In this 2026 edition of Experiencing Healthcare, we open the year with a deceptively simple leadership question: who's the hardest person to lead? The answer isn't a teammate—it's yourself. Through humor, real-world CEO moments, and hard-earned reflection, Matt unpacks why self-leadership is often overlooked, how boundaries are actually discipline in disguise, and why emotional regulation is the foundation for every decision you make—especially in a “heavy” industry like healthcare. The takeaway: if you want to lead others well this year, start by leading you with intention.

This episode flips the script as producer Jimmie Markham interviews host Dan Karnuta about the year's biggest healthcare themes. They revisit discussions form previous episodes on AI governance, operational adoption, workforce resistance, healthcare education pipelines, insurance pitfalls, nursing workflow redesign and physician well-being. Karnuta emphasizes that rapid technological change demands strong communication, thoughtful change management and a workforce prepared to partner with AI. They also explore deeper systemic issues, including healthcare's "klugeocracy" — a system built on decades of quick fixes. Despite the complexity and inefficiency this creates, Karnuta ends on a hopeful note: the industry is talking openly about its challenges, experimenting with new solutions and developing leaders committed to improving the system. Karnuta is an associate professor in the Naveen Jindal School of Management's Organizations, Strategy and International Management Area as well as director of its Professional Program in Healthcare Management.

They discuss how the U.S. healthcare system fails to support physicians' well-being, both mentally and spiritually. Taylor describes how doctors struggle with the stress, isolation and burnout of leadership expectations although they do not get trained in that aspect of their careers. They also discuss how the U.S. healthcare system prioritizes profit over true health, creating a structure where sickness — not wellness — drives revenue, leaving doctors feeling voiceless and disconnected from their calling. Toxic Exposure: The Monsanto Roundup Trials, and the Search for Justice," reveals the dark side of the world's most widely used herbicide. Jessica Aldridge interviewed Dr. Chadi Nabhan in 2023, who offered his expert insights on the link between glyphosate and cancer, the landmark legal battles against Monsanto, and the ongoing struggle for environmental justice. Tune in for a compelling narrative that exposes the failures of regulatory agencies and the courage of individuals standing up to agrochemical giants. For years, Monsanto declared that their product Roundup, the world's most widely used weed killer, was safe. But in 2015, scientific studies concluded that glyphosate, the active ingredient in Roundup, is probably carcinogenic. The Environmental Protection Agency (EPA) and Monsanto disagreed with the findings, as scientists worked to understand the link between glyphosate and cancer. Dr. Chadi Nabhan's book, Toxic Exposure [https://chadinabhan.com/mybooks/], tells the true story of his role as an expert physician witness who testified in multiple state and federal trials against Monsanto. His book recounts the heartbreaking stories of numerous patients who developed the cancer non-Hodgkin lymphoma, after regularly using Roundup on yards and school grounds. Monsanto is now owned by Bayer, one of the largest agrochemical companies in the world. These companies and the EPA downplayed the health dangers of Roundup and the active ingredient glyphosate even after Monsanto lost numerous court cases (owing billions in judgements) and settled out of court for more than $11 Billion for more than 100K patients. In this interview we discuss the history of Roundup, the dangers of glyphosate, the trial stories and verdicts, and what the everyday person can do to fight for justice against this agricultural behemoth. Dr. Chadi Nabhan is an expert in lymphoid malignancies and treating and diagnosing cancers. He is author of Toxic Exposure: The True Story behind the Monsanto Trials and the Search for Justice [http://www.chadinabhan.com]. He received his medical degree from Damascus University in Syria. After performing basic science research at Massachusetts General Hospital/Harvard Medical School, he completed his internal medicine residency as well as an MBA in Healthcare Management at Loyola University in Chicago. Dr. Nabhan maintains active medical licenses in five states, and has over 300 peer-reviewed articles and abstracts. He is also a sought-after speaker, moderator, facilitator, and the creator and host of his own podcast, "Healthcare Unfiltered" [https://www.youtube.com/channel/UCjiJPTpIJdIiukcq0UaMFsA]. Welcome to another episode of the Sustainable Clinical Medicine Podcast! In this episode, Dr. Sarah Smith sits down with Dr. Jeff Cohn, a retired hematologist-oncologist whose journey took him from the patient bedside to the halls of hospital leadership and, ultimately, to coaching fellow physicians. Dr. Cohn shares deeply moving stories from his clinical days—like orchestrating a final, meaningful anniversary for two dying patients—as well as the challenges and motivations that led him to transition into healthcare quality improvement. Through engaging anecdotes, Dr. Cohn reveals how he championed transformative approaches to organizational change, particularly the innovative use of “positive deviance” to tackle MRSA infections in the hospital. He explains how listening to frontline healthcare workers and drawing out internal best practices led to remarkable reductions in hospital-acquired infections—by 85%! Along the way, you'll learn about practical facilitative techniques, such as “liberating structures,” that empower teams and ensure every voice is heard. The episode also delves into the realities of clinician burnout, the complexities of driving system-wide change, and Dr. Cohn's current passion: coaching physicians in positive intelligence and mental fitness. Whether you're a healthcare professional or simply interested in sustainable, people-centered medicine, this conversation offers inspiring perspectives and actionable insights on making systems—and ourselves—work better. Here are 3 key takeaways from this episode: Harnessing Frontline Wisdom: Dr. Cohn described the transformative power of inviting frontline staff to identify solutions—a process called “positive deviance.” By genuinely asking nurses, transporters, and other care team members for their ideas (and truly listening), his organization achieved an 85% reduction in MRSA infections, all through internally sourced solutions. Liberating Structures Make Meetings Matter: Traditional hospital meetings too often drain energy and stifle creativity. Dr. Cohn's team used “liberating structures”—simple yet structured facilitation tools—to ensure every voice was heard, which sparked engagement and uncovered innovative solutions. Prioritizing Wellbeing for Sustainable Change: Through his coaching work, Dr. Cohn now helps physicians leverage mental fitness and positive psychology. He emphasizes that systemic change should support—not overwhelm—the clinical workforce, and that even within challenging systems, there are individuals and habits from which we can all learn. Meet Dr. Jeff Cohn: I'm a native Philadelphian, trained as a hematologist/medical oncologist. I practiced/taught/did clinical research full-time for the first 15 years of my career. I then was asked to assume various non-clinical roles (division head, interim Chair of Internal Medicine, Chief Quality Officer). The Chief Quality Officer role ultimately became a full-time role and I held that position for 12 years. Since then I've led a non-profit organization for three years (Plexus Institute) and worked with a couple of design teams as the medical director. I'm now working as a Mental Fitness and Leadership coach with physicians. Along the way I obtained a Masters in Healthcare Management from Harvard School of Public Health. ADHD and other brain-based conditions such as depression, autism, and even schizophrenia are increasingly understood as inflammatory disorders of the brain—deeply rooted in the complex interplay between genetics, environment, and lifestyle. Many individuals go undiagnosed for years, internalizing harmful labels like “lazy” or “difficult,” until the weight of unaddressed symptoms finally becomes too much. But there is hope: Functional Medicine offers a radically different approach by uncovering and addressing root causes such as nutritional deficiencies, gut microbiome imbalances, food sensitivities, toxin exposures, and lifestyle factors. Stories of transformation emerge when these root drivers are corrected—children once labeled disruptive begin to thrive, adults regain focus and emotional balance, and families find relief. This emerging paradigm offers not just symptom management but the possibility of genuine healing through personalized, whole-body care. In this episode, I discuss, along with Dr. George Papanicolaou and Dr. Jaquel Patterson, how ADHD and other mental health challenges are often rooted in inflammation and imbalances in the body—and how addressing diet, lifestyle, and root causes can lead to real healing. Dr. Papanicolaou is a graduate of the Philadelphia College of Osteopathic Medicine and is Board Certified in Family Medicine from Abington Memorial Hospital. Over time as the healthcare system made it harder for patients to receive personal care, Dr. Papanicolaou decided a change was needed. He began training in Functional Medicine through the Institute of Functional Medicine. In 2015, he established Cornerstone Personal Health—a practice dedicated entirely to Functional Medicine. In August 2017, I invited Dr. Papanicolaou to join The UltraWellness Center and we've been successfully helping people together ever since. Dr. Jaquel Patterson is a nationally recognized naturopathic physician, success coach, three-time Amazon bestselling author, and Forbes contributor. She owns a successful medical practice, Fairfield Family Health in Connecticut, and brings over 16 years of clinical experience, specializing in Lyme disease, autoimmune conditions, integrative psychiatry, and environmental medicine. A sought-after speaker, Dr. Patterson has been featured in USA Today, New York Magazine, and Forbes, and on CBS, Fox, NBC, and ABC. She's the past president of the American Association of Naturopathic Physicians and currently serves on the board of the American Academy of Environmental Medicine. Certified by the Institute for Functional Medicine (IFM), she's also completed a fellowship in functional medicine with a focus on psychiatry and ADD/ADHD. Plans like healthcare sharing ministries, limited benefit plans and short-term insurance can appear similar to conventional healthcare insurance but they lack essential consumer protections like coverage for pre-existing conditions, essential health benefits, and balance-billing safeguards. At age 39, Francesca Montillo had a thriving healthcare career at a prestigious Boston hospital—but her heart was in Italy, dreaming of pasta, wine, and sun-soaked adventures. If you've ever wondered what might be waiting on the other side of “what if,” this conversation is for you.Guest Bio Francesca Montillo grew up in Italy until the age of 11, upon which time she moved to Boston with her family. She spent every summer after the move back in Calabria, enjoying time at the beach, local markets and spending time with childhood friends. It was during her trips back that her love for Italy grew even more intensely. Her initial career ambitions were to work in law enforcement and she holds a Bachelor's degree in Criminology and Law. After a short stint in the field, she went on to earn a Masters in Healthcare Management instead. After a successful 22+ year career in healthcare management for both private and governmental sectors, Francesca reinvented herself again and took the leap and started her own business, combining her love of Italian travels and cooking. Via her business, Lazy Italian Culinary Adventures, Francesca is able to showcase her native land and its cuisine.
