On this weeks show, Brock McLean joins the Under Pressure Podcast to take a look back at his career, injuries and difficult periods during that time.  Brock started off his career playing for the Dees and all was rolling really well his first few seasons. But in 2008, his first major came as he tore his Syndesmosis, which saw him have to wear a moon boot for the following 6 months.  Which then continued on into some further issues as he was injured, the self-worth and the doubt started to creep into his mind. He had to distract himself from these things, which lead him down the path he certainly didn't want to be down and not using the right coping strategies to help himself.  He talks through his thoughts he had during this difficult period in his life and gives great insights into what its like and what you can do when you're facing challenges like these yourself and know there are ways to help and assist.  We talk about the amazing premiership drought that was broken this year by the Dees and how connected you still feel to your club post career and how important it is to keep those relationships moving forward in life.  We finish off the podcast as we always do, with a draft. This time, we are especially excited as we are having two awesome guests with us, Benedikte Ræder and Mette Andersen. They are the first and senior authors of two of the most outstanding trials comparing suture button stabilization to single syndesmotic screw fixation in unstable syndesmotic injuries. One study is entitled “Randomized trial comparing suture button with single 3.5 mm syndesmotic screw for ankle syndesmosis injury: similar results at 2 years.” published in the Acta Orthopaedica and the other RCT is entitled “Better outcome for suture button compared with single syndesmotic screw for syndesmosis injury: five-year results of a randomized controlled trial”  published in the Bone and Joint Journal.  As indicated in the title of the two studies, they found conflicting results. And today we have the great privilege to discuss these with them.

Dr. Barg specializes in the care of the foot and ankle, as well as reconstruction of traumatic injuries to the foot, ankle replacement, and joint preserving procedures. Prior to coming to the University of Utah where Dr. Barg has set up a very impressive biomedical research lab, both in terms of size and the breadth of projects, he was the head of the Orthopedic Department at the University of Basel in Switzerland. Dr. Barg currently serves as a reviewer for numerous medical journals including Foot & Ankle International, Journal of Biomechanics, Clinical Anatomy, and BMC Musculoskeletal Disorders, and is well published in foot and ankle replacement. Currently, Dr. Barg is working with a variety of other researchers on a series of weight-bearing CT projects, including two cadaver research studies. On working with Dr. Arne Burssens on a templating method, Dr. Barg said: “we’re able to compare the healthy side versus the injury side and can detect very small differences in imaging using weight-bearing, which we’re not able to do using conventional radiographs and MRI.” Give this podcast a listen to hear Singh and Dr. Barg break down his recent presentation on evaluating syndesmosis, his discoveries on the effect that torque plays in syndesmosis measurement, and whether this findings could translate to imaging in the clinical setting.

Ankle syndesmosis injuries following forced ankle dorsiflexion and lateral rotation of the foot require very specific management and rehabilitation to achieve great treatment outcomes. If you approach management of ankle syndesmosis injuries in the same manner as a lateral ankle ligament injury it will likely result in long term pain and dysfunction for your patients.
