In episode 83 of Going anti-Viral, Dr Steven Grinspoon joins host Dr Michael Saag to provide an update on guidelines for statin use in people with HIV based on results from the REPRIEVE trial. Dr Grinspoon is a Professor of Medicine at Harvard Medical School and a clinician in the Neuroendocrine and Pituitary Tumor Clinical Center and faculty member at the Massachusetts General Hospital. He is a clinical researcher who studies hypothalamic control of body weight and fat distribution in obesity and lipodystrophy with a focus on the metabolic and cardiovascular consequences of visceral fat accumulation. Dr Grinspoon is the author of more than 200 peer-reviewed publications and serves as the chief of the Massachusetts General Metabolism Unit and the director of the Nutrition Obesity Research Center at Harvard. Dr Grinspoon and Dr Saag discuss the latest findings from the REPRIEVE trial and provide updates on statin guidelines for people with HIV as well as providing insights into cardiovascular risk management. They discuss the safety and efficacy of statins and the role of cardiovascular risk calculators in clinical practice in addition to discussing the impact of low-density lipoprotein (LDL) cholesterol and inflammation on cardiovascular risk. Send us Fan MailConcern is one of the most socially approved forms of anxiety, and that's exactly why it can be so hard to see. We talk about the moment “I'm just worried about you” stops being love and starts becoming pressure, projection, and control. When concern turns into rumination, diagnosing, or mentally rehearsing what someone should do, it doesn't deepen connection. It quietly wedges the ego into the relationship and steals the ease that makes love feel safe. We also get concrete about how this shows up in real life: snapping a correction that carries irritation, spiralling about money and logistics, judging health choices, or building rigid protocols that create inner battles. Using an A Course in Miracles frame, we explore a simple practice: treat any upset or disturbance as a signal that the mind needs correction, not proof that the world is dangerous. That shift is not bypassing feelings. It is learning what feelings are actually for. From there we zoom out to bigger fear narratives including politics, rent, media programming, and the urge to “save the world.” The invitation is direct: you are sustained by the love of God, not by circumstances, and nothing outside you has power unless you give it power. We end with a way to live this without perfectionism by choosing “happy games” over fixing and letting everything you perceive become useful for returning to peace. Full Text of Readings Memorial of Saint Benedict, Abbot Lectionary: 388 The Saint of the day is Saint Benedict Saint Benedict's Story It is unfortunate that no contemporary biography was written of a man who has exercised the greatest influence on monasticism in the West. Benedict is well recognized in the later Dialogues of Saint Gregory, but these are sketches to illustrate miraculous elements of his career. Saint Benedict was born into a distinguished family in central Italy, studied at Rome, and early in life was drawn to monasticism. At first he became a hermit, leaving a depressing world—pagan armies on the march, the Church torn by schism, people suffering from war, morality at a low ebb. He soon realized that he could not live a hidden life in a small town any better than in a large city, so he withdrew to a cave high in the mountains for three years. Some monks chose Saint Benedict as their leader for a while, but found his strictness not to their taste. Still the shift from hermit to community life had begun for him. He had an idea of gathering various families of monks into one “Grand Monastery” to give them the benefit of unity, fraternity, and permanent worship in one house. Finally he began to build what was to become one of the most famous monasteries in the world—Monte Cassino, commanding three narrow valleys running toward the mountains north of Naples. The Rule that gradually developed prescribed a life of liturgical prayer, study, manual labor, and living together in community under a common abbot. Benedictine asceticism is known for its moderation, and Benedictine charity has always shown concern for the people in the surrounding countryside. In the course of the Middle Ages, all monasticism in the West was gradually brought under the Rule of Saint Benedict. Today the Benedictine family is represented by two branches: the Benedictine Federation encompassing the men and women of the Order of St. Benedict; and the Cistercians, men and women of the Order of Cistercians of the Strict Observance. Reflection The Church has been blessed through Benedictine devotion to the liturgy, not only in its actual celebration with rich and proper ceremony in the great abbeys, but also through the scholarly studies of many of its members. Liturgy is sometimes confused with guitars or choirs, Latin or Bach.
