The Flexor Hallucis Longus (FHL) muscle is a common site of injury in dance - although rare in other sporting/athletic populations. It is a an important stabilizer of the 1st ray during toe off and when on demi-pointe, making it susceptible to injury in dancers. In this episode, I talk about the anatomy and function of the FHL, and discuss why it is often misdiagnosed by healthcare professionals who aren't experienced in working with dancers. Welcome back to part 2 of our injury conversation. From prevention to treatment, in this episode we're answering all of your questions about anything to do with injuries. We collected these questions from you the listeners (and Instagram followers) and our Physical Therapist John is back to answer everything we didn't get to in part 1. Topics discussed in this episode include: Lower back pain and what kinds of exercise help with it. Tendonitis, especially looking at the achilles and the patellar tendon. John even provides a very specific achilles tendonitis protocol you can use. IT band syndrome, including what aggravates it and what treatments do and don't help. How to build strong and durable hamstrings, as well as what types of activity tend to injure people's hamstrings. Warm ups, specifically looking at how effective research suggests they are, and some specific strategies you can try. And finally we talk about non-physical components of injury rehab, including nutrition, sleep, and the mental aspect.

Strengthening tendons and muscles in our workouts is a must. Like it or not, as we age many of us find ourselves dealling with more tendon issues. Tennis Elbow, runner's knee, Achille strain, Patellar Femoral Tendonosis, etc… What's going on here? As we age our tendons are not trained like our muscles. We don't hang from trees or run sprints like we used to. But, we may still find ourselves going to the gym in performing traditional muscle strengthening exercises. This is not all bad, but as we see the muscles getting stronger we may have not taken the time to allow the tendons to strengthen with the muscles. Before you know it you have a muscle that is stronger than the tendon can handle. This could be due to strength or over use. We have come to find out that completely laying off and stopping all exercises is actually not the way to properly heal and get stronger. We want to continue to workout, but change our focus on what we are working out. Let's start to train the tendon before we get any stronger in the muscles. Advice -Eccentric vs Concentric motionsConcentric is what most of us think about when we are building muscles. We “contract” the muscle to shorten it and flex the joint. The muscle bulges and gets shorter and fatter. Eccentric is the lengthening part of the motion. The extension of the elbow in the curl. This means the muscle is lengthening. Here's the key, if we are getting tendon issues we are likely working the concentric phase too hard and the muscles are contracting with great tension on the tendon. The muscle is able to handle it, but the tendon begins to break down or even tear. So, let's move into placing a sufficient, but not too heavy of a load on the tendon and allow the tendon to be stressed in order to be stimulated for growth, but not so stressed that it tears. The muscle will likely not work as hard but we are working the tendon at a load that it can handle. Ben Patrick, Knees over Toes Guy, was one of the guys that I began to follow closely because he has really stressed the importance of strengthening the knees in order for the muscle to be most effective in jumping and running. Sore knees are oftentimes caused by irritated and weak tendons, typically the patellofemoral tendon or the hamstring tendons. A key point also is to not be afraid to train the joints through their fullest range of motion. For years coaches told their athletes to not squat with their knees going out past their foot. Between this and sitting in chairs with our legs bent only at 90º our PFT lost its range of motion and became weak when placed in a movement beyond its trained motion. For me, this meant sore knees all the time. Two knee replacements later. The second part of the video is demonstrating with the Kettle Bar varieties of movements that make the body work through multiple planes. Maybe it’s not an issue at all and there’s just a portion of the population who don’t dream? I find it very strange as I dream every night very vividly. Looking forward to hearing your experience with this! Thanks! Katie: Hi Dr Cabral, Could you please help with some advice for my teen sons acne. After seeing a Naturopath he has been off sugar, dairy and processed foods for 4 months with not too much change. He currently takes zinc and a probiotic and does clay masks.The only option he has been given from his gp is 3 to 6 months of antibiotics and Accutane which we want to avoid due to side effects and long term damage. As well as the pimples he already has alot of scarring and he is only 14. Thankyou for your time we appreciate it :) Lindsay: I’d love to hear your thoughts on a shortened follicular phase. This is something I have been struggling with for the last 7-8 years. I wear an Oura ring and can clearly tell when I ovulate based on temperature change, vaginal discharge, and mittelschmerz. I typically bleed for 5 days at the beginning of my cycle and ovulate around day 10, resulting in a 24 day cycle. My luteal phase is always 13-14 days in length. In November I ovulated on day 7! I couldn’t believe it, I bled for 5 days, ovulated on day 7, and then had a 14-day luteal phase for a 21-day cycle. No history of birth control or health struggles to speak of, and not on any medication. No obvious hormonal issues: clear skin, healthy blood sugar levels, healthy weight, sleep well, and exercise regularly. 39 years old. Kelsi: hi dr. cabral! a question about sauna - is it still beneficial for someone who experiences elevation in blood sugar during / after a session? thank you. Aaron: I've noticed that I'm more prone to tendonitis and muscle strain as I get older. I try to lift weights 2 to 4 times a week when I'm not injured. I try to eat whole food, try to exercise whole body workout daily, trying to reduce stress (big problem for me), I'm working on reducing toxins, worst sleep (my biggest problem I'm working on), very vata/anxious, can't take DNS because I'm sensitive to B6, but I do maintain my methylation, vit D, and magnesium, healthier now than I have been.
