In this powerful episode of the Prolonged Field Care Podcast, Dennis sits down with Viktoriia, Chief Medical Officer, responsible for medical care across more than 150 kilometers of active front line. A former OB/GYN who served as a combat medic in 2014–2015 and later on Medevac, Viktoriia shares the raw, real-time evolution of Ukrainian military medicine from outdated Soviet-era training to a battle-hardened system built for drone-saturated battlefields, prolonged evacuations, and extreme resource constraints. They dive deep into how Ukraine rapidly expanded medic scope of practice, trained every infantryman to Combat Lifesaver (CLS) level, pushed aggressive hypothermia management, and achieved a groundbreaking policy change allowing combat medics to deliver low-titer group O whole blood and dry plasma at the point of injury after a specialized 32-hour course. This podcast was recorded in partnership with Leleka Foundation, an American-Ukrainian charitable initiative committed to helping frontline medics in Ukraine save lives. This project creates a vital platform for Ukrainian frontline medics to share firsthand trauma care experience from the battlefield with their American counterparts, strengthening knowledge exchange. Key Takeaways:Train everyone to CLS level — it's the only scalable solution to medic shortages and the targeting of medical personnel.“Prolonged Field Care under fire” is the new reality: limited interventions, maximum security, and ruthless prioritization because the battlefield is completely transparent.Aggressive hypothermia management and intraosseous access have become frontline skills for regular infantry due to hours-to-weeks-long evacuations in freezing trenches.Decision-making under fire is the most critical (and trainable) skill — technical abilities mean nothing without the judgment of when to act.Other militaries should stop preparing for the last war. Use Ukraine's “lessons identified” now, while you still have time to adapt training proactively.Whether you're a combat medic, unit leader, medical educator, or just obsessed with what actually works when everything goes wrong, this episode delivers hard-won wisdom you won't find in any textbook.Chapters00:00 — Welcome & Introduction to Viktoriia Kovach, Chief Medical Officer02:03 — The Core Lesson of the War: Radical Adaptability to Save Lives03:11 — Pre-2014 Medical Training: Soviet-Era Foundations & “Grandfather's Bandages”06:36 — The 2014 Shift: NATO Cross-Training Lands in Ukraine09:38 — Expanding Medic Scope of Practice: Blood, IO Access & Advanced Interventions11:14 — The Modern Training Pipeline: 55-Day Basic General Military Training + Adaptive Period16:01 — Combat Lifesaver (CLS) for Every Infantryman — The Scalable Solution19:39 — Solving the Combat Medic Shortage: Positioning, Internal Instructors & CLS Emphasis23:52 — Drone Warfare Reality: Training Rapid Decision-Making Under Constant Aerial Threat29:07 — Prolonged Field Care Under Fire: Hypothermia Priority, Wound Care & Limited Interventions31:49 — The Blood Transfusion Revolution: 32-Hour Course, Low-Titer O Whole Blood & Dry Plasma36:23 — Lessons Identified: Advice for Commanders Preparing for the Next War39:39 — Closing & ResourcesFor more content, go to www.prolongedfieldcare.orgConsider supporting us: patreon.com/ProlongedFieldCareCollective or www.lobocoffeeco.com/product-page/prolonged-field-care

A recent investigation has unearthed previously unknown information about the death of a female Ukrainian journalist who had been looking in to war crimes in the conflict between Russia and Ukraine. Viktoriia Roshchyna, herself Ukrainian, was captured in 2023 whilst reporting from the occupied territories. She died in 2024 after a year in detention, where she was held without charge and without legal assistance. Editor on the Investigations team at The Guardian, Juliette Garside, joins Anita Rani to discuss the report. Have you ever wondered how much impact the name you were given has had on your life? Florence Knapp's debut novel The Names begins with a dilemma; Cora is about to register the birth of her baby son, but should she call him Bear, the name chosen by her nine-year-old daughter Maia, Julian, which is her choice, or Gordon, the name she is expected to give him and also the name of her abusive husband and his father before him? Florence joins Anita to discuss her new book and the fateful decisions we make every day. It's been a big week for Chelsea who last night won the Women's Super League title for the sixth year in a row, beating Manchester United 1-0 after a goal from Lucy Bronze. The victory marks the first Women's Super League title for Chelsea manager Sonia Bompastor, who took over from Emma Hayes last summer. It also follows a defeat against Barcelona in the Champions League over the weekend. BBC Sport's Emma Sanders joins Anita to discuss Chelsea's season. With the Met Gala fast approaching, we take a closer look at the business behind the red carpet with International Style Correspondent for the New York Times, Elizabeth Paton, and Fashion Editor for Glamour Magazine, Rosie Lai. If we're open to it, fibromyalgia can be a path to self-discovery and personal growth. It teaches us about ourselves and how to let go of perfectionism and unrealistic expectations. It highlights the value of embracing small victories. It reminds us that rest is an intentional practice, not a passive activity. Over time, our bodies change, and we have to adapt. Life doesn't always go the way we expect it to, and the best thing we can do is to create systems that support us in our lives and that allow us the flexibility to adjust when we need to.  Becoming a Certified Fibromyalgia Advisor® or Certified Fibromyalgia Coach® can help with this by educating, encouraging, and supporting you in your journey with fibromyalgia. As people with fibromyalgia ourselves, we can give you real tips and tricks from our own lives and things that have helped us with our fibromyalgia. Today, in the tenth installment of our Stories of Hope and Healing, Tami shares inspiring excerpts from our January 2025 event. You'll hear from three graduates about their stories and how they've been able to improve their fibromyalgia symptoms. On average, our graduates improve their symptoms by about 50% during the program, and in this group, their symptoms were improved by a staggering 63%. All of these graduates ended with a revised fibromyalgia impact questionnaire score of 26 or less, which is very close to what some would consider to be remission. In this episode you'll hear about Clara's recognition that self-care means more than fine-tuning nutrition and exercise, insights from working with two different coaches and how each helped her in different ways, how Clara's Book of Me helped her to better understand her body, why Clara decided to become a Certified Fibromyalgia Coach® to help other women veterans, the main factors that contributed to her 84% improvement in symptoms, Kim's perspective on the gift of feeling better and the joy of helping others through coaching, why Kim decided to become a Certified Fibromyalgia Coach® despite having multiple other coaching certifications, the unique hands-on experience and the rounded curriculum of the program, avoiding burnout while managing fibromyalgia and maintaining a sustainable approach, adapting to life's changes, how a life crisis impacted Kim's symptoms, lessons learned from a recent fibro flare, celebrating your progress and day-to-day accomplishments, the various strategies that contributed to Kim's significantly improved fibromyalgia symptoms, how Kim has helped clients overcome insomnia and manage sleep apnea, why she loves working with women with strong personalities, encouragement for you to seek support and guidance in your fibromyalgia journey, Viktoriia's decision to join the Certified Fibromyalgia Coach® program, understanding the importance of self-compassion, adapting your life to your fibromyalgia reality, lessons that fibromyalgia teaches us, essential elements in Viktoriia's recovery journey, the importance of flexibility and adaptability in managing stress and fibromyalgia symptoms, how coaching helped Viktoriia on her journey, the value of community and support among fibromyalgia patients, differentiating coaching from therapy, focusing on proactive management of fibromyalgia rather than coping strategies, the lack of resources for non-English speakers and the importance of bridging this gap in fibromyalgia support, and more.     Note: This episode is not meant to be medical advice. Every person and every situation is unique. The information you learn in this episode should be shared and discussed with your own healthcare providers. Kudaibergen, a prominent Qazaq researcher of colonialism and a sociologist at the University of Cambridge, who tackles the commemoration of the genocide in Qazaqstan and underscores the need to elevate indigenous voices in these discussions, and Viktoriia Grivina, a Ukrainian writer and a cultural researcher from Kharkiv, who exposes Russia's colonial rhetoric, with her home city being the victim of it.Dive into "Matryoshka of Lies" with Maksym Eristavi, author of the illustrated guidebook "Russian Colonialism 101," and Ukrainska Pravda.
