We welcome Foot & Ankle Specialist and Residency Director of Loyola University Medical Center, Dr. Adam Schiff, MD to discuss suture button versus screw fixation for ankle syndesmotic injuries.  We review the study “Comparison of Suture Button and Syndesmotic Screw for Ankle Syndesmotic Injuries: A Meta-Analysis of Randomized Controlled Trials“ published in OJSM in January coming to us from Southwest Hospital of Army Medical University in Chongqing, China.  In the study, Dr. Kang-Lai Tang and his team found no difference in terms of malreduction, however the suture button group had significantly lower rates of local irritation and unplanned reoperation.  Dr. Schiff lends his expertise on this topic, lays out the pros and cons of each approach, the historical perspective, and what situations might be best suited for each treatment construct.

Historically, results of direct ACL repair are poor. As such, it has been viewed as a substandard surgical option in comparison to ACL reconstruction, the modern gold-standard approach. However, ACL reconstruction is not without its flaws, and as technology and techniques have advanced, the idea of ACL repair has reemerged. Coming to us from Tripler Army Medical Center, Drs. Christian Cruz and Craig Bottoni share their results with ACL repair in their military population. Like studies from the past, the failure rate was high at 26%. However, those that succeeded had good to excellent results in an average return to full duty time that beats what is expected with ACL reconstruction. Drs. Cruz and Bottoni share their experience with this active population and why ACL repair may make sense in certain situations as well as when to avoid it.

Au sein de ce premier de deux (au moins) épisodes à propos de la diaspora vietnamienne, Eléonore Tran nous parle de recherche en histoire de l'art qu'elle a effectuée à propos des travaux de six artistes de cette diaspora: Bao Vuong, Prune Phi, Jacqueline Hoàng Nguyễn (qui avait participé au podcast de The Funambulist en 2019), Binh Danh, Quynh Lam et Dinh Q. Lê. Au sein d'un mémoire intitulé "Suture(s): Collecte et utilisation de photographies vernaculaires dans la pratique artistique du Vietnam et de sa diaspora" (2021), elle avait ainsi montré de quelle manière ces artistes mobilisent la mémoire (et son absence) par l'intermédiaire de la photographie (familiale et autre) afin de faire diaspora et questionner celle-ci. Eléonore Tran est doctorante en histoire de l'art contemporain à l'université Paris 8. Diplômée de Lettres modernes (Université Paris Cité) et d'Histoire de l'art (Paris 1 Panthéon Sorbonne) ses recherches mêlent à la fois étude artistique, sociologique et historique. Sa thèse porte sur les différentes manières dont la photographie sert de matériau de mémoire au sein de la diaspora vietnamienne. À travers l'analyse des œuvres d'artistes contemporains originaires du Vietnam, elle considère la photographie à la fois comme un fragment permettant de reconstituer le passé et comme un médium permettant d'établir de nouveaux récits de soi et de la communauté.

If you're like, my guest, Nabeel Cajee, then it was probably like, you know what, I got this, he had a background in embroidery Jaz's IntroductionBut for me, I was absolutely hopeless. And to be fair, I've improved a lot since the first days in dental school, but there's so much I still need to improve on, which is why I found you one of the best people to teach on sutures. This guy is brilliant. He's got a whole online portfolio of videos to watch. And it is something that's really helped me and I'm continuing through his course. And I want to bring him on to share with you some nuggets of inspiration to improve your suturing come Monday morning. We covered this episode. But no, there are two types of courses, there are courses that you do, which will ultimately, it may not be the main goal, but ultimately it will improve your return on investment, or it will give you a return on investment. So I know that if I start placing implants, and I got an implant year long course, it might cost me a lot of money on this one year long course. But I know that there'll be a return on investment, I can offer my patients implants. And that should reflect my bottom line. That shouldn't be the reason I do the course, ideally, but it will have an ROI. There are other courses that you do, and other things that we've purchased like microscopes for example, or suturing, for example that you do because you know that you can give the highest quality of your care to that patient. One of the things that Nabeel says that when you place a suture, that is your signature of the surgery. I love that saying, right? And to be fair, from my experiences with suturing, I can definitely do with a helping hand hence why I've come to this sort of episode, I think I can share this with everyone to help you all.
