Play Episode Listen Later Jun 16, 2026 45:55

Dr. Peter Sorensen joins hosts Dr. Jeffrey Jensen and Dr. Johanna Richey for a wide-ranging conversation about artificial intelligence in podiatric education and practice. Recorded as Sorensen finishes his second year of residency at Ascension St. Vincent in Indianapolis, the episode traces his path from a college ankle fracture into the profession, through his early experiments with ChatGPT in 2022, and into the platform he now builds for students and residents. This episode is sponsored by Bako Diagnostics! The importance of using AI responsibly is Dr. Sorensen's main message. He and the hosts discuss how to let AI save time and sharpen thinking without quietly eroding the skills that make a good clinician and surgeon. Along the way the conversation turns to hallucinations and active learning, the risk of “de-skilling,” AI as a thought partner, and how these tools can lower the steep barrier to entry into organized podiatry.Sorensen started using ChatGPT the week OpenAI released it to the public in November 2022, while he was still in podiatry school. The early versions, he admits, let him down — they couldn't even browse the internet in real time — but he kept a running mental list of things he wished the tools could do. Over the last several months, he says, the capabilities finally caught up to those ideas. The most quoted idea in the episode is also its most countercultural: skepticism isn't a reason to avoid AI — it's the reason to use it. “I always tell people to always doubt the output of AI,” Sorensen says, because hallucinations are real. But that doubt, he argues, turns a passive user into an active learner. If something feels off, you go to the primary source, and modern tools are increasingly good at telling you where their claims came from. The hosts press on the harder question — cognitive offloading, or “de-skilling.” Dr. Richey makes the point that real learning has to be a little hard: “We can't biohack knowledge, but we can make it more accessible.” She and Jensen return to a distinction they've discussed before — that knowledge is becoming a commodity in the palm of our hands, while wisdom is not. Sorensen agrees that the journey matters more than the destination, and that offloading every hard thing would leave the next generation unable to do the work. His answer isn't to abstain; it's to be intentional.Beyond time saved, Sorensen describes the use case he now values most: a thought partner. Claude, he says, is his go-to here — a soundboard that helps him clarify what he actually thinks while keeping the ideas his own. As a senior resident beginning to form his own positions on patient care and professional politics, he finds the tools useful for clearing mental clutter and organizing scattered thoughts into something coherent. He speaks from recent experience. Through the APMA Emerging Leaders Program, he traveled to Washington, D.C., to advocate on Capitol Hill, and came away struck by the tireless, often-underappreciated work that actually moves the needle — including the efforts of APMA staff. Attending the House of Delegates as a young physician observer was, he says, one of his most eye-opening experiences, and also where he started using Claude in earnest, to make sense of what he was watching.About Dr. Peter SorensenPeter Sorensen, DPM, MHA, is a podiatric surgery resident at Ascension St. Vincent in Indianapolis. He earned his DPM and MHA from Des Moines University (graduating in 2024) and holds a BA in Spanish from Southern Utah University; he previously worked as a medical Spanish interpreter. He is a member of the APMA Emerging Leaders Program and served on the APMA Re-imagining Education Task Force. He is the publisher of The Guidewire, where he writes about artificial intelligence in podiatric medicine and works to lower the barrier to entry into the profession for students and residents.

In this episode of the Dean's Chat Learning Series, Dr. Johanna Richey welcomes podiatric dermatology expert Dr. Leland Jaffe for an in-depth discussion on diagnostic miscues, biopsy decision-making, and the importance of maintaining a broad differential diagnosis in podiatric medicine. Drawing from his experience teaching dermatology and working extensively in wound care, Dr. Jaffe shares how his threshold for performing biopsies has significantly lowered over the years as he recognized how many serious conditions can masquerade as common foot and ankle pathology. The conversation explores how chronic wounds, nail disorders, inflammatory skin conditions, and even routine “ingrown toenails” can occasionally hide malignancies or autoimmune disease. Dr. Jaffe emphasizes that when a patient fails to improve with appropriate standard-of-care treatment, clinicians must pause and ask, “What am I missing?” rather than simply repeating unsuccessful therapies. Through several compelling clinical examples, including wounds later diagnosed as squamous cell carcinoma, basal cell carcinoma, and autoimmune-mediated lesions, he highlights how biopsy can completely alter a patient's treatment trajectory and, in some cases, save lives.Dr. Jaffe also provides practical insight into biopsy technique selection, discussing when he prefers punch biopsies, shave biopsies, or excisional biopsies depending on the suspected diagnosis. He explains why inflammatory conditions often require full-thickness punch biopsies, while suspected melanomas may warrant more nuanced excisional approaches to ensure accurate staging. Importantly, he reassures clinicians that “some biopsy is better than no biopsy,” encouraging providers not to let uncertainty or fear prevent them from obtaining tissue when something feels atypical.A major theme throughout the episode is collaboration. Dr. Jaffe and Dr. Richey discuss the value of working closely with dermatopathologists, oncologists, dermatologists, and wound care specialists to optimize patient outcomes. They stress that diagnosing a concerning lesion is not the end of the clinician's role, but rather the beginning of a team-based process that benefits both patients and providers.#podiatry #podiatricmedicine #podiatricdermatology#skincancerawarenessmonth #skincancerawareness #skincancer #melanoma #melanomaawareness#PICA #BakoDx

In this episode of Mental Man Monday, Izzy Baker is joined by Houston's own 713-E for a conversation about relationships, masculinity, boundaries, and the psychology behind some of the internet's biggest viral moments. As a men's mental health podcast focused on culture, emotional intelligence, and accountability, this episode explores how social media continues shaping the way people view dating, loyalty, privacy, and self-control. The conversation begins with Nick Cannon going viral after admitting he allows his son to date while being far more protective over his daughter. Izzy and 713-E unpack the debate surrounding gender roles, parenting styles, and whether certain forms of protection eventually become control. The discussion explores how different expectations for sons and daughters can impact confidence, decision-making, and emotional development — especially once children finally experience freedom on their own. From there, the episode shifts toward Boston Richey responding to explosive allegations from his ex, leading into a broader discussion about relationships, social media exposure, and why so many personal conflicts now become public entertainment. Izzy questions why people increasingly run to the internet instead of handling situations privately and examines how online reactions can blur the line between accountability, humiliation, and performative responses. The livestream also dives into a deeper conversation surrounding fetishes, boundaries, curiosity, and cultural influence. Referencing ongoing internet conversations and reactions to comments made by Trick Daddy, Izzy and 713-E discuss how early exposure, social normalization, and online culture can shape attraction and behavior. The conversation asks an uncomfortable but important question: just because something becomes normalized online, does that automatically make it healthy?Throughout the episode, the discussion continuously returns to mental health, emotional discipline, and intentional living. Whether discussing dating standards, oversharing online, temptation, or relationship boundaries, the larger focus remains on how men navigate modern culture without losing self-awareness, accountability, or emotional stability in the process. Because sometimes the biggest lessons aren't hidden inside the viral moment itself. Sometimes the real lesson is learning how to move with discipline in a culture that rewards impulsive behavior, public reactions, and constant validation.Listen. Reflect. Join us as we listen to Dr. Andrew Mastay, Dr. Patrick DeHeer and Dean's Chat Co-host Dr. Johanna Richey as they discuss “all things podiatric medicine and surgery training!” Tune in, as we explore the evolving challenges in medical education, particularly around feedback, motivation, and training culture in podiatric medicine. The conversation centers on a perceived shift in how students and residents respond to feedback, with the panelists emphasizing that modern learners still value feedback but often need it delivered with greater balance, clarity, and psychological safety.Tune in as we highlight that today's learners benefit from a mix of positive and constructive feedback, noting that exclusively corrective feedback can feel threatening and counterproductive. We emphasize the importance of timing, privacy, and framing, as well as involving learners in self-reflection by asking what they think went well and what they may do differently before offering critique. This approach helps align expectations and fosters a coaching environment rather than a punitive one.We highlight the importance of maintaining high standards while creating a supportive learning environment. Psychological safety does not mean lowering expectations but rather enabling trainees to think critically, ask questions, and learn from mistakes without fear of humiliation. In high-stakes settings like the operating room, we advocate for calm, step-by-step guidance, prioritizing patient safety while using real-time teaching moments constructively. A major theme throughout the discussion is the importance of individualized teaching. The panel emphasize understanding each learner's unique personality, challenges, and motivations. Whether addressing anxiety, confidence issues, or external life stressors, tailoring support to the individual is key to unlocking performance and growth. They also stress that confidence-building is essential, as both under confidence and overconfidence can negatively impact learning and patient care. The panel challenges the notion that younger trainees are less motivated, instead suggesting that they are motivated differently—often placing greater value on well-being, balance, and meaning in their work. Misalignment in expectations between educators and trainees is identified as a common source of friction, highlighting the need for open communication and mutual understanding. Finally, the conversation touches on motivation, with the conclusion that true motivation is internal and cannot be forced—only supported and inspired. Building relationships, showing genuine investment in learners, and creating opportunities for meaningful engagement are key to helping trainees reach their full potential. Practical strategies discussed include structured feedback frameworks, after-action reviews following cases, simulation-based learning, and creating consistent educational systems within residency programs.Overall, the panel underscores that while the landscape of medical education is evolving, the core principles of respect, communication, structure, and individualized support remain central to training competent, confident physicians. We hope you enjoy!

Laura opens up about how COVID upended her thriving wedding flower business, which had grown to 60 to 100 weddings a year, and how a pair of paper lips sitting in her living room sparked an entirely new creative direction. After a famous lip artist spotted her work on Instagram and invited her to collaborate, Laura's paper lip art took off in ways she never expected. "Anything 3D grabs attention. Anything that comes off the wall, people seem to gravitate towards." — Laura The conversation covers the realities of running two brands, managing massive production orders as a solo artist, and the physical toll that large-scale paper crafting takes on your hands, back, and mental energy. Laura, Sara, Jessie, and Quynh get into the details that only paper artists understand: how many flowers you can realistically assemble in a day, why Cricut mats wear out faster than you think, and how chopsticks became Laura's most essential tool. "At this time in my career, it is okay to say no and it is okay to give them your feedback." — Laura They also discuss the challenges of working with marketing companies and event coordinators who often reach out with unrealistic timelines and tight budgets, and why paper artists deserve to be brought into projects early rather than treated as a last-minute addition. What You'll Hear in this Episode: From wedding florals to paper lip sculptures and how Laura's 12+ year journey in paper flowers took a turn she never saw coming The Instagram DM from a celebrity lip artist that changed everything What it actually looks like to work with major brands and the difference between going through a marketing agency versus landing a direct brand partnership The reality of large-scale production: 600 lips for Too Faced, 4,000 roses for Lancôme, and what it takes to pull that off The physical and mental toll of making the same thing hundreds of times Running two brands when one is your passion and the other pays the bills and what happens when they start pulling in opposite directions Card stock versus crepe paper: why the medium you work in matters more than you'd think The tools Laura can't live without: chopsticks, Cricut machines, vinyl picker tools, and kebab sticks (yes, really) Cricut mat maintenance, buying in bulk, and building a machine workflow that actually holds up under pressure Laser cutters versus Cricut machines Where Laura is headed: teaching, fine art lip sculptures, and a creative practice that's evolving on her own terms Making it work as a maker and a mother — and why your workspace has to go wherever your family needs you Why setting realistic expectations with clients around timelines and budgets isn't just good business but necessary

Terbinafine resistance is on the rised. PCR testing (Onycho.ID) provides novel and specific real-time TaqMan PCR testing to detect 12 mutations of T. rubrum and T. mentorophytesWhat is MolDx?The MolDX (Molecular Diagnostic Services) Program is a Palmetto GBA-managed Medicare program that evaluates, registers, and determines coverage/reimbursement for molecular diagnostic tests. It ensures tests are clinically valid, assigns unique DEX Z-Codes for identification, and sets policies for over 28 states to ensure Medicare covers only medically necessary, evidence-based tests. MolDX States (Strict Molecular Testing Review)These states follow the MolDX Program run by Palmetto GBA, which requires DEX Z-codes and technical assessment for PCR tests.Southeast• Alabama• Georgia• Tennessee• North Carolina• South Carolina• Virginia• West VirginiaMidwest• Indiana• Michigan• Iowa• Kansas• Missouri• Nebraska• Kentucky• OhioWestern States• California• Arizona• Nevada• Utah• Oregon• Washington• Idaho• Montana• Wyoming• North Dakota• South Dakota• Alaska• HawaiiAdditional comments from Dr. Scherer, Bako Consultant - who will be joining us on a future episode:“The main benefit of Bako Diagnostics' screening and reflex PCR testing for nails is that it provides a very cost-effective method for PCR analysis. With this approach, the laboratory first performs a screening test. If a dermatophyte is detected during screening, only four PCR probes (cycles) are run in the reflex test, and the test is billed only for those probes. However, if a different type of fungus is identified during screening, the reflex test expands accordingly. For example, if a non-dermatophyte mold is suspected, the lab tests for seven different organisms. Similarly, if yeast is indicated, the reflex panel focuses on yeast organisms. The advantage of this screening-and-reflex algorithm is that the laboratory limits the number of DNA probes used to only the genus or group identified in the initial screening. This targeted approach reduces unnecessary testing and ultimately produces the lowest possible cost for PCR toenail diagnostics.”

In this episode of Dean's Chat, Dr. Jeffrey Jensen and Dr. Joanna Richey welcome Professor Dr. Lyndon Mason, an internationally recognized foot and ankle orthopedic surgeon based in the United Kingdom. The conversation offers a comprehensive look at his career journey, the structure of UK surgical training, and the foundational principles that have guided his success in clinical practice, research, and education.Dr. Mason, shares insights into his early path into medicine. Unlike the United States, where pre-medical education precedes medical school, Dr. Mason entered medical training directly at age 18. He outlines the UK system, which includes medical school, foundation years, core surgical training, registrar-level specialization, and fellowship.A central theme throughout the discussion is the profound impact of mentorship. Dr. Mason credits several key mentors with shaping his career trajectory, not only by providing guidance but also by challenging him to pursue opportunities beyond his comfort zone. He emphasizes that mentorship is not a short-term relationship but a lifelong influence, often continuing decades after initial training. These relationships also play a critical role in helping trainees navigate self-doubt and imposter syndrome—an experience Dr. Mason openly acknowledges as part of professional growth.Dr. Mason's career has been marked by extensive international experience, including training and observerships in Australia, Germany, and the United States. These opportunities exposed him to high-volume, complex pathology and diverse surgical approaches. Importantly, he highlights that the greatest value of these experiences lies not just in learning surgical techniques, but in understanding how different systems and surgeons approach problem-solving and patient care. With over 300 publications and more than 50 awards, Dr. Mason's academic productivity is driven by what Dr. Richey calls “insatiable curiosity.” He underscores the importance of questioning established practices, encouraging trainees to ask “why” at every step rather than accepting tradition at face value. The discussion also explores differences between the UK and U.S. healthcare systems, particularly in relation to industry collaboration. Dr. Mason explains that opportunities for device development and commercialization are more limited within the UK's National Health Service, where intellectual property is often institutionally held. As a result, relationships with industry differ significantly from those in the United States.Education remains a cornerstone of Dr. Mason's career. He describes a deep passion for teaching, especially at the level of early learners, and has developed numerous educational resources over time. During the COVID-19 pandemic, he launched a YouTube channel to make high-quality lectures accessible to trainees who might otherwise be restricted by paywalls. For him, teaching and curiosity are inherently linked—those who are driven to learn are naturally inclined to teach.Ultimately, Dr. Mason finds the greatest fulfillment in mentorship and the success of his trainees. He takes pride in seeing students progress through their careers, with the hope that they will surpass his own accomplishments. This long-term investment in others reflects his broader philosophy of growth, both personal and professional. The episode concludes with advice for students and early-career professionals. Dr. Mason encourages aspiring clinicians to question everything, work diligently, and pursue opportunities with persistence. He emphasizes that background does not define potential, and that growth often occurs in moments of discomfort. Hard work, curiosity, and a willingness to step outside one's comfort zone, he notes, are the keys to building a meaningful and impactful career.

Drs. Jensen and Richey discuss with Dr. Lehrman a coding summary, an educational tool that can benefit every client, practicing podiatrist, resident, and student. The summary can be seen in graphic form at the end of the podcast.o Developed to provide general coding guidance based on commonly accepted standards and interpretations of ICD-10 diagnostic coding and CPT procedural coding principles.o To be used solely as a guideline to assist practitioners. Final coding decisions remain the responsibility of the provider.o Significant thought, expertise, and clinical experience has gone into the development of this summary. BakoDx enlisted the insight of Dr. Jeffrey Lehrman, a well-respected advisor and certified cod, and with support from Dr. Wayne Bakotic, Dr. William Scherer, as well as Shawna, Jasmine and compliance approval from Donna Millard.o Ensuring broad distribution supports consistency, compliance awareness, and informed clinical documentation practices across the podiatric medical community. Our goal is to provide clarity, enhance understanding, and empower providers with reliable reference material, while reinforcing that this document remains a guideline and not a substitute for independent coding judgment or payer specific policies.Enjoy!!

While medical training often follows a structured and predictable framework, many guests emphasized the importance of recognizing opportunities, embracing change, and leaning into the aspects of practice that bring genuine joy. It has been especially inspiring to hear the wide range of ways podiatrists have found fulfillment—across medicine, surgery, education, leadership, and life beyond the clinic—by sharing their unique talents and expertise.Another clear theme has been the exceptional quality of our students. Today's podiatric students are highly motivated, deeply engaged, and purpose driven, and they are helping to shape and advance the future of the profession. We are also continually reminded that our profession is rich with talent. From clinicians and educators to innovators and advocates, podiatry thrives because of the collective strength of its people. Our sponsors play a vital role in this ecosystem as well—supporting advocacy efforts, workshops, leadership development, and continuing medical education. No one succeeds alone, and together we are stronger. Finally, leadership has emerged as a central topic, defined by courage, integrity, and passion. We highlight the diverse ways our guests have demonstrated leadership and made meaningful contributions to the profession. We are grateful for this journey and deeply appreciative of you—our audience—for your time, enthusiasm, and thoughtful feedback.
