For the second year, Yvonne and Rafael identify ten issues to frame their conversations with guests as they kick off a new season. Here we go with Season 4! Are you ready? Our issues are...Provider Shortages - Healthcare jobs are growing faster than any other industry, yet there is a shortage of experienced doctors and clinicians to fill the openings. This is straining the entire healthcare system; we certainly feel it in workers' compensation. https://kffhealthnews.org/news/article/health-202-primary-care-shortage-data/https://fortune.com/2024/02/14/america-shortage-primary-care-doctors-work-ai-physician-burnout-crisis/Chronicity - definition: the fact or quality of being long-lasting, as a disease, condition, or symptom; psychological influences, such as stress, can significantly affect the severity and chronicity of the illness (or injury). Is workers' compensation contributing to chronicity? We are going to discuss it. Economics of Inflation - we carried this one over from last year as inflation persists. There is talk that we may be at a tipping point regarding how inflated medical costs impact healthcare, particularly with hospitals and other facilities. Mental health and well-being - this is a favorite topic of ours. We are happy to see this issue becoming more commonplace at conferences and events in the workers' compensation industry. Embracing AI - the future is here, and it is time to embrace how workers' compensation can benefit from artificial intelligence. How is AI already being used in WC, and how can it help us do the work smarter, faster, and more efficiently? Need for good education - this is hands down one of our favorite topics to discuss. It's not to say there isn't any good education out there, but we need more. We aim to uncover resources and encourage more good education. Acknowledging bias - we all have it. Are you aware of your unconscious bias? How does your bias impact your day-to-day decisions in WC, regardless of your role? How do your decisions affect a person's life (and the family attached to that person)?Federal benefits in workers' compensation - this will be one of Rafael's favorite issues because it is where he spends his time every single day! Here, we will discuss federal laws and processes that crossover into workers' compensation: Medicare, Medicaid, Social Security, ERISA, Veteran's Admin, tri-care, the Affordable Care Act, and more. Expanding workplace issues - there are so many! Starting with the ongoing talent shortage, workplace violence, our changing workforce - telecommuters, gig workers, aging workforce, high-stress levels, lack of leadership, need for hybrid work strategies (work at home vs. office), rules and regs + expanding presumptions, safety issues, inexperienced workers and so much more! https://www.uschamber.com/workforce/understanding-americas-labor-shortageDiversity, Equity, Inclusion and Belonging (DEIB) - one of our favorite topics to explore together. DEIB conversations have fizzled; some refer to diversity "fatigue," but we will not let it go. Rest assured, we will have several guests on to talk about DEIB. In Texas and Florida, DEI is under attack, and it is in other states as well. https://www.axios.com/2024/01/04/dei-jobs-diversity-corporateBONUS! Presidential election year. There is much to discuss as we enter the election year cycle. Who wants to talk politics?¡Muchas Gracias! Thank you for listening. We would appreciate you sharing our podcast with your friends on social media. Find Yvonne and Rafael on Linked In or follow us on Twitter @deconstructcomp

J., Serino, M., Moqrich, A. Abstract: Over the past decade, the gut microbiota has emerged as an important regulator of nervous system's health and disease states. Yet, its contribution to the pathogenesis of chronic somatic pain remains poorly documented. Chronic pain is a heavily debilitating disease affecting more than 1.5 billion people worldwide that can manifest through a long-lasting hypersensitivity to mechanical and/or thermal stimulations. Maladaptive responses of dorsal root ganglia (DRG) neurons and spinal cord (SC) interneurons to tissue injuries and also of non-neuronal cells including DRG macrophages and SC microglia are acknowledged as important drivers of sensory symptoms underlying chronic pain. Recent evidence shows that signals from gut microbiota are required for the initiation of injury-induced sensory hypersensitivity, via the ability to interact with the immune system. However, whether and how gut microbiota promotes pain chronicity remains unknown. Here, we report that male mice lacking Myosin1a (KO) raised under single genotype housing conditions (KO-SGH) are predisposed to develop chronic injury-induced mechanical pain. We demonstrate that this predisposition is caused by their dysbiotic gut microbiota, which sustains the immune response in the DRG following neuropathic injury. Parental antibiotic treatment modifies gut microbiota composition and completely rescues the injury-induced chronic pain and associated DRG inflammatory response in male KO-SGH offspring. Together, our data establish a causal relationship between a dysbiotic gut microbiota and the predisposition to injury-induced chronic pain. Copy rights belong to original authors. Australian academic, writer, and poet, Michael J. Leach reads from and talks about his latest poetry book Chronicity. We talk about such things as the relationship between the visual/concrete poems on the page and their sonic qualities in live readings, the sensual, visceral nature of the work, how he chooses and works with constraints, his use of humour and the way he plays scientific precision against emotion, the way poetry enables him, as a scientist, to work better with the complexities of the real world, and lots more. Welcome to Episode 52 of Nooks and Crannies! Chronicity Teaser or Two Scoops of Chronic Illness with Other Paul and Moxie :::::::::::::::::::::::::::::::::::::::::::::::::::::::::::::: Well howdy there folks, we have a meaty course for you all today in Chronic Illness Narratives, Care-Giving and Experience! I promise the next few will be lighter. I have jumped back into transcribing the Chronicity Series *17 interviews with chronic illness sufferers mashed up into an 8 part series, so I must have had pain and illness on my mind when I recorded both of these interviews on the same day, with Moxie (http://yourbrainonfacts.com/) and Other Paul from Varmints! which he co-hosts with the amazing Donna (https://www.podchaser.com/podcasts/varmints-230694) Paul kicks things off by talking to Matty about his migraines and being diagnosed Bi-Polar 2 years ago before some new interlude music from Cullah *links below (13:30), then it’s Moxie’s turn to talk Care-Giving to a person with a severe pain condition and the strains it puts on relationships. More new interlude music (28:52) Then ol’ Capt. Concussion dishes reflexively on, you guessed it, CONCUSSIONS! Oh you betchya there is more interlude music, (42:30) before Moxie tells us why her hips certainly don’t lie and why Cats are such loveable bastards. Welcome to Episode 29 of Nooks and Crannies! 29–Maple Syrup’ed LED Misunderstandings and the Rise of German Fascism :::::::::::::::::::::::::::::::::::::::::::::::::::::::::::::: Welcome folks to another Dazzling episode of Nooks and Crannies! Albeit a slightly more downcast endeavor (0:24) (3:08) To keep this promise Evan informs Matt of the reemergence of German-Style Fascism in the recent regional elections in Thuringia. We spin this into a discussion of societal apathy, the rise of polemic populists, how off-base Francis Fukuyama was and the ways xenophobia is used to deepen existing divides. Fun! (17:52): Pull up an arm chair, stoke that fire and adopt a pretentious expression…it’s time for another Canadian Heritage Minute with Evan and Matty! This week we have Jacques Cartier Misnaming Canada and the “Discovery” of Maple Syrup by apparently a bunch of starving settler colonialists, or something. And here is a link to that random story Matt stumbled upon about the Oklahoma Farmers turned away by the Canadian government in the mid 1800’s (https://www.youtube.com/watch?v=M9h51JxiC6U ) (24:41) Hey is that an prematurely Old Man on a tin roof shouting at the moon?!? Nah, its just old man Matty with his surprisingly well researched rant on the increasing prevalence of LED headlights and the dangers they pose to old folks like himself. Here are the sources for my irrational anger: https://www.telegraph.co.uk/news/2018/03/24/drivers-complain-dazzling-headlights-led-bulbs-installed-cars/ https://www.nhtsa.gov/sites/nhtsa.dot.gov/files/vanderlofske.pdf https://www.theguardian.pe.ca/news/provincial/rcmp-warns-about-use-of-aftermarket-led-lights-1521/ https://www.nytimes.com/2008/01/27/automobiles/27LIGHT.html https://www.psychologicalscience.org/news/minds-business/cognition-at-the-speed-of-led-lights.html https://www.cnn.com/2019/05/16/health/blue-light-led-health-effects-bn-trnd/index.html https://www.ledsmagazine.com/smart-lighting-iot/smart-cities/article/16699866/light-and-human-health-led-risks-highlighted https://www.researchgate.net/publication/296621681_A_summary_of_LED_lighting_impacts_on_health (41:18): And to put a cap on this one, Matty in rather graphic detail, describes to Evan the nature and dimensionality of his long standing (pun absolutely intended) Knee injury, the physio he just got, for some reason Matt ties in mind-body dialectical phenomenology, all in a sad attempt to segue into an update on the Chronicity series *halfway done the transcriptions, like 30% done on the whole series. Be well, check you on the flip side, aren’t you lucky?! **If you read this far, congrats..as a special reward you are the first to know that we have another minisode upcoming! Welcome to Episode 21 of Nooks and Crannies! Apologetic Antiquing, Challenging US Military Norms and Livestream 3.0 Redux Welcome back folks and thanks for staying subscribed :) Evan tries to kick things off this week but failed miserably, instead Matty was forced to shoehorn in a discussion of oolong tea and acupuncture, which he deftly maneuvered towards an in-depth conversation about his Favorite Post-Concussion Show: Antiques Roadshow UK! (10:22) We hear from the thought-filled former Host of the sorely missed podcast: Spouse Spouts, Susan Reynolds, whose show was dedicated to advocacy and awareness around the issues faced by Military Families. This segment was taken from the start of her Chronicity interview, we were talking about the nature of her podcast and, no surprise here with Susan, she launches into a fascinating discussion about the challenges and misconceptions of the challenges faced by US Service Personnel in regards to their mental health. I have honestly never heard this “take” before but am not surprised that Susan would have made such a thoughtful connection. Please re-launch Spouse Spouts!! :) (22:00) For those of you who missed the 3rd Annual Livestream for the Cure, podcastathon to raise funds for the Cancer Research Institute https://fundraise.cancerresearch.org/fundraiser/1947095) There is still time to donate if you feel so inclined and are able. In this episode we will talk about taking verbal and non verbal cues from patients. These cues can make or break the future of this doctor/patient relationship. We also dive into pulling out the Chronicity of a problem to prevent minimizing.
Matt sat down with disability advocate, educator, public speaker and podcaster Jahmeelah Gamble. If you like going down rabbit holes, this is the episode for you. We chatted about balancing conflicting cultural expectations in an educational context, accidently landing a TV show for 6 seasons and the ways Jahmeelah has endeavored to be a humanist rather than a knight in shining armor. Matt shares a little confession, while Jahm talks about her brand. Spoiler: it’s intersectional-reflexive applied empathetic-humanism! Matt also gets Jahm to explain how and why she became such a strong education advocate and how advocacy extends well beyond the classroom to permeate all aspects of life. As a public speaker, Jahmeelah shies away from intentionally trying to inspire while being mindful of the power in setting an example. Wrapping up the conversation, Matt explores how Jahm’s faith intersects with her speaking and advocacy. Jahm gives us an impassioned description of the struggles facing educational aid workers in Canada. Jahmeelah’s message of empowerment and positivity is downright infectious, so be sure to check out the Slay the Mic Podcast. There comes a moment in time when the things that we could once do, the things that we did easily and without much effort, become more difficult. Maybe even impossible. It is in those moments that the reality of what chronic pain and illness can do to the body and mind becomes apparent. The experience of the first episode or attack is now interwoven into a web that has transformed our lives. This episode tells the stories of first encounters with what would become a chronic pain or illness. The act of remembering pain and illness, however, is not linear: the past becomes the present, and the future is a mixed bag of unknowns. Living in chronicity entails loathing time, but also cherishing it more than anything else. Like a stich, both time and our bodies have begun to unravel.
