Seabury Resources For Aging Community Relations Manager Jennifer Brown joined Raquel and Ryan on The Senior Soup Podcast to discuss geriatric care management, which is also known as Aging Life Care Management.  Founded in 1924 in Washington, D.C., Searbuy Resources began as a women's retirement home with deep roots in the faith-based community. Next April, Seabury will celebrate its 100th birthday - and yes, they are throwing a big party!  At the beginning of the podcast, Jennifer discussed her background in the healthcare industry and her role as Seabury's community relations manager. Jennifer discussed the care management services that Seabury offers older adults, and she shared the appropriate time for someone to reach out to a care manager for help with their aging loved ones.  The care management field is relatively new, Jennifer explained. It started locally about 30 years ago and is a specialty area of licensed social workers who are available to assist older adults and their families with various care needs, social needs, and housing needs. Care managers are hired to provide the same care and support that a next of kin would do for an older adult with no next of kin or a family far away. Care managers develop care plans to connect older adults and their families with home care agencies, primary care physicians, specialists, therapists, elder law attorneys, daily money managers, house cleaning services, and so many other resources.  Aging life care managers also advocate for older adults, ensuring that their needs and safety are considered. They help mediate and arbitrate between family members who may have different opinions about the best course of action for an older adult.  Families can contact aging life care managers when an older adult's situation declines or if the extended family is having difficulty managing their loved one's care. Care managers' hourly rates run between $150 to $170. It was 5:30 in the afternoon and dinner was in the oven. When I told her it was in the late afternoon, a look of confusion crossed her face that I will never forget. That moment was frozen in time. Sure there were the little things that I had noticed before, like “where did I put the keys”, or “what time are we going to the doctor?” But as things progressed I began to think, ‘is there something not right?'.  But on this afternoon, I knew ‘something was wrong this was the beginning of our journey with Mama's battle with dementia.Over the next 11 years, we cared for our dear Mama at home, helping her with small things that grew into big things. She was at times agitated and demanding. Her mood swings were enough to cause whiplash. She pushed our patience at times to a point that we had to just “get a break” from the constant care. But, we also learned that the heart knows no limits to love. Over time as her condition worsened, we knew we needed outside help to manage her care.​As we looked for caregivers and doctors to help we found it very difficult to find a comprehensive or cohesive approach to help us manage my mother-in-law's overall care and in particular her dementia-related care needs.Why Premier Care Management?After this experience and over a decade of working with seniors as an RN, and Executive Director in independent, assisted, and memory care communities, I turned this knowledge, experience, and passion exclusively to the practice of Geriatric Care Management. Social connection has long been associated with good quality of life, as well as positive physical and mental health outcomes for seniors. With the COVID-19 pandemic, most seniors are struggling to balance that quality of life with life itself. Social isolation and loneliness were already prevalent in our senior population. Today even those living in retirement communities are advised to stay-at-home, which is exacerbating an already serious problem. Families and professionals are seeing a significant increase in isolation related problems such as depression, physical/cognitive decline and emotional distress. To address this growing crisis, I will be joined by Crystal Littlejohn, a Geriatric Care Manager and founder of a program specifically designed to address the need for engagement with older adults. We will discuss social isolation, its' impact on seniors and ways that thoughtfully engaging loved ones can benefit their overall well-being.

Episode 7: Living Well with DementiaDementia - changes in memory and thinking that are beyond what is normal for age. It is progressive (worsens over time).Treatment can slow progression of dementia, but there is no cure. It is felt that regular exercise, following a healthy diet such as the Mediterranean diet, and staying socially and cognitively engaged can help prevent dementia. Controlling chronic diseases such as high blood pressure, high cholesterol, and diabetes can also help with prevention.Today we are joined by Deb Kaul, the owner of geriatric consulting service “Dignity Care” and one of the Co-founders of “Memory Café of the Red River Valley.” Deb has a bachelors degree in Business Administration from UND and a BSN from the University of Mary. She also has a Master’s certificate in Geriatric Care Management from the University of Florida. Deb considers the lessons she learned while caring for her parents - both of whom lived with dementia, the most valuable education she has ever received.Deb provides recommendations for living with and caring for people living with dementia:Keeping the diagnosis of dementia a secret from the person with dementia robs the individual of their autonomy. It also perpetuates the stigma related to dementia.It is possible to live well with dementia.Our responses to the diagnosis impact how a person responds to the disease. If they are diminished and de-humanized, they lose hope.How to engage with a person living with memory loss: Greet them with a compliment or information that reminds them about their life. Tell them their story (“That is a beautiful/handsome sweater.” or “You did such a great job raising your children.”) Avoid quizzing them (“Did ___ come to visit you this morning?”)For caregivers: Communicate about what’s occurring. Allow the person with memory loss to set goals for themselves. Avoid arguments with people living with dementia. They lose the ability to rationalize and reason, and it’s our job as caregivers to think creatively to solve problems. Caregivers can apologize, redirect, distract to work around conflicts or disagreements “Live their truth.” People with dementia are living in the present moment, so living with them in their truth/reality can help them and reduce conflicts. Engage trustworthy friends to build a village of support around them. People living with dementia should continue to have opportunities to make friends and maintain some independence. Communicate goals with physicians and other medical providers. Continue to pursue joy in life (both caregiver and person living with dementia).Available resources:“What the Hell Happened to my Brain?” by Kate Swaffer. The author’s diagnosis resulted in “prescribed disengagement” by her physician. She was told to “get her affairs in order” when diagnosed at age 49. It is recommended that this group gets revaccinated.If you were born in the 1950s or early 1960s, you may not have been vaccinated. Individuals born before 1957 are presumed to have immunity (likely had measles infection and so are immune to further infections) and do not need the vaccine (there are exceptions to this).It’s safe to revaccinate if you’re uncertain of your vaccine history. Healthcare workers who received the vaccine before 1967 or didn’t receive it should be vaccinated. Geriatric Care Managers or Aging Care Specialists, are an important member of an elder's care team. They are the conduit between the patient, family, and medical team. They assist is determining the best use of resources based on individual needs and because they go into the home, they can see the big picture. They advocate for the patient with discussion about what the patient's wishes are. They can be nurses, social workers, or elder care workers with a special certification. They can work for insurance companies, physicians, or can be independently employed. Tara Cunningham RN, shares her perspective as an eight year veteran Geriatric Care Manager and as a nurse. Martha & Mary has served the West Sound region for the last 124 years. This not-for-profit organization currently serves people from 4 weeks old to end of life with child care, early childhood education, senior housing, in-home services to help clients stay independent; and physical rehabilitation, long term care and memory care in a skilled nursing setting.   Betty, however has only served seniors for the last 35 years in all capacities from individual caregiver to Care Director. She joined Martha and Mary in December 2010.  She speaks often on the topics of The Continuum of Care, From Full Independence to Last Breath, Fall Prevention and Senior Housing Options.
