About the Speaker: Dr. Abigail Chaffin is a Professor of Surgery and Chief of the Division Plastic and Reconstructive Surgery at Tulane University. She is also the Program Director of the Tulane University/Ochsner Clinic Plastic Surgery residency program. She currently serves the Medical Director of the MedCentris Wound Healing Institute at Metairie. Dr. Chaffin is Board-Certified by the American Board of Plastic Surgery and is Board-Certified by the American Board of Surgery. She is also Board-Certified by the American Board of Wound Medicine & Surgery, and the American Board of Wound Healing. Dr. Chaffin is a Fellow of the American College of Surgeons. She is also a Certified Wound Specialist Physician. Due to her clinical and research excellence in wound medicine, she has been honored to be named as a Master of the American Professional Wound Care Association. Dr. Chaffin is a graduate of the University of Michigan, Ann Arbor, where she received a Bachelor's Degree in Biology. She then received M.D. degree at Wayne State University School of Medicine in Detroit, Michigan. After this, she completed a five-year residency in General Surgery at the Wayne State University/Detroit Medical Center program.  She then completed a two-year fellowship in Plastic and Reconstructive Surgery at Tulane University, serving as well as Chief Administrative Resident. Dr. Chaffin has been in practice for over 18 years. She focuses her practice on wound medicine and wound reconstructive surgery, in addition to general reconstructive plastic surgery. She has a particular clinical interest in complex wound surgical reconstruction. She has been honored to receive a Top Doctor award by New Orleans Magazine for the past seven years. As well, she has received the New Orleans Magazine Exceptional Women in Medicine award for the past four years. Dr. Chaffin has published over 65 peer-reviewed publications in wound medicine and plastic surgery. She is a section editor for the ePlasty journal for the reconstructive surgery section. She also serves as an invited peer-reviewer for the Advances in Skin and Wound Care journal, the Journal of Wound Care, and the International Journal of Tissue Repair. She has served as Primary Investigator or Co-Investigator for numerous clinical trials at Tulane University.  She is on the Editorial Board of the International Journal of Tissue Repair. She is an internationally and nationally recognized speaker at scientific conferences, and she frequently serves as a course faculty member and speaker for wound medicine scientific meetings including the Boswick Wound and Burn Symposium, the CAMPs Summit, SIITRAL, and the Symposium on Advanced Wound Care. She is a Board Examiner for the American Board of Plastic Surgery.  She serves as committee chair for several national plastic surgery societies including the American Society of Plastic Surgeons, the Southeastern Society of Plastic and Reconstructive Surgeons, and the American Council of Academic Plastic Surgeons. For ACEPS, she currently serves as Chair of the Research Committee. Dr. Chaffin is currently the Chair of the Assembly of State and Regional Societies for the American Society of Plastic Surgeons, and she is presently serving as a member of the American Society of Plastic Surgeons Board of Directors. Maddie Barrett is a long-time runner, and these days she focuses primarily on the marathon. In this episode, she shares her whole running journey, including going through a time of disordered eating. Now Maddie sees food as a way of healing her body. Her story also includes her experiences of going through these recent two major surgeries and subsequent recoveries, and how that impacted her running life. Maddie's taken lessons from past periods of recovery—most notably from childbirth—and this learned wisdom has served her well.  Maddie's story is very personal, and it's also one that so many women can resonate with. In a conversation with CancerNetwork®, Kandace P. McGuire, MD, and Paschalia Mountziaris, MD, PhD, highlighted the use of L-Dex bioimpedance spectroscopy as a method for detecting lymphedema earlier in patients who undergo surgery for breast cancer. The experts discussed seamlessly integrating this novel modality into standard vitals workflows and detailed other considerations for improving long-term survivorship outcomes via proactive lymphatic care.McGuire began by breaking down why detection of lymphedema typically occurs later after its development, describing how a sentinel lymph node biopsy and additive radiation can cause lymphatic damage and obstruction that correspond with symptoms months to years down the road. At her institution, bioimpedance spectroscopy, the use of a small electrical signal measuring “bioimpedance”, is employed at various points before and after breast cancer surgery to easily determine the likelihood of developing lymphedema through a nursing visit. According to Mountziaris, having a noninvasive method like this provides a “valuable tool” for informing patients of their risks of experiencing lymphedema.The experts also discussed a need to develop a more nuanced method for detecting potential lymphedema among patients with a higher body mass index and spoke about fostering communication across the breast surgery oncology team, the plastic/reconstructive team, and physical rehabilitation specialists to monitor abnormal fluid changes in patients. Looking ahead, they emphasized making bioimpedance spectroscopy more accessible as a key goal in lymphedema care.“We are privileged that McGuire and I have a great team and that I have the equipment that we're able to provide these things for our patients. From my standpoint, some of these patients appear—to me, by measurements, and everything else—to have been cured of their lymphedema after these interventions,” Mountziaris stated. “Getting an L-Dex score on them is just another way to demonstrate that we did bring them to stage 0 or no lymphedema.”McGuire is professor of surgery and chief of breast surgery at Virginia Commonwealth University (VCU) Massey Cancer Center. Traditional surgery has relied on standardized implants made of titanium and involves lengthy operations. The OsseoLabs approach reduces surgical planning and operating time, improving patient outcomes by achieving a better fit and enhanced bone integration through the use of advanced implant materials. AI is used to automate and accelerate the design process for these custom devices, enabling a scalable and cost-effective approach to create complex, regulated medical devices. Vikram explains, "We want to use our engineering know-how, especially in 3D printing, to improve the quality of the outcome of the surgeries and also help the students to be able to offer a better solution at a speed, and also the outcome that is a little bit more precise. And that's what we have been working on for quite a long time. We want to use the knowledge of 3D printing, biomechanics, and also AI to help, especially in complex surgical cases. So both the patients and the surgeons are better off with the outcome. And the main gap that we are seeing here is not only in the Asian countries where we operate, but also in the US. And the use of 3D presented implants, even though it has been around for quite some time in the US, we still see a lot of gaps that can be improved in what materials are being used, and how the implants are designed. We help the surgeons and also the hospitals throughout this process." Joe elaborates, "Yes, for such a device, they all look the same way, but the patients look different. So we see this gap between the available instrumentation and the actual treatment that the surgeons want to conduct. And we want to break that barrier by being able to decide which instrument is required individually, and I mean that in the day, we could not do that. The price can be competitive because all the manufacturing technology is used to support production. But we have to depend on how they matured in the past few years. We are able to adopt the technology, which allows us to decide and print the instrument for specific care and specific surgery. I think the speed and cost become much more practical, and all we need to look into is the 3D printer. But eventually, when you come into something like customization, you talk about the speed. Dr. Kris Shewmake was born in Pine Bluff, Arkansas. He earned his bachelor's degree in biology from Hendrix College in Conway and a master's degree in natural science from the University of Arkansas at Fayetteville. He received his medical degree from UAMS, where he also completed a residency in general surgery. He was president of his class and elected into the Alpha Omega Alpha Medical Honor Society (top 10% of the class). His eight years of surgical training after medical school included a General Surgery residency at the University of Arkansas for Medical Sciences (UAMS) Medical Center and a Plastic and Reconstructive Surgery residency in Dallas at the University of Texas-Southwestern Medical Center. Dr. Shewmake then spent an extra year at UCLA working with children and adults affected by facial birth deformities. Upon completion of his fellowship in 1992, Dr. Shewmake returned to his native state of Arkansas as Chief of Plastic Surgery at UAMS and Arkansas Children‘s Hospital. In 1996 he left UAMS and entered private practice. In 1999 and in 2009 he was named best plastic surgeon in Arkansas by his colleagues. In 2017 he rejoined UAMS as the director of the Division of Plastic and Reconstructive Surgery and also serves as an associate professor there. He is married with three daughters.

She is the author of the new book “The Second Mouth”, which addresses the powerful mind-body connection in functional urology.  Dr. Fleischmann serves as Director of FPMRS at White Plains Hospital Center where she has been in clinical practice for the last 25 years. She holds an active appointment as Assistant Clinical Professor in Obstetrics and Assistant Clinical Professor of Gynecology and Urology at Albert Einstein College of Medicine. Additionally, she is a member of the American Urologic Association and Society for Urodynamics and Female Urology (SUFU). Dr. Fleischmann is the author of several research publications and presentations.
