1) Cosecha: Hace un tiempo escuché una historia que cambió el chip de mi pensar. La charla se llamaba “Las tres cosas que aprendí mientras se estrellaba mi avión”. Rick Elias iba en un avión con 200 personas. El avión hizo una maniobra extraña. Rick le preguntó a la azafata y ella lo calmó diciendo que era algo normal (habían perdido un motor). Al poco tiempo hace otro ruido y ahí el piloto anunció en alta voz “Prepárense para el impacto”. Rick ya sabía que se iba a morir y dice que en ese momento pensó tres cosas: 1) todo cambia en un instante. Parece que no nos damos cuenta pero es verdad y pensó en todo lo que quería hacer y no hizo, en toda la gente que quería haberle dicho que la quiere y no lo hizo. En ese momento aprendió que no hay que aplazar nada en la vida. Cada minuto de la vida hay que disfrutarlo porque vivimos de espaldas a la muerte y cada minuto puede ser el último. 2) pensó en la cantidad de tiempo que había perdido por culpa de su ego. Desperdiciar el tiempo en cosas que no importan con gente que sí importa. Hay veces que en vez de pelear por la razón es conveniente pelear por la felicidad. Entre tener razón y ser feliz, elige ser feliz. 3) morir no da miedo, dice Rick que es como que toda la vida estamos preparándonos para ello. Morir no da miedo, pero te sientes muy triste porque amas la vida. Finalmente sobrevivo Rick aquel día, ya que el piloto logró salvar a la mayoría de la tripulación (es el avión que aterrizó en el Río Hudson). Rick termina la charla diciendo que todos volamos hoy y que no sabemos si nuestro avión se estrellará esta noche. 2) Poder: Lo más difícil del mundo creo, es conocerse a uno mismo. Y lo más fácil es hablar mal de los demás. Descubrirse es lo más difícil, por eso debemos trabajar el ego y no dejar que nos gane. Por eso, aprende a descubrir que el mayor poder que puedes tener es aceptarte como eres, saber tus límites y comprender que necesitamos de la ayuda de Dios. 3) Vayan: La clave es saber que eres anunciador de buena noticia, que las personas pueden ser ayudadas con tu testimonio y que puedes lograr tocar vidas con tu simple sonrisa o con un simple escuchar. Por eso, evangelizar es mostrarte cercano y simple, sin tener que resolver la vida de los otros, sino más bien mostrando que se puede vivir a pesar de los golpes que te lastiman otros. Algo bueno está por venir.

What if a world without HIV wasn't just a dream — but a decision we could make right now? In this compelling episode of Viva La Vulva, Dr. Cara Quant picks up where the conversation left off, diving deep into PrEP — the once-a-day pill (or injectable option) that can reduce the risk of HIV transmission by up to 99%. Yet despite its proven effectiveness, PrEP remains one of the most underutilized prevention tools among the very communities that need it most. For Black women — who are 10 times more likely to receive an HIV diagnosis than their White counterparts — the gap between what's possible and what's accessible is not a medical failure. Il y a une semaine, l'Afrique du Sud lançait officiellement le lénacapavir, un antirétroviral utilisé dans le traitement de l'infection par le VIH, jugé révolutionnaire. Avec une seule injection tous les six mois, ce médicament préventif atteint une efficacité qui frôle les 100%. « Un signe d'espoir » s'est réjoui Cyril Ramaphosa, président du pays le plus touché au monde par le VIH, avec plus d'un Sud-Africain sur dix porteurs du virus. Un signe d'espoir également pour toute l'Afrique, qui concentrait, en 2024, 65% des personnes vivant avec le VIH dans le monde, selon les données d'Onusida. La Camerounaise Ndong Essomba Bitchoka, infectiologue à l'unité de prise en charge des patients VIH/SIDA à l'hôpital de Deido, à Douala explique ce que ce traitement peut changer. RFI : Les Sud-Africains porteurs du VIH peuvent désormais bénéficier du nouveau traitement Lenacapavir. Pourriez-vous expliquer comment fonctionne ce traitement ? Dr Ndong Essomba Bitchoka : Le Lenacapavir est un médicament antirétroviral. Il va empêcher l'assemblage des molécules ARN du virus du VIH, donc empêcher la formation de nouveaux virus du VIH. C'est à peu près comme ça qu'il fonctionne. Pour simplifier un peu, quelle est la grande avancée du Lenacapavir par rapport à la PrEP, le traitement le plus couramment utilisé ? La PrEP est un médicament qu'on va administrer aux personnes non-infectées au VIH pour leur éviter de le contracter. Il est beaucoup plus indiqué pour les personnes qui sont plus à risque de développer le VIH. Le Lenacapavir, par contre, peut être utilisé dans le traitement de l'infection au VIH pour contrôler le virus chez certaines personnes. C'est-à-dire qu'au lieu de prendre des comprimés tous les jours par exemple, vous allez avoir des injections qui vont permettre de limiter la réplication du virus dans votre organisme pendant environ six mois. Donc la PrEP est destinée à titre préventif aux personnes non-porteuses du VIH, tandis que le Lenacapavir peut aussi être un traitement pour les personnes porteuses. Et sa grande différence est qu'au lieu de comprimés, on donne des injections deux fois par an… Exactement ! La personne doit déjà au préalable être contrôlée avec des médicaments oraux, avec des comprimés. Puis le Lenacapavir vient appuyer l'action des antirétroviraux qui étaient déjà présents à la base, qui contrôlaient le virus. Sauf que ce même contrôle va se faire sur une longue durée d'action associée à d'autres médicaments. Donc le Lenacapavir pour une personne déjà porteuse du VIH, il faut déjà qu'il y ait un traitement antérieur au Lenacapavir ? C'est cela. Si on a un patient qui est dépisté séropositif au VIH aujourd'hui, le Lenacapavir ne sera pas la première intention qu'on pourra lui proposer en termes de traitement. Hormis l'Afrique du Sud qui cumule le plus grand nombre de personnes atteintes du VIH dans le monde (8 millions de Sud-Africains, NDLR), une grande partie des infectés vivent en Afrique. Vous avez une estimation du coût du traitement ? Comme je n'ai pas le coût exact, je ne pourrai pas vous donner une réponse exacte. Mais dans la majorité des cas, si je parle par exemple de mon pays, le Cameroun, le Lenacapavir n'est pas encore accessible par exemple. Mais pour l'instant, les États avec les partenaires financiers font le nécessaire pour que les médicaments soient subventionnés. Du coup, les patients ne les paient pas. Dans le cas de l'Afrique du Sud, je ne sais pas encore si ce sera le cas ou pas, mais je pense que les États se battent pour voir dans quelle mesure subventionner, ne serait-ce qu'en partie, l'accès à ces médicaments antirétroviraux. Pour l'instant, seules 40 000 doses ont été mises à disposition en Afrique du Sud. Le président Cyril Ramaphosa annonce la fourniture d'un million de doses d'ici fin 2027. Sauf que l'organisation Health Gap estime qu'il en faudrait le double de ces doses chaque année pour répondre aux besoins du pays. Au-delà du cas sud-africain, une pénurie plongerait-elle les patients bénéficiaires dans un nouveau désarroi ? Quelles seraient les conséquences s'ils étaient forcés de suspendre le traitement ? C'est bien de pouvoir commencer avec un certain nombre de doses et de sélectionner les patients qui sont vraiment nécessiteux par rapport à ce traitement au Lenacapavir. En fonction de comment la réponse à ce traitement se fera chez ces patients, on pourrait envisager d'augmenter le nombre de doses disponibles et même d'augmenter la population qui pourrait y avoir accès. Le risque, effectivement, c'est qu'en disponibilisant beaucoup de doses de médicaments et qu'après l'Etat ne puisse plus s'en procurer, ça va effectivement créer une rupture dans le traitement et la survenue de résistance à cet antirétroviral, qui est plutôt salutaire pour la prise en charge des patients. On rappelle que le Lenacapavir n'est pas un vaccin. Pourquoi y a-t-il plus de recherches sur le traitement plutôt que sur un vaccin ? Est-ce si compliqué à élaborer ? Le VIH est un virus qui est un petit peu sournois, dans le sens où il a beaucoup de mutations. Imaginez un cambrioleur qui visite différentes maisons, mais avec un mode opératoire différent. Il est identifié comme mesurant 1m80 quand il frappe dans la maison A, mais quand il va dans la maison B, il mesure plutôt 1m50. Ce qui lui permet d'entrer dans certaines ouvertures inaccessible pour un cambrioleur de 1m80. Il y a beaucoup de mutations. On n'arrive pas encore à identifier un vaccin qui pourrait bloquer l'action de ces différentes souches mutantes du VIH. C'est ce qui rend la chose difficile pour trouver un vaccin. Mais il y a quand même des essais qui donnent un petit peu espoir. Notamment l'essai BRILLIANT 011 en cours justement en Afrique du Sud, pour lequel on espère avoir de bons résultats d'ici quelques années.
