Because resolutions often focus solely on taking action without considering the underlying intentions, thoughts, beliefs, and emotions.I share a recent coaching experience where we explored the power of feeling emotions in the body, emphasizing the importance of understanding our emotions rather than suppressing them. The key is to move beyond mere willpower, recognizing that true change requires a deep dive into our thoughts and beliefs.As we navigate through the episode, I challenge the conventional approach to resolutions, especially those centered around weight loss, fitness, or eliminating habits. Instead, I encourage you to question why you desire these changes and whether the underlying beliefs align with your true desires.I open up about my own journey, recounting a time when I believed clearing my acne would bring happiness, only to realize that self-acceptance and approval were the true goals. This serves as a reminder that external changes don't guarantee internal fulfillment.The core message of the episode revolves around the concept of an "anti-resolution." I'm not against creating new results in your life; however, I am against mindless action and setting resolutions without understanding why you want them. I urge you to ponder the question: Why do I want this result? And go beyond the superficial answers.I introduce a crucial aspect of the anti-resolution approach – focusing on emotions. Let's face it: we have to work to live. The unfortunate reality is that those who identify with symptoms of what is known as borderline personality disorder (BPD or EUPD), as well as other disorder labels like complex post-traumatic stress disorder (cPTSD), often find it hard to “hold down a job.” These individuals are often highly sensitive, reactive, creative, intuitive, and intense. The workplace can be highly triggering for anyone grappling with trauma recovery. It's all too common to feel that we have to take/do jobs that we hate and end up sacrificing our creative passions. In this episode, we will discuss the common issues and triggers that can arise for us in the workplace and how to best handle and move through them. We'll also dive into how we can move towards work that we love – something that can feel impossible but doesn't have to be. This one's for you if you feel like you jump from job to job and find yourself repeating patterns that cause chaos and discontentment in your professional life. The key to better understanding dissociation rests in normalizing it. In the third of the 5 Critical Crisis Conversations, Manaan explores how the person's agenda of ensuring short term safety through admission may be quite different to the long term focus of resilience for the professional.Both want safety but the route to it is different. Being able to show the similarity of the overarching goal amidst the dissimilarity of the immediate outcome is a critical conversation to have with the person in distress. If they are able to understand that the professional is not rejecting their needs or abandoning them and on the contrary is selecting a much longer harder route to walk with them one can bring the person on board for safety planning and taking charge of maintaining their safety. Or else a person with BPD will feel the pressure to prove the professional that they are in deep distress and the risk will escalate as the threats to harm oneself intensifies.

Impulsivity is a core feature of BPD. Impulsivity in BPD has been defined as 'rapid and unplanned action.' However, a preference for immediate gratification and discounting of longer-term rewards might better describe the impulsive behaviors that people with BPD use to regulate their emotional distress.The most up-to-date research proposes that BPD develops due to a complex interplay of genetics, increased affective sensitivity/reactivity, and stressful life events. The idea is that these factors prevent those of us with BPD from developing healthy emotional coping mechanisms, which results in a severe form of emotional dysregulation.Our difficulty regulating our emotions is what leads to our increased use of impulsive coping strategies. Usually, without even being conscious of it, we are using these impulsive behaviors to help self-soothe (also called "down-regulate) and further repress our deep feelings of shame and self-hatred. The key here is that for people with BPD, impulsivity usually occurs in response to the stress and negative emotions we've taught ourselves to avoid. Sometimes to the outsider, we can seem quite successful, giving little indication of the almost constant negative discourse going on inside our own heads. Kabir identifies as a nonbinary person. Non-binary is used to describe people who feel their gender cannot be defined within the margins of gender binary. Instead, they understand their gender in a way that goes beyond simply identifying as either a man or woman.A 2011 study found that patients with BPD were approximately twice as likely to report having a sexual relationship with a same-sex partner as comparison subjects with other personality disorders. It is a well known fact that queer people have poorer mental health outcomes than the heterosexual population, mainly due to discrimination, finding it difficult to access services, etc.After learning more about Kabir's story, I thought the podcast audience would appreciate their story, journey and perspective. Around 1 in 20 people is estimated to have a personality disorder, and it is a neglected and under-resourced area of our healthcare system. In this week's episode, we discuss the stigma surrounding personality disorder, which can often manifest itself in high levels of anxiety for both patients and GPs, when it comes to diagnosing and managing it, and how to help a patient come to terms with their diagnosis. With suicidal ideation being experienced by many people with a personality disorder on a regular basis, we also talk about how we may best manage a situation of a patient in crisis presenting in primary care. Our guests: Leisha Davies is a clinical psychologist, originally from South Africa, who currently works in private practice. Soumitra Burman-Roy is a consultant psychiatrist at the South London and Maudsley NHS Foundation Trust. He also works for Maudsley Learning, an organisation which provides mental health training for primary care. Marie Stella McClure, who was diagnosed with borderline personality disorder at the age of 38, is the author of ‘Borderline: a Memoir', a book about her life and experiences of BPD.
