Millions of women across the globe have undergone breast augmentation using a medical device called a breast implant. This is done for reasons such as cosmesis, sometimes called enhancement of the breast, but also for trauma to the chest wall or for oncological reasons meaning breast cancer. Any surgery is not without risks. Using a medical device like a breast implant is not different. This is the focus of our discussion on this episode of the DiepCJourney podcast. My guest is a leading expert in the field. We are going to discuss the medical definition, symptoms, diagnosis, and treatment BIA/ALCL. This stands for breast implant associated anaplastic large cell lymphoma. It is an uncommon form of ALCL. I am speaking to Dr. Mark Clemens. After receiving his Medical Doctorate from Pennsylvania State College of Medicine, Dr. Clemens completed his Plastic Surgery residency at Georgetown University. In 2010, he came to the University of Texas MD Anderson Cancer Center in Houston, Texas where he completed a fellowship in Microvascular Reconstructive Surgery. Dr. Clemens is an active researcher whose interests include BIA-ALCL, long term implant outcomes, robotic-assisted, microvascular, and reconstructive surgery of the breast. He is the author of the book, Phoenix Rising: Empower & Transform Through Breast Reconstruction. I am honored to list this book on the DiepCJourney Resources tab under Books. Breast implant-associated anaplastic large cell lymphoma, also called BIA-ALCL, is a rare type of cancer of the immune system that can develop in the scar tissue capsule and fluid surrounding a breast implant. In some cases, it can spread throughout the body. BIA-ALCL is curable in most patients if it's diagnosed early and treated appropriately, but a small number of women have died from the illness. Listen to the episode to hear Dr. Zafar explain: what we currently know about BIA-ALCL and its causes treatments for BIA-ALCL the differences between the three types of capsulectomies available reconstruction options for women who have implants removed

Over time, we've seen the standard of "beauty" shift when it comes to breast augmentation.  In 2021, "natural" is in, and the "adult entertainer" look is out.  Regardless of what look you are going for, the internet is full of recommendations and opinions on how to get the result you want.  This means YOU, the patient, feel like you have to make a ton of decisions that are your responsibility.  Feelings of overwhelm , confusion, and anxiety about this operation are the result.  In this episode, we'll explore the ins and outs of breast augmentation and the role patients play during consultation through recovery. We'll also discuss implant types, shapes, textures, and placement - why certain combinations yield different results - and why your final outcome (not the different implant, incision, and shape choices) should be your main focus going into your consultation. In this episode, we cover: Implant Type: The pros and cons of saline and silicone implants and how to determine the best option. (03:19) Implant Shape: An overview of round and anatomic implant shapes and how rotation may impact the decision to go with one or the other. (06:39) Implant Profile: Profiles can go from low to ultra high, but the most common is the moderate plus profile. (08:23) Implant Texture: How smooth and textured implants differ and a brief look at BIA-ALCL, also known as breast implant-associated anaplastic large cell lymphoma. (10:08) Implant Position: Patients either have over the muscle or under the muscle placement. The combination of texture and position will also affect the capsular contracture rate. (14:01) Incision Choices: A look at inframammary, periareolar, transaxillary, and TUBA incisions and why inframammary and periareolar are recommended. (18:56) Pre-consultation: How to prepare for your breast augmentation consultation and what to ignore when researching online. (22:10) Consultation: During your consultation, patients and surgeons discuss goals, 3d photos are taken, and a digital simulation of the surgery is performed to narrow down the implant size and profile. (29:48) Surgery and recovery: For most patients, avoiding activity during recovery is the hardest part! The key to a long-term, stable breast shape and size, is avoiding those urges and following the recommended recovery guidelines. In medical circles, we say, “When you hear hoofbeats, look for horses, not zebras,” as a reminder that when people present with a given set of symptoms, the diagnosis is generally something common. But sometimes it IS a zebra, and patients sometimes DO have a rare condition like the one we're exploring in this episode—breast implant associated lymphoma or BIA-ALCL. On this episode of Inside the Lab, our hosts Dr. Dan Milner and Ms. Kelly Swails are joined by Dr. Kirill Lyapichev, MD, Molecular Genetic Pathology Fellow at MD Anderson Cancer Center in Houston, Texas, and soon-to-be Hematopathology Director at the University of Texas Medical Branch in Galveston, and Dr. Deniz Peker, MD, Associate Professor in the Department of Pathology and Laboratory Medicine at Emory University in Atlanta, Georgia, to discuss breast implant associated ALCL. Dr. Lyapichev shares the history of breast implants in general and BIA-ALCL specifically, walking us through two case studies of the entity with very different outcomes. Dr. Peker describes her approach to diagnosing breast implant associated ALCL, and our panelists explain the clinical signs associated with the rare condition. Listen in for insight on the treatment options available to patients and learn what pathologists need to know about the epidemiology of BIA-ALCL.  Topics Covered  · Two contrasting case studies of patients with breast implant associated ALCL· What we know about the epidemiology of BIA-ALCL and its relationship with chronic inflammation· Why BIA-ALCL is considered a very rare entity· Diagnosing BIA-ALCL and hesitations around making a diagnosis based on cytology alone· The role molecular clonality test plays in diagnosing BIA-ALCL and how common clonality is in published cases of the entity· How to recognize the clinical signs of breast implant associated ALCL· Insight on the history of breast implants in general and BIA-ALCL specifically· The excellent prognosis for patients diagnosed with BIA-ALCL in its early stages (and why reconstructive surgery patients with the condition are caught earlier than cosmetic cases)  · What treatment options are available for patients with breast implant associated ALCL· The latest developments in BIA-ALCL and why pathologists need to be familiar with it Connect with ASCP ASCPASCP on FacebookASCP on InstagramASCP on Twitter Connect with Dr. Lyapichev Dr. Lyapichev on TwitterDr. Lyapichev on LinkedIn Connect with Dr. PekerDr. Peker on TwitterDr. Peker at Emory Winship Cancer Institute Connect with Dr. Milner & Ms. SwailsDr. Milner on TwitterMs. Swails on Twitter Resources Thomas Cronin & Frank GerowInside the Lab in the ASCP Store

Have you ever heard of Breast Implant Illness? If not, you are not alone. I had not heard of it either until few friends of mine publically announced their intention to have “explant” surgery after they started to suffer from inexplicable health challenges over the past few years. One such friend is Michelle Rycroft, a dedicated health coach and personal trainer who I have had the pleasure of knowing for several years. When Michelle started to share her challenges with what she believes to be breast implant illness, I was intrigued about the illness and started to do some of my own research. I came across hundreds of stories just like Michelle’s and I strongly feel that this is a topic that women need to be better informed about, especially in light of the FDA stepping forward last month to notify the public that “individuals with breast implants have a risk of developing breast implant-associated anaplastic large cell lymphoma, or BIA-ALCL, a type of non-Hodgkin’s lymphoma (cancer of the immune system).” Breast augmentation is one of the most popular cosmetic surgery procedures done in the United States. While I am not for or against plastic surgery, I am all for helping women to make educated decisions about their health.
