Dr John Guiliana has had a unique career path, moving from Pharmacy to Podiatry and eventually into Healthcare Management with NEMO Health. Right now, during COVID-19, he has been assisting podiatry business owners across North America to move through the Fear, Learning and Growth Zones, and helping them understand how they can have lower patient volumes, yet still, enjoy improved profits. On this episode we discuss: How a D+ student found his intellectual groove Why he decided to leave Pharmacy and become a Podiatrist Nemo Health - A Practice Improvement Company COVID-19 Covid-19 has affected everybody, on both a business and personal perspective, and it has been global. Fear, Learning & Growth Zones During this pandemic, it's essential to get out of the Fear Zone, move through the Learning Zone, and eventually get into the Growth Zone You need to learn how to maintain profitability in your business with lower patient volumes, and some podiatrists will come out of the pandemic in a better than when they went in. It's about learning to do more with less. Why is there is a low patient volume? Patients are also going through the three zones of fear, learning and growth. There is also the Paradox of Thrift - which occurs when you have people that have been economically hurt, and because of this, they become more thrifty with where and when they spend their money. Cash flow is the key right now for every business. Getting paid faster and being more careful where you spend your money, only then can you work on profits. "The actions you take right now are going to help you with the long-term profitability of your business." Action Steps with Lower Patient Volumes Spend time reinventing your practice: Go through your policy and procedure manuals. Engage and collaborate with your staff and be a leader, and remove those team members that will not change. Look at your clinical protocols: You need to increase your PVR - Per visit Revenue. This is why lower patient volume will help increase long-term revenue because of the more thorough patient and evidence-based protocols. These will well offset lower patient volumes. Focus on what you enjoy doing. Get rid of the things they don't like to do. Looking beyond the chief complaint: What other treatment or service can you ethically provide. Dentists have been doing this forever. They explain things you cannot see Does your patient with an ingrown toenail have bunions or chronic knee pain? Creating Care Plans results in better patient outcomes and higher PVR. Final Tip Search for lessons learned. And the #1 lesson learned from this pandemic is the importance of liquidity. You need to have six to nine months of cash reserves for the next economic crisis. Shaun Bergin and George Koutsioufitis are the owners of Fairfield Podiatry in Victoria, Australia, and they have created a podiatry business overflowing with professionalism, fun and a business culture that would make many podiatrists envious. I first noticed they were different when I came across their website, and from that point, I knew I had to have them as guests on this podcast. Today we discuss: How and why they develop a friendly website, unlike many other podiatry websites. The importance of making sure their website was a representation of who they were. Boring was not an option. Partnerships Why George thought Shaun was going to be the right business partner. He acted like a business owner before he became the business owner. Sharing the same values Having the right work ethic Being part of the International Medical Team at the 2004 Olympics in Athens What to look for when you’re employing someone else. They've got to be there right fit. Part-time employees are never 100% invest because their mind is in more than one place. Don't try and fill a void with anyone; you're best to plan ahead BusinessLessons Things take much longer to implement than expected. It much harder to get the team on the same page as you when changes a made. The power of spending time together as a team and telling stories. Final Tips Explore the different options and pathways available to you when you graduate. Work out what you like and works for you; this will keep you in the profession. There are still a few too many podiatrists that don’t look outside the box, and they make the profession boring and not that great for new graduates to work with. You need to think outside the box and make your work interesting and expose yourself to as many areas as you can in podiatry so you can think laterally. Today’s Guest After two and a half years of interviews and solo shows on this podcast, I'm super excited to introduce a new format. You may know Jo Casey from previous episodes: I interviewed her for episodes 21 and 121 (no, we didn't plan the numbering there...but I like it!), and Jo interviewed me for episodes 31 and 115. Today's guest joining us on the Join Up Dots free podcast is a lady who like so many of us decided that she wanted something in life, and then found out that she hated it once it was achieved. As she says in her own words “My lifelong goal was to be a manager. I hit the corporate fast track and became a manager in record time – and found out I hated it! The long hours, corporate politics and endless budget meetings drained my energy and enthusiasm for life. What I really wanted, I decided, was a life where I could make my husband and kids my top priority – so I quit the corporate world to run my own business. I built and ran several businesses but something was still missing.” That statement is so interesting to me on so many levels, which we will discuss on the show. But our guest worked on living a life with status and prestige, but then changed direction. So she became an entrepreneur which is the direct opposite to where she was and still it wasn't right. These decisions seem to be directed by her head, or her survival instincts, yet once she listened to her heart, and really listened to what the old blood pumper was telling her then it all came together for her. And now she works with clients all over the world in such wonderful places as Holland, Belgium, Australia, U.K., Hong Kong and more! She has coached and mentored hundreds of entrepreneurs helping them create 6 figure businesses and beyond. Even achieving six figures in one day. So does she look back at her career in corporate land and thank the gods that even though she was climbing up the wrong ladder, it still taught her the skills to build her own ladder and start climbing again? And how does she balance this increased success with also being a Mum and Wife. Could she see a time when once again the ladder turns out to be wrong?
