In this episode, I sit down with Dr. Jillian Spencer, a child and adolescent psychiatrist from Brisbane, Australia, who has found herself at the center of a heated debate surrounding so-called “gender-affirming care” for children. After raising concerns about the practices at her hospital's gender clinic, Jillian was suspended and is now embroiled in legal battles that could redefine workplace protections for those with gender-critical beliefs. What led her to take such a bold stand, and what are the implications for mental health care?We explore the concepts in a not-yet-published paper Jillian recently co-authored with Dr. Roberto D'Angelo, analyzing the interpersonal dynamics between gender clinicians and their patients using psychodynamic theory. How do psychological dynamics such as repression, collusion, codependency, projection, and reaction formation play into relationships between distressed, vulnerable patients and the professionals who “affirm” them? And what are the risks involved in psychiatry's shift away from psychodynamic thinking?We analyze the alarming trend of fast-tracking youth with suicidal ideation to gender clinics and the dangerous message this sends to vulnerable adolescents, potentially incentivizing personality disordered behavior and risking permanently entrenching vulnerable people into problematic problems for life that they could have grown out of in adolescence if only provided with the proper support.Jillian Spencer is a child and adolescent psychiatrist in Brisbane, Australia. In April 2023, she was stood down from her job at the Queensland Children's Hospital for raising concerns about gender interventions for children. Since that time, she has spoken out in the Australian media about her concerns which has led to further allegations of breaching the employee Code of Conduct. She remains suspended from her job. Jillian has launched legal action claiming political discrimination in the workplace due to her belief in biological reality, that people can't change sex, in the hope of achieving a Forstater decision in Australia. This court case will occur in early 2025. She is also awaiting an employment commission outcome as to whether she is officially a ‘whistleblower'. It often means watching trusted people in positions of authority turn into bullies and, worse, watching friends and colleagues turn into cowards.That dark reality makes it all the more important to understand those people who do the right thing and stand up for what's right. One of them is James Esses, a British attorney in his early thirties who was kicked out of a training school for therapists for raising concerns about the medical mistreatment of children confused about their gender.As far as cancelations go, Esses's wasn't particularly dramatic or noteworthy. He wasn't a famous actor, musician, or writer. He was just someone who, early in his career, decided he didn't want to be a lawyer anymore and instead wanted to help people with mental health problems.Given that protecting the institutions of civilization requires more ordinary people, without the resources of famous artists and authors, to stand up against bullies, we should seek to understand why they do it so that we might encourage more of it.Esses' journey began in 2020 when he was in his third and final year of getting his therapist's degree from Metanoia Institute and volunteering for a charity to staff a mental health hotline.“I was on the cusp of setting up my own private practice,” he says. “I had children coming through on this helpline saying they were trapped in the wrong bodies and that they wanted to use breast binders and take puberty blockers. They were younger and younger.”The charity told Esses “to kind of just affirm” the pseudoscientific and dehumanizing idea that some children are born into the wrong bodies.“Many had come across this stuff online,” he said. “Many of them were being taught it in school. Children have been taught from a very young age that it's possible to be born in the wrong body and that you can essentially change your sex.”Esses started reading about children being medicalized and given drugs and surgeries. “I couldn't believe what I was reading. We were damaging, irreparably, children in the name of an ideology that isn't founded in evidence or fact. I couldn't believe it."“The message from the training institutions and our regulatory bodies as therapists was, essentially, affirm,” Esses explained. “Don't explore. Don't challenge. Affirm transitioning, no matter what. And to me, that flew in the face of proper therapeutic ethics and the Hippocratic oath. So I couldn't simply abide by that. I felt compelled to start speaking out about it.”Esses cofounded with some colleagues a new group, Thoughtful Therapists. “I wrote a petition to the UK government,” he said. “I started engaging on social media for the first time about this, doing some interviews, and writing some articles. And then, out of the blue, one day in May, I received an email from my institution telling me that they were expelling me with immediate effect.”Esses says the experience was humiliating. “It was a two-paragraph email that simply said that there had been some complaints about my writing and my advocacy and that I had brought them into disrepute, and so they were expelling me with immediate effect.“They blocked my email and my access to the university Intranet portal,” he said. “And they had, on Twitter, publicized the fact that they had expelled me.”Esses was shattered. “I was in an awful state. In a single email, my entire future life plans went up in smoke. I hadn't done anything wrong. All I had done was raise concerns essentially about child safeguarding.”Esses had done the right thing and was now paying a heavy price. “For the first for the first couple of days, I didn't want to get out of bed. You know, I was really that low.”

In this week's episode, I am joined by Licensed Marriage and Family Therapist Christine Sefein to discuss her experiences in higher education that led her to a deep questioning of critical social justice and its anti-human sentiment.Christine is a Licensed Marriage and Family Therapist specializing in grief and trauma work. Two years ago, Amy Gallagher was told she was unfit to engage with her patients since she disagrees with Orwellian principles foundational to woke ideology that view white people as the “problem of our time.” Today, I am joined by Amy Gallagher to discuss the shortcomings of this subtly vicious ideology with inherently detrimental impacts on psychotherapy as a profession along with the unparalleled negative outcomes for patients who are subjected to it. Amy Gallagher is a Mental Health Nurse who has been working in the UK National Health Service (NHS) for seven years. She has worked with adolescents and adults with a range of issues such as eating disorders, personality disorders, anxiety, depression, and psychosis. Amy has a postgraduate diploma in psychodynamic psychotherapy and is currently suing the NHS for suspending the final part of her training to become a psychotherapist because she disagreed with Critical Race Theory. Amy argues that psychotherapy as a profession has been hijacked by an ideologically-driven oppression of viewpoint divergence, threatening the professional development of therapists, and as in Amy's case, open discrimination against Christian counselors and their patients. Amy sheds light on how the profession of psychotherapy suffers as a result of ideological principles lacking in an evidence base being taught as fact, and we discuss the negative effects and experiences patients endure as a result. Amy's case presents a unique point in time where people of certain demographics are discriminated against by progressive puritanicals who believe that simplistic and divisive narratives are the only answer to the complexities of the human condition. Half of our listeners are in the US and more than a quarter are in the UK, while the news on gender matters is rapidly evolving in both countries. So the time is ripe for a check-in with a gender critical therapist friend “across the pond,” James Esses. Last year, James was on track to become a therapist when his masters' program expelled him for expressing concerns about the impact of gender ideology and medicalization on youth. He is now taking the issue to court, and spends his free time further advocating for caution and common sense in the treatment of gender confused youth.In this collegial conversation, James and I touch on recent events in the UK — the Tavistock GIDS, Stonewall, Mermaids, Cass Report, Maya Forstater, Allison Bailey, and the Gender Recognition Act — as well as US news including the Biden Administration's destruction of Title IX. While attending school and volunteering for a national helpline, James was taken aback by the rise in gender dysphoria among children and the resulting therapy prescription that not only advocated but also pushed for gender transition as the only and best solution for children struggling through the confusion of puberty. James Esses, a former a Criminal Barrister and currently a Trainee Psychotherapist, discusses the current social dilemma at the heart of the gender identity debate where countries are banning “conversion therapy” while dismissing the beneficial uses of therapeutic approaches to gender dysphoria. Analysing the deeper issues central to the refusal to embrace anything but affirmation, Esses points out how it the proposed ban on conversion therapy in terms of gender dysphoria is “very dangerous” to the client-therapist relationship as he underscores the need to push against the stereotypes being recycled by the gender identity lobby asking “What is wrong with being gender atypical?” Claiming that we need to spend more time addressing gender stereotypes, Esses lays bare some of the fundamental dangers at the heart of a petition and he and other members of Thoughtful Therapists addressed to the British government, namely the conflation of “conversion therapy” with beneficial therapy which effectively mandates the affirmation of transitioning.
