Join us in Miami Beach next week for the StartUp Health Festival @ ViVE 2022 to meet the MouthWatch team and dozens of other Health Transformers: startuphealth.com/festival — Thanks to COVID-19, the telemedicine industry saw an unprecedented boom over the last two years. Millions of people who never thought they'd see their doctor over a video call were introduced to the idea and discovered it was often pretty convenient. This phenomenon has gotten plenty of air time. But what about tele-dentistry? You may have to see your dentist in person if you want to get a cleaning done, but it turns out there's a whole world of virtual dental care that has exploded in the last two years, following the same trend line as telehealth. At the forefront of this movement is our guest today, Brant Herman, the CEO and Co-founder of MouthWatch. This company – which StartUp Health backed in 2015 – saw the need years ago and has designed an intraoral camera that can help visualize what's going on in the mouth and send that information to a remote specialist. They've also built Teledent, a leading platform for managing teledentistry practices. For this work they've won a host of awards, and for two years in a row have been on Inc's list of fastest growing private companies. We'll get into all of that in the interview, hear about the company's journey, and talk about the critical role of oral health in holistic health. — Join us in Miami Beach from March 6-9 for the StartUp Health Festival @ ViVE 2022 to meet Brant and dozens of other Health Transformers: startuphealth.com/festival — Connect with the team at MouthWatch. Teresa Duncan is a speaker and writer with over 20 years experience in healthcare whose expertise includes proven accounts receivable and insurance methods, and helping doctors and managers establish solid management systems. A recipient of the Educator of the Year Award by the Association of Dental Implant Auxiliaries, Teresa is also an author and a frequent contributor to a variety of publications.  She has been named one of the Top 25 Women in Dentistry by Dental Products Report Magazine, and is recognized yearly as a Leader in Consulting by Dentistry Today. Jamie Collins, RDH-EA,BS, is a clinical practicing hygienist who has been in the dental field for twenty years. In addition to clinical practice Jamie is also an educator, has contributed to multiple textbooks, curriculum development and creation, and contributes as a key opinion leader with multiple companies. She is an active member of the American Dental Hygienists Association, a member of the Dental Codeology Consortium Committee for dental coding, and a colleague of Michelle's at MouthWatch.   In today's episode, our hosts and guests discuss CDT codes in general, where dentistry has gone wrong in coding, the ADA and carriers, and the new staging and grading and which codes may apply there. They also delve into diagnosis coding, internal/unspecified codes and tracking, outcomes based payments, code 4346 and 4355 usage, the importance of educating consumers, and some code suggestions from our experts for particular scenarios. They finish up by looking at developments in coding, our experts' takeaways for listeners, and the issue with assigning codes to products.     Interview starts: 4:32         CDT codes   Where dentistry has gone wrong in coding   The ADA and carriers   The new staging and grading and codes   Diagnosis coding   Internal/unspecified codes and tracking   Outcomes based payment   Code 4346 and 4355 usage   Educating consumers   Code suggestions for particular scenarios   Developments in coding   Takeaways for listeners regarding coding   Assigning codes to products     QUOTES:    “There's so many offices that code for what they think that they're going to get reimbursed for, rather than coding for the procedures that are actually performed.”   “Dental offices don't want to lose patients, they still want the revenue so they're going to kind of give the customer what they want.”   “I think there are times that the ADA could have been a little bit stronger in their representation against the carriers.”   “You can't worry about the insurance, you just have to treat, diagnose treatment.”   “Here's the thing with insurance carriers is when a guideline gets put out, it usually takes them about a year or two to catch up to that.”   “They're going to look at redoing the CDT completely.”   “We don't have the data in order to sufficiently say, ‘Hey, I need more money for this because I do a lot of it and it saves you money on this down the road.'”   “If we're going to perio chart, we chart it all.”   “Are there guidelines for 4355 of how much crap we knock off people's teeth?”   “You don't go into a doctor's office and dictate that they take out this tumor or not this tumor, you don't do that. So I just think we're very scared of patients, and I just don't get it.”   “There's this thought, ‘well, it's not going to get paid, we really shouldn't even bill it.'”   “I like to get paid for things.”   “The more you bill for it, you create that metric, they're seeing it coming down the road, the more apt they're going to be to cover it.”   “I really believe that clinicians should not be aware of a patient's insurance status when they're back there.”   “Just because it's a benefit doesn't mean that it's rooted in any kind of science.”   “Start saying you probably won't get paid for it, but you can do what you think is best clinically.”   “We need to code for what is needed, and what we need to do, not what the insurance will cover or won't cover for them.”      LINKS:   A Tale of Two Hygienists homepage - https://ataleoftwohygienists.com/     AToTH on Facebook - https://www.facebook.com/aTaleOfTwoHygienists/     AToTH on Instagram - https://www.instagram.com/ataleoftwohygienists     AToTH on LinkedIn - https://www.linkedin.com/company/atoth/?originalSubdomain=ca        PDT Homepage     American Academy of Periodontology Homepage     Email Jamie   My Dental Educator Homepage      Odyssey Management Homepage   Odyssey Management Newsletter   Nobody Told Me That! It’s the final installment of Andrew and Michelle’s four-part cariology series! To cap off our journey with caries, we’re stepping away from private practices to learn how dental professionals in the public health sector deal with all things related to tooth decay. Sharity Ludwig, RDH, is the Director of Clinical Innovations and the Director of Community Dental Programs for Advantage Dental, which provides dental care to Medicaid recipients throughout Oregon in partnership with DentaQuest. Sharity and her team work specifically with expecting mothers and parents of young infants to ensure dental health and caries prevention from a young age. She and her colleagues wish to debunk the idea that cavities are inevitable for young children. In this episode, Sharity discusses the differences between public and private dental practice, explains how she and her team perform community outreach, and reveals her technique for silver fluoride application.   Mouthwatch is the official sponsor the Cariology Series this month. As we near the final stretch, this episode's specific focus is on the tools and methods used to figure out if that little recess is a cavity or something else. Paresh Shah, DMD, has been practicing privately in Winnipeg, Manitoba, for nearly three decades. Outside of his clinics, he's a prosthodontic instructor at the University of Manitoba Dental School and an editor for the Canadian Journal of Restorative Dentistry. Paresh's years running a clinic have taught him the importance of using the latest diagnostic tools and listening to the input of his seasoned hygienists. In this episode, Dr. Shah weighs the pros and cons of different caries diagnostic methods, reveals how he's learned to spot false positives, and explains why he takes a collective approach to his clinic staff. EPISODE HIGHLIGHTS: Interview starts: 12:00 - Dr. Shah explains how he fosters a collaborative environment in his practice and what lengths his team goes to in order to identify caries.  - Some helpful tips for visually identifying caries during examination. - Why Dr. Shah and his team still stand by tactile exploration during caries diagnosis. - Newer clinical tools that get the patients themselves more invested in the diagnosis process. - How hygienists can more effectively ‘hand off' patients to the dentist for caries diagnosis.  - Why time and experience are the best experience for dealing with false positives. - The reason Dr. Shah will still swear on a caries dye after all these years. - The wide array of diagnostic tools Dr. Shah and his staff use at his clinic. - Why Dr. Shah values his hygienists' clinical judgment and encourages them to speak up.   QUOTES: “A lot of hygienists can be more than tooth-scrapers.” “Our traditional way of looking at caries is through visual inspection and tactile sense.” “If it sticks, we're treating it. It's as simple as that.” “If there's something I feel is not necessary to treat, I make sure I let the patient know the why of not wanting to treat it.”  “You have to stretch your mind and you have to work collaboratively.”   LINKS: Dr. Paresh Shah's homepage - https://www.drpareshshah.com/ Dr Shah on Instagram - https://www.instagram.com/drpareshshah/   Mouthwatch is the official sponsor the Cariology Series this month.
