Reginald Santiago is an inpatient care coordinator and case manager at Duke University Hospital's Raleigh campus. Over the past 30 years in healthcare, he has served in roles as a paramedic, adjunct paramedic instructor, tactical medical instructor, and Special Operations Forces Medical Instructor specializing in prolonged field care (PFC).  Additionally, Reginald is the creator and host of the Paramedic2RN podcast. Reginald's educational background includes an Associate's degree of Applied Sciences in Emergency Medical Science, an ADN from a Paramedic to RN bridge program, and as well as his BSN. Reginald is currently pursuing an MSN in Adult-Gerontology as a Clinical Nurse Specialist at East Carolina University. He holds board certifications as a CEN, TCRN, CCRN, CFRN, CTRN, and is a national registry paramedic. Transitioning between careers can be difficult and Reggie is no stranger to difficult challenges. On episode 501 of The Nurse Keith Show nursing and healthcare career podcast, Keith interviews Dr. Christine Frazer, PhD, CNS, CNE, a Clinical Nurse Specialist, Certified Nurse Educator, and academic educator. Throughout her nursing journey, Dr. Frazer has worked in various fields including neonatal ICU, pediatrics, post-partum, adult critical care, cardiac care, emergency, gerontology, and academia. In the course of their conversation, Keith and Dr. Frazer dig deep into how microaggressions and implicit bias can negatively impact the nursing profession, patient care, and the larger healthcare industry, and what we can do about these troubling phenomena. Over the last two decades, Dr. Frazer has taught in all degree-level programs and played a vital role in developing and coordinating new programs. In 2008, Dr. Frazer joined Walden University's College of Nursing and teaches in the graduate nursing program. She is a distinguished recipient of the university's Faculty Excellence Award and serves the university and College of Nursing by serving on various committees and by leading groups. Before coming to Walden, Dr. Frazer taught for nearly a decade at Penn State University, and served as Coordinator for their Clinical Nurse Specialist program. Dr. Frazer was one of ten nurse professionals selected and inducted into the 2023-2024 National League of Nursing year-long Leadership Academy. During her time with the academy, Dr. Frazer developed an Implicit Bias toolkit, which guides nurse educators in directly influencing the trajectory of nursing students' lives, advancing the greater good of healthcare delivery, and creating a more socially just society. Throughout Dr. Frazer's professional career, she has researched and published in peer-reviewed journals on a variety of topics including nurse burnout, student learning, teaching effectiveness, online learning, leadership, neonatal, pediatrics, and critical care nursing. Currently, Dr. Frazer serves on the Editorial Board of Teaching and Learning in Nursing, the Official Journal of the Organization for Associate Degree Nursing. Sepsis it seems is always in the news, never far from anyone who works in CDI or coding circles. And here we are again, back in the headlines due to its inclusion in the Office of Inspector General (OIG) Work Plan. Sepsis has been discussed so much due to the complexity of disease, the cost of treatment, complexity of coding, and increased regulatory spotlight. What else is there to say? But with it back in the spotlight I wanted to turn from the usual insular conversations in the mid-revenue cycle to someone working on the front lines of care. Amanda Hart is an ED nurse and a sepsis clinical nurse specialist at Tower Health, a regional integrated healthcare system based out of Pennsylvania. She spends a substantial amount of time battling this deadly disease, and her work has much to offer anyone who works to get it accurately documented and coded. On this episode we discuss: • Amanda's background in the military and EMS, path into nursing and eventually the ED • Her role as sepsis clinical nurse specialist and how it overlaps with CDI/coding • Pathophysiology—clinical indicators of sepsis and what makes it such a deadly disease • Problems inherent in lack of uniform definitions, what Tower Health uses, and her own take on Sepsis-2 vs. Sepsis-3 debate • The importance of early screening, including processes, technologies and people. In previous episodes, Talking About Kids has addressed the need for trauma-informed care and the importance of involving youth in the design and oversight of programs for them. This episode combines the two. Recently, Yehudis Stokes, Marjorie Robb, and their colleagues published a paper on the perspectives of youth, caregivers, and other stakeholders on implementing trauma-informed care in an inpatient mental health program. Yehudis Stokes is a Registered Nurse at the Children's Hospital of Eastern Ontario and a Clinical Nurse Specialist with the Psychosocial Oncology team at Princess Margaret Cancer Centre, and Marjorie Robb is the Acting Chief of Psychiatry at Children's Hospital of Eastern Ontario and the Division Chief of Community-Based Psychiatry. The advice comes as 3,000 people are hospitalised with COPD, or chronic obstructive pulmonary disease, ever year across Clare, Limerick and North Tipperary. The condition accounts for up to 45% of patients attending respiratory clinics and usually affects people over the age of 40 who smoke or have smoked for an extended period. Clinical Nurse Specialist at University Hospital Limerick Noelle Ryan says in addition to refraining from smoking and using inhalers, a special programme of exercise can be an effective way of fighting the disease.

Jacob Miller is a Nurse Practitioner, Clinical Nurse Specialist, and Nationally Registered Paramedic with a background in flight, critical care transport, and emergency nursing, in addition to over 15 years of EMS practice. Jacob received his Master of Science in acute care from the University of Maryland, a post-master's certificate in advanced flight nursing from Case Western Reserve University, and his Doctorate in Nursing Practice from the University of Cincinnati. Jacob currently works as a clinical educator and critical care transport clinician in Cincinnati, Ohio. Jacob holds multiple certifications including the CFRN and CTRN from BCEN. He is active in several professional organizations including serving as an at-large director on the Air & Surface Transport Nurses Association board of directors, chair of the Emergency Nurses Association's advanced practice advisory council, and co-chair for the EMS Special Interest Group with the American Academy of Emergency Nurse Practitioners. One of Jacob's recent accomplishments is being accepted as a Fellow in the Academy of Emergency Nursing. The American Heart Association relayed that the 1960s saw the first rise of coronary care units (CCU) in response to heart attacks. Today, cardiac ICUs, both medical and surgical, provide intensive care to a breadth of fatalities, ranging from arrest to post-open heart surgeries. The CVICU is seen to be a home for a "special breed" of nurses with its required list of complex skillsets, such as the titration of vasopressors drips and management of life support and assistive devices (e.g., ventilators, chest tubes, LVAD, ECMO, IABP, CVVH). How true is the "toxic" environment of surgical ICUs? What is the main reason for extended patient stay in the ICUs? Why are CVICU nurses usually given a bad rep on social media?We are joined today by Dr. Danielle LeVeck, a dual board-certified Adult-Gerontology Acute Care Nurse Practitioner and Clinical Nurse Specialist. She received her BA in Communications from Purdue University in 2007, second degree of BS in Nursing from Indiana University in 2011, and DNP from the University of Maryland in 2018, with her doctoral research focusing on implementing palliative care for patients with end-stage heart failure. PREVALENCE - Of 5.8 ICU admissions, 4.8 million survive - Of the 4.8 million survivors, 50-80% will beet diagnostic criteria - COVID has brought PICS to forefront. In the course of their conversation, Keith and Dr. Whitehead discuss the role of the CNS, how to become a CNS, and the goals of the NACNS to diversify the CNS profession. Dr. Phyllis Whitehead is a clinical ethicist and clinical nurse specialist with the Carilion Roanoke Memorial Hospital (CRMH) Palliative Care Service and Associate Professor at the Virginia Tech Carilion School of Medicine. She initiated the Palliative Medicine and Moral Distress Consult Services at CRMH, and is certified in pain management and as an advanced practice hospice and palliative care nurse. Dr. Whitehead is an expert presenter on the subjects of pain and symptom management, opioid-induced sedation, moral distress, and patients' end of life preferences. Her research interests include moral distress and improving communication with seriously ill patients, and her research has received funding from numerous grants. In 2020, Dr. Whitehead was elected as a Distinguished Practitioner Fellow in the National Academy of Practice in Nursing. She is a graduate of Radford University where she earned her BSN and MSN, and earned her doctorate degree at Virginia Tech. Our guest for this episode is Alice Benjamin, a board-certified Clinical Nurse Specialist and Family Nurse Practitioner, as well as an author, podcast host, health advocate and a regular on-air medical contributor for CNBC, Dr. Oz, The Doctors, CNN and the BBC.  Alice's career as a nurse was inspired by the death of her father from a heart attack when she was a child. Since then she's been on a mission to educate and empower the public about the pursuit of health and wellness. Alice was the first nurse to chair the American Heart Association Health Equity Taskforce, as well as the first black nurse to serve on the California board of directors for the American Nurses Association. Today she and Dan talk about her career, the portrayal of nurses in the media and patient safety. As a nurse who is still at the bedside and in the wake of the Randa Vaughnt case, this is a topic that Alice is particularly passionate about, and they get into the frequency of near misses in healthcare, why the “just get it done” culture of nursing is ripe for errors, and how nurses can engage with hospital leadership to help create a safer environment for everyone.
