In this special episode of Australian VBAC Stories, co-host Katelyn, doula and VBAC educator, openly shares the story of her three daughters' births, and the many ways they've shaped who she is today.Her first pregnancy was incredibly challenging. She found herself in a private care model that didn't feel aligned, facing repeated pressure around sweeps and induction. Her labour became intense and overwhelming, with syntocinon, limited support, a posterior baby, and growing concerns for her baby's wellbeing. It ended in a cesarean, followed by separation from her baby and confusing, inconsistent breastfeeding advice. It left her feeling anxious, disconnected from her instincts, and unsure of her voice.From there, Katelyn began to seek something different. She immersed herself in VBAC education and found a supportive community. For her second birth, she planned a VBAC at hole with a private midwife. She laboured for a long time and gave so much of herself to the process, but when progress stalled, she transferred to hospital and ultimately had another cesarean. This time, though, things felt different - she had more agency, more support, and a stronger sense of being part of the decision-making, even as she continued to process the grief of how things unfolded.By her third pregnancy, there was a real shift. Katelyn focused deeply on her emotional wellbeing and letting go of expectations. She laboured at home again, grounded and supported, before transferring for an epidural and cesarean when her baby still wasn't descending. Afterwards, she learned there were physical factors at play - significant adhesions and a velamentous cord insertion - which helped bring clarity and understanding to her experiences.What stands out most in Katelyn's story is the evolution of how she felt within each birth. Through it all, she speaks with honesty about the complexity of grief and healing, but also with a sense of peace, empowerment, and deep gratitude. Her journey has not only shaped her as a mother, but also inspired the work she now does supporting others as a doula and through her VBAC education course.

Artistas populares que con un vasto repertorio de monólogos, diálogos, sones, rumbas y guarachas, salpicados de humor criollo y sátira política, acapararon un espacio nada despreciable en aquellos catálogos de principios del siglo XX. En la memoria dos grandes referentes de aquel delicioso teatro criollo. Ambos hicieron época encarnando al célebre "negrito": Arquímedes Pous, tempranamente desaparecido en 1926, y Ramón Espígul. Este último, hasta finales de los años 30, mantuvo una notable producción de grabaciones dejando huella entre sus contemporáneos. Pedro Guida nació en Sagua la Grande el 10 de enero de 1902 y a partir de los primeros años de la década del 30, radicado en la capital, incidió favorablemente en el movimiento de los jazz bands criollos. Sus condiciones como saxofonista, clarinetista, flautista, arreglista y compositor le permitieron figurar en importantes alineaciones de ese formato como la "Hermanos Castro" y la "Hermanos Palau". Rememorando su paso por esta última banda escogemos dos piezas suyas en las voces de Radeúnda Lima, Dulce María Fresnedo y Francisco Cruz, conocido como "El Indio". En la memoria: Pedro Guida, esta vez al frente de su propia banda de comienzos de los 50. Al piano: Felo Bergaza. La frontera sonora de los años 80 a los 90. Xiomara Laugart con su banda XL y un repertorio memorable marcado especialmente por las piezas del trovador Alberto Tosca. Entra ellas "Canción de un viejo trovador" y "Ni un ya no estás". Las cualidades vocales de "La Negra", y su inmensa capacidad interpretativa, le permitieron salirse muy pronto del encasillamiento que traía consigo el pertenecer al movimiento de la llamada "nueva trova", consiguiendo desarrollar años más tarde una sólida trayectoria radicada en los Estados Unidos. Xiomara Laugart y aquellas canciones que nos recuerdan su despegue discográfico, incluidas en un álbum producido por EGREM en 1990. "Que manera" de Luis Ríos, con arreglo del pianista Omar Sosa, fue otro de sus grandes éxitos. Y nada más cubano que el arco sonoro trazado magistralmente desde una danza de Cervantes, pasando por un danzón de nuevo ritmo hasta desembocar en un chachachá, sirviendo de opening al álbum "Habana Report" del pianista Ernán López Nussa. Otra joya de colección que vio la luz en 2002. En las orillas del nuevo milenio así se escuchaba el Jazz AfroCubano gracias al maestro con la complicidad de Tata Güines en tumbadoras, Pancho Terry en la percusión afrocubana, Changuito en los timbales, y Jorge Alexander "Sagua" en contrabajo, creando la atmósfera precisa para que Joaquín Oliveros en flauta, Alexander Brown y Julito Padrón en trompetas y Orlando Sánchez "Cubajazz" en clarinete completaran una fabulosa travesía por ciertos clásicos que siempre valdrá la pena traer a la memoria.

The literature suggests that both arthroscopic posterior Bankart repair (APB) and posterior bone block (PBB) are effective procedures in the short to medium term, although recurrence and revision rates do not appear to be negligible. However, fewer studies, especially comparative ones, are available regarding the long-term outcomes of these procedures.   In conclusion, at a mean follow-up of 5 years for APB and 12 years for PBB, the APB group had a similar recurrence rate but fewer complications and revisions compared with the PBB group. Today's episode addresses a common pattern in maternity care: past birth experiences being used by providers to justify predetermined interventions in subsequent pregnancies.One listener asked, do women need to get out of the tub for fetal heart rate decels, and is this based on policy or physiology? Is Pitocin given after heavy bleeding in a prior birth justifiable for routine use in a subsequent birth? Additionally, we explore the long-term impact of episiotomies and membrane sweeps performed without consent, and address myths about posterior (OP) babies and whether those conditions are mother-specific or unique to each pregnancy. Oxytocin has become known for having anti-anxiety and affiliative behavioral effects. That's why clinicians and researchers are excited about using oxytocin as a potential therapeutic. Brian Trainor is a professor at UC Davis, and his lab has been studying this complexity for the past decade. For an animal model, they work with a territorial, aggressive, monogamous rodent species called California mice. The high rates of failures and reoperations (up to 50%) after arthroscopic posterior Bankart repair (APBR) remain a concern.   In conclusion, in patients with recurrent posterior instability, the presence of posterior glenoid erosion was the main risk factor for failure after APBR. Contributor: Meghan Hurley, MD Educational Pearls: 1. Initial Assessment Start with a physical examination: Determine if the bleed is anterior or posterior. Perform a primary survey: assess airway, breathing, and circulation (ABCs). Airway compromise = intubation immediately. If the patient is stable, have them blow out any clots, then re-examine the nares. 2. Topical Medications Anesthetics: provide local anesthesia and pain relief. Lidocaine Tetracaine Vasoconstrictors: reduce bleeding. LET (Lidocaine, Epinephrine, Tetracaine) is ideal because it provides anesthesia and vasoconstriction. Cocaine pledgets (less common). Tranexamic acid (TXA). Oxymetazoline (Afrin). Cautery (Chemical): If an anterior bleed is visualized, silver nitrate can be applied for cauterization 3. Technique Tips Use a nasal speculum. Spread up and down rather than side to side to avoid injury to the septum. Place LET-soaked gauze in the nares. Apply a nasal clamp for ~15 minutes to compress the vessels. Note that pledgets may cause upper lip numbness 4. Reassessment After 15 minutes, remove materials and inspect for a source of bleeding. If still bleeding and a source is identified, cauterize the site. Observe for 15 minutes to monitor for recurrence of bleeding. 5. Packing If the above measures fail to control bleeding: Anterior packing: Nasal tampon (Merocel) Convenient for outpatient removal. Balloon device Inflate the anterior balloon for compression. Posterior packing:  More complex, should consult ENT for additional assistance. 6. Disposition & Follow-Up Although rare, toxic shock syndrome is a possible complication of nasal packing. Antibiotic prophylaxis is controversial, but may be considered in high-risk patients. Outpatient follow-up if stable: Tampon: The patient can remove it at home. Balloon: Return to ED for removal. 7. Risk Factors for Epistaxis & Prevention Deviated septum, dry environments, and anticoagulant use Advise on humidifier use, nasal saline, and medication review to minimize future episodes. In today's episode, we will be concluding our discussion of the biomechanics of spinal fusions by discussing posterior constructs.  

Despite advancements in surgical techniques, the recurrence rate of anterior cruciate ligament (ACL) rupture after reconstruction remains between 2% and 20%. An increased posterior tibial slope (PTS) is associated with higher ACL rupture and reconstruction failure rates.   In conclusion, ACW-HTO combined with ACL reconstruction shows promising long-term outcomes, significantly reducing PTS and improving knee stability and function with an acceptable complication rate.   Click here to read the article.
