Dr del Rio is a Distinguished Professor of Medicine in the Division of Infectious Diseases at Emory University School of Medicine. He has worked more than a decade with hard-to-reach populations including people with substance use disorders to improve outcomes of those with HIV and to prevent infection with those at risk. Dr del Rio discusses steps his clinic has taken to provide care to hard-to-reach populations including understanding why patients miss appointments. Dr Saag and Dr del Rio also discuss the unique challenges that foreign-born individuals have in accessing care and the impact of current immigration enforcement on people in need of care and on the broader healthcare workforce. Full Text of ReadingsMemorial of Saint Benedict, Abbot Lectionary: 387The Saint of the day is Saint BenedictSaint Benedict’s Story It is unfortunate that no contemporary biography was written of a man who has exercised the greatest influence on monasticism in the West. Benedict is well recognized in the later Dialogues of Saint Gregory, but these are sketches to illustrate miraculous elements of his career. Benedict was born into a distinguished family in central Italy, studied at Rome, and early in life was drawn to monasticism. At first he became a hermit, leaving a depressing world—pagan armies on the march, the Church torn by schism, people suffering from war, morality at a low ebb. He soon realized that he could not live a hidden life in a small town any better than in a large city, so he withdrew to a cave high in the mountains for three years. Some monks chose Benedict as their leader for a while, but found his strictness not to their taste. Still the shift from hermit to community life had begun for him. He had an idea of gathering various families of monks into one “Grand Monastery” to give them the benefit of unity, fraternity, and permanent worship in one house. Finally he began to build what was to become one of the most famous monasteries in the world—Monte Cassino, commanding three narrow valleys running toward the mountains north of Naples. The Rule that gradually developed prescribed a life of liturgical prayer, study, manual labor, and living together in community under a common abbot. Benedictine asceticism is known for its moderation, and Benedictine charity has always shown concern for the people in the surrounding countryside. In the course of the Middle Ages, all monasticism in the West was gradually brought under the Rule of St. Benedict. Today the Benedictine family is represented by two branches: the Benedictine Federation encompassing the men and women of the Order of St. Benedict; and the Cistercians, men and women of the Order of Cistercians of the Strict Observance. Reflection The Church has been blessed through Benedictine devotion to the liturgy, not only in its actual celebration with rich and proper ceremony in the great abbeys, but also through the scholarly studies of many of its members. Liturgy is sometimes confused with guitars or choirs, Latin or Bach. We should be grateful to those who both preserve and adapt the genuine tradition of worship in the Church. The elections have been viewed as a reflection on the administration of President Ferdinand ‘Bongbong' Marcos, Jr. and as especially consequential for the future of Vice-President Sara Duterte. She was impeached by the House of Representatives in February 2025, setting the stage for a trial by the Senate, but with her continuing popularity making her a serious contender for the presidency in 2028. To interpret the mid-term elections, Dialogues on Southeast Asia has turned to Dr. Sharmila Parmanand, an Assistant Professor in Gender, Development and Globalisation in the Department of Gender Studies at the London School of Economics and Political Science (LSE) and also an Associate and member of the Management Committee of the LSE's Saw Swee Hock Southeast Asia Centre. Dr. Parmanand's research focuses on the intersection of gender and politics in the Philippines, ranging from sex work and migration policies to the connections between gender, nationalism, and democracy. She is currently working on her first book, titled Saving Our Sisters: The Politics of Anti-Trafficking and Sex Work in the Philippines, but she is also busy conducting research, writing, and publishing on other fronts, including a new collaborative project on queer activism across Southeast Asia. Despite decades of investment, innovation, and policy reform, yields on African small farms remain significantly below those in high-income countries. While the limitations of smallholder models, that doesn't mean that the problem is easy to solve, not least because the way that land is owned my make consolidation impossible. The result: fewer opportunities for structural transformation and rural development. Dr Maldonado is a Professor in the Stanford University School of Medicine where she has achieved national and international recognition for her scholarship in the epidemiology and control of infectious diseases. She has served on several national and international committees including, until recently, the CDC Advisory Committee on Immunization Practices (ACIP). Dr Maldonado and Dr Saag provide an overview of RSV including the history of RSV vaccine research and development and provide an update on the availability of RSV vaccine in adults but not in children. They also discuss the risk of severe disease in children and the limited treatments available. Dr Maldonado and Dr Saag also discuss the process for vaccine approvals in the United States. Specifically, they review the role of the ACIP committee in vaccine approvals and provide an update on the recent dismissal of all of the prior members of the committee, which included Dr Maldonado.
