American psychiatry has built a sociological armor around itself that protects it from reform. The armor has two parts. Reverence and complexity. Together they form the most effective institutional defense system in American professional life. And the apparatus, in 2026, has evolved its most refined defensive move yet, the DSM-6 roadmap, which absorbs the entire body of structural critique against the field by publishing thoughtful documents acknowledging the critique is correct, while channeling an entire generation of reform energy into bureaucratic processes that will conclude, eventually, with the publication of a new manual that incorporates the language of the critique without changing what the manual does. Why the apparatus persists despite forty years of evidence it is failing. How residency capture, modality capture, and credentialing capture work together to produce a workforce whose tolerance for the mystery of the work has been systematically lowered. What would have to change. And why none of the obvious answers are actually answers. This episode covers: Of Two Minds. Tanya Luhrmann's anthropology of American psychiatric residency. How young doctors who enter training wanting to think across biological and psychological registers get formed, by the reward structure of training itself, into single-register practitioners. The question of whether you are more scared of yourself than of not seeing life clearly. The wager that even if humans always refuse, professional psychology should stop being the most refined refusal in the culture. Even in university and placements, and my Master's, and now being a therapist, so it's been a long time in this field!' - Paula Gonzalez When explaining how Paula came into the world of therapy, and what encouraged her to pursue becoming a therapist and working in mental health, Paula recalls her family lineage and that she came from a long line of women who worked in various helping professions.  'My mom being a social worker, my grandmother was a teacher, you know, my aunt is a doctor, so it's always been a part of my life to be in the helping profession. For me, having gone through my own experiences, like immigrating to Canada at a young age … That experience was very difficult for me … That made it clear … that I really wanted to become the kind of person I wish I had had at that time to get me through that.' - Paula Gonzalez Starting a private practice  Paula completed her Master's degree in 2016, and then she started working in community mental health, where she stayed for four years.  While Paula loved the community aspect of working with people in her city, it was difficult to find a full-time job at the time, especially since virtual sessions were not yet popular or common.  But when COVID hit in 2020, Paula started thinking about opening up her own solo virtual private practice.  'I did start in community mental health, and then I worked at a group practice. As a new grad, very new territory for me. Eventually, I decided, "I miss community mental health!" So I decided to go back. But then COVID hit. What led me to open up my own … during COVID … I felt so called to doing my part in working with folks directly.' - Paula Gonzalez  And so, Paula launched her online private practice! She would see clients in the evenings or even on weekends while she continued working in community mental health. Scaling from solo to group private practice  'Every day I wake up, and I'm like, "How, I can't believe I get to do this!" What if a major driver of today’s mental health crisis isn’t simply “more disorders,” but more people who feel unseen, unheard, and alone? Jackie explains how the Soulwell Center began: while teaching counselor “helping skills,” she realized many of those relational tools could be taught in a lay-friendly way to parents, pastors, and everyday Christians. The result is a training approach that combines practical listening techniques with the neuroscience of relationships—equipping people to hold a safe space where others can feel truly “seen and known.” James and Jackie discuss a trend Jackie has observed across decades in the mental health field: in the last 10–15 years, more clients have been coming not primarily with severe pathology, but because they don’t have anyone who listens. Therapy becomes a paid place of connection—something that should not be rare in Christian community. The conversation explores how technology can create distance (including the rise of AI-mediated communication), why many people lack a “mental model” for deep listening, and how shame and perceived “threat” can make relational closeness feel unsafe. Jackie introduces the concept of “eyes of delight”—the nonverbal experience of being attended to with warmth—and explains why nonverbal presence often does more than words. They also connect listening to the broader formation of disciples: without embodied, relational connection, people drift into isolation, cope through substitutes, and struggle to develop distress tolerance—the ability to endure discomfort and stay engaged through conflict, hardship, and the messiness of real relationships. The result is not only loneliness, but fragility and retreat from vocation, mission, and spiritual maturity. On November 7, Commonwealth Club World Affairs of California, the nation's oldest and largest public affairs forum, will host The Asian American Foundation's (TAAF) first-ever AAPI Youth Mental Health Summit. Under the theme “Sparking Solutions Together,” the summit will convene hundreds of experts, advocates, funders, and business executives to address the urgent and often overlooked mental health challenges facing Asian American and Pacific Islander (AAPI) youth. From 2018 through 2022, suicide was the leading cause of death among Asian Americans aged 15–24, and the second leading cause of death among Native Hawaiians and Pacific Islanders. Yet despite being deeply impacted by the nation's mental health crisis, AAPI youth remain largely invisible in the national mental health conversation, and the data needed to understand their mental health is scarce at best. To fill the gap, TAAF released "Beyond the Surface" in December 2024, the most comprehensive study to date on AAPI youth mental health, which revealed: Nearly 1 in 2 AAPI youth screen positive for moderate depression; 1 in 3 have planned or attempted suicide; Stigma, family pressure, and silence keep many from seeking help; Only 53 percent feel comfortable talking with their parents; Just 1 in 4 have accessed formal care; and 46 percent have never seen a mental health provider.
