Happy Juneteenth! It's another edition of our monthly Midday Healthwatch, with Dr. Leana Wen. The latest surge in the COVID-19 variant known as omicron is beginning to show signs it is receding, but the highly transmissible BA.2 variant has led the CDC to estimate that nearly 60% of Americans have already been infected. For children, the infection rate is even higher: closer to 75%. And help for our youngest children is on the way. On Friday, an advisory committee of the Food and Drug Administration emergency use authorization (EUA) to the Moderna and Pfizer-BioNTech coronavirus vaccines for children under the age of five. The CDC signed off on the panel's recommendation on Saturday. This comes as a relief to many parents, and it presents a dilemma for many others, who may be vaccinated themselves, but who are hesitant about vaccinating their kids. Dr. Wen is herself the parent of two children under the age of five. On the 11th of April, the testing positivity rate for COVID 19 in MD stood at 2.42%, nearly twice what it was three weeks earlier. It's nearly doubled again in the last three weeks. The BA.2 variant is highly transmissible. The CDC estimates that nearly 60% of Americans have already been infected. For children, the infection rate is even higher: closer to 75%. And now, two more variants, BA 4 and BA5 are causing big spikes in South Africa. What's on the horizon for the US? Of course, with the rise in infections, many fewer people are becoming seriously ill than in the past. But serious illness and death for immunocompromised patients is still a major concern. It's the Midday Healthwatch, our monthly focus on public health issues with Dr. Leana Wen. For weeks, the state testing positivity rate for COVID 19 has been below 2%. But it's starting to creep up again. As of this morning, it stands at 2.42%, nearly twice what it was three weeks ago. At last week's Gridiron Club dinner in Washington, about a dozen political A-listers, including Attorney General Merrick Garland, Commerce Secretary Gina Raimondo and Congressmen Adam Schiff and Joaquin Castro tested positive for the coronavirus. Do the rising numbers and high profile cases indicate that we should once again avoid large gatherings? Dr. Wen says, “No.” She writes in the Washington Post, “At this point in the pandemic, we have to accept that infections will keep occurring.” Is another Coronavirus surge in the cards? The BA.2 sub-variant strain is even more transmissible than the original omicron variant, which itself spread much more quickly than the delta variant. During the last surge, nearly half of Americans contracted the virus, but many fewer people became seriously ill than in previous surges. In the middle of January, the US was reporting more than 900,000 new cases of COVID 19 every day. The daily case count now averages less than 50,000. Here in Maryland, two months ago the testing positivity rate was nearly 27%. Now, it stands at less than 2%. Maryland and the District of Columbia have the nation's lowest daily case rate. The vaccination rate is creeping up very, very slowly. More than a year into vaccine availability, only about 65% of Americans are fully vaccinated, well behind dozens of other countries. The US Centers for Disease Control and Prevention (CDC) has revised its guidelines for masking in public spaces, changing the metrics it emphasizes as they determine what is best practice at this stage of the pandemic. About a month ago, the Food and Drug Administration authorized booster shots of the Pfizer-BioNTech coronavirus vaccine for 12- to 15-year-olds, and it lowered the time frame for everyone getting a booster from six months to five months after the second dose of the Pfizer vaccine. The CDC made a final recommendation to allow it soon thereafter. About a month from now, it's expected that the Pfizer vaccine will be approved for children as young as six months old.There is some progress to be happy about in the fight against COVID. In mid-January, the United States was reporting more than 900,000 new cases of COVID 19 per day. Over the weekend, that number had receded to about 304,000. The positivity rate here in MD was nearly 27% last month. As of this morning, it stands at just above 6%. But more than a year into vaccine availability, only about 64% of Americans are fully vaccinated. Dozens of other countries have much higher rates of vaccination. With vaccines now available for virtually everyone except newborns, the question is, will Americans take full advantage of the opportunity? And will wealthy countries like the US assist less wealthy countries to get their people fully vaxed? Today, in our first show of the new year, it's the Midday HealthWatch.  Tom's guest is Dr. Leana Wen, who for years has been so generous sharing her time and expertise with Middaylisteners. Dr. Wen is an emergency physician and former Baltimore City Health Commissioner. She teaches at the George Washington University School of Public Health. She's also a columnist for the Washington Post, a medical analyst for CNN, a Senior Fellow at the Brookings Institution and the author of Lifelines: A Doctor's Journey in the Fight for Public Health. This morning, the US Food and Drug Administration (FDA) authorized booster shots of the Pfizer-BioNTech coronavirus vaccine for 12 to 15-year-olds, and it lowered the time frame for everyone getting a booster from six months to five months after the second dose of the Pfizer vaccine. The Centers for Disease Control and Prevention (CDC) will review the FDA panel's ruling. They're expected to make an official recommendation on Wednesday. Over the New Year's weekend, the US was reporting nearly 300,000 new cases of COVID-19 every day. That number has climbed to more than 400,000 daily cases as of today and it's likely much higher, given the number of people who are self-testing for the virus, and not reporting positive results. And it's not all due to the omicron variant. The Delta variant still accounts for more than 41 percent of cases in the United States, as of the week that ended on Christmas Day. The positivity rate in Maryland is nearly 27% and Maryland hospitals are under orders to limit various procedures so that scarce medical personnel can concentrate on patients with severe COVID-related disease. Maryland reported more than 14,000 new cases in the last 24 hours. More than 2,700 people are currently in Maryland hospitals; that's nearly 200 more than on Sunday. As we mentioned, children over the age of 12 will soon be eligible to get a booster. But, first things first: nationally, only 32% of kids above the age of 5 have received their first shot. And, a year into vaccine availability, only about 62% of Americans are fully vaccinated, well behind dozens of other countries. The trends in the coronavirus pandemic are heading up again, and this is before any possible uptick that may or may not happen with the new omicron variant. The positivity rate here in Maryland has been above 5% for about a week, and the number of daily new cases in our state has hovered around 17 or 18 hundred each day, many more than in the past several months. Children over the age of five are now eligible to get a jab, and everybody over the age of 18 qualifies for a booster. Nationally, nearly a year into vaccine availability, only about 60% of Americans are fully vaccinated, well behind 60 other countries. Places like Portugal, Cuba, Cambodia, Iceland, and Australia all have much higher rates of fully vaccinated people. And of course, now, with booster shots recommended for those who have received the original dose, the definition of what “fully vaccinated” is may have to change. Today on Midday, it's the Midday HealthWatchwith Dr. Leana Wen, who is always so generous with her time and willingness to answer our questions about COVID-19. Today on Midday, it's the Midday Healthwatch with Dr. Leana Wen, an emergency physician and former Baltimore City Health Commissioner who joins us each month to talk about public health issues, and who has been providing us with timely updates on the COVID-19 pandemic. Trends in the pandemic are headed in a relatively positive direction. Nationally, cases are down about 20% in the last couple of weeks, but that still amounts to about 114,000 new cases every day. With 45% of the country still unvaccinated against the coronavirus, many people who received the Pfizer vaccine six months or more ago, are now cleared to get a third dose. There was some confusion last week when an advisory panel of the CDC made a recommendation as to who should qualify for a booster shot. CDC Director Rochelle Walensky broadened the group that her advisors had identified. The people who are now eligible include people who are immunocompromised, at least 65 years old, or who work in front line jobs like medical personnel and teachers. That group is likely to expand to include people who received the Moderna and Johnson & Johnson vaccines, but the FDA and CDC have not yet approved boosters of those vaccines. Today on the Midday Healthwatch, Dr. Leana Wen will help us sort out these developments. Dr. Wen teaches at the George Washington University Milken Institute School of Public Health.   "This year, the Fourth of July is a day of special celebration, for we are emerging from the darkness of a year of pandemic and isolation, a year of pain, fear and heartbreaking loss. Just think back to where this nation was a year ago, think back to where YOU were a year ago, and think about how far we've come.” — President Joe Biden, speaking at the White House on Sunday, July 4. Maryland is one of only 20 states to reach President Biden's goal of vaccinating 70% of adults with at least one dose of the coronavirus vaccine by last weekend. Nationally, fewer than half of Americans have been inoculated. And states where vaccination rates are lower are experiencing a surge in cases, as the Delta variant of the COVID-19 virus takes hold and continues to infect unvaccinated people. Data from Johns Hopkins shows that the Delta variant, a mutant strain of COVID-19 that is twice as transmissible as the original virus, has led to almost three times as many new COVID cases in states where vaccination rates are low.Is it time to start requiring people to be inoculated against SARS CoV2, the virus that causes COVID-19?
