It's been seven years since the e-scooter companies unceremoniously dumped fleets of Limes and Birds onto the streets of Denver, and many Denverites are still upset about it! But at the same time, many others have grown to rely on these micro-mobility machines. Councilman Chris Hinds, in particular, has been pushing for tougher regulations for years, and last week he put forth a new proposal to the rest of City Council, citing Denver Health data showing a huge increase in scooter-related accidents in 2024. Every child has an individual healthcare experience, but for marginalized communities like migrant families, they face some unique challenges in our healthcare system. From dealing with trauma to navigating a language barrier, working to get the right care can be complicated. At this year's annual Children's Hospital Colorado podcast live event, Charting Pediatrics took a close look at this topic. In this episode you will learn about acute clinical challenges, resource constraints, social needs, moral distress, impact on staff, child health policy, combating misinformation and more. Steven Federico, MD, the Chief Government and Community Affairs Officer at Denver Health joined us for this special episode. His passion for improved and equitable healthcare is fueled by his experiences as a pediatrician and primary care physician at Denver Health, where he has worked since 2002. In his past role as medical director, he oversaw three community health centers and 19 school-based clinics that provide comprehensive physical and mental health care to 70,000 children across Denver. Her perspective on the systemic, normalized abuse and mistreatment of childbearing people is aligned with advocacy workers, interdisciplinary scientists, and all interested and affected parties working to end obstetric violence and achieve birth equity.   Dr. Brie Thumm is an Assistant Professor at the University of Colorado College of Nursing. She has been practicing midwifery domestically and internationally since 2001 when she completed her Masters in the Science of Nursing at Yale University. She obtained her MBA in Healthcare Administration at Baruch College in New York City and her PhD in health systems research at University of Colorado College of Nursing. Her area of research is perinatal workforce development to address disparities in maternal health outcomes and improve the well-being of health care professionals. Prior to her current position, Brie provided care at Planned Parenthood of New York City, served as the Assistant Director of the Sexual Assault Response Team for the Manhattan public hospitals, conducted mental and behavioral health research at the Rocky Mountain Regional Veteran's Affairs Medical Center, and led the clinical and research arms of the Maternal Mortality Prevention Program at the Colorado Department of Public Health and Environment. She continues to practice clinically at Denver Health. This week we talk with Dr Lindsey Fish, Medical Director of the Peña Southwest Urgent Care Clinic at the Denver Health and Hospital.  She is an Associate Professor of Medicine at the University of Colorado School of Medicine. We also discuss the new legislation in some states that has changed the way that parents with substance use issues are treated.  Since it's been proven that punitive measures do not lead to change with people who have this complicated problem, new laws were put into place with the hope that women with substance use issues start prenatal care earlier. You'll also learn how educators and other birth workers can offer resources for those who want to seek help.     Our guest, Dr. Kaylin Klie, MD, MA is an addiction medicine specialist who works at UCHealth in Colorado as an Assistant Professor of Family Medicine and Psychiatry with a special focus on perinatal addiction. She is the founder of the Denver Health and University of Colorado OB Addiction Medicine clinics, which provide integrated peripartum care and substance use assessment and treatment for pregnant and mothering women.  Dr. Klie has been on a steering committee member for the State of Colorado Substance Exposed Newborn task force, and the co-chair for Provider Education. In this podcast episode we want to introduce you to our BCEN Friend, Adam Sorenson. Graduating in 2012 with his BSN, Adam Sorenson began his career as a pediatric nurse before transitioning to a role as a pediatric intensive care nurse. After a few years in the PICU, Adam shifted to working in an adult surgical intensive care unit (SICU) at a Level 1 trauma center, where he continues to work today. Adam's professional development includes advanced training, starting with Trauma Nursing Core Course (TNCC) and Advanced Trauma Care Nursing (ATCN) to obtaining his certification as a Trauma Certified Registered Nurse (TCRN). In 2014, during the Ebola outbreak in Central and West Africa, Adam joined the Highly Infectious Treatment Team (HITeam) at Denver Health Hospital, an all-voluntary group established to handle such crises. Denver Health was designated as an Ebola regional treatment center in 2015. In 2021, Adam took on a part-time role as a Biocontainment Unit Nurse Educator, focusing on training, education, and preparedness for viral hemorrhagic fevers, rare respiratory viruses, and novel viruses. Additionally, Adam has served as a nurse in the Air Force Reserves for the past five years. Dean's Chat host, Dr. Jeffrey Jensen welcomes Dr. Kristine Hoffman to Dean's Chat. Dr. Hoffman is currently the director of the Denver Health Podiatric Medicine and Surgery Residency Program, Chief of Podiatry, and an attending physician in the Department of Orthopedics at Denver Health Medical Center. Additionally, she is an Assistant Professor in the Department of Orthopedics at the University of Colorado School of Medicine. Kristine is a Colorado native who received her Bachelor of Arts degree with distinction from the University of Colorado at Boulder. She completed her Doctorate in Podiatric Medicine at Rosalind Franklin University of Medicine and Science in 2007. She returned to Colorado for her post-graduate training and completed her 3-year surgical residency at the North Colorado Medical Center Program based in Greeley, Colorado. Following her post-graduate studies, Kristine joined a podiatry group in Boulder, Colorado where she worked from 2010 to 2016. She transitioned her career to academic medicine in 2016 when she joined Denver Health – a leading academic hospital, Colorado's primary safety net hospital, and level-one trauma center. Outside of podiatry, Kristine enjoys trail running and fly fishing. She is a four-time “Troutwoman” – having successfully completed this challenging event that involves running at least 26.2 miles with a minimum of 3000 feet of elevation gain and catching all 4 species of Colorado trout in under 12 hours. Kristine is currently the president of Running Rivers a nonprofit organization with the mission to conserve and restore native fish ecosystems by educating and inspiring recreationists to engage in conservation. She also has a long history of competitive rock climbing which enabled her to travel the world and compete. She currently lives in Boulder, Colorado with her husband, daughter, 2 dogs, and 11 chickens. Dr. Jeffrey Jensen chats with Dr. Kristine Hoffman and they reflect on her leadership roles and success in the field, highlighting the importance of training in shaping her career.   The mouth is a fundamental part of the human body. It allows us to eat, drink, breathe, speak, smile and express ourselves. Naturally, that means oral health is connected to our broader physical health, but in the United States, medical and dental care are structurally separate. Despite their connections, the two have entirely different systems of professional training schools, insurance coverage and care networks. This is referred to as the dental divide, and it results in varied levels of access and health outcomes. The repercussions of these barriers are significant, especially for children in our most vulnerable communities. To discuss this important topic, we are joined by Patricia Braun, MD. Dr. Braun is a primary care pediatrician at Denver Health, as well as a professor of pediatrics at the University of Colorado School of Medicine. She is the immediate past chair of the Section on Oral Health for the American Academy of Pediatrics, and her research focuses on oral health outcomes and program evaluation. The breach led to issues with claim submission, as some payers who relied solely on Change as their intermediary were unable to process claims. This created a backlog of claims and disrupted the revenue cycle for many healthcare organizations. Some providers resorted to manual processes, such as printing and mailing claims, which added complexity and delays to the billing process. Cash flow was significantly impacted by the breach, especially for smaller healthcare entities like physician groups and community hospitals. With claims not being processed efficiently, these organizations faced financial strain due to delayed payments and uncertainty about when the situation would be resolved. The lack of cash flow threatened the financial stability of these smaller entities, highlighting the importance of having contingency plans and backup systems in place. Data security concerns arose as a result of the breach, with questions about the extent of the data compromised and the potential implications for patient privacy. The breach raised questions about the adequacy of security measures in place and the vulnerability of healthcare systems to cyber threats. The need for enhanced data security protocols and proactive measures to prevent future breaches became a pressing issue for healthcare organizations. Experian's Cyber Incident Response team and Cyber Threat Intelligence teams have been engaged and are closely monitoring the situation. We have not seen evidence of a threat to Experian. Out of an abundance of caution and consistent with Experian's security practices, Experian has temporarily severed connectivity to Change Healthcare. During the period of this severed connectivity, there will be an impact to batch and EDI authorizations, claims and remits, claim status, eligibility, and notice of care transactions that Experian Health sends to Change Healthcare. Experian will resume connectivity to Change when it receives assurances from Change Healthcare that it has adequately contained the threat. Payers work with a variety of clearinghouses to process data. In many cases, multiple payer connections are available although sometimes payer connectivity may be limited or even exclusive to a single clearinghouse. In the majority of cases, transactions from Experian Health route directly to the requestedpayer. When connectivity is limited or certain routes are unavailable, however, it is common to route traffic through another clearinghouse. Below is a list of potentially impacted payers. Digital content is at the center of our society and its prevalence continues to grow. Storytelling on digital platforms allows the medical community to showcase groundbreaking research and connect with audiences, who otherwise may not be privy to this information. Experts like Joshua Williams, MD, are at the forefront of some of this innovation. In October 2023, he received the Advancement in Research Award at the American Academy of Pediatrics National Conference.  Dr. Williams is an assistant professor of pediatrics at the University of Colorado School of Medicine, as well as a practicing primary care pediatrician with Denver Health. He is the principal investigator on a National Institutes of Health grant to examine the impact of digital storytelling on influenza vaccination equity. Over the last three years, Colorado has spent nearly $2 billion on homelessness, concentrated largely in the Denver metro region. While metro areas across the country have seen a surge of people experiencing homelessness, the Denver metro has some of the highest levels of growth among the areas the U.S. Department of Housing and Urban Development tracks. There has been a 58% increase in the Denver metro's unhoused population since 2016. More become homeless for the first time, and more are chronically homeless. Joining Host and Chairman Earl Wright is Donna Lynne, CEO of Denver Health. Donna was the Lieutenant Governor of Colorado from 2016-2019. Prior to that role, she served in executive leadership positions at Kaiser Foundation Health Plan and Kaiser Foundation Hospitals. She has a unique view of the homelessness issue, the recent influx of migrants, the need for a large coalition of stakeholders to address the issue, and a lot more. We will look at what the future looks like in medical imaging in breast cancer. I'm speaking to Dr. Randy Miles. Dr. Miles serves as the chief of breast imaging at Denver Health with oversite over the breast divisions clinical, research, and educational programs. He also serves as the associate director for research in the radiology department. He earned his medical degree from Mayo Clinic College of Medicine. Dr. Miles has achieved multiple awards throughout his career and served on medical missions in Haiti and the Dominican Republic. He completed his master's in public health degree from Harvard School of Public Health. He completed his diagnostic radiology residency training at the University of Illinois, Chicago where he served as chief resident. He went on to complete his breast imaging fellowship training at the University of Washington Medical Center, Seattle Cancer Care Alliance. Dr. Miles has been the recipient of numerous research grants and has published over fifty papers. He talks nationally on his work in artificial intelligence, health literacy, and breast cancer disparities. Board Certified in Internal Medicine, she completed her undergraduate degree at Harvard, medical school at Boston University School of Medicine, and her internal medicine residency and chief residency at Yale, where she won numerous clinical awards. Dr. Gaudiani moved to Denver in 2007, choosing Colorado with her husband because its emphasis on the outdoors, the incorporation of nature into daily life, and the importance of family activity time as it offered a great way to foster work-life balance. In 2008, she was one of the founding team members of the ACUTE Center for Eating Disorders at Denver Health. After seven great years there, she left as its Medical Director to pursue her vision of outpatient care, founding the Gaudiani Clinic. During her years at ACUTE, Dr. Gaudiani became a recognized internist for her work on the medical complications of eating disorders.
