In this podcast, we are joined by Dr Alex Crichton, researcher at the Trisha Roy Vascular laboratory at Houston Methodist Hospital, Texas, to discuss how QISS MRA can be used to image tibial vessels that are often missed by more conventional forms of imaging such as CT angiography, ultrasound duplex imaging or digital subtraction angiography. Diabetic patients can be especially challenging in selecting target vessels for treatment because of the presence of calcium in the vessel wall and the distal nature of the disease. Furthermore, many of these patients have impaired renal function where the use of contrast media can be detrimental. In this episode, Dr Crichton explains the work being done with QISS MRA to enable visualisation of these so called “hibernating vessels” and also how it can be used to characterise plaque morphology and the suitability for endovascular intervention.

Despite advancements in surgical techniques, the recurrence rate of anterior cruciate ligament (ACL) rupture after reconstruction remains between 2% and 20%. An increased posterior tibial slope (PTS) is associated with higher ACL rupture and reconstruction failure rates. In conclusion, ACW-HTO combined with ACL reconstruction shows promising long-term outcomes, significantly reducing PTS and improving knee stability and function with an acceptable complication rate. Sagittal-plane slope-reducing high tibial osteotomy (HTO) can reduce the risk of anterior cruciate ligament (ACL) injury in knees with a high posterior tibial slope. The biomechanical effect of slope-reducing HTO on tibiofemoral kinematics and force carried by the ACL remains less well understood. In conclusion, slope-reducing HTO decreased ACL force linearly by reducing ATT with compression and also by decreasing coupled ITR with combined compression and valgus. Shin splints are a real pain in the tibia. If you have ever taken up running as a new hobby or you have jumped back into activity a little too soon you have probably had this soreness along the front or inside of your lower leg. While it is a frustrating injury it is also one you can address in a preventative fashion and you can correct. It takes some time and patience but if you address all the factors it will be well worth it in the end. The Medial Meniscus is a known secondary stabilizer to the ACL in terms of anterior tibial translation in the knee. So what happens after ACL reconstruction if the medial meniscus is deficient? Dr. Andrew Fithian, MD joins us to discuss his study addressing this very question. Coming to us from Kaiser Permanente San Diego, Dr. Fithian and his team investigated postoperative anterior tibial translation in autograft ACL patients with an intact medial meniscus, a repaired medial meniscus, or a partially resected medial meniscus. The results may guide decision making when facing a situation with an irreparable medial meniscus tear in the setting of an ACL tear, particularly with graft choice.

When you get a stress fracture, one of the earliest visible indications on an X-ray or an MRI or a CT scan is a thing called periosteal elevation. Your doctor might see it on ultrasound, x-rays or MRI studies....way before your ever see a crack in the bone. Since it's one of the earliest changes in the bone when you start to get a stress fracture, I thought it might be useful to talk about the term "periosteal elevation" really means.
