In the presence of anterior glenoid bone loss (aGBL), options for bony glenoid augmentation include Latarjet procedures and free bone block transfers. Bone graft placement is challenging, and malposition causes complications, such as recurrent instability or osteoarthritis.   In conclusion, in the presence of aGBL, flush bone block placement restores osteochondral glenohumeral stability, while a 1-mm medialized bone block fails to increase stability. Pearls and Quotes - Obesity & Cardiovascular Disease Risk The durability of metabolically healthy obesity (i.e., normal A1c, lipids, LFTs, BMP, normotensive) is limited. Within 5 years, a third of adults with “metabolically healthy” obesity will develop a cardiometabolic complication. The biomechanical and psychosocial complications of obesity are just as important as the cardiometabolic complications. Biomechanical and psychosocial complications, including obstructive sleep apnea, joint pain, and mood disorders also influence cardiovascular disease risk. Weight loss is not always the patient's goal. Meet patients where they are and understand their challenges, concerns, and long-term goals with respect to their cardiovascular health and obesity. This information provides an opportunity to frame the conversation in a supportive and engaging way that allows for patient education. Body mass index (BMI) is a screening tool for obesity, but is not sufficient for providing individualized care. Obesity management methods that result in rapid weight loss may not be appropriate for all patients. These methods, such as bariatric surgery and GLP1-receptor agonists, require regular monitoring, follow-up, and multidisciplinary care (e.g., nutritionist, exercise physiologist, endocrinologist, cardiologist, psychologist, etc.). Show notes - Obesity & Cardiovascular Disease Risk Is it possible to be healthy at any size? Whether an individual can be healthy at any size depends on the definition of health and its durability.Approximately 10-15% of adults with obesity are metabolically healthy.The risk for developing cardiometabolic disease is higher in obese versus non-obese adults. One in three adults with metabolically healthy obesity will develop cardiometabolic complications (i.e., insulin resistance/diabetes, hyperlipidemia, hypertension) within five years. Thus, metabolically healthy obesity may represent a transient phenotype with adverse long-term consequences. Consider non-metabolic health consequences of obesity that also influence cardiovascular disease risk. Obstructive sleep apnea, joint pain leading to decreased physical activity, and mood disorders are key considerations here and encompass the biomechanical and psychosocial consequences of obesity. Does large, rapid weight loss result in poorer long-term weight loss than slower, gradual weight loss? When approaches to weight loss are not sustainable, such as extremely low-calorie diets or extreme fitness regimens, the results and associated health benefits are less likely to be durable. Lateral extra-articular tenodesis (LET) is being performed more frequently with anterior cruciate ligament (ACL) reconstruction (ACLR) to decrease graft failure rates. The posterior tibial slope (PTS) affects ACL graft failure rates. The effect of ACLR + LET on tibial motion and graft forces with increasing PTS has not been elucidated.   In conclusion, increased PTS was confirmed to increase graft forces linearly. Although ACLR + LET reduced graft force compared with ACLR alone, slope reduction had a larger effect across all testing conditions. No other clinically significant differences were noted between ACLR with versus without LET in regard to graft force, ATT, or IR.   To read the article, click here.

He works for the US Olympic and Paralympic Committee as a Sport Physiologist, and he conducts academic research with the University of Michigan. His work has focused on running biomechanics and performance. Geoff is a runner himself and competes internationally in ultramarathons. He was the 2016 USA national champion in the 100km and has finished 5th at the 100km World Championships, twice, in 2016 and 2018.  Dustin Joubert has a PhD in Exercise Physiology from Texas A&M University (2015). He was a faculty member in the Department of Kinesiology at Stephen F. Austin State University from 2015-2022 before landing at his current academic home at St. Edward's University in the great running community of Austin, TX. After publishing the first super shoe comparison study, he realized there was a great interest in objective running shoe performance reviews, independent of brand partnerships. He started an Instagram page in September 2021 with the goal of sharing such data. After some success there, he decided to start his website in September 2022 to better catalog the testing and reviews he has done and continue to do with the goal of also reaching a larger audience beyond Instagram alone. Your process, how you interact with your clients, how you like to very carefully gauge where you gather your reference material to keep you more authentic and not just echoing the voices of other artists.    I feel like you have to develop yourself as an artist, and you can become an amazing artist, but once you are an amazing artist, have you also done some personal development in the meantime?   If you arrive at 40, 45 and you feel like, oh, I feel like I'm still 20 because I haven't really done anything but just, like, tattooing and art and tattooing and art and tattooing and art, then at some point you're like, maybe there's space for some other stuff here that makes me grow as a person.   I still draw, I still paint, but not nearly at this pace I used to, only because I'm striking a balance, like, gosh, can I do something else today besides art? Can I go ride? I like the mountain bike, too, and anything. Just go for a walk.

Richard J. Schwab, MD, DABSM, Head of Sleep Medicine, University of Pennsylvania, and the Co-Medical Director, Penn Sleep Center, discusses the pathogenesis of obstructive sleep apnea, and obstructive sleep apnea treatment. Podcast Points: What is obstructive sleep apnea? What causes sleep apnea in some, when others seem to avoid it, and what can you do about it? How do soft tissues impact sleep? Dr. Schwab provides some background on his work and the various sleep problems that exist. He talks about sleep apnea, and why there is so much left to understand. Why does it happen when we sleep? He discusses soft tissue structures and lateral walls and other factors that can lead to airway paths collapsing. Dr. Schwab's extensive research seeks to target the pathogenesis of obstructive sleep apnea by using advanced upper airway imaging techniques. His studies help further explain the motion of various key structures of the upper airway and the role they play in airway closure. Dr. Schwab talks about the biomechanics of apneic events. He provides information on how they utilize magnetic resonance imaging and electronic beam computed tomography during sleep, as well as wakefulness, to study patients. Dr. Schwab talks about abnormal craniofacial structures as well as soft tissue, and how they can potentially impact sleep apnea occurrences. He discusses mouth breathing, studies they conducted on tongue fat, and how it all could impact breathing issues. As he states, if you naturally have a narrow airway, as movement occurs when you sleep, apnea could be initiated. He provides an in-depth discussion of how tissues move, and studies they have done on wakefulness. But he states there are more studies on sleep and breathing that they plan to do in the near future. Dr. Schwab, through his exhaustive research annually, collaborates regularly with members of the Departments of Radiology and Biomechanical and Computer Engineering. And together, the scientific researcher/developers have designed an extremely advanced, computer graphics-based analysis software that can assist with modeling, in three dimensions, of the biomechanical interrelationships that exist between soft tissue structures and the upper airway. Recently someone asked me if Biomotional and Biomechanical are synonymous? In this episode I consider the two, coming from the fact that movement is not an intellectual process!! In my world we are not biomechanical at all ever...so sticking "bio" infront of a word and describing different hinges and levers to justify using  "levers" as synonyms for "joints" in the human body doesn't make us mechanical. I'm struggling with this metaphor at the best of times...!! Do you know what I mean?
