Welcome to The Sports Docs Podcast! On each episode we chat about the most recent developments in sports medicine and dissect through all the noise so you know which literature should actually impact your practice. On today's episode we're focusing on patellofemoral cartilage defects with Dr. Cassandra Lee, an orthopaedic sports medicine surgeon, team physician for the UC Davis Aggies and Sacramento Republic FC, and chief of the sports medicine service at UC Davis. Dr. Lee has published on and spoken a lot about cartilage – both at the basic science level as well as clinical application – so we're excited to have her join the discussion today. We have some great articles for you that contribute well to our conversation on the surgical treatment of patellofemoral cartilage disease. As always, links to all of the papers that we discuss on this show can be found on our podcast website. The first article is a systematic review published just this month in OJSM titled “Cartilage Restoration for Isolated Patellar Chondral Defects.” Ronak Patel and his colleagues at the Illinois Center for Orthopaedic Research and Education summarized the results and complication rates of various patellar cartilage restoration techniques. They concluded that osteochondral autograft transplantation and autologous chondrocyte implantation were the most studied procedures for isolated patellar chondral defects. The article also touches upon newer techniques such as augmented microfracture, but the authors state that there is wide variability in indications and techniques that must be clarified in future higher-level studies. Then, from the upcoming June issue of Arthroscopy this year, we review a retrospective cohort study titled “Utilization of Autologous Chondrocyte Implantation in the Knee Is Increasing While Reoperation Rates Are Decreasing Despite Increasing Preoperative Comorbidities.” Drew Lansdown and his team at UCSF observed a significant increase in the use of ACI since 2017 with a significant decrease in the rate of 90-day and 2-year reoperations for ACIs performed after 2017. Older age and tobacco use were predictors of increased risk of conversion to arthroplasty. Male sex was associated with decreased risk of reoperation. We are joined today by Dr. Cassandra Lee, a board-certified fellowship-trained orthopedic sports medicine surgeon and Chief of the Sports Medicine service at UC Davis. Dr. Lee obtained her medical degree from Boston University. She completed her residency training at Wake Forest University and sports medicine fellowship at Washington University in St. Louis, serving as team physician for the Washington University Bears, St. Louis Rams, and St. Louis Blues. Dr. Lee was a 2017 Lars Petersen Travelling Fellow for the International Cartilage Regeneration and Joint Preservation Society and a 2022 American Orthopaedic Society for Sports Medicine / Asian-Pacific Knee, Arthroscopy and Sports Medicine Society Traveling Fellow. She currently serves as team physician for collegiate and professional teams including the UC Davis Aggies and the Sacramento Republic FC. Her research interests are in modulation of post-traumatic osteoarthritis, currently funded by an NIH R01 and Department of Defense grants.

Danny Vitiello is a professional goalkeeper for Sacramento Republic FC in the USL Championship, the 2nd tier of US soccer. Danny played at the university of Albany before signing with at the time USL Championship side Nashville SC. After spending his first professional season adjusting to the pro game, Danny signed for Pittsburgh Riverhounds where he was awarded USL Championship's Golden Glove. The USL Championship 2023 season is finally here! The Crew are back to talk about El Paso Locomotive FC's disappointing 1-0 loss to visitors Sacramento Republic FC on opening night of Season 5 at the SWUP. There were green shoots of optimism though, as the Crew dissected Brian Clarhaut's new controlled play style, the impact of the debutants, and the goalscoring chances that Los Locos had. The Crew also highlight the second half pressure Los Locos put on the visitors to try to win the game and odd refereeing decisions or lack thereof. Orlando City went and did the thing, winning the 2022 Lamar Hunt U.S. Open Cup. The Lions claimed a trophy with a resounding (but difficult) 3-0 victory over Sacramento Republic FC thanks in large part to the efforts of Facundo Torres and Benji Michel in the final 15 minutes plus stoppage time. Most of The Mane Land staff was on hand to see this historic event and we're all still basking in the glow of a championship and the prospect of Orlando playing in the Concacaf Champions League. However, this is Central Florida and a little rain is going to fall, obviously. In this case it was allegations of spying lodged before the game by Sacramento. We'll have to keep an eye on what transpires from the investigation but at this point we're not expecting anything major to come from it and we hope that is the case. With the good and the bad comes the ugly, and this past week it was the treatment of Taylor Twellman on social media for the high crime of breaking unwelcome news. Others reported it without much more detail without the vitriol. Having just had a phone conversation with Twellman about Orlando City earlier in the day and having heard the compliments he gave our city and fans just a few hours before that news broke. He reiterated that online but with the unfortunate timing of it being right after his news-breaking tweet and it was met with some true ugliness. That was unfortunate. The Lions went to Philly and took a worse beating than we expected but not a surprising one, all things considered. We'll take the trade-off since it was always going to be a difficult fixture. Orlando City will turn its attention toward a vital two-game homestand now. The Pride lost their second straight — this one at home against the Portland Thorns. Seb Hines' team has no margin for error in the playoff chase and must get a result in Louisville to realistically stay in the fight. OCB played like a team that had just been eliminated from playoff contention with a 6-1 beating at Philadelphia Union II. Honestly, the less said about the Young Lions' match on Sunday, the better. This week's mailbag asked whether college sports are best enjoyed at home or in person. Orlando City SC found a new way to win last Wednesday, falling behind 2-0 to the Seattle Sounders — a team the Lions had never previously beaten — and then rallying back from two goals down to win, 3-2. That happened despite Mauricio Pereyra missing a penalty kick in the first half that would have pulled the Lions level before the break. The team continues to show resiliency and found a way to win despite rotating the squad and falling behind. We break down Orlando's fourth consecutive victory and second straight at home and make our selections for Man of the Match. Another Lion made MLS Team of the Week, so we mentioned that as well. Benji Michel dropped by to talk about the team's U.S. Open Cup final tonight against Sacramento Republic, which is a special moment for the Orlando native and OCSC Homegrown Player.
