Pseudotumor cerebri (PTC), also known as idiopathic intracranial hypertension, is a condition caused by elevated cerebrospinal fluid pressure in the brain. The symptoms of this condition mimic those of a brain tumor, giving it the name pseudotumor cerebri, meaning "false brain tumor." The increased pressure can cause swelling of the optic nerve and can result in loss of vision. Medications can often reduce the pressure and headache, but surgery is necessary in some cases.Alexander M. Check out today's questions: Cheryl: Dr. Cabral, I have a 23 year old daughter with Pseudotumor cerebri. She has been on a diuretic for the past 8 months. What we have researched online it seems that the only constant is being overweight and of child bearing age. When this was originally diagnosed she was having vision problems and had to have a spinal tap to remove fluid to ease the pressure. Is there anything you can recommend? Or is weight loss the best course of action? She is probably about 5'8" and weighs between 170 to 180. She had loss some weight after she was first diagnosed and was able to get off the diuretic for about 2 months. Since then she has put some of the weight back on and is now back on the diuretic. We worry about long terms effect of the diuretic. Thank you in advance for your help. Die große Variationsbreite tumoröser Prozesse im anatomisch eng begrenzten Raum der Orbita stellt eine Herausforderung in der augenärztlichen Praxis dar. Das diagnostische und therapeutische Procedere gestaltet sich, je nach Grunderkrankung, höchst unterschiedlich, und ist für den weiteren Krankheitsverlauf und das “outcome“ von entscheidender Bedeutung. Bei der niedrigen Inzidenz von Orbitatumoren finden sich in der deutschen Literatur zu diesem Thema keine Erhebungen mit größeren Fallzahlen. Die publizierten internationalen Veröffentlichungen kommen, je nach geographischer Herkunft, erfassendem Zentrum und Studiendesign, zu sehr unterschiedlichen Ergebnissen (Albert et al 2003). Das Ziel dieser Arbeit ist die Erfassung von Häufigkeitsverteilung und diagnostischen Charakteristika der verschiedenen orbitalen Prozesse, die an der Augenklinik der Universität München von 1991 bis 2000 histologisch verifiziert wurden. Hierzu erfolgt retrospektiv die Auswertung von 149 Patientenakten. In Hinblick auf die Fallzahl stellt diese Arbeit unseres Wissens nach die umfangreichste bisher veröffentlichte deutsche Erhebung zu diesem Thema dar. Es können insgesamt 42 verschiedene Entitäten nachgewiesen werden. Fast zwei Drittel der Fälle (63,7%) machen Neoplasien aus, und Malignome bestehen bei knapp der Hälfte (71 Fälle, 47,6%) der Patienten. Die häufigsten Arten sind: Non-Hodkin-Lymphome (18,7%), Dakryoadenitis (10,7%), kavernöse Hämangiome (5,4%) und Dermoidzysten (5,4%), Plattenepithelkarzinome (von den Lidern ausgehend) (4%), pleomorphe Adenome (4%), Aderhautmelanome (mit orbitaler Ausdehnung) (3,3%), Basaliome (von den Lidern ausgehend) (3,3%), Metastasen (3,3%), der Pseudotumor orbitae (3,3%) und das Rhabdomyosarkom (3,3%). Mehr als 50% der Orbitatumoren treten innerhalb der sechsten bis achten Lebensdekade auf. Abhängig vom Lebensalter variiert die Prävalenz der verschiedenen Tumoren sehr deutlich. Die in diesem Kollektiv am häufigsten genannten klinischen Zeichen und Symptome sind: Tastbarer Tumor (53,7%), protusio bulbi (42,3%), Lidödem (35,5%), Schmerzen (32,8), eingeschränkte Motilität / Doppelbilder (32,1%), Ptosis (18,1%), Visusminderung (16,1%). Bei den Bildgebenden Verfahren besteht beim Ultraschall in 65% der Fälle, beim MRT in 47% der Fälle und beim CT in 41% der Fälle eine Übereinstimmung zwischen dem erhobenen Befund und der Histologie. Eine Exenteratio orbitae ist fast ausschließlich bei sekundären Orbitatumoren notwendig. Um diesen radikalen operativen Eingriff zu vermeiden, empfiehlt sich die Behandlung aller sekundären Orbitatumoren oder ihrer Vorstufen in einem frühen Stadium.
