It is our esteemed pleasure to host Dr. Ciku Mathenge, MD PhD, an eyecare leader in Africa (specifically Rwanda) who has earned adulation and respect among her patients and the global ophthalmology community for her clinical care as well as research. Dr. Mathenge originally spent many years as an ophthalmologist in Nakuru Eye Unit at Rift Valley Provincial Hospital in Kenya where she transformed care. She served as the Regional Medical Advisor for the Fred Hollows Foundation (FHF) and started programs in Kenya, Rwanda, and Burundi. She was awarded the first Sir John Wilson Fellowship by the British Council for Prevention of Blindness and received her PhD from the acclaimed London School of Hygiene and Tropical Medicine where she conducted research in glaucoma, diabetic retinopathy and ARMD in Africa. She has over 30 scientific papers in peer reviewed journals. Currently, Dr. Mathenge has been working diligently on developing the Rwanda International Institute of Ophthalmology (RIIO), a center that aims to become the top center for training and research for all eyecare specialists in the region. Among the many committees Dr. Mathenge has served on, some of them include the ICO advisory committee as well as the ICO Diabetic Retinopathy Task Force. We have much to learn from this episode as Dr. Mathenge narrates her journey and shares the incredible path that is forged with hard work, integrity, and a pursuit to make her residence a place of progress and ingenuity!

Dr. Jeff and Cole discuss Glaucoma, what signs may be presented when you get Glaucoma. What’s your cupping?It may not be what you think it is. Dr. Jeff introduces the visual of the doughnut and why your cupping is so important. Dr. Jeff answers questions that our patients are asking us on a monthly basis. Can you get Glaucoma from your mom and dad?Yes Can you get Glaucoma from your neighbor?No What conversation am I having with my mother / grandmother, that will help us notice signs?Patients, for the most part, don’t have pain. They also won’t have symptoms - it’s less symptomatic for high blood pressure. What’s happening with your optic nerve?This is what we are looking at every time you come in for an exam. When you have Glaucoma (garden variety type), it’s bi-lateral (in both eyes) but asymmetric disease - one eye proceeds the other. General, people treat both eyes at the same time. The invert to Glaucoma is Age-Related Macular Degeneration (ARMD). Glaucoma loses vision around the circumference of the eye and conversely ARMD loses vision in the center of the eye. Is my vision going to be full restored?With Glaucoma, when you lose your vision; we can’t bring it back. We’re looking at millions of nerve wires and seeing if they are thinning to see if you are trending to Glaucoma, over time. We’re watching the nerve, structure and function by the OCT and side-vision test are part of it as well. 12 to 20 is the average eye pressure - it varies day to day and time of day. 4 Types of Glaucoma Congenital Glaucoma Unique to You Closed Angle (painful type, estimate 3%) - very unique - sudden eye pressure rise, blurred vision, comes on very quick. 45-65 years old Open Angle ("Garden Variety" is 85%) majority of Glaucoma patients You won’t feel pressure in your eyes until it’s in the mid-40s range. What does Glaucoma suspect mean? It means you have risk factors (age, African American, Rx, Diabetes, thickness of your cornea, performance of side-vision test) and that we are watching your measurements, pressures and other risk factors. Does it mean I will get Glaucoma?No.   9 out of 10 people will not get Glaucoma Will I get Glaucoma because I’m Diabetic? No. Having your annual eye exam is so important to consistently watching the health of your optic nerve, watching your high pressure and the general aging and health of your eyes. We can see so much more today, just by looking into your eyes.

Let me say it again, your genes are not your destiny. Carlyle and I have been working with people for a couple decades now, and we see people in all different types of domains of their life. What we find of most people is they assume if it runs in their family; they will have and do the same thing their parents did. Your genes are not your destiny.CarlyleInstead of medical hexing, like we have talked about, we can say it is like a family or gene hexing. As opposed to seeing the pattern and looking to change it. Dr. MillerI see people respond to it if we can put it in some familiar ways. Without getting into a significant talk about genetics and epigenetics, consider the most common of these eye problems. Take the most common form of macular degeneration, called age related macular degeneration or ARMD. As it says in the name, the degeneration shows up in people when they are in their 60s, 70s, or 80s. From the time they are kids to the time they retire, there are no problems with their eyes. All of a sudden, their eyes start to fall apart. You need to be asking yourself if these diseases have a strong underpinning of some genetic predisposition, meaning you have a gene that predisposes you to the disease. We know that almost everybody who gets macular degeneration has some sort of abnormal gene. How is it that you can live to be 60, 70, or 80 years old, and have no idea that you have any kind of a genetic problem? You have had the gene from birth and your body has kept it suppressed. If it starts to rear its head, your body is able to fix things faster than they could break. This is true of many other diseases. Another bit of data that is interesting are the genetic testing kits, which look for genetic abnormalities. It is so common now, for only $90 you can get an at-home test kit that shows you any tendencies you may have for all these different dispositions.

Here are some links from this week's episode  https://ankeny18th.extremebodyshaping.com/ http://cafe-diem.com/ https://www.firefliesaglow.com/ https://www.iowachiroclinic.com/ https://www.weestees.com/ http://www.firetrucker.com/ Full Transcript: Hi, welcome to the Ankeny Podcast I'm Tony Guerra and Ankeny nonfiction author professor and triplet dad I'm bringing you the people places and events in Ankeny Iowa and you can find past episodes and links at Ankeny podcast.com welcome to the show Who am I I'm Tony I came here from Washington DC in Baltimore I met my wife who's from Ankeny and we lived in Iowa City for a year but I've lived in Ankeny for almost 10 years now and my daughters are about to turn seven so they're at that age that they can self entertain for a bit give me some time to do the podcast and and this summer I'm gonna be launching the Ankeny podcast which highlights the businesses people in places in Ankeny Iowa I've been podcasting for two years but I've started my own podcast last December that podcast the pharmacy leaders podcast it's about 15 to 25 thousand downloads a month and it's super niche it's just really for pharmacists and people interested in pharmacy but I found that I wasn't really talking to the people that were in my own town and that was a little bit disconcerting to me so I thought boy that'd be really cool to just start another podcast and but I know how to do it so I'm gonna start talking to the people in my neighborhood that really helped me out so I'll probably broadcast from cafe X that's right next to the trail point aquatic and Wellness Center it's about 50 yards from my office where I teach um I teach chemistry and pharmacology down d-mac in Ankeny and I also teach a chemistry and Newton in the fall in the spring but it was barrels last November where I kind of signed up again and and just kinda got back into it and the Farrell's though people that we know but we go back we're like this is where you meet your neighbors you're like oh my gosh where do you live you live right down the street and you don't realize that your neighbors because you know we're so busy and so much often we come home and we just go right in our garage so it just kind of dawned on me that maybe I think they'll start talking to other people and ice talked to Steph Wilson who's the owner of the Farrell's ankeny north and Farrell's Ankeny South and if you don't know Farrell's if you've ever seen people running down 18th Street between the come-and-go and northeast Elementary those are the members doing their 10 weeks and the first day or just before the first day then at five weeks into ten weeks you do a mile run and you'd be amazed the improvement is exponential but I'll be talking to Stephanie Wilson in a future episode who owns that and then dr. Aaron dr. Sean and dr. Vicky at the Iowa chiropractic clinic on 1st Street you know I want to thank them they keep me running I just I ran the Nashville half a month ago and then the Drake half last week but just kind of keeping me from getting too far off so that I can keep running anyone who's a runner knows that it's not if you're gonna get injured you do but certainly minimize it by going to those guys and then I met someone from ways teas who I was like oh my gosh so we need a Mart market to market shirts for our run coming up here in a couple weeks actually so eight of us from d-mac professors and people are working at rail point come together to run 75 miles from way I can't remember the starting point that's bad I've been there two times but we come down to downtown Des Moines but I really appreciate we's tees for helping us out there and it was really kind of serendipitous I I talked to Stephanie Wilson about coming on the show from Farrell's and then she's like oh you should come to this business group just talk to them for a little bit about podcasting and then I was like oh my gosh well we need t-shirts and we just need them and you know I think it was like two or three weeks and we need custom t-shirts for our team so it's just kind of cool that that connection happened but if you know somebody that details cars that's where I need some help both of our cars well I'll just leave pharmacology books or chemistry books whatever it is and that's how I support the show so I I'll just bring a microphone and laptop down I mean I'm in Northeast Ankeny so it's never more than a 10-minute drive to wherever you're at in terms of events this was what really prompted me to start the show because we went to the Ankeny 's authors fair and the only reason I seen that it was gonna happen was because I happened to go to Kuykendall and return a book and then I saw oh wow that's on Saturday it's two days away at the Pinnacle Club at Otter Creek and I my wife I think had to work that Saturday so I took my daughters down there and it was really cool because you'll hear our three six year olds coming in and there and I wasn't sure you know what authors were gonna have books we had but there were a number of kids that were I wanna say 12 and 14 two of them and my daughter's engaged with them bought books from them and they were just kind of really inspired to see that these authors were so young so Zoe Olsen I think she's 12 year old and she wrote Harry and Larry tale of two Canaries and puppy days and then Elijah Olsen the stick book family the stick family book he's 14 and we have those books in our house so but it's just cool that they're on Amazon too and and that you can buy the books and the thing about the podcast is that my pharmacy leaders podcast actually goes to a hundred countries or a hundred countries are listening to it so it's I think it's called glocal where you act globally but work locally so I've talked to a very successful business owner and I said wow you know but it must be really tough to get your items to these other countries and things like that and he's like no the the u.s. absolutely wants you sending things out it's getting things in that's tough so in terms of exporting you know maybe you think your market is here but you know for those for that group of young authors your reach could be global I mean it's there's kids everywhere in the world and the same thing with a number of the businesses in Ankeny while you know you serve us as that are in Ankeny that really the the opportunities are way out there I'm one of the best-selling books in the UK and also in Australia especially Australia because they're an island and they don't really get it's really expensive to get product there so a lot of times this print on demand that we have now makes it a lot easier to get items to them so again opportunities I think around the world in terms of business so just really really cool the the podcast is just a really really cool medium to kind of have it's kind of a cross between radio and talk show and then in terms of events I got to go to Drake Relays last night saw Ankeny Centennial Ankeny high compete in the four by one I didn't get to see too much else after that but again you know I'm so busy with the kids I kind of want to do this show just so I know when things are happening so you know we'll talk about Summerfest certainly when that's coming up in July I know fire trucker has a bunch of rides a lot of what I'll talk about probably is gonna have to do with riding and running as well and then of course the Ankeny optimist club has their Ankeny 5k that Sunday July 15th Prairie Ridge Aquatic Center so I want to make sure that I don't miss those events but also that I'm again connecting with people that are going to be at those events so this is just a little bit of an introduction what I'm hoping to do with the Ankeny podcast again please get in touch with me if you want to be on the Listen in as Dr. Dan Roberts, founder of the Macular Degeneration Support group, shares with us important information about macular degeneration, ARMD, and wet macular degeneration. The Top Two Misconceptions About Macular Degeneration The top one is that you’re going to go blind. That needs to be cleared up immediately. Dr. Roberts tells us that that’s why he retired too early. He shares that he thought he was going to go blind. He was a music teacher, and what good is a music teacher who can’t see the music? That was the very first issue he approached with his organization. It was to make sure that the first thing out of their mouths was, “You’re not going to go blind.” The next misunderstanding is that they can deal with it on their own. We have become very active in the field of low vision rehabilitation. Dr. Roberts has served on a committee with the other academies to build a rehabilitation model. He tells us that that’s what they’re really promoting in the patient community. It’s to realize there is a direction they can go. Dr. Roberts’ organization uses the model the pharmaceuticals use on the television commercials where they say, “Ask your doctor.” They’re educating the patient. Then they’re putting the monkey on the doctor’s back so when the patient comes in and says, “I just heard about such-and-such a pill,” that means the doctor has to educate himself so he can answer the questions. Dr. Roberts’ support group organization followed that model with the rehabilitation education. They educate the patients with it and say, “Go to your doctor and say, ‘Here’s where I am on the model.’” Dr. Roberts tells us that he checked with doctors on this. He didn’t want to put monkeys on their back that they can’t handle, but all the doctors he’s asked about that approach have said, “Definitely. Let them come in and ask us where they’re supposed to go because that’s going to make sure we remember to refer them on to a support or rehabilitation center.” That’s one of the issues. Don’t Rely On Your Doctor Too Much One of Dr. Roberts’ basic philosophies is that we need to take care of ourselves and not expect the doctor to know everything about us and do everything for us. We have to take at least 50 percent of the initiative. Some people, especially the older generation with whom I work, tend to put all their trust in the doctor and let him do all the thinking for them. That’s one thing Dr. Roberts stresses. You have to do some thinking for yourself. You know what’s good for you. Start taking care of your body. Start eating better. Look around because not everything is a clinically proven, FDA-approved approach. Every treatment started out as an alternative treatment. Choosing an Alternative Eye Treatment for Macular Degeneration When you’re looking around at an alternative treatment, consider if it has a history of safety and effectiveness. Have you heard reports otherwise other than from the company itself or the doctor himself? Is the practitioner reputable and following acceptable practice and protocols? Do you have the time and money to invest in that treatment in case it doesn’t work? Once you’ve decided you’re going to do it, is it safe? Have you gotten opinions from professionals and other patients about their experiences with it? Have you fully educated yourself? Do you have the money to invest in it? Do you have the time to really check it out?

Macular degeneration research has indicated that there is a subset of ARMD patients who actually do worse with certain vitamins. This study was done with macular degeneration, but it can be related to other eye diseases and health issues. We’re going to be covering some examples of risks and benefits of some of the best eye vitamins. We’re also going to look at methods of testing. And most importantly, how do you determine if you’re part of this subset that may do worse with vitamins? We’re also going to be looking at the homeopathic approach to eye disease, the value of customized vitamins, and how to obtain your particular customized vitamin formula so you can obtain the maximum benefit of the nutritional supplements that you take. There isn’t just one homeopathic medicine for age-related macular degeneration or glaucoma. There isn’t just one eye drop or one injection that’s going to cure your symptoms. We must look at the individual person. We will look at another study indicates which of the 20% of people with early macular degeneration will progress to advanced disease. Who is in this group? How can we identify them? Finally, we'll explore the hard truth that what is one person’s medicine is another person’s poison. Even a common ingredient can lead to a 135% increase in macular degeneration in some patients with a certain genotype. The job of your ophthalmalogist is to test each ingredient to see if it’s beneficial or detrimental for you and your specific eye problems.
