balance with estradiol for wellbeing and fertility during the premenopausal years,  and for lifelong good health. Summary of the problem There are 2.2 billion people in the world who are visually impaired. There are two main causes of visual impairment. The first is medical problems, such as cataracts or vitamin A deficiency, which require surgery or medical treatment to treat them. The second is refractive errors, which means that the eyes are healthy, but the person needs glasses in order to see clearly. Uncorrected refractive errors (URE) are the leading cause of visual impairment worldwide. In 2020, more than 1.1 billion people in the world were visually impaired simply because they did not have access to glasses, and this number is expected to rise to 1.8 billion by 2050. This essay will focus on addressing URE, barriers to providing glasses, and new technology that might greatly expand the scope of care. Refractive errors, such as myopia, hyperopia, astigmatism, and presbyopia, are easily corrected with glasses. Refractive error is an extremely common problem; 75% of American adults wear glasses or contact lenses for correction. In high-income nations, the incidence of visual impairment from uncorrected refractive errors (URE) is less than 10%, since most people have access to glasses through their medical insurance. However, the burden of visual impairment from URE is more than four times greater in low- and middle-income countries compared to high-income countries. In western, eastern and central sub-Saharan Africa, 80% of the population suffers with uncorrected refractive errors. While the global incidence of blindness has decreased in the past 30 years due to improvements in medical treatments of trachoma and river blindness, this has been offset by the increasing age of the population. There has been a shift from medical visual impairment to refractive visual impairment as the leading cause of disability. Presbyopia, which is the need for reading glasses or bifocals, develops in 100% of people by the age of 50. Without reading glasses or bifocals, people over age 50 cannot perform tasks requiring near vision, such as reading, removing pebbles from beans or rice, or spotting holes in their malaria bed net. There are 826 million severely impaired and 1.8 billion moderately impaired people who need, but do not have access to reading glasses, which would allow them to easily perform these tasks. This includes up to 94% of the population in some countries in Africa and Asia. (6, 19) Importance of solving the problem Disability URE is the number one global cause of disability.(4) According to the Global Burden of Disease 2013 study, the global burden of URE was 11.3 million disability-adjusted life years (DALYs). The number of DALYs increased 43% from 1990 to 2013. Highest DALYs (241 DALYs per 100,000) were found in low-income areas, including southeast Asia, eastern Mediterranean, and Africa. Women over the age of 40 had higher DALYs from URE compared to men and younger people. Older people had higher DALYs which increased with age. URE burden is inversely proportional to a country's Human Development Index, so countries with the least-developed health and education provisions were more affected by URE.(5) Morbidity URE is not just about inability to work. Because vision is the dominant of the five senses, people fear blindness more than many lethal conditions such as cancer. Mobility and autonomy are usually dependent on vision.
