Presentatie: Sophie Derkzen

'n Wonderlike geskenk. Sy geskenk van lewe. Dink 'n paar keer elke dag daaraan - dit verander alles.Die Gees van God het my gemaak, die asem van die Almagtige het my die lewe gegee.Dis Sy Woord. Rib-sparing DIEP flap refers to a technique used during Deep Inferior Epigastric Perforator (DIEP) flap breast reconstruction surgery where the surgeon preserves the rib and surrounding muscle when accessing the blood vessels that supply the flap.Quick Breakdown:    •    DIEP Flap Reconstruction:    •    Uses skin and fat from the lower abdomen to create a new breast after mastectomy.    •    Preserves the abdominal muscles (unlike TRAM flap).    •    Requires connection of the flap's blood vessels (deep inferior epigastric vessels) to chest vessels, usually the internal mammary vessels (IMVs).    •    Traditional DIEP:    •    Often involves removal of a portion of rib cartilage (typically the 2nd or 3rd rib) to reach the internal mammary vessels for microsurgical connection.    •    This can lead to post-op pain, risk of chest wall weakness, or delayed healing.    •    Rib-Sparing Technique:    •    Avoids rib removal by carefully dissecting around the rib or using intercostal spaces.    •    Benefits:    •    Less post-op pain.    •    Preserved chest wall integrity.    •    Potentially quicker recovery.    •    More cosmetic outcome (less chance of chest wall contour deformity).Clinical Note:    •    Rib-sparing is technically more challenging and requires a skilled microsurgeon.    •    Not all patients are candidates; it depends on anatomy, vessel quality, and other surgical considerations.Connect with Midwest Breast on Instagram: Instagram: Instagram: @MWBreastStay Connected with Dr. Deepa Halaharvi:TikTok: @breastdoctorInstagram: @drdhalaharviTBCP Instagram: @thebreastcancerpodcastWebsite: https://drdeepahalaharvi.com/YouTube: https://www.youtube.com/@deepahalaharvi5917Instagram: @thebreastcancerpodcast

Sheila Romanski is with us today after overcoming two diagnoses of breast cancer and the removal of a tumor in her left shoulder thanks to an autologous stem cell transplant.  She tells the @CancerInterviews podcast her initial of Stage 1A breast cancer in 1996 came after her doctor suggested a routine mammogram at age 36 when at the time mammograms were not performed on women that young.  Radiation treatment successfully addressed that diagnosis, but the following year, the cancer had metastasized to her shoulder.  That brought on the stem cell transplant, plus chemotherapy and more radiation.  All seemed well until 2008 when the breast cancer returned as Stage 4 triple negative infiltrating ductal carcinoma.  Shiela opted for a double mastectomy with reconstruction, which was followed by a second, more aggressive form of chemotherapy.  She has been pronounced NED (No Evidence of Disease), and now enjoys a healthy lifestyle, leading a non-profit that aids cancer victims.   Sheila Romanski advises everyone to do self-examinations for breast cancer.  She admits had she done so, her breast cancer would have been caught sooner.  In 1996, it was rare that 36-year-old women went in for routine mammograms, but Sheila's doctor suggested such an exam.  Thanks to that suggestion, a 2cm tumor was found.  She underwent a lumpectomy and radiation treatment.   While it appeared cancer was in her rear-view mirror, in 1997, a lump was found in her collar bone area.  That was treated with four chemotherapy treatments and autologous stem cell transplant, a procedure involved her own stem cells.  The chemo and the transplant last four months.   For Sheila, this time in her life was very challenging.  She was no only battling cancer, but she was raising four small children.  However, she says she was able to get through because of support from her friends and her church, and because attending to the needs of her kids took her mind off cancer.   Sheila's health appeared to be on the right track until 2008 when another routine mammogram revealed micro-calcifications in the same breast as her original diagnosis.  She decided a double mastectomy would be her best option. It would include a procedure called a diep flap, which involves taking skin from other parts of the body.  However, her surgeon said Sheila would have to lose twenty pounds before diep flap could be performed.  It took Shiela ten years to lose the weight and by that time, her surgeon said she no longer performed diep flap.  However, she said she could get Sheila “fixed up” and in 2021, thirteen years after mastectomy, reconstruction was complete.   Sheila Romanski has gone on to found Crystal Roses, a non-profit that seeks to aid those diagnosed with cancer.   Additional Resources:   Support Group:   Crystal Roses  https://www.crystalroseshelps.com

"My quest always has been how to make peace more profitable than war… I think it's going to be my life legacy..."Imagine a world where creating peace is more profitable than engaging in conflict. A world of 'tech for good' , where technology bridges divides, fosters collaboration, and drives equitable solutions. This vision isn't just a dream; it's the driving force behind innovators like Yip, a leader in promoting peace, purpose, and equitable collaboration within the AI and Web3 spaces.A brilliant conversation full of purpose, curiosity, hope and opportunities. We explore the concept of making peace more profitable than war, the importance of democratizing access to skills and capital, and the vision behind yip's platform, Jedi, in fostering collaboration across emerging technologies. Remaining present and fully conscious of our actions and impact is key to this process. What if we could understand and implement regenerative economic and societal models that prioritize long-term impact and equity over short-term gains, particularly democratising access for all to more regenerative funding models. We discuss the idea and importance of technical and "human" readiness, to equip and educate people in this common challenge. Yip is running her first Zebracorn reatreat in June, to start bringing together these systems and minds. Link to find out more : https://www.systain3r.com/Yip shares her story, insight and dreams of the world she is consciously and collectively crafting to bridge the gap between digital and human for the common good. The man insights you'll get from this episode are : - Bridging gaps through a collective approach to complex problems at societal level. Inspired by Muhammed Yunus' efforts for (financial) inclusion for unbanked people, overcoming the limitations of capitalism with thoughtfulness.- Also inspired by Thich Nhat Hanh's promotion of compassion in communication and shaping corporate culture by bringing mindfulness to tech companies; a mindset of peace feeding into capitalism and technology to engage with them.- Constantly reinventing/rethinking and democratising access can change conversations in systems and communities; the vision for the J3d.AL platform is to make peace more profitable than war.- As an advisory platform, it invites people to become a Jedi or find their inner Jedi, sharpening their intuition and standing up for their values. It helps governments identify collaboration opportunities with each other and the private sector.- As a (market) intelligence platform, it helps identify opportunities for collaboration in emerging fields of technology not yet mature enough for the mainstream media, e.g. quantum computing, cybersecurity, space technology.- The philosophy behind it is to generate a body of knowledge and translate it on a semantic basis from one concept to another, facilitating mutual understanding to break down silos for collaboration to accelerate innovation.- J3d.AI has three areas of operations – technology, health and sustainability. Conferences and workshops teach the skills of foresight, wisdom, compassion, and understanding technology and regenerative systems thinking.- A ‘zebracorn' marries a zebra (a company that is both black and white, i.e. both profitable and works to improve society) with a unicorn (a billion-dollar market valuation company characterised by VC funding and exponential growth).- Zebracorns are totally doable in an age of tech and globalisation as tech brings down costs and its wise use can enable business; they remain open-minded about...

This interview on the DiepCJourney podcast is with three nationally known medical tattoo artist. It will also be available on the DiepCFoundation educational YouTube channel for viewing. Our conversation today is on medical tattooing with three of the leading medical tattoo artists in the United States. I begin with the President and founder of AMT, The Alliance of Medical Tattooing, Ms. Nicole Rizzuto, a licensed tattoo artist and I speak to her about her vision and why she created AMT. Next is Christina Siegel who is a registered nurse and licensed tattoo artist and is the vice-president of AMT. Last and certainly not least, is the medical tattoo artist in our beautiful city of Seattle, WA., Mr. Eric Eye. Eric and I are both advisory board members of AMT. As a two-time cancer survivor who had successful DIEP flap breast reconstruction after a double mastectomy, I wanted “body art,” a story, a strong reminder of who I am, what I do, and a symbolic message to celebrate my own personal Journey through breast cancer. When I describe this as body art, it is much more that that because I went to a medical tattoo artist. I chose Eric Eye because of his artistic background, credentials and someone who works with breast cancer patients. Going to a tattoo artist is an emotional process that is transformative, having a profound impact on breast cancer survivors. In a recent cross-sectional study, the researchers examined the relationship between medical tattooing as a complementary cosmetic intervention and body-image distress and mental health outcomes among breast cancer survivors following surgery. The findings show medical tattooing may be a valuable tool in improving body-image distress and mental health for those who wish to pursue it. The study also states more research is needed. We discuss the services medical tattoo artists provide, safe practices and what patients should be asking for, the planning and shared decision-making conversations with your medical tattoo artist, and how each of them finds balance in this work of passionate care for breast cancer patients.

How much do you know about robotic surgery for DIEP flap breast reconstruction? What areas in the U.S. are performing these types of robotic surgery for those affected by breast cancer? What if I told you robotic DIEP flaps are being performed in Hawaii? It is important to share with you surgeons I meet to provide access to care for those affected by breast cancer. Thank you for listening to the podcast to find out who these surgeons are. We are going to discuss robotic surgery and DIEP flap being performed in Hawaii with my guests on this edition of the DiepCJourney podcast. Be sure to watch our conversation on the DiepCFoundation YouTube channel when it is published, and you can sit with us and watch our conversation. I first met Dr. Daniel Murariu at the American Society of Plastic Surgeons in San Diego in September of 2024. What impressed me at our first meeting was his interest in the resources breast cancer patients provide. We talked about the resources provided by DiepCJourney Foundation, but we also discussed the many other breast cancer support groups helping patients navigate their decision through treatment, surgery, and recovery. I was also impressed with the time he spent listening and I thought to myself, if I were a patient of Dr. Murariu, I think I would feel very much a part of my decision process with his thoughtful demeanor. He is a TEDx UC Davis speaker, Rotarian, speaks three languages, has published multiple papers, an impressive resume to be sure!! Further, Dr. Murariu practices the co-surgeon model at his private practice office in Honolulu, Hawaii at Athena Clinic Plastic Surgery. He collaborates with his team member, Dr. Peter Deptula. We are going to learn more about the work of Dr. Deptula and his training in the interview. They are both board-certified reconstructive surgeons who perform autologous breast reconstruction, lymphedema surgery, complex abdominal wall reconstruction, and complex microsurgery focusing on head and neck and breast reconstruction. We wrap up our conversations with an announcement by Dr. Murariu. It is exciting to know that he and Dr. Deptula are organizing the RAMSES 2027 meeting in Honolulu, Hawaii. RAMSES stands for The Robotic Assisted & Microsurgical Endoscopic Society, “The group represents the ‘early adaptors' of robotic technology in the field of microsurgery.” He graciously invites surgeons to visit them to observe their work and learn more about robotics in plastic surgery.

Welcome to cutting-edge approach to Breast Reconstruction: "The Nerve Sparing DIEP flap"A "Nerve sparing DIEP flap" refers to a surgical technique used in breast reconstruction where the surgeon carefully dissects and preserves the nerves that supply the rectus abdominis muscle when harvesting tissue from the lower abdomen using the deep inferior epigastric perforator (DIEP) flap, minimizing potential complications like abdominal weakness or bulging at the donor site by avoiding damage to these motor nerves; essentially, it prioritizes preserving the function of the abdominal muscles while taking the necessary tissue for breast reconstruction. Key points about nerve sparing DIEP flap:•Muscle preservation: Unlike older techniques like the TRAM flap, a DIEP flap aims to completely spare the rectus abdominis muscle, only taking the skin, fat, and blood vessels supplied by the perforating vessels from the deep inferior epigastric artery. •Nerve identification: During surgery, the surgeon carefully identifies and dissects around the nerves that innervate the rectus muscle to minimize their damage. •Benefits: By preserving the nerves, a nerve-sparing DIEP flap can help maintain abdominal wall strength, reduce the risk of hernias, and improve post-operative function and body contour. Connect with Midwest Breast on Instagram: Instagram: Instagram: @MWBreastStay Connected with Dr. Deepa Halaharvi:TikTok: @breastdoctorInstagram: @drdhalaharviTBCP Instagram: @thebreastcancerpodcastWebsite: https://drdeepahalaharvi.com/YouTube: https://www.youtube.com/@deepahalaharvi5917Instagram: @thebreastcancerpodcast

Listeners of the DiepCJourney podcast are familiar with the DIEP flap breast reconstruction procedure where the abdominal skin, tissue, and underlying blood vessels are used to replace the breast tissue lost due to mastectomy after being affected by breast cancer. On this episode we are going to discuss the “stacked flap” in breast reconstruction. I'm speaking with Dr. Tim Matatov of Southwest Breast and Aesthetics in Phoenix, AZ. He is a board-certified plastic surgeon who is specifically trained in microsurgery. It is the specialty of microsurgery that is required to perform a stacked flap procedure. Dr. Matatov and his team perform these and why I reached out to him for this interview. Dr. Matatov tells us the original definition of a stacked flap is that you put one flap on top of the other and place those two flaps into a surgical site, in this instance, the breast area to recreate a breast after mastectomy. It has evolved to a flap from the abdomen, a bipedicle flap with two blood vessels that are connected to the entire abdomen and taken as one piece. He points out that at his practice it is used for skin resurfacing in the radiated patient where a lot of the damaged tissue would have to be removed. This type of flap provides enough skin to repair the area damaged and needing repair from radiation. Another purpose for a stacked flap is in the setting of a single mastectomy; when a patient chooses to remove only one breast and keep the other and there is not enough tissue from a single flap from the back, thigh, or abdomen. In this instance, the stacked flap provides the volume needed to achieve symmetry with the existing breast.  Dr. Matatov goes on to tell us where on the body theses blood and tissue sources are from as well as what the recovery looks like for patients. Today's witnesses are from Matthew West's website called popwe.org. If you don't know who Matthew West is, he is a singer, songwriter, and storyteller. This website is for the non-profit that he has with his father, a pastor. Matthew and his father encourage people to share their stories. They have various categories of stories. Today, I chose two testimonies from the Healing Category. Often, we can hear our own story in someone else's story. When we listen to others tell their story, it helps us see that we are not alone. When we hear how God worked in their situation, it gives us hope that He will work in ours, too! I pray when you hear these testimonies, you get the faith and hope to believe that miracles can happen in your situation, also. I pray you know that God is there with you, and if you invite Him into your situation, He can help. I hope you enjoy these testimonies.Karen:  In 2004 my mom suffered kidney failure and started dialysis at 75. It was very hard on her while my older sister tested to be a donor and was ruled out. Then my older brother tested and was unable to donate. It's important to note that they are both biological children and I was adopted. I was also a single mom raising two kids at 37, so needless to say I was scared to donate, but I prayed and my growth group prayed with me. Come to find out, a man sitting beside me had donated his kidney to his daughter and was doing great 18 years later. I also kept seeing signs to remind me that “greater love hath no man than to lay down his life for another,” so I stepped up and was deemed acceptable but had to lose 40 pounds to do so.Mom was getting more drained by the day. I lost the weight in 8 weeks and surgery was scheduled for October, but mom suffered a hematoma and surgery was put off till February for her healing. The waiting was tough. It made me second guess but I still did it.  In February 2005, she was able to stop dialysis after I donated my kidney to her and my prayer was that God would take care of us. It's because of seeing the toll mom and my grandma went through on dialysis I knew I could never do that. Lesson learned. Never tell God what you can't do!Fast forward 14 years. I got kidney cancer and had a partial nephrectomy in September of 2019. Unfortunately surgery had complications and I had a collapsed lung, respiratory distress, and internal bleeding that resulted in a 34 day hospital stay including a medically induced coma for 2 weeks. My family was unsure if I would make it or not. I remember asking God to take me as I was on a ventilator and unconscious, but He said it wasn't my time yet, He had great plans for me. My kids still needed me so I asked Him to heal me. Within 2 days I was awake, and the next day the ventilator came out. A few days after that I left ICU for a regular room. That only lasted 2 days before being discharged to rehab for 2 weeks where I learned how to walk again.Unfortunately, my kidney never woke up after that surgery even though I still had 78% left, but they think the medications they gave me for the coma were too strong and could have damaged it further. Thus began my time on dialysis and boy was it miserable. Three times a week I would go in and be hooked up to a life-saving machine for 3 hours, then be exhausted after with muscle cramps, nausea, high blood pressure, headaches, and overall feeling miserable. I had numerous surgeries for peritoneal dialysis but it never worked. I had numerous hospital stays for high blood pressure, headaches, abdominal pain, and bowel obstructions. It was the roughest 33 months of my life.I was finally approved for a transplant in April of 2022 and on Mother's Day May 8, 2022 I got a new kidney! Praise God for only He could schedule that! After giving my mom a kidney in 2005, He gave me one on Mother's Day- amazing! It's now several months later and although it hasn't been a picnic I am so thankful to be on the road to health and healing. Blessed be the name of the Lord!We celebrated my parents 69th wedding anniversary this past Sunday and I'm so thankful for all that God has done and continues to do on a daily basis for me! To God be the glory!Emily:  Hello, I'm Emily. I'm a 10 time Breast Cancer Survivor. Yes, you read that right, ….TEN.It all began with a stomach ache in 2008. As I went to my doctor for a routine exam, she found the Cancer! Of course, I had to go through an ultrasound and mammogram first, but my doctor had saved my life by detecting this early, I thought.I was 33, had 3 kids, (ages 6, 2, and 8 months) and a full-time job teaching 1st graders at a public school. Cancer? Who has time for Cancer? I was told it is an aggressive form and difficult to treat, because doctors don't know much about it. I have Triple Negative Breast Cancer. I fought with everything I had through chemo, radiation and a lumpectomy.God and I spent a lot of time talking about how I was going to win this battle and with His help, I did, for 4 1/2 years. The Demon itself returned in March of 2012, just before my 5 year clear mark. I was devastated, scared, angry, frustrated and MAD at God. Why was this happening to me again? Why do I have to go through this again? This isn't fair! God carried me through the second battle, just as He did the first with just as much grace and comfort.Fast-forward to 2022. I've now had TWO double mastectomies, a lumpectomy, ten biopsies, a DIEP flap reconstruction surgery, 3 more rounds of chemo (78 total treatments), 3 different types of radiation (67 total treatments), and Proton therapy (35 treatments) to try and kill this cancer. It has reared its ugly head 10 times and this is going to be the last!!!Just a month and a half ago, I learned about a new drug that people were having success rates with. It was a Godsend and true miracle. An email delivered to my inbox revealed something we've been waiting for for 15 years!!!! The product is working and I will have another scan in February to hear that there is NED (No evidence of disease)!!! I am positive after reading the success stories!I tell my story to say this… God is the only reason I am still here!!!! Nobody survives Cancer 10 times without God. Period.  If you are battling Cancer of any kind, just know that God is on your side and He is the ultimate healer!!!! Without Him and our faith, we are nothing!! I'm still standing AND teaching for 15 years and 10 battles later!God is so good and Matthew West, you're my favorite!!! Your music inspires me and the children in my class from rough homes every single day!! Our favorite is “How Good Of God!”Thank you both so much for sharing your stories. I know it can be hard to be vulnerable and share your story with other. When you are able to do so though it helps other people see they are not alone. It provides hope to those who hear it.
