Dr. Paul Offit is a pediatrician specializing in infectious diseases and an expert on vaccines, immunology, and virology. He is the co-inventor of a rotavirus vaccine that has been credited with saving hundreds of lives every day. Dr. Offit is the Maurice R. Hilleman professor of vaccinology, professor of pediatrics at the Perelman School of Medicine at the University of Pennsylvania and director of The Vaccine Education Center at Children's Hospital of Philadelphia (CHOP). Offit is currently a member of National Institutes of Health (NIH) working group on vaccines, a subgroup of the "Accelerating COVID-19 Therapeutic Interventions and Vaccines" (ACTIV) comprised of experts to combat COVID-19. He is also a member of the FDA's Vaccines and Related Biological Products Advisory Committee (VRBPAC). Previously, he was a member of the Centers for Disease Control and Prevention's (CDC) Advisory Committee on Immunization Practices. The medical safety net ensuring vaccine safety is historic, says Henry Bernstein, DO, MHCM, FAAP, a voting member of the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC), and a professor of pediatrics at the Donald and Barbara Zucker School of Medicine at Hofstra/Northwell.  00:55 - Dr. Bernstein explains his role on the ACIP 03:02 - He explains the four reporting programs they use to monitor and learn from data on possible reactions to vaccines. Three of those have been around for many years, with the CDC's V-safe after vaccination health checker a new addition during COVID-19.  V-safe: a text messaging-based system that enables people who received the COVID-19 vaccine to report any post-vaccine side effects via a free app. Users receive weekly messages for up to six weeks; then three-, six-, and 12-month check ins after the final dose of vaccine. Users are also reminded to get the second vaccine dose and a check-in afterward about their experience. VAERS (Vaccine Adverse Event Reporting System): a national database to which providers, vaccine manufacturers and the public can report side effects. The "early warning system" provides the CDC and FDA with information that helps identify health concerns and ensure vaccine safety. Clinical Immunization Safety Assessment Project (CISA): a national network of vaccine safety experts from the CDC's Immunization Safety Office (ISO) and medical research centers. There are seven centers around the county that serve as consultants around these less common events and also do research relating to vaccine safety, as well as work with health care providers when vaccine safety questions arise, or adverse events following immunization occur domestically. An influential Covid-19 model from the University of Washington predicts there will be 562,000 deaths by April 1st. If 95% of people wore masks, 55,000 lives could be saved. Dr. Chris Murray is the director of the group at the University of Washington that’s been working on these models throughout the pandemic. He joins AC360 to discuss when he thinks the vaccines will make a major dent on the daily death rate. Plus, the FDA has issued emergency use authorization for Moderna’s Covid-19 vaccine. Dr. William Schaffner is a professor at the Division of Infectious Diseases at Vanderbilt University Medical Center and is also a member of the CDC Advisory Committee on Immunization Practices. He tells AC360 that getting the Moderna vaccine to rural hospitals and health departments will be much easier because it doesn’t have the intense, cold storage requirements that the Pfizer vaccine has. Listen Now The Emergency Medical Treatment and Labor Act (EMTALA) was passed in 1986 under the Reagan administration to help prevent patient dumping.  The law requires hospitals to provide emergency medical treatment to anyone regardless of citizenship, legal status or ability to pay.  In recent years however hospitals have begun to impose upfront emergency room fees.  Today approximately half of all hospitals do so.  Hospital executives claim these fees reduce ER overcrowding by diverting patients with non-emergency needs.  Patient advocates claim the fees undermine EMTALA's intent and causes patients' health conditions to worsen.  For example, in 2011 one large national hospital chain saw 80,000 patients leave their emergency rooms untreated when faced with a $150 use fee.  During this 17 minute podcast Professor Rosenbaum explains what generally EMTALA requires, when ER fees can be legally solicted or collected, the negative effects of fee collection, she questions the legitimacy of the industry's argument that fees help to encourage more appropriate site of care use and what can be done to provide better oversight and enforcement of EMTALA.     Professor Sara Rosenbaum is the Harold and Jane Hirsh Professor of Health Law and Policy and Founding Chair of the Department of Health Policy at The George Washington University School of Public Health and Health Services.  Professor Rosenbaum is best known for her work on the expansion of Medicaid and community health centers, patients' rights in managed care, civil rights and health care, and national health reform.  She is the lead author of Law and the American Health Care System, a landmark textbook that provides an in-depth exploration of the interaction of American law and the U.S. health care system.  She has received numerous national awards for her work, serves on governmental advisory committees, private organizational and foundation boards, and is a past Chair of AcademyHealth. She is a member of the CDC Director's Advisory Committee, the CDC Advisory Committee on Immunization Practice and a Commissioner on the Medicaid and CHIP Payment and Access Commission (MACPAC).  She received her BA from Wesleyan and her JD from Boston University.
