İşte bu manada olmak üzere Allâhü Teâlâ, "Allâh'ın zikrine koşun" (Cum'a s. 9) ifâdesi ile namazı "zikir" diye adlandırmıştır. In this episode, the we welcome back guest host, Dr. Neelou Weeker, and ED nurse, Leigh Clary, to discuss the critical intersection of language barriers, patient equity, and emergency care. Through two powerful clinical scenarios, the team explores the “gold standards” of medical translation, the challenges of resource-limited community settings, and how TeamSTEPPS tools—specifically closed-loop communication and situational monitoring—can be leveraged to ensure true informed consent and patient safety. The Gold Standard vs. Clinical Reality Providing equitable care means ensuring every patient, regardless of language or culture, fully understands their medical team. While academic centers are often highly resourced, executing communication seamlessly remains a universal challenge. 1. Translation Tools and Hierarchy The Gold Standard: Video- or audio-based professional interpretation tablets allow face-to-face or direct vocal translation. The Secondary Backup: In-house dual-handset “blue phones” connect directly to professional phone lines when tablets experience connectivity issues. The Tertiary Backup: Multilingual staff members can help act as a bridge. Many institutions feature language fluencies on staff ID badges. Note: Staff members should only be used to establish initial rapport or identify the required dialect, not as official medical interpreters. The Danger of Family Interpreters: While family members bring invaluable cultural context and an understanding of the patient’s baseline, studies show they only correctly interpret medical dialogue 19% of the time. The Bottom Line: Always utilize the official route first. When technology fails, do your absolute best—never settle for “good enough” when better communication is possible. 2. Academic vs. Community and Rural Settings Emergency medicine requires extreme adaptability. In resource-limited community or rural hospitals, finding an interpreter for less commonly spoken languages can take upwards of 30 minutes. Physicians must sometimes physically carry translation phones from room to room while managing other patients just to maintain an open line with a rare-dialect interpreter. Applying TeamSTEPPS to Patient Communication We routinely use TeamSTEPPS tools to communicate with our fellow clinicians, but we must remember that the patient is the most important member of the healthcare team. 1. Closed-Loop Communication & The Teach-Back Method To confirm true patient understanding, avoid simple “yes or no” questions, nods, or smiles. Instead, utilize the Teach-Back Method, requiring the patient to repeat the instructions or choices back to you in their own words. How to Phrase It (Taking Responsibility): “I want to make sure that I have been clear in what I’ve said to you. To help me feel reassured that I communicated everything correctly, could you tell me what you understand is going on?” Clinical Value: This is particularly vital for high-stakes decisions and ED discharge instructions. Multimodal Approach: In high-stakes moments, combine professional translation, family context, and teach-back to minimize errors. 2. Situational Monitoring Resuscitative environments are chaotic, and the primary physician trying to run a cod or secure an airway has immense cognitive load. The Team Safety Net: Other team members (nurses, techs, scribes) can help monitor the situation and catch critical communication errors. Reconciling Clinical Urgency with Informed Consent How do you balance the immediate need to save a life with the time-consuming process of formal translation? The ABC Priority: First and foremost, secure Airway, Breathing, and Circulation. If a patient presents to the ED in extremis and cannot communicate, clinicians must operate under the assumption that the patient wants life-saving measures performed. Task Delegation: While the medical team manages the immediate ABCs, immediately task support staff (such as social workers) with finding an official interpreter, locating family members, and gathering background information. Next Steps: Once the ABCs are stable, the team has the time and space to pause, establish formal translation, and dive deeper into informed consent for further procedures. Key Takeaways Acknowledge the Bias of Urgency: Time pressure can tempt us to bypass official translation channels. Guard against this by maintaining an equity-first mindset. Close the Loop with Patients: Ensure they can paraphrase their care plan or consent choices. Protect the Team via Shared Roles: Trust your teammates to monitor the big picture and catch subtle communication gaps during high-stress resuscitations. Nûh (a.s.) her gün bayılıncaya kadar dövülürdü. O buna rağmen, yine her vurulduğunda, "Allâh'ım kavmime hidayet et, çünkü onlar ne yaptıklarını bilmiyorlar" derdi. Bizim Peygamberimiz (s.a.v.) bu cümleyi sadece bir kere söylemiştir. Halbuki Hz. Nuh (a.s.) bir günde aynı cümleyi defalarca söylemiştir. Bu nedenle, Hz. Nûh (a.s.)'ın Peygamberimiz (s.a.v.)'den üstün olduğunu söylemek gerekir?" denilirse, buna bizim cevabımız şu olur: Cenâb-ı Hakk'ın "Bizi, dosdoğru olan yoluna ilet" sözünden maksadı, kendisinden bu üstün ahlâkın istenmesi olup, Hz. Peygamber (s.a.v.)'de Fatiha'yı her gün şu kadar defa okuduğuna göre, Hz. Peygamber (s.a.v.)'in bu kelimeyi söylemesi, Nûh (a.s.)'ın söylemesinden daha çok olur. Mümin Allâh (c.c.)'u tek bir delille tanıyınca, bütün varlıklarda O (c.c.)'un varlığına, ilmine, kudretine, cömertliğine, rahmetine ve hikmetine delâlet eden nice deliller olduğunu anlar. Buna göre kulun, "Bizi, dosdoğru olan yoluna ilet" sözünün manası: "Ey Râbbimiz! Her şeyde, Senin zâtına, sıfatlarına, kudretine ve ilmine delâlet eden nice deliller bulduk" olur. Bu takdire göre, söz konusu olan sual kendiliğinden bertaraf edilmiş olur. Cenâb-ı Hakk "Muhakkak ki sen, doğru bir yola hidayet ediyorsun, göklerde ve yerde bulunan her şeyin kendisine ait olduğu Allâh'ın yoluna" (Şûra s. 5253) "Bu, benim dosdoğru olan yolumdur, öyleyse ona uyunuz!" (En'âm s. 153) buyurmuştur. "Sırât-ı müstakim" insanın, Allâh (c.c.)'dan başka her şeyden yüz çevirmiş: bütün kalbi, fikri ve zikriyle Allâh (c.c.)'a yönelmiş olmasıdır. Guslün farziyyeti Kur'an-ı Kerim ile sabittir. Cenâb-ı Hâkk şöyle buyuruyor: "Cünüp olursanız iyice temizlenin." (Maide s. 6) Gusülde bedenin tamamını yıkamak farzdır. Efendimiz (s.a.v.)'in hanımı Meymune validemiz şöyle anlatıyor: "Resûlullâh (s.a.v.) ayakları dışında aynen namaz için abdest alır gibi abdest aldı. Ardından avret mahallini ve bedenine isabet eden yıkanacak şeyleri yıkadı. Sonra üzerine su döktü. Daha sonra ayaklarını uzatıp yıkadı." Buna göre gusül alan kişi önce ellerini ve avret mahallini yıkar, varsa bedendeki necaseti giderir sonra ayakları yıkamayı tehir ederek abdest alır. Daha sonra baştan başlayarak her defasında kuru yer kalmayacak şekilde bütün bedeni üç kez yıkar. Son olarak ayaklarını yıkar. Ayaklarını anlattığımız şekilde tehir etmesi gusledilen yerde ayaklara değecek şekilde müstâmel suyun birikmesinden ötürüdür. Eğer böyle bir durum söz konusu değilse tehir etmesi gerekli değildir. Suyu normal kullanmak yani ne çok israf etmek ne de çok az kullanmak, ilk yıkamada bütün azâları ovalamak, kimsenin görmeyeceği bir yerde gusletmek ve sonunda bir mendil (havlu) ile kurulanmak müstehâbtır. Yıkanmasında meşakkât olan yerler istisna edilmiştir. Dişler arasında ve diş kovuklarındaki kalıntıların çıkarılmasında meşakkât olduğundan en sahih görüş guslün sıhhâtine mani olmamalarıdır. Burunda sıvı haldeki sümüğün gusle mani olmayacağı, kuru olanın ise mani olacağı söylenmiştir. Aynı şekilde tırnaklar arasındaki macun ve benzeri yapışkan maddeler de gusle manidir. Toz ve topraklar ise gusle mani değillerdir. Abdest uzuvlarına yapışmış olan hamur, mum, çapak gibi şeyler gusle manidir. Pire veya sinek pisliği ise mani değildir. Su, saç diplerine ulaşırsa kadınların örgülü saçlarını açmalarına gerek yoktur. (Suâlli-Cevâplı İslâm Fıkhı, c.1, s.231-233)

Her kim de birini (hayatını kurtararak) yaşatırsa, sanki bütün insanları yaşatmıştır. Şüphesiz peygamberlerimiz onlara apaçık deliller getirdiler. Ama bundan sonra da onların çoğu yeryüzünde taşkınlık göstermektedirler.” (Mâide 32)Medine yahudileri Hz. Peygamber'i ve sahâbeden bazılarını öldürmek için tuzak peşindelerdi. Bu sebeple yüce Allah onlara adam öldürmenin ne kadar büyük bir cinayet olduğunu göstermek için Hz. Âdem'in iki oğlunun kıssasını anlattıktan sonra onların kutsal kitaplarında da haksız yere bir insanı öldürmenin bütün insanlığı öldürmek; bir canı kurtarmanın da bütün insanlığı kurtarmak gibi olduğunun yazılı bulunduğunu haber vermiştir.Bütün dinler, hukuk ve ahlâk sistemleri haksız yere adam öldürmenin, cana kıymanın büyük bir suç olduğunda birleşmişlerdir. Ancak bu suçu önlemek için alınan caydırıcı tedbirler farklıdır. İslâm, haksız yere adam öldürmeyi önlemek, toplumun can güvenliğini sağlamak, onları huzurlu ve mutlu yaşatmak için bu suçu işleyenlere dünyada kısas cezasını öngörmüştür.“Cenâb-ı Hak burada, tek bir canı öldürmenin, bütün insanları öldürme gibi olduğu hükmünü vermiştir. Şüphe yok ki bu sözden maksad, kasten ve haksız yere adam öldürmenin cezasını iyice anlatmaktır. Bunun bu şekilde anlatılmasındaki gaye de, yahudilerin böylesine bir tehdidin olduğunu bilmelerine rağmen peygamberlerini öldürmeye yeltenip, öldürdüklerini belirtmektir. İşte bu, onların kalplerinin son derece katı ve itaattan uzak olduklarını gösterir. Bu kıssaların anlatılmasından maksad, daha evvel de bahsettiğimiz gibi, yahudilerin Hazret-i Peygamber (sallallahü aleyhi ve sellem) ile güzide sahabelerini öldürmeye niyetlendikleri hadise ile ilgili olarak, Hazret-i Peygamber (sallallahü aleyhi ve sellem)'i bir teselli.Müslümanlara karşı savaşan kâfirin küfrü ve imandan sonra küfre girip irtidad etmesi veya haydutluk yaparak yol kesmesidir. "Allah ve Resulüne harb açanların cezası ancak öldürülmeleridir" (Mâide.33) âyeti ile muradı budur.Tek bir insanın öldürülmesini, bütün insanların birden öldürülmesine benzetmek, bir kimseyi haksız yere kasten öldürme işinin ne kadar mühim bir suç ve büyük bir günah olduğunu iyice beyân etmek içindir. Yani nasıl bütün insanların öldürülmesi herkesçe çok büyük bir cürüm ve suç ise, aynı şekilde tek bir insanın öldürülmesinin de çok büyük ve korkunç bir suç olması gerekir. Binâenaleyh bu ifâde ile kastedilen, her ikisinin de büyüklük ve vahamet bakımından birbirine benzediğini ortaya koymak olup, büyüklüğün miktarı bakımından denk olduklarını beyân etmek değildir. Bu, nasıl çok büyük ve vahîm bir suç olmasın ki? Çünkü Cenâb-ı Hak, "Kim bir mümini kasten öldürürse, cezası, içinde ebedi kalıcı olmak üzere cehennemdir..." (Nisa. 93) buyurmuştur.Bir kimse haksız yere kasten bir kimseyi öldürmeye teşebbüs ettiği zaman, şehvet ve gazap tarafını, taat tarafına tercih etmiş olur. Durum böyle olunca, bu tercih her bir insana nisbetle meydana gelmiş olur. Böylece onun kalbinde, kendisi ile arzu ettiği şeylerden herhangi biri hususunda çekişen her ferdi, gücü yettiği takdirde öldürme düşüncesi yer alır. Halbuki mü'minin hayırlar hususundaki niyetleri, amellerinden daha hayırlıdır. Bunun gibi, mü'minin şerler hususundaki niyet ve düşünceleri de, şer amellerinden daha şerlidir."Yahudilerin çoğu, bundan sonra, yani peygamberlerin gelişinden ve onlara öldürmeyi haram kılışımızdan sonra, yine de haddi aşmışlardır, yani adam öldürmenin büyük bir suç olduğuna aldırış etmemişlerdir."Âyette, "Kim de onu diriltirse, bütün insanları diriltmiş gibi olur..." buyruğundaki "onu diriltmekten" murad, o insanı yangın, boğulma, açlık, soğuk ve sıcak gibi öldürücü ve yok edici şeylerden kurtarmaktır.”

Bu gün özür dilemeyin! Siz ancak yapmakta olduklarınızın karşılığını görüyorsunuz.” 7Dünyadaki ateşler taşı yakamaz. Cehennem ateşi nasıl bir ateş ki yakıtı insan ve taş?“Keşşâfda, bu ifadeye, "Günahları terkedip, taatları yapmak ve ailenizi, kendinizi sorumlu tuttuğunuzşeylerle sorumlu tutmanız suretiyle, koruyun" manası verilmiştir. Yine bu ifadeye, "Kendinizi, nefsinizidavet ettiği şeylerden koruyun. Çünkü nefis, size kötü şeyleri emreder" manası verilmiştir."Bu ateşin başında iri gövdeli, sert tabiatlı melekler vardır." Cenâb-ı Hakk bu ifadeyle, on dokuzzebânî (cehennem bekçisi melek) ile onların yardımcılarını kastetmiştir. Bunlar, alabildiğine büyük, haşin vesert meleklerdir. Onların bu şekilde yaratılmış olmaları yadırganacak bir şey değildir. Yahut da onlar, Allah'ındüşmanlarına karşı, alabildiğine şiddetli, Allah dostlarına karşı alabildiğine merhametli oldukları için,hilkatlerinde değil de işlerinde böyle serttirler. Nitekim Hak teâlâ (mü'minleri vasfederken), "(Onlar),kâfirlere karşı alabildiğine sert; birbirlerine karşı ise son derece merhametlidirler" (Fetih 29) buyurmuştur.Ayetteki, "Ne emrolundular ise onu yaparlar" ifadesi. işin gerektirdiği ortamdan ötürü onların, çok çetin vesert olduklarına delâlet eder. Çünkü onlar Allah'ın emirlerini yerine getirme ve düşmanlarından intikamalma hususunda asla şefkatli davranmazlar. Burada, meleklerin, âhirette, Allahü teâlâ'nın emir ve yasaklarıile mükellef olmaya devam ettiklerine bir işaret vardır. Çünkü onlardan sâdır olacak isyan. Allah'ın emir veyasağına muhalefet olur."Ey kâfirler, bugün özür dilemeyin" buyurmuştur ki bu, "Onlara, "Bugün mazeret beyan etmeyin" denilecek"takdirindedir. Çünkü mazeret beyan atmek, tevbe etmek demektir. Tevbe ise, cehenneme girdikten sonra,artık makbul değildir. Binâenaleyh mazeret beyan etmek, onlara bir fayda vermez. yani, "Sizin yapmışolduğunuz o kötü amelleriniz, hikmet-i ilahiyye gereği size bu azabı gerekli kılmıştır."Allahü teâlâ, "Eğer yapamazsanız, ki kesinlikle yapamayacaksınız, o halde yakıtı insanlar ve taşlar olan oateşten korununuz. Zira o, kâfirler için hazırlanmıştır" (Bakara, 24) buyurarak, cehennemin kâfirler içinyaratıldığını bildirmiştir. Öyleyse burada, mü'minlere böyle hitap etmesinin hikmeti nedir? Deriz ki:Fasıkların derekeleri, kâfirlerin derekelerinin üstündedir. Çünkü fasıklar da kâfirlerle birlikte, aynı yerde,yani cehennemdedirler. Bundan dolayı mü'minlere, "Bu ateşin kendileri için hazırlandığı kimselerlebirlikte olmamanız için, fısk-ı fücurdan (günahtan) alabildiğine kaçınmak suretiyle kendinizi bu ateştenkoruyun" denmiştir. Bu hitapla, Cenâb-ı Hakk'ın onlara, mürtedlikten korunmalarını emretmiş olması dauzak bir ihtimal değildir.” RaziAli (radıyallahü anh), Katade ve Mücahid şöyle demişlerdir: Yaptığınız işlerle kendinizi koruyunuz, onlarayapacağınız tavsiyelerle de aile halkınızı koruyunuz.el-Kuşeyrî'nin zikrettiğine göre bu âyet-i kerîme nazil olunca, Ömer (radıyallahü anh) şöyle demiş:Ey Allah'ın Rasûlü! Haydi kendimizi koruduk diyelim. Peki aile halkımıza ne yapabiliriz?' Peygamber şöylebuyurdu: "Allah'ın size yasakladığı şeylerden onları alıkoyarsınız, Allah'ın emrettiklerini onlara daemredersiniz.""Sen aile halkına namazı emret, kendin de sabırla ona devam et" (Taha, 20/132)"Önce yakın akrabanı uyar." (eş-Şuara, 26/214)"Çocuklarınıza yedi yaşında namaz kılmalarını emrediniz. (Kılmazlarsa) on yaşında onları (hafifçe)dövünüz ve yataklarını birbirinden ayırınız." Ebû Dâvûd, I, 133; Hâkim“Hepiniz çobansınız ve hepiniz sürüsünden sorumludur. İnsanların başındaki İmâm (İslâm devletininyöneticisi) bir çobandır ve o, onlardan sorumludur. Adam aile halkı üzerinde bir çobandır ve onlardansorumludur."

Heather Bartlett, BSN, RN, CEN, CNML, is a nurse educator and supervisor in the Emergency Department at MyMichigan Medical Center Midland in Midland, Michigan. She is the person responsible for orienting every nurse, ED tech, and unit assistant who joins their team, equipping each one with the confidence, competence, and humanity to show up for patients on their worst days. Her mission is deeply personal. When Heather was 15, her father died of a massive heart attack at age 42. In the immediate aftermath, an emergency department nurse was her rock and her ally. A man in a high speed, rollover motor vehicle crash experiences a multitude of symptoms including hemorrhagic shock and profound respiratory distress. As our team assesses him, they attempt multiple needle decompressions to relieve pressure and improve breathing. None are successful.
