This week, our hosts travel to the annual American Veterinary Medical Association (AVMA) convention in Austin, Texas. Discover the heated debates and critical updates on veterinary medical ethics arising from the AVMA's House of Delegates approval of a recently revised Principles of Veterinary Medical Ethics (PVME). Join hosts Dr. Ernie Ward and Becky Mosser, MPA, RVT, as they delve into some of the changes to the PVME that they believe will affect many veterinary professionals. Our duo starts by discussing the newly-introduced ethical usage by veterinarians of the term “veterinary technician,” some significant changes to the ethical definition of “evidence-based medicine,” why the omission of a single one matters to them, and the controversial topic of animal depopulation and ventilation shutdown (VSD+) receives an ethical okay from the AVMA HOD. The controversies began before the official opening of the 2023 AVMA Convention in Denver. One debate that resurfaced this year was expanding the scope of practice, review of licensed veterinary technicians' roles, and their permitted professional duties. Despite having no regulatory or enforcement abilities, the AVMA proclaimed it would pledge “to defend against expanding the scope of practice,” particularly in the role of a proposed midlevel practitioner. Unfortunately, the discussions quickly grew to include debates about preventing any establishment of a VCPR by a licensed veterinary technician, going against legislation passed in several states. Let's dive into the world of veterinary mentorship with today's guest, Dr. Kate Boatwright. An experienced small animal general practice and emergency veterinarian, Dr. Boatwright is also a celebrated speaker and author. She walks us through the importance of mentorship in the veterinary field, the hurdles new graduates often face, and the idea of a mentorship agreement. We explore her latest book, The Veterinary Mentorship Manual, a comprehensive guide for establishing effective mentorship programs within practices. Furthermore, Dr. Boatwright paints an inspirational picture of her shift into ER, highlighting the significant role a supportive environment played in shaping her journey....More about Dr. Boatright!Dr. Kate Boatright is a 2013 graduate of the University of Pennsylvania and has worked in small animal practice since graduating. She has worked at multiple practices in Pennsylvania and is currently employed part-time in a rural general practice clinic (Penn Ohio Veterinary Services) and provides limited relief services to emergency clinics. Her clinical interests include feline medicine, internal medicine, surgery, and emergency medicine. She also supervises and mentors pre-veterinary and veterinary student shadows as well as new graduate veterinarians. During veterinary school, she was the 2011 National VBMA Vice President and led the development of the original business certificate program with fellow board members. She remains involved with the VBMA as an Honors portfolio reviewer, speaker, and founding member of the VBMA Alumni. In 2018, she began freelance writing and speaking. She wrote the Finding Balance column for Today's Veterinary Practice in 2020 and 2021 and the Beyond the Clinic Column in 2022. She regularly writes for dvm360 and AAHA publications, including a new column on spectrum of care for NEWStat. She published the Veterinary Mentorship Manual in March 2023 to support individual veterinarians and clinics in providing strong mentorship to early career veterinarians. Dr. Boatright has spoken both live and virtually to numerous veterinary schools and at state and national conferences, including dvm360® Fetch conferences, AVMA Convention, AVMA's Veterinary Leadership Conference, and ACVIM. She founded KMB Veterinary Media LLC in 2021 to continue her work in educating veterinary professionals through speaking, writing, and mentorship. In addition to her clinical work, she has been active in organized veterinary medicine since shortly after graduation. She serves as the secretary for her local association (NWPVMA), has been a District Trustee on the PVMA Board, and currently serves as the alternate delegate for Pennsylvania to the AVMA's House of Delegates. In case you missed the first AVMA convention after the COVID hiatus, Dr. Heller and his guests -- Amy Lloyd, VMD, Chaundra Schofield, VMD, and Kelly Dunham, DVM -- cherry pick some of the interesting tidbits they gathered from the lectures they attended in Philly. Show notes: Toxicity: a. Don't ever give cats hydrogen peroxide!!!! Hemorrhagic gastroenteritis/death. NO SAFE AT HOME REMEDY in cats. b. Use dexdomitor 7mcg/kg IM to cause 81% of cats to puke. Hydromorphone SQ can also work 75% of the time. Xylazine less likely. c. Cholestyramine is a great drug to use when trying to detox a patient who ingested a drug or poison that undergoes enterohepatic recirculation. It's a bile acid sequestrant. d. ILE – Intravenous Lipid Emulsion – helps draw out fat soluble toxicants. (most commonly made of soy bean oil) – great for marijuana toxicosis. Hyperlipidemia: a. Fenofibrate (micronized form) 4-10mg/kg BID – should resolve 80-100% of cases by 1 month. Will require lifelong therapy. b. Low fat diet will treat only 25% of cases. Dermatology update: a. Chlorpactin – study showed powdered chlorine 0.4% solution kills MRSP topically in just 10 secs. Chronic UTI: Nitrofurantoin – given nightly for 4 months can help with deep seeded chronic infections. Feline arthritis: a. Dr. Kristen Shaw gave some great resources for both vets and owners to help to identify cats affected by arthritis during her lecture: i. zoetisus.com/oa-pain/feline-exam-videos.aspx (you can also link to dog OA resources from here) ii. Catoachecklist.com iii. Solensiavetteam.com Carrie Jurney gave a lecture on CPR for the veterinary soul – she broke down veterinary personality types, individual tendencies as well as common work place stressors. a. American foundation for suicide prevention – https://afsp.org/ d. www.betterhelp.com/nomv - Get matched with a licensed therapist and enjoy a FREE MONTH of professional online counseling e. Psychology today's therapist finder Many filters including language, gender (including option or non-binary), insurance type, price, ethnicity served, sexuality, and faith Injection site sarcomas a. Can include extraskeletal osteosarcoma (I did not know this!) b. 1-2-3 rule: 1. Increasing in size after 1 month 2. Larger than 2 cm 3. Still present 3 months after vaccine c. 42% recurrence rate with COMPLETE EXCISION d. 21.7% metastatic rate Mary Telle – Keratoconjunctivitis Sicca a. **Neurogenic can be caused by long-term otic preparations like Osurnia and Claro** i. Other causes include trauma, neoplasia, otitis, hypothyroidism ii. Usually unilateral and associated with a crusty ipsilateral nostril iii. Pilocarpine 2% ophthalmic solution given orally at a dose of 1 drop per 10 kgs PO bid to start, but may need to increase based on response iv. Since it is a parasympathomimetic drug, need to watch for toxicity including salivation, vomiting, diarrhea, bradycardia, and weakness

Dr. Jon Congdon shares his insight into how to navigate your way to the top of a specialized field while maintaining your relationships, and sanity. Dr. Congdon started from the bottom cleaning cages and worked his way up to being Director of Anesthesiology at one of the top 3 schools in the country, NC State Veterinary. He is one of only 230 board-certified veterinary anesthesiologists in the country, one of my former classmates and a personal friend. Recorded live on location at the 2018 AVMA Convention in Denver Colorado, we discuss the progression of veterinary medicine, the highs and lows of being a specialist, and how to accept vulnerability in your knowledge. Dr. Congdon lays out his pragmatic practical approach to how to make everybody’s veterinary experience better with anesthesia. Getting into the meat of his veterinary story, the struggle of residency and the toll his career had on his relationships, Dr. Congdon relays his advice for those starting out on their journey and anyone interested in anesthesia. Putting importance on learning to think for yourself, searching for information and being prepared, Dr. Congdon brings a no judgment attitude to the field of anesthesiology. And they may make mistakes and it may look bad from the outside, but it's not like any of us are trying to make mistakes or are trying to be lazy, we're just busy. And cases come to us that we think are straightforward and then they are not and we have to adapt on the fly. This idea of community and this podcast is a huge part of that.” (40:46) “Anesthesia is super cool for me because I can identify a problem, I can administer a treatment, and I get to fix it right away. You’re probably already aware that your mindset determines a lot of what you experience in life, including your career in vet med. But how you create and nurture that positive mindset? So many of us struggle with issues like impostor syndrome, guilt, general negativity and so much more that affect our relationships, careers, and lives, and today, Dr. Cherie Buisson walks us through how to break out of a mental rut and craft an attitude that will pay dividends in all aspects of your life. At the recent AVMA Convention in Denver, Colorado I’ve have had the opportunity to meet so many wonderful new people, including Dr. Buisson. Dr. Buisson is a certified hospice and palliative care veterinarian, participates in Helping Hands Pet Hospice and founder of A Happy Vet. After deciding that private practice wasn’t for her, she practiced as the medical director for a shelter before finding her true passion in hospice and palliative care, and has never been happier. And thank goodness I finally figured out what I am supposed to be doing, but it was a long road. So A Happy Vet is kind of dedicated to hoping that someone else won't waste 8 years of their life trying to do something that doesn't suit them, and realizing that there are all kinds of things you can do in vet med.” (3:57) “It's all of us, we all feel like ‘maybe I just snuck in under the radar’ and it just cracks me up because we think we are not smart enough to get into vet school and deserve this career, but we think we were smart enough to fool everybody.” (16:43) “I would cut open a patient and go, ‘I can cut something and sew it back up and it usually doesn't die’. Every now and then it strikes me, and it still does today, 18 years later, that this is such an amazing profession.” (23:33) “If you had $100 and you lost $2 would you spend $98 to get the $2 back? And that's what we do. Every single day we waste 98% of the good stuff that happens, harping on the 2% that was lousy. And that's what we tend to remember and I say we chew on it till our teeth hurt, and we don't think of the good stuff.” (25:58) “Its part of the trap of hospice care and palliative medicine is you get them feeling better and then you forget that you haven't fixed them, you are just making them feel better. And so then when they do die, you have to get used to the fact that they are supposed to die. But that was really hard for me when I had such dramatic improvement and then to lose them felt like a failure. And so I had a hard time adjusting to that, so again that's what I teach, ‘this is what I am struggling with, how about you?’” (44:34) “Because we are perfectionists we tend to turn down things that we don’t know that were are good at. And those are the things we should be pursuing. You might not realize, however, the impact of our career pursuits on other areas of our lives. Dr. Jon Congdon shares his insight into how to navigate your way to the top of a specialized field while maintaining your relationships, and sanity. Dr. Congdon started from the bottom cleaning cages and worked his way up to being Director of Anesthesiology at one of the top 3 schools in the country, NC State Veterinary. He is one of only 230 board-certified veterinary anesthesiologists in the country, one of my former classmates and a personal friend. Recorded live on location at the 2018 AVMA Convention in Denver Colorado, we discuss the progression of veterinary medicine, the highs and lows of being a specialist, and how to accept vulnerability in your knowledge. Dr. Congdon lays out his pragmatic practical approach to how to make everybody’s veterinary experience better with anesthesia. Getting into the meat of his veterinary story, the struggle of residency and the toll his career had on his relationships, Dr. Congdon relays his advice for those starting out on their journey and anyone interested in anesthesia. And cases come to us that we think are straightforward and then they are not and we have to adapt on the fly. This idea of community and this podcast is a huge part of that.” (40:46) “Anesthesia is super cool for me because I can identify a problem, I can administer a treatment, and I get to fix it right away. Anesthesia, it’s real-time physiology.” (1:04:54) “You will never get the credit. You will never be at the forefront of the story. But it shouldn't matter. We are not here to get accolades, we're here to get them through. We're not here to judge if they should be anesthetized, we're here to get them through. Other doctors are having those conversations with the clients, it's not our role to judge.
