Shin splints are a common type of leg pain caused by repetitive stress on the bones and muscles in the lower leg, particularly around the tibia. The condition, also known as medial tibial stress syndrome (MTSS), is characterised by pain and tenderness along the inner or front of the lower leg. It is often triggered by activities like running, jumping or other high-impact sports. Medial gutter impingement may compromise the results of an otherwise well-fixed total ankle arthroplasty (TAA), but no previous study has assessed predisposing factors. This case-control study sought to investigate potential risk factors and the role of talar component downsizing in decreasing medial impingement. In conclusion, talar component downsizing correlated with an 82% reduction in the probability of medial gutter impingement. Postoperative varus alignment, an elevated joint line level, and medially translated and internally rotated talar component were more prevalent in patients reoperated for medial impingement. Die Headline ist eine von mir sehr bewusst gewählte Provokation und mit meiner Tagesdosis, hier bei „apolut“, möchte ich sie erklären und mit Fakten untermauern, denn in „Sachen Ukraine“ geht es immer noch um's Ganze, sprich um „Krieg und Frieden“. Eine epische Gegenüberstellung, die schon als Roman zu Weltruhm gelangt ist, wohlgemerkt von einem Russen verfasst. Der Roman "Krieg und Frieden" wurde von Lew Nikolajewitsch Tolstoi verfasst. Er wurde zwischen 1865 und 1869 veröffentlicht und gilt als eines der bedeutendsten Werke der Weltliteratur.Derzeit haben wir eine politische Gegenüberstellung, die „nachkriegs-geschichtlich“ gesehen, also nach den beiden Weltkriegen, absolut aussergewöhnlich und noch nie da gewesen ist. Dazu birgt diese ganze „Ukraine-Causa“ die Gefahr eines dritten Weltkriegs in sich oder den Weg zu einem Frieden in der Ukraine und eventuell sogar darüber hinaus. Die USA und Russland wollen Frieden und Zusammenarbeit allerdings wollen Europa und die Ukraine weiter Krieg, bis zum, wie es bei ihnen heißt, „gerechten Frieden“ ohne anschliessende Zusammenarbeit mit Russland natürlich, von „Aussöhnung“ will ich gar nicht anfangen. Hier in Europa geht es um Aufrüstung und Abschreckung, denn die Russen wollen ja ganz Europa erobern und darum kann und darf man ihnen nicht trauen, also „Kalter Krieg 2.0“.Medial tobt dazu ein Informationskrieg zwischen den „Mainstream-Medien“, die als „einzig seriös gelten“, so bescheinigen sie es sich unentwegt selbst und den „freien Medien“ im Internet, die vom „seriösen Mainstream“ als „rechts und/oder verschwörungstheoretisch“, ja man muss es so unumwunden formulieren, „gebrandmarkt“ werden und das in einer Tour. Der Mainstream will den „gerechten Frieden“ und Aufrüstung, die „freien Medien“ wollen einfach Frieden und Abrüstung. Und SIE, liebe Leser, haben die freie Wahl zwischen diesen beiden Angeboten, was ihre Informationsquellen angeht, über die Sie sich Ihre Meinung bilden.Vorweg eine für diesen Artikel unumgängliche Einordnung des Begriffes „Wahrheit“: Der Begriff „Wahrheit“ ist eine heikle Angelegenheit, wenn es um Politik geht und wenn es um Krieg geht erst recht. Denn es heißt ja so schön und absolut richtig: „Das erste Opfer des Krieges ist die Wahrheit!“ Und in der Politik ist „Wahrheit“ immer nur ein „Annäherungswert“, dessen Distanz zur objektiven, faktischen „Wahrheit“ durchaus sehr stark variieren kann, denn im politischen Kontext ist der Begriff „Wahrheit“ das wohl am meisten in Anspruch genommene und gleichzeitig am meisten missbrauchte Wort überhaupt. Der deutsche Philosoph Martin Heidegger (1) hat mal gesagt, damit wir uns wieder wirklich verstehen, müssten wir eigentlich eine neue Sprache erfinden. Selten war das so zutreffend wie in unserer Zeit, denn die Bedeutung vieler Worte und Begriffe ist zurzeit so weit von ihrer „Ursprungsbedeutung“ entfernt, was unter anderem oder vor allem durch ihren Einsatz im politischen Diskurs zustande gekommen ist, noch verstärkt durch ihre Verwendung im Kontext von Meinungsbildung und Propaganda. Medial meniscal repair performed at the time of primary anterior cruciate ligament reconstruction (ACLR) has been shown to be significantly associated with subsequent surgery, and subsequent surgery has been associated with increased Knee injury and Osteoarthritis Outcome Score (KOOS) pain score and decreased patient satisfaction. In conclusion, successful medial meniscal repair performed at the time of primary ACLR decreased clinically significant knee pain 10 years postoperatively. However, the mediating effect of subsequent surgery was significant and diminished the overall contribution of medial meniscal repair in decreasing the likelihood of KOOS pain. Continued efforts should be made to decrease the likelihood of subsequent surgery after medial meniscal repair performed at the time of primary ACLR. Click here to read the article.

Shin splints are a real pain in the tibia. If you have ever taken up running as a new hobby or you have jumped back into activity a little too soon you have probably had this soreness along the front or inside of your lower leg. While it is a frustrating injury it is also one you can address in a preventative fashion and you can correct. It takes some time and patience but if you address all the factors it will be well worth it in the end. Focus on form, technique and rehab and you'll be good to go in helping correct this extremely annoying injury. In this episode, we're going to continue our discussion with Dr. Aaron Krych and focus on surgical advancements to address meniscus extrusion and optimize postoperative outcomes for patients.Our conversation picks back up with an article published last month in Arthroscopy Techniques titled “Arthroscopic Centralization of the Extruded Meniscus With Posterior Root Tear: A Technique Using Meniscotibial Ligament Fixation.” Most meniscus centralization techniques involved anchoring the meniscus body directly to the tibia, which can limit the normal anatomic motion of the meniscus. Dr. Krych and his team described a novel technique for meniscus centralization that maintains natural meniscal motion by utilizing the meniscotibial ligaments to centralize the meniscus body.Then, from the May 2024 issue of Arthroscopy, we review an article titled “Satisfactory Clinical Outcome, Complications, and Provisional Results of Meniscus Centralization with Medial Meniscus Root Repair for the Extruded Medial Meniscus at Mean 2-Year Follow-Up.” This case series concluded that patients undergoing medial meniscus root repair with meniscus centralization demonstrated significant improvements in pain, function, satisfaction and quality of life. Additionally, there was no evidence of significant arthritic progression on postoperative imaging. No patients underwent revision meniscus surgery or total knee arthroplasty.

On today's episode we're focusing on medial meniscus root repairs with Dr. Aaron Krych, Chair of Orthopedic Surgery and Professor of Orthopedic Surgery at The Mayo Clinic in Rochester, Minnesota. We have some great articles for you that contribute well to our conversation on the optimal treatment of medial meniscus root tears, including recent advancements in surgical technique to optimize outcomes. We'll start off our discussion today with systematic review article from the February issue of Arthroscopy this year titled “Root Repair Has Superior Radiological and Clinical Outcomes Than Partial Meniscectomy and Nonoperative Treatment in the Management of Meniscus Root Tears.” This review included 56 studies with over 3000 patients. The authors concluded that root repair demonstrated the least amount of postoperative joint space narrowing and the greatest increase in patient reported outcome scores including IKDC, Lysholm, KOOS pain and activity scales.Then, from the February 2022 issue of OJSM we review the publication titled “Prospective Consecutive Clinical Outcomes After Transtibial Root Repair for Posterior Meniscal Root Tears.” This multicenter case-control study concluded that patients who underwent posterior meniscus root repair utilizing a transtibial pullout technique had significant improvement in clinical outcome scores at 2 years with an overall low complication rate. Increased age, increased BMI, cartilage status, and meniscal extrusion did not have a negative impact on short-term functional outcomes, but age greater than or equal to 50 years and extrusion negatively influenced patient activity level.We finish up our conversation today with the publication titled “Biomechanical Performance of TranstibialPull-Out Posterior Horn Medial MeniscusRoot Repair Is Improved With KnotlessAdjustable Suture Anchor–Based Fixation.” This controlled laboratory study, published in the March issue of OJSM this year, compared four different fixed transtibial pullout repair techniques to a knotless adjustable repair technique. This study found that the knotless adjustable repair resulted in higher tissue compression and less tissue displacement compared to traditional fixed repair, which is great for healing. Additionally, the rip-stop Mason-Allen suture configuration provided higher resistance to suture cut-through, withstanding repair failure. We are joined today by Dr. Aaron Krych, Chair of Orthopedic Surgery and Professor of Orthopedic Surgery at the Mayo Clinic. He is also the Chair of the Division of Sports Medicine at Mayo Clinic and team orthopedic surgeon for Minnesota Timberwolves. Dr. Krych received his medical degree and completed his orthopedic surgical residency training at the Mayo Clinic. He then went on to complete a fellowship in sports medicine at the Hospital for Special Surgery. Dr. Krych has published extensively on the topic of meniscal pathology, and specifically meniscus root repair, so we are very excited to have him join our discussion today.
The Medial Meniscus is a known secondary stabilizer to the ACL in terms of anterior tibial translation in the knee. So what happens after ACL reconstruction if the medial meniscus is deficient? Dr. Andrew Fithian, MD joins us to discuss his study addressing this very question. Coming to us from Kaiser Permanente San Diego, Dr. Fithian and his team investigated postoperative anterior tibial translation in autograft ACL patients with an intact medial meniscus, a repaired medial meniscus, or a partially resected medial meniscus. The results may guide decision making when facing a situation with an irreparable medial meniscus tear in the setting of an ACL tear, particularly with graft choice.
