Cloud DX CEO Robert Kaul joined Steve Darling from Proactive to share exciting updates with Proactive regarding the renewal of Master Deployment Agreements with paramedic services in Ontario. These agreements, spanning 12 months with an Ontario County Paramedic Service and 24 months with the Guelph Wellington - City of Guelph Paramedic Service, signify the continued partnership between Cloud DX and these healthcare providers. In Ontario, Community Paramedic Services leverage Cloud DX's Connected Health Remote Patient Monitoring (RPM) services to offer daily monitoring of vital signs, physical symptoms, and chronic conditions to seniors. Through the Cloud DX mobile app, patients can interact with paramedics, facilitating close oversight and empowering patients to manage their own care effectively. Of particular note is the Community Paramedicine for Long-Term Care (CPLTC) program, fully funded by the Ontario provincial government to provide additional care for seniors before admission into long-term care facilities. The recent announcement of an additional $174 million CAD in funding to expand the CPLTC program from 2024 onwards underscores the importance and growth potential of this initiative. Technologist Jonathon Feit has often said that the greatest innovations serving Mobile Medicine (Fire & EMS) come from rural spaces. The most obviously reason, perhaps, is that agencies have fewer resources to begin with, and it takes longer for their neighbors to arrive on-scene, therefore they have no choice but to learn how to do more for themselves. This has always been the hallmark of those who live and work where the wild spirit of self-reliance abides. But there is something else, too: big skies, clean air, and the ability to better afford life and spend less time in traffic tend to attract those who need to be able to afford aspects of life that would be impossible where the norm is sitting in traffic going back-and-forth from long hours in an office: for example, the time to care for a child or parent who is seriously ill or disabled and need more regular attention. For this reason, while they have fewer available resources that most cities, countrysides tend to attract a higher concentration of people who themselves — or whose parents or children — are more likely to need emergency healthcare. That sets up a Catch-22 and an important risk of health inequity that rural Mobile Medical agencies navigate around the clock. Reuben Farnsworth, who spearheaded Community Paramedicine, ET3, and other specialty care programs at Colorado's Delta County Ambulance District — an agency that has won statewide honors, including as the first ambulance service to interoperate with the Quality Health Network health information exchange and local hospitals — will explain how a data — and economics-centric management style has powered his service's success, from clinical medicine, to education and policy development, to advancing legislative priorities with partners.

She has served in clinical roles as a Pharmacist-In-Charge for Target Corporation and within the Florida Department of Health in Manatee County. In 2016, Dr. Reinhartz established a Practice Site within Emergency Medical Services, and is the Consultant Pharmacist for the Manatee County Community Paramedicine Program. Within this role, Dr. Reinhartz and her pharmacy students resolve medication issues or disease factors which are contributing to frequent 911 use, ER visits, or hospitalization. For her work in EMS, Dr. Reinhartz has been recognized with a Chief’s Commendation Award and a Congressional EMS Unit Citation Award for exceptional care provision and leadership within EMS. Dr. Reinhartz has garnered national attention from the United States Public Health Service for innovation within EMS and Community Paramedicine. Most recently, she was named the 2020 Next Generation Pharmacist Civic Leader, which is awarded to one pharmacist nationwide whose vision for interprofessional care best spotlights the needs of underserved communities. This episode of the ESCN podcast features an insightful conversation with senior healthcare leaders to discuss an initiative aimed to optimizing transfers from Long-term Care Facilities to an Emergency Department. The ESCN is partnering with Seniors Health, RAAPID, Community Paramedicine, and Long-term Care (LTC) Facilities. The solution is a centralized and standardized LTC-ED care and referral pathway for LTC facilities considering transfer to ED. In conjunction with the implementation of this new pathway, targeted INTERACT® (Interventions to Reduce Acute Care Transfers) tools will be implemented within LTC. This new standardized LTC-ED care and referral pathway will help to better optimize use of both the emergency department and the community paramedicine program, in a way that is patient-centered. Studies have shown that the provision of community paramedicine within LTC can significantly reduce transfers to emergency. This initiative will be implemented within 36 LTC facilities in the Calgary Zone between August 2019 and March 2022. If the evaluation of this change demonstrates improvements to both LTC residents and the health care system outcomes, it will be spread to other zones within the province, starting with the Central zone. We had an opportunity to sit down with Mel House, the Executive Director of the Ohio Dept. of EMS. Director House wanted to share with our listeners what their responsibilities are and how they go about fulfilling those responsibilites. Director House also took time to give some facts about certifications, the recertification process, and to also dispelled some rumors. We also took this opportunity to discuss the new legistlation passed by the Ohio House of Representatives that allows paramedics to practice in "non-emergency" situations. This new legislation opens the door for community paramedicine in Ohio. Check out the "ORC 4765.361 Information Sheet" released by the Ohio Department of EMS for more information.
