Dr. Philip J. Landrigan is a pediatrician and a public health doctor. He is Professor of Biology, Director of the Program for Global Public Health and the Common Good, and Director of the Global Observatory on Planetary Health at the Schiller Institute for Integrated Science and Society at Boston College. He is also Professor Emeritus of Pediatrics and Preventive Medicine at the Icahn School of Medicine at Mount Sinai. In his work, Phil's goal is to better understand how toxic exposures in the environment harm human health. As a pediatrician, he focuses primarily on the impacts on children's health. On the public health side, he aims to harness science to prevent toxic exposures to keep kids healthy and prevent disease. His research has spanned many areas, including the health impacts of lead poisoning, air pollution, pesticides, plastic pollution, and chemical pollution. When he's not working, Phil loves to spend his time outdoors, particularly hiking and kayaking. He is also an avid reader and enjoys spending the evenings sitting down with a good book. Phil completed his undergraduate studies in biology at Boston College and earned his MD from Harvard Medical School. After an internship at Cleveland Metropolitan General Hospital, he completed his residency at Boston Children's Hospital. Phil also earned a MS degree with distinction in Occupational Medicine from the London School of Hygiene & Tropical Medicine and the University of London. Phil worked at the Centers for Disease Control and Prevention, the National Institute for Occupational Safety and Health, and Icahn School of Medicine at Mount Sinai before joining the faculty at Boston College. He also spent a sabbatical working at the U.S. Environmental Protection Agency. Phil has earned numerous awards and honors throughout his career, including the Gold Medal for Distinguished Service to Humanity from the National Institute of Social Sciences, Lifetime Achievement Award from the National Council for Science and the Environment, the Lifetime Achievement Award from Healthy Child Healthy World, the Child Health Champion Award from the U.S. Environmental Protection Agency, the Meritorious Service Medal of the U.S. Public Health Service, multiple Commendation Medals from the Navy and Marine Corps, the Secretary of Defense Medal for Outstanding Public Service, the National Defense Service Medal, and many others. For many people, mould becomes a major concern in winter and the health problems mould can cause have been under more scrutiny since the death of two-year-old Awaab Ishak who died in 2020 from long-term exposure to mould at his home in Rochdale.At Imperial College London a study called WellHome is underway to monitor levels of mould and other indoor pollutants in homes across the west of the capital. Presenter James Gallagher meets Mum of four Roxanne who had monitoring equipment installed in her house to collect data that could lead to guidance on housing quality and inform how healthcare professionals look for and treat conditions affected by mould. On this week's program, we bring you some highlights from the 2024 Environmental Justice Conference hosted by the West Jefferson County Community Task Force (WJCCTF) on Saturday, Oct. 19th, 9am - 3pm, at the University Club at the University of Louisville and online. Funding for this free public conference was provided by the Environmental Protection Agency RATHA Grant and the Louisville and Kentucky Branch of the NAACP. The Theme of this 8th Annual Conference was “Environmental Health: Knowledge Is Power.” Participants enjoyed a day of information with opportunities to ask questions concerning the three-year Rubbertown Air Toxics and Health Assessment (RATHA) research grant funded by the Environmental Protection Agency (EPA), learned about other topics of environmental interest, and participated in an Environmental Roundtable. The research partners for the RATHA grant include the West Jefferson County Community Task Force in collaboration with the Air Pollution Control District, the Christina Lee Brown Envirome Institute of UofL, the Louisville Metro Department of Public Health and Wellness, and the Park DuValle Community Health Center. EJ Groups, Concerned Citizens, and Residents of West Louisville neighborhoods and areas near Rubbertown are encouraged to work together to address air toxics and their impact on our health. Bio's Erin Browkoski Erin has been working in the world of higher education disability services since 2013. She holds a master's degree in clinical mental health counseling and is a Nationally Certified Counselor and Licensed Professional Counselor in Illinois. Erin is a proud graduate of the Docs With Disabilities Initiative (DWDI) Disability Resource Professional Academy. Erin currently serves as Assistant Director of Medical School and Clinical Accessibility at UI College of Medicine for students on all three campuses: Chicago, Rockford and Peoria. Suzanne Hawks Suzanne joined the Center for Learning Access and Student Success at Wake Forest University in early 2018. She received her BS in Psychology from Presbyterian College and spent several years working with children and adults with Autism Spectrum Disorder before joining the field of higher education in 2012. While Suzanne serves students across all programs at Wake Forest, she is the primary contact for students within the School of Medicine. In her spare time, Suzanne enjoys spending time with her family (which includes the cutest little boy in the entire world), really fun friends, and their rescue pit/heeler mix Tommy Boy.   Karyn LaTurner Echols Karyn is a Student Disability and Student Services professional currently serving as the Disability Officer at Rocky Vista University College of Medicine in Ivins, Utah. Her extensive experience includes roles such as Disability Access Consultant at Arizona State University and Associate Director for Student Disability Services at the University of Chicago. She holds a Master of Education from Arizona State University and is multilingual, fluent in English, German, Italian, and conversant in Japanese.   Christine Low Christine serves as the Director of Disability Services at the Icahn School of Medicine at Mount Sinai in New York City. Beyond her role as Director, Christine holds a faculty position in Environmental Medicine and Public Health and serves as Adjunct Faculty at Columbia University, School of Social Work. Christine devotes significant time to disability inclusion efforts for medical education. She is an active member of the Access in Medicine (AIM) program within the DocsWithDisabilities Initiative and from 2016 to 2023 served on the Board of Directors for the Coalition for Disability Access in Health Science and Medical Education. With a national presence, Christine has delivered presentations on various disability-related topics. Her contributions extend to scholarly work, co-authoring significant publications, including "The Performance and Trajectory of Medical Students with Disabilities: Results from the Pathways Project.” She also contributed to the leading text on the topic of disability inclusion in health professions education authoring chapters in "Disability as Diversity" and "Equal Access for Students with Disabilities: The Guide for Health Science and Professional Education, Second Edition."  Christine lives in northern NJ with her husband and her Bernese Mountain Dog, Diesel. Sarah Triano Sarah is the Director of the Center for Learning Excellence at Geisinger College of Health Sciences where she oversees the disability services for the school's nursing, Doctor of Medicine, and master's in biomedical sciences programs. Sarah is the former chair of Access in Medicine (AIM), a program of the Doctors With Disabilities Initiative (DWDI), a working group focused on creating resources and guidance to improve equitable opportunities for students with disabilities and is passionate about creating a welcoming, inclusive, and accessible space for students with disabilities in medicine and health science programs. In both his clinical practice and as a member of the Board of Directors of the American Academy of Environmental Medicine, he considers detoxification as a critical foundation to health. During this conversation, Dr. Morrison emphasizes that toxins in the environment are a major cause of disease and that our bodies are not equipped to handle the vast array of chemicals present in our modern environment. He explains that detoxification is a natural process where cells push toxins into the connective tissue, which then moves them through the lymphatic system, liver, gallbladder, and digestive tract for elimination (or through sweat or breath). However, if the body is overwhelmed with toxins or if the detoxification process is hindered, symptoms and illness can occur. Most often, this looks like visible inflammation, aches and pains, fatigue, skin issues and brain fog.   Dr. Morrison recommends supporting detoxification through practices such as regular exercise, anti-inflammatory eating, drinking plenty of water, and maintaining regular bowel movements. Pearls and Quotes - Obesity & Cardiovascular Disease Risk The durability of metabolically healthy obesity (i.e., normal A1c, lipids, LFTs, BMP, normotensive) is limited. Within 5 years, a third of adults with “metabolically healthy” obesity will develop a cardiometabolic complication. The biomechanical and psychosocial complications of obesity are just as important as the cardiometabolic complications. Biomechanical and psychosocial complications, including obstructive sleep apnea, joint pain, and mood disorders also influence cardiovascular disease risk. Weight loss is not always the patient's goal. Meet patients where they are and understand their challenges, concerns, and long-term goals with respect to their cardiovascular health and obesity. This information provides an opportunity to frame the conversation in a supportive and engaging way that allows for patient education. Body mass index (BMI) is a screening tool for obesity, but is not sufficient for providing individualized care. Obesity management methods that result in rapid weight loss may not be appropriate for all patients. These methods, such as bariatric surgery and GLP1-receptor agonists, require regular monitoring, follow-up, and multidisciplinary care (e.g., nutritionist, exercise physiologist, endocrinologist, cardiologist, psychologist, etc.). Show notes - Obesity & Cardiovascular Disease Risk Is it possible to be healthy at any size? Whether an individual can be healthy at any size depends on the definition of health and its durability.Approximately 10-15% of adults with obesity are metabolically healthy.The risk for developing cardiometabolic disease is higher in obese versus non-obese adults. One in three adults with metabolically healthy obesity will develop cardiometabolic complications (i.e., insulin resistance/diabetes, hyperlipidemia, hypertension) within five years. Thus, metabolically healthy obesity may represent a transient phenotype with adverse long-term consequences. Consider non-metabolic health consequences of obesity that also influence cardiovascular disease risk. Obstructive sleep apnea, joint pain leading to decreased physical activity, and mood disorders are key considerations here and encompass the biomechanical and psychosocial consequences of obesity. Does large, rapid weight loss result in poorer long-term weight loss than slower, gradual weight loss? When approaches to weight loss are not sustainable, such as extremely low-calorie diets or extreme fitness regimens, the results and associated health benefits are less likely to be durable.
