Nous sommes vers l'an 965. Thorvald Ásvaldson, qui s'est rendu coupable d'un crime terrible, est banni de Norvège. Il prend alors les chemins de l'exil en passant par les côtes sauvages de l'Islande avec son fils adolescent, Erik, surnommé le Rouge. Quelques années plus tard, Erik, à son tour, fait couler le sang et se retrouve proscrit, condamné à errer sur les mers. Mais des rumeurs de terres mystérieuses à l'Ouest lui sont parvenues aux oreilles : Erik s'enfonce alors dans le brouillard et les glaces pour découvrir une contrée qu'il baptise Groenland (« Terre verte »), nous verrons bien les raisons stratégiques de ce nom. C'est dans ce sillage qu'en 985, un marchand nommé Bjarni Herjólfsson s'égare lors d'une tempête et devient le premier Européen à apercevoir, sans y débarquer, les côtes boisées d'un continent inconnu. Quinze ans plus tard, le fils d'Erik le Rouge, Leif l'Heureux, décide de transformer la découverte visuelle de Bjarni en réalité tangible. Il achète le navire du marchand et navigue vers ces rivages mystérieux, découvrant des terres qu'il nomme Helluland et Markland, avant de poser le pied au Vinland, le « pays du Vin », un véritable paradis où la vigne pousse spontanément. L'épopée vire pourtant au cauchemar lorsque les colons se heurtent aux autochtones, les affrontements sont sanglants. On April 18th, hundreds of tenants gathered inside a school cafeteria to help shape Toronto's first city-wide tenant's union – the TTU. There were elections, robust policy discussions and plenty of stories from tenants. Blueprints of Disruption was there to capture it all, and provide analysis afterwards.On-scene interviews from inside the Toronto Tenant Union's founding Convention start off with Ricardo Tranjan, housing policy expert and author of The Tenant Class. Tranjan talks about his expectations for the day, and his excitement at the prospect of “taking another step” towards building the tenant class consciousness he wrote about in his book.There are also interviews with tenant organizers from different buildings across the city – including Jerry and Steve from 240 Markland. They talk about their saga with an REIT buying their building, applying for five consecutive above-guideline rent increases (AGIs) and trying to push legacy tenants out of the building.We also interview Bridgette, a resident of the Caseway building – outside of which convention participants rallied after the day's more official proceedings. Episode 220: Approach of Insomnia in Adults     ARREAZA: Today we are going to talk about how to approach sleep issues in adults — from the initial assessment all the way through treatment. And I think what is going to surprise a lot of our listeners is the changes in management in recent years, especially what is recommended as first-line therapy. So, let's jump right in. MOIRA: Sleep is one of those topics that touches every specialty, but Primary Care in particular, so I'm excited to dig into this. ARREAZA: Let's start with the big picture–statistics. How common are sleep problems in adults? MOIRA: Incredibly common. Chronic insomnia affects roughly 10% of the general population, and that number has only grown in recent years . During the COVID-19 pandemic, for instance, prevalence rates of insomnia symptoms were reported globally at 20 to 45% (wow). And, importantly, those sleep problems did not simply resolve once infection rates dropped, insomnia symptoms and fatigue have continued even as mood improves in people recovering from COVID-19 infection.  ARREAZA: Incredible that we are in 2026 and still talking about COVID-19. And we clinicians need to understand that insomnia isn't just an annoyance. It has long-term consequences. Also, financially, insomnia causes direct and indirect costs of up to $100 billion each year. MOIRA: Exactly. Insomnia is both a risk factor for, and a symptom of, several psychiatric disorders, and it is a predictor of death by suicide, making it an important target for intervention. It's highly comorbid with medical and psychiatric disorders and is associated with significantly increased healthcare utilization and costs. People with insomnia also perform more poorly on complex cognitive tasks. So, we're talking about a condition that affects cognition, mental health, physical health, and quality of life. ARREAZA: And yet, it still gets overlooked in many clinical encounters. Let's be honest, dealing with insomnia is not easy on patients… and doctors! MOIRA: That's the paradox. Primary care practitioners are often poorly informed about sleep disorders, which remain underdiagnosed and sub-optimally managed. In one Italian epidemiological survey, insomnia was reported by 64% of over 3,000 patients interviewed under general practitioners, with 20% reporting both nighttime and daytime symptoms. So, the patients are there, we're just not always asking the right questions or knowing what to do when they tell us about their sleep. ARREAZA: Great. Let's talk about assessment. In my experience, we need a full encounter to address sleeping issues. Patients tend to mention insomnia as you start walking out of the room. Let's say a patient tells us, "Doctor, I can't sleep," how de we approach this? MOIRA: The first step is a comprehensive sleep and health history. Clinical assessment should describe the sleep disturbance and elicit etiological and exacerbating factors. You want to understand the nature of the complaint; is it difficulty to fall asleep, difficulty staying asleep, early morning awakening, or some combination? How long has it been going on? What's the impact on daytime functioning? ARREAZA: That's why I think it should be addressed in a full encounter, if possible, because understanding the full extent of the problem requires time. We need to think about contributing factors too. MOIRA: Absolutely. Factors such as medications, medical disorders, and psychiatric disorders can all increase the risk for insomnia. You need to screen for comorbid conditions, depression, anxiety, PTSD, and chronic pain. Insomnia is actually both a risk factor for and a symptom of several psychiatric disorders. You also want to rule out other primary sleep disorders. Comorbid insomnia and sleep apnea, for example, is highly prevalent and debilitating. If someone has both insomnia and obstructive sleep apnea, treating only one without addressing the other may lead to suboptimal outcomes. ARREAZA: Now that you mention comorbid conditions, let's mention nocturia. I feel like it's very common with my older patients. MOIRA: Great point. Nocturia (waking from sleep at night to void) and chronic insomnia frequently co-exist in older adults, contributing synergistically to sleep disturbance. Treatments typically target either nocturia or insomnia rather than simultaneously addressing the shared mechanisms for these disorders. There's emerging work on integrated cognitive-behavioral treatment programs that address both conditions simultaneously, which is a promising direction. Vikingarnas resor till Nordamerika blev den sista länken i en nordatlantiska kedja av nordbornas bosättningar. Norge, Island och Grönland bands ihop via havet i en väldig kedja. När Erik Röde år 985 grundade den grönländska kolonin skapades en utgångspunkt för vidare färder västerut.Vinlandsresor var först nordiska sagor, för att sedan 1960-talet ta en allt tydligare form med hjälp av nya arkeologiska fynd och innovativa mättekniker. Nu kan vi datera ett av vikingarnas besök på New Foundland till år 1021 – 471 år före Columbus.I detta avsnitt av podden Historia Nu samtalar programledaren Urban Lindstedt med Fredrik Charpentier Ljungkvist, professor i historia, med särskild inriktning på historiska geografi vid Stockholms universitet, om nordbornas närvaro i Nordamerika.Vinlandsresorna är en berättelse om logistik, resurser och risk: små samhällen i Grönlands fjordar som behövde timmer, järn och prestigevaror – och som till slut pressades av klimat, ekonomi och avstånd.Grönlänningasagan, berättar om en rad expeditioner, där Bjarni Herjólfsson obeserverar det amerikanska fastlandet första gången år 986 efter att ha drivit ur kurs under en resa från Island till Grönland. Hi, this is Heidi Markland with Ancient Spring Acupuncture in beautiful Coeur d'Alene, Idaho. Today I'm gonna talk about a question that I often get asked when someone first finds out that I'm an acupuncturist. So, while most people are familiar with the idea of acupuncture treating pain, many people ask what if I feel pretty healthy overall, why should I try acupuncture? First, if we look at the historical Chinese texts about acupuncture, we know that it was originally used as a preventative medicine. Dating back to 3 rd century B.C., the Yellow Emperor's Classic speaks of treating disease by preventing illness before it even begins. 1 Chinese medicine is about keeping the body in balance. A good practitioner does this by observing a patient. So, imbalances can be seen in inspecting facial complexion, body type, fingernails, tongue appearance, and feeling the patient's pulse, among other things that we can observe. Let's look at an example of someone with very few to no health complaints. She's an active 56-year-old that seeks acupuncture because she wants to be able to stay as active as possible. In looking at her fingernails, one may notice some dryness and brittleness. Her complexion may show some redness in the cheeks and the tongue may be slightly redder than average. Along with some patient questioning, we may want to start acupuncture treatment focused on nourishing the yin. This will help prevent further imbalance and because the patient wants to stay active, we would also add points that support the sinews and muscles. Furthermore, the patient wants to make sure that her thinking remains sharp so we can add points to actually boost brain and marrow. Maybe the patient mentions that she feels her eyesight is beginning to lose its sharpness. We can add points for that! We can add points to help clear the eyes and strengthen them. If you're looking for more scientific reasoning……well, research shows that the insertion of acupuncture needles can create effects due to modulation of the somato-sensory system and of the autonomic nervous system. Have I lost you? Basically, this means that science proves that acupuncture has a whole-body effect by tapping into the nervous system. Ear acupuncture, specifically, has been shown to stimulate the vagal nerve, which is responsible for the regulation of internal organ functions. And if these reactions weren't enough, it's proven that the body releases feel good endorphins with the insertion of acupuncture needles. So, at the very least, you should leave an acupuncture session with a sense of well-being and stress reduction. [2,3] Do you have more acupuncture related questions?
