In this high-energy episode recorded live at the 2026 Chicago Midwinter Dental Meeting, Alan once again sits down with Whitney DiFoggio, better known as teethtalkgirl to discuss the evolving landscape of dental communication. Whitney shares her journey from YouTube pioneer to a leading voice in dental public health, emphasizing her "relatability equation": how building trust through shared interests leads to better patient compliance. The duo dives into the dangers of social media "DIY" dental trends, the psychology of combatting misinformation, and why Alan has officially become a convert to interproximal brushes. Whether you're looking for social media strategy or tips on finding career longevity in the "op," this conversation bridges the gap between clinical excellence and modern patient connection. This throwback episode is actually an older episode of Very Dental featuring one of the amazing co-hosts of Very Clinical! In this wide-ranging "shop talk" session, Alan is joined by Zach to revisit the early days of dental social media and the evolution of their clinical workflows. From the "crushing it" culture of the old Dentaltown forums to the practicalities of the modern operatory, they dive into the tools that actually make a difference. Zach shares his love for the spade proximator and the Swiss-Army-knife utility of Teflon tape, while Alan makes a case for the PDL syringe. They also tackle the high-stakes debate of the dentist's lunch hour and offer a deep dive into the "goofy draw" strategies required when prepping crowns on teeth with deep facial undercuts. "Instagram is dentistry and Facebook is where friends and family are... Instagram is like meth, I can't get off it." — Alan Mead Alan sits down with a powerhouse trio—Dr. Russell Schafer, Dr. Mitch Hopkins, and Dr. Matt Standridge—live from Voices of Dentistry 2026. They bridge the gap between "Instagram Dentistry" and the daily grind of running a productive practice, covering everything from why 2D photography still beats 3D face scanning to the hard truths about dental school debt and clinical mastery. The Guest List: Dr. Russell Schafer: The man who helped name the "Group Function" and a recurring voice on the network. Dr. Mitch Hopkins: A first-time guest and longtime listener, sharing his journey into implant training and full-mouth rehabs. Dr. Matt Standridge: The "Tom Hanks of the Very Dental Podcast," returning to discuss the discipline of dental photography. Key Conversations: The Gateway to Comprehensive Care: Photography The group discusses Matt's presentation on photography. While high-end DSLRs create "dental porn" for Instagram, the real value lies in using photography as a diagnostic tool for every new patient. The "Dummy" Solution: Discussion on the ShoFu camera as a simpler alternative to complex DSLR setups. The IG Satire: A hilarious pitch for "Garbage Mouth Instagram"—high-end photography of the most neglected cases to stick it to the "AACD" types. 3D Scanning vs. The "Uncanny Valley" Despite the push for $10,000 3D face scanners, the consensus remains: 2D photos are more useful for lab design. * 3D renderings often fall into the "uncanny valley"—looking almost human but just "off" enough to be distracting. Photos are essential for designing smiles that actually fit the patient's face, not just their arches. Advice for the "Mid-Career" and New Dentists The conversation turns to the crushing debt and clinical gaps facing new grads. The All-on-4 Trap: Why jumping straight into full-arch implants because of debt can be a recipe for malpractice. The Power of Bread and Butter: A defense of molar endo, quadrant fillings, and the "Lean and Mean" model of dentistry. The PVS Skillset: Why learning analog impressions (and packing cord!) is still a vital skill in a digital world. Leveling Up Your Clinical Skills The episode wraps with a "lightning round" of advice for improving your work tomorrow: Design Your Own Crowns: Use software to marginate your own preps. It's the fastest way to realize where your technique is failing. Magnification & Lighting: Whether it's jumping from 2.5x to 4.5x loops or investing in high-quality surgical headlamps, if you can't see it, you can't treat it. Kevin and Zack welcome back Dr. Alan Mead for a discussion of modern geriatric dentistry. It's not as much a recipe book for treating older patients but how the guys have seen their geriatric patient population change over time. They discuss the massive divide between "spry" seniors and those struggling with late-life health declines, the ethics of aggressive treatment in nursing homes, and how clinical rules must change when the goal shifts from "permanent fix" to "comfort and function." Episode Highlights Genetics vs. Lifestyle: Why some 80-year-old patients have the periodontal health of a 20-year-old, while some 30-year-olds are already on multiple medications. The "Caries Explosion": A discussion on why a sudden spike in decay in older patients is often a systemic "red flag" for overall health decline. To Treat or Not to Treat: Navigating the difficult conversations with the adult children of aging patients. When is a denture a "death sentence," and when should you just reach for the SDF? Clinical Tools for the Aging: The role of Silver Diamine Fluoride (SDF) and Glass Ionomers in "patching" and maintaining quality of life without invasive surgery. Alan dives deep into his decades-long journey with high magnification dentistry. In this episode, Alan breaks down why high-powered magnification isn't just for practitioners with "bad eyes" and why it has become the most humbling yet rewarding tool in his operatory. From the technical evolution of the varioscope to the critical importance of specialized lighting, Alan shares why he believes every restorative dentist should consider making the leap to the microscope to rediscover the joy and precision of their craft. The Eyesight Myth: Why magnification isn't a crutch for poor vision, but a tool to see beyond human limits—and why Alan wouldn't let a "naked eye" dentist work on him. Light vs. Magnification: A look at why lighting is arguably as important as the lens itself, including Alan's preference for Enova surgical headlamps over traditional overhead lights. Beyond Endodontics: Debunking the idea that microscopes are strictly for root canals and exploring their value in restorative diagnosis and execution. The Varioscope Revolution: How variable objective lenses have drastically lowered the learning curve for new microscope users compared to the "static" lenses of the past. The "Humbling" Reality of 15x: Why seeing your work under high power can be frustrating but ultimately raises the bar for your foundational skills and polishing techniques. Mastering Documentation: Practical tips on using beam splitters, keeping your camera in focus, and why a dedicated scope monitor is a game-changer for your workflow. Patient Perception: The surprising truth about why patients are often more impressed by 3D scanners than live microscope feeds. This special Saturday episode of the Very Dental Podcast is all about the upcoming Chicago Midwinter Dental Meeting! Alan Mead sits down with Dr. Phil Schefke, President of the Chicago Dental Society (CDS), to preview the 2026 Midwinter Meeting. Whether you're a 30-year veteran of the meeting or a dental student looking for your first experience, this episode dives into why Chicago remains the premier destination for dental innovation, collegiality, and—believe it or not—puppies. The Guest: Dr. Phil Schefke Dr. Schefke serves as the President of the Chicago Dental Society, the largest component of the ADA. With a board of 15 members and a dedicated staff, Phil has spent nearly a decade on the leadership ladder to help orchestrate a meeting that hosts attendees from all 50 states and over 60 countries. What's New at the Midwinter Meeting? The CDS team has spent three years redesigning the attendee experience based on direct feedback. Here are the new features and highlights discussed in this episode: Streamlined Registration: No more waiting for badges in the mail. Attendees will receive a QR code to print their badges at self-service stations at McCormick Place or at three partner hotels (Hyatt Regency on Wacker, Hyatt McCormick, and Marriott McCormick). Enhanced Exhibit Floor Layout: The floor has been redesigned with shorter aisles for easier mobility and "crisscross" navigation. Refresh & Recharge Stations: Scattered throughout the floor, these areas offer water and phone charging ports to keep you hydrated and connected. Grab-and-Go Dining: New food stations on the exhibit floor designed for quick lunches between CE courses. The "Hadley" Puppy Corner: A dedicated space in the back of the exhibit hall to play with puppies and melt away the stress of the day (named after the first CDS president). Early Career Dentist Lounge: A comfortable space featuring couches and networking opportunities specifically for the next generation of clinicians. CDS "Swag" Booth: For the first time, attendees can purchase official CDS-branded jackets, sweaters, and coffee cups. Tech Upgrades: A brand-new, intuitive registration system that prevents double-booking courses and high-quality, reliable Wi-Fi throughout the venue. On-Site Mobile Clinic: The Illinois Masonic Hospital will have their mobile dental van on the floor for tours and demonstrations. Free Professional Headshots: Available at the CDS footprint for attendees to update their professional profiles. Special Events & Keynotes Thursday: Keynote speaker Chris Chelios, three-time Stanley Cup champion, discussing leadership and championship-level teamwork. (Plus, Garrett's Popcorn giveaways!) Friday: "Sip, Suds, and Sales" (beer and wine on the exhibit floor) followed by the Beats and Bites DJ party. Saturday: The traditional President's Dinner Dance, a black-tie event featuring a live band to celebrate the week's successes. The "Fourth Level" of Dentistry Alan and Phil discuss the "secret sauce" of CDS membership. At just $125 (for ADA members), membership provides free registration to the meeting, offering incredible value compared to standard registration fees. It's a tradition that spans 161 years, proving that while technology changes, the value of face-to-face collegiality remains priceless. Zach and Kevin welcome Alan back to the show by commiserating over those moments where you look at your schedule and wonder why you (or your past self) agreed to a specific procedure. Kevin's #16 Crown Prep: Kevin recounts the dread of seeing a crown prep on a third molar (#16) at 8:00 AM. Despite the technical difficulty and the "Is this a joke?" feeling, it ended up being successful with a rubber dam. Zach's "Self-Extracting" Molars: Zach shares a cautionary tale from his early career working with his father. He attempted to crown a second molar that was tilted so far distally it was practically trying to extract itself. The result? A lost contact, a lost crown, and eventually, a lost patient. The Bioclear Pivot: Alan discusses a recent case where he planned a crown on #15 but found decay so deep it was near the crest. He pivoted to a side-by-side Bioclear composite, which was harder and more time-consuming but ultimately the right clinical choice. Categorizing the "Resurrected" Patient The main theme of the episode focuses on patients who reappear after years—sometimes a decade—away from the chair. Alan and the hosts break these down into tiers: 1. The S-Tier (The Best Case) These are patients who didn't leave because of a conflict; they simply "had a gap in life." Examples: Moving away and moving back, going through a divorce, or the "COVID-12" (people who stopped coming in 2020 and are just now emerging). The Dynamic: They still feel like "your" patients. They often have a high follow-through rate because they are relieved to be back in a familiar, trusted environment. 2. The A-Tier (The "Grass Wasn't Greener") Patients who left for a specific reason—usually insurance—and realized that "a dentist is a dentist" isn't actually true. The Insurance Trap: Many leave because the office went out-of-network, only to find that the "in-network" experience lacked the technology or relationship they valued. The Result: When they return, they often have a newfound appreciation for the higher level of service and clinical care provided. 3. The Lower Tiers (The Friction Cases) The Billing "B-Tier": Patients who disappeared because they owed money. They usually only reappear when they have an emergency and are suddenly willing to pay their old balance to get out of pain. The Personality "C-Tier": Patients who left after a confrontation or a bad review. Professional Growth & "Hard Nos" Alan reflects on how his clinical confidence has changed over the years. When a patient returns after 10 years, they aren't seeing the same dentist; they are seeing a version of Alan that is more skilled, has better tools (like new Bioclear matrices), and more experience. When is a "Resurrection" a "Hard No"? Abuse of Staff: Rudeness, yelling, or cussing out the team. Chronic No-Shows: People who are "constitutionally incapable" of making it to an appointment. The Dismissed: If a formal dismissal letter was sent, the door remains closed regardless of the patient's willingness to pay. Join the Very Clinical Facebook group! Alan discusses his "whirlwind" travel plans for the Chicago Midwinter Meeting, reviews the new Bioclear TSS Matrix system, and shares his first impressions of the remarkably small Shining 3D Elf scanner. Key Highlights The Chicago Midwinter Strategy Alan is opting for a surgical strike on Chicago this year: The Schedule: Working Thursday, driving to Indiana, hitting the McCormick Center all day Friday for recording, and driving back Friday night. The Goal: Maximizing "content in the can" by recording with attendees and exhibitors without the expense of a multi-day hotel stay. In this episode of the Group Function podcast, host Alan Mead sits down with Dr. Alex Kantor and Dr. Eric Applesies from the Just a Couple of Dentists podcast to unpack the "regular ass dentist" experience of buying, starting, running and selling dental practices! Fresh off their presentation at the 2026 Voices of Dentistry conference, the Eric and Alex (a married couple of dentists) dive into the logistics of optimizing a practice for sale, emphasizing that maximum value is built in the 18 to 24 months before a transition by tightening systems and improving hygiene "show rates." The conversation balances technical advice on EBITDA and overhead reduction with a candid—and occasionally spicy—critique of the post-COVID hygiene labor market, ultimately highlighting how dentists can prepare their businesses for a successful handoff to either a private buyer or a DSO. In their session and conversation with Alan, Dr. Alex Cantor and Dr. Eric Applesies highlighted several key levers to pull when prepping a practice for sale. Their core philosophy is that "a dollar saved is $4 to $6 earned" during a valuation. Key Optimization Strategies The 12-to-24-Month Runway: Valuations are typically based on the last 12 months of profit (EBITDA). To ensure your systems are fully "flushed out" and consistent, start implementing changes 18 to 24 months before you intend to sell. Hygiene "Show Rates": Increasing your show rate from 70% to 90% is considered "low-hanging fruit." Because hygiene has low variable costs (mostly just prophy paste and floss), nearly every dollar of a kept hygiene appointment goes directly to the bottom line, which is then multiplied during a sale. Aggressive Appointment Confirmation: Use automated texts to keep appointments top-of-mind. The "48-Hour Rule": If a patient hasn't confirmed 48 hours out, inform them they will be removed from the schedule. Follow Through: Actually remove unconfirmed patients to give your team two days to fill the spot with someone who wants to be there. Overhead Reduction (The "Spend" Side): Supplies: Move away from the "old school" model of buying through a single rep who brings donuts. Use platforms like Crazy Dental or Net 32 to treat supplies as a commodity and secure the lowest price. Labs: Look for efficiency gains through model-less workflows and digital scanning, which can reduce lab fees and turnaround times. Clinical Extraction: If you want to retire immediately after the sale, stop working clinically one year prior. This proves to a buyer (especially a DSO) that the practice's revenue is sustained by systems and associates, not just your individual production. Know Your "Game": Private Buyer: Better if you want a quick exit (6–12 months) and have a smaller practice. DSO Buyer: Better for high-revenue practices ($5M+ range), but usually requires you to stay on for several years as an associate. In this episode Alan sits down with Dr. Abe Hoellrich in Scottsdale, Arizona, during the Voices of Dentistry conference! Abe's practice focuses on sedation dentistry in Columbus, Ohio. He offers an insider's perspective on the "turf battles" surrounding anesthesia privileges, weighing the tensions between general dentists, oral surgeons, and medical doctors. The discussion highlights the nuances of patient safety versus access to care, with Abe arguing for the superior safety of reversible IV sedation compared to oral methods and emphasizing the need for stringent training to preserve the future of dental sedation. Recorded live from VoD 2026 in Gilbert, Arizona Alan is joined by Jay Glazer from Crazy Dental to tackle the rising costs of running a dental practice. Together, they dismantle the outdated model of relying on sales reps and "special" pricing lists, advocating instead for the transparency and efficiency of online ordering. Jay shares his philosophy on overhead control using the "14-dollar milkshake" analogy, illustrating how small savings on supplies can compound into massive wealth over time. Tune in to learn how to ditch the donuts, stop overpaying for the same products, and utilize exclusive promo codes to slash your overhead immediately. This throwback episode was originally recorded live from the main stage at Voices of Dentistry 2022 where the original Dental Hacks—Dr. Alan Mead and Dr. Jason Lipscomb—reunite for a hilarious and nostalgic session. The duo kicks things off by catching up on life since their "breakup" and going head-to-head in a competition to determine who has endured the worst patient horror story of the pandemic era. Later in the show, the classic Brain Trust format returns as Dr. Mark Costes and Dr. Justin Moody join the stage for a round of "Ask Us Anything." The panel covers a wide range of topics, including their surprising hobbies outside of dentistry, pandemic takeaways, and critical advice regarding practice startups versus acquisitions for the next generation of dentists. In this classic episode Dr. Mike DiTolla returns to the show to break down the clinical realities of working with zirconia. After sharing a humorous and humbling story about his recent shoulder surgery and the universality of missed nerve blocks, Mike shifts focus to the material science of modern dentistry. He details the specific use cases for full-strength versus translucent zirconia, explains why he treats cement margins like narrow Class V restorations, and offers precise protocols for adjusting and polishing without damaging the ceramic. From the "Lipscomb Scale" of removal difficulty to the critical importance of decontamination after try-in, this conversation offers practical, universal solutions for simplifying the zirconia workflow. This classic Dental Hacks episode features a long-form interview with Dr. Rella Christensen of TRAC Research (formerly CRA). The discussion covers the history of independent product evaluation, the challenges of maintaining objectivity in dental research, and significant findings regarding restorative materials and caries management. The original interview had really poor sound quality but a little help from an Adobe AI filter really cleared it up. This interview was worth another listen, I think! Key Interview Highlights 1. The Origins and Ethics of TRAC Research History: The organization began in 1976 (founded by Gordon Christensen) with the goal of collecting subjective data from clinicians to create objective "Consumer Reports" style rankings for dentistry. Unbiased Stance: Dr. Christensen emphasized that TRAC is a non-profit foundation that accepts no funding from manufacturers. If a company refuses to provide a product for testing (often fearing a negative review), TRAC purchases it on the open market to ensure the data is captured. The "File Drawer" Effect: They discussed how negative research is rarely published in journals due to corporate influence in dental schools and editorial boards. TRAC aims to publish the truth, even when it is negative. 2. Restorative Materials: The Zirconia Revolution 10-Year Study Results: Dr. Christensen detailed a 10-year study comparing various ceramic systems. The Failure: Zirconia substructures (like Lava) survived well, but the veneering ceramics (porcelain layered on top) had massive failure rates (chipping and cracking). The Winner: The study pointed toward monolithic zirconia (specifically BruxZir) and lithium disilicate (e.max) as the most durable options. Translucency Warning: She noted that as manufacturers push for more translucent (anterior) zirconia, they often lower the flexural strength (from ~1200 MPa to ~600 MPa). She cautioned that the long-term fracture toughness of these newer, "prettier" zirconias was still unknown at the time 3. The "Bombshell": Enamel Remineralization & Caries The Study: TRAC performed an in-depth clinical study on 330 orthodontic patients (high risk for white spot lesions). They tested various protocols including MI Paste, Fluoride Varnish, Ozone, and Xylitol against a control group. The Result: None of the products performed statistically better than the control group (who did nothing special), with the exception of PreviDent 5000, which was only marginally better. The Conclusion: Dr. Christensen concluded that you cannot arrest caries or remineralize enamel if the patient's diet is not addressed. She stated that dental caries is "the only infectious disease we attempt to treat while allowing the cause [sugar] to continue." In this candid episode, Alan announces he is taking a hiatus from recording new content for the Very Dental Podcast Network. Feeling momentarily "tapped out" after nearly 12 years of podcasting, Alan reflects on the evolution of the medium and his desire to avoid becoming repetitive. Starting in January, the show will move to a weekly Friday release schedule featuring remastered "Greatest Hits" episodes, beginning with a classic 2015 interview with Dr. Rella Christensen. While he plans to step back from the microphone for a bit, he will continue to produce episodes for hosts Kevin and Zach and intends to record at the upcoming Voice of Dentistry and Chicago Midwinter meetings. Alan concludes by asking the audience for feedback on future formats, including potential co-hosts or narrative storytelling. Message me on Facebook or email me if you've got thoughts. In this throwback Dental Hacks episode, I am joined by Dr. Jason Lipscomb to welcome two absolute legends in dental education: Dr. Frank Spear and Dr. Gregg Kinzer. This interview was originally released back in 2018 and it marked the first time Jason, Frank, and Gregg sat down together on the podcast, delivering a conversation packed with history, philosophy, and actionable practice management advice. The conversation traces the evolution of dental continuing education, moving from Frank's early days of disjointed lectures to the structured, curriculum-based approach of the Spear Center today. The group discusses the dynamics of Frank and Gregg's successful 20-year partnership, emphasizing the importance of shared vision over finding a "clone." They provide deep insights into patient communication, specifically how to filter new patients using the "Emergency Room vs. Mayo Clinic" analogy and how to re-engage long-time patients in comprehensive care without making them feel neglected. Finally, they cover the importance of fighting burnout through clinical growth and the future of restorative dentistry, including the integration of airway health and online team education. Join hosts Zach and Kevin for the annual Very Clinical holiday wrap-up! This week, they welcome back the recurring year-end duo, Dr. Erin Elliott and Dr. Gary Holtzclaw. It is officially a tradition as Erin and Gary join the guys to break down how they are spending the holidays and what has changed in their lives over the last twelve months. In this festive episode, the group shares their unique Christmas traditions, ranging from Icelandic rice pudding games to seafood feasts and tongue-in-cheek "virgin sacrifices." Beyond the holiday cheer, they dive into significant life updates. Kevin discusses the transition of selling his practice to become an associate and his growing obsession with pickleball. Erin shares the joys of empty nesting and her quest to visit every NFL stadium. Gary reveals why he deleted his social media accounts to reclaim his attention span and his commitment to taking real vacations, while Zach opens up about the realities of taking over full ownership of his family practice. It is a candid look at the personal and professional evolution of four dentists at different stages of their careers. Join the Very Clinical Facebook group! In this holiday edition, Alan starts off feeling the weight of the Winter Solstice and a bit of "podcasting burnout," questioning whether an audio veteran needs to pivot to video to stay relevant. However, he refuses to stay in a funk. Alan shares his real-time strategy for getting his head straight for the holidays, including a change of recording venue, hacking his own social media algorithms to remove toxicity, and finally getting back to the gym. He also shares updates on his practice's workflow, his renewed passion for Bioclear, and why you need to be in Arizona this January. This throwback Dental Hacks episode features one of Alan and Jason's classic airway discussions with Dr. Jeff Rouse. This was recorded at the 2018 DS World held in Orlando. Dr. Rouse elaborated on the Seattle Protocol and the essential approach to airway management in dentistry. Jeff broke down the differences between airway issues and sleep apnea, emphasizing the importance of nasal breathing and the role of night guards. He also addresses screening techniques using high-resolution pulse oximetry and cardiopulmonary coupling and the importance of interdisciplinary collaboration involving ENT specialists and orthodontists. Kevin and Alan are joined again by Dr. Dhaval Patel to discuss the significant shifts occurring in digital dentistry, moving beyond the traditional CEREC workflow. Dhaval explains that his highly popular online group, "Keep CERECing," was recently renamed "Keep CAD/CAMing" after he consulted with Dentsply Sirona's legal team regarding trademark usage, which prompted him to broaden the group's scope. This change has led to a natural expansion in technology focus, now encompassing the wider CAD/CAM landscape including 3D printing and 5-axis milling. Dhaval details how he uses 3D printing for models, surgical guides, and night guards, and leverages a 5-axis mill for larger, multi-unit cases and zirconia restorations, highlighting the cost efficiency and ability to run large jobs overnight. The conversation also explores the rise of dedicated, outsourced digital designers and the potential for artificial intelligence (AI) to handle design in the near future. Alan explores the state of prevention in dentistry, inspired by his constant sense of smelling periodontitis everywhere. He argues that while the profession talks a good game about prevention, its core focus remains surgical intervention. Alan stresses that the success of dental health hinges on the 363 days of patient home care outside the office, yet dentists often fail to properly educate patients on effective techniques beyond generic instructions to "brush and floss." Drawing on insights from hygiene experts, he calls for a radical shift to front-load prevention—moving past a reliance on periodic exams to actively equipping patients with the customized tools and knowledge needed for effective, daily self-care. AI is in the conversation all the time these days. Most people are convinced that the productivity gains that AI promises are all to the good, but is that really true? Alan is joined again by Dr. Gary Holtzclaw for a conversation that needs to be had. The conversation includes the role of AI in dentistry, skepticism towards new technologies, the evolution of social media, and the impact of sports betting on the integrity of sports. They explore the benefits and potential threats of AI, the changing landscape of social media interactions, and the implications of gambling in sports. The conversation highlights the need for critical thinking in the face of overwhelming information and the importance of maintaining integrity in both technology and sports. On the Very Clinical podcast we're suckers for a good story, and our guest, Dr. Dhaval Patel tells one of the best ones we've ever heard! Kevin and Alan (sorry, no Zach this episode, he's busy mourning the record of his Kansas City Chiefs) welcome Dr. Dhaval Patel to the show. Our plan was to talk about his journey into CAD-CAM dentistry and don't worry, we get there. But he tells the story of how he became a dentist in the US after his training in India and the story is AMAZING. Let's just say that Dhaval has taught us that you need to ask for the things that you want. Following this amazing story, Dr. Patel recounts his unexpected introduction to CEREC technology in 2006, starting with a dust-collecting Red Cam unit at his Pacific Dental Services office. Despite initial skepticism from colleagues about "shitty crowns," a half-day training session with Dr. Bob Conrad ignited his passion for the technology, leading him to fully embrace it, become an owner-doctor, an early adopter of the Omnicam, a renowned speaker, and the founder of the popular Keep CERECing Facebook group in 2016. He concludes by announcing the upcoming Third Annual Keep CAD Camming meeting in Austin, October 15th-17th, 2026, which will expand beyond CEREC to cover a wider range of CAD/CAM and digital dentistry topics. In this episode of the Very Dental Student Podcast, host Mohamed Abo-Basha and Dr. Alan Mead discuss his journey through the fall semester of his D4 year. We discuss Mo's recent experiences with dental boards and externships as he approaches graduation. Mo shares that the dental boards have recently changed, moving away from live patient exams to mannequin-based clinical testing (Prosthodontics, Endodontics, Periodontics already completed, Operative to follow) and computer-based written boards. He expresses that this change is a positive step ethically. Clinically, Mo is nearing completion of his requirements, only needing a few more Scaling and Root Planing (SRP) cases. He also details his externships, which provided valuable experience in high-volume operative and extraction procedures at a low-fee clinic and a mobile unit, significantly boosting his speed and confidence in procedures like administering anesthesia and performing quick, effective patient exams. Looking ahead, his final semester will primarily involve a Surgical Implantology class and serving as a Teaching Assistant (TA) for first-year operative students, which he anticipates will be a rewarding experience. They discuss the journey of writing the book, the importance of taking actionable steps, and how to overcome overwhelm in both personal and professional life. Alan opens the episode by discussing his experiences teaching his two sons, particularly his youngest, Jacob, how to drive. This experience, which he finds unnerving and anxiety-producing, has highlighted how much of his own driving is now automatic "muscle memory" that he struggles to verbalize. He notes the beginner's mindset—or lack thereof, particularly in confident teenage boys—and how new drivers often don't realize what they don't know, struggling with basic things like staying in the lane. Alan draws a parallel between his son's learner's permit and a new dental license, reflecting on his own lack of clinical experience after graduating dental school in 1997 with a minimal number of completed procedures. He suggests that a new dentist's license is essentially a "learner's permit," and advises both new practitioners and experienced dentists approaching new techniques (like his recent Bioclear review) to embrace humility and the beginner's mindset to learn effectively without existing biases. This episode features host Alan welcoming back guest Dr. David Clark, the founder of the Bioclear Method, for an insightful and entertaining discussion. The main focus is the launch of the new Tooth and Surface Specific (TSS) matrix system, which David describes as a "game-changer" that significantly simplifies complex composite restorations, particularly for teeth with normal papillae. They also cover the rising value of Bioclear procedures, with David sharing a story of a patient who expected to pay $10,000 per tooth for comprehensive treatment. Finally, they dive into a "King of the Nerds" conversation about the use of operating microscopes in dentistry, drawing parallels to the early resistance faced by a pioneering neurosurgeon, and discussing the challenge of using high magnification on an unstable patient—like a scene from the movie Twister. In this episode of the Very Clinical Podcast, hosts Zach and Kevin welcome back special guest Dr. Frank Clayton for a deep-dive conversation sharing essential advice on navigating a dental career from its beginning stages to retirement. The discussion kicks off with lighthearted banter about alternative dream careers—Zach as a baseball play-by-play announcer, Kevin as a morning radio host, and Frank as a game show host. For the early career dentist (Years 1–7), Frank advises working for a couple of years, then investing heavily in CE to establish a clinical niche (like his focus on form, function, and aesthetics), and, most critically, living well below one's means while investing consistently with a dental-specific financial planner—advice that Kevin reinforces by telling listeners to "gamble on yourself." During the middle-game grind (Years 10–25), Frank recommends shedding unhappy or unprofitable procedures to embrace the "boring" but profitable, repetitive work, while utilizing advanced tax-sheltering strategies like Cash Balance Plans; Kevin shares that he reinvigorated his middle career by learning implants, which provided financial and time freedom. Finally, addressing the last third of a career (Years 25+), Frank stresses the importance of viewing the practice as a valuable asset and advises starting the exit planning process five years out, focusing on selling at the top—by knowing the office's maximum production capacity (the Law of Diminishing Returns)—rather than limping to the finish line. In retirement, Frank finds liberation in being "just Frank," free from the burden of daily high-stakes decisions, and encourages other dentists to find a new focus rather than fearing the loss of the "doctor" identity. Alan reflects on his self-described "average" skill level as a dentist and explores how adopting prescriptive, systematic approaches can help practitioners in the middle of the bell curve achieve consistently excellent patient outcomes. Inspired by his recent experience at a Bioclear course, he argues that a systemized methodology, like the one taught by Bioclear, is often more reliable than relying on individual "superhero" hand skills or talents. Dr. Mead shares how the course's emphasis on a specific, step-by-step process—from using warmed composite and specific matrices to post-operative polishing—resulted in universally great outcomes for all attendees, regardless of baseline skill. He strongly recommends that fellow dentists, particularly those who struggle with consistency or implementation after CE, consider adopting systems and maximizing magnification (like using a microscope or high-power loops) to elevate their practice. Finally, he encourages listeners to try a piece of the system, specifically the Twin Ring Universal for posterior composites, and offers a 15% off code (VERYDENTAL8TON) for Bioclear products. This episode is a special interview where Alan is the guest! He's interviewed by his long-time friend and podcasting colleague, Andrew Johnston, the host of A Tale of Two Hygienists, as Andrew wraps up his run on that show. The conversation is a blend of dentistry, business, and the endurance of creative work. Dr. Mead shares his experience attending a Bioclear course and the challenge of changing established habits in dental practice. The core of the discussion centers on the evolution and future of dental podcasting, including the shift from audio-only to video/social content, and the role of AI in show production. They also dive into deeper topics about the future of the dental profession, the pressures of student debt on new dentists, the importance of longevity and permanence in practice, and the struggle to find social fulfillment outside of one's career. Dr. Mead offers sage advice for new podcasters, stressing the importance of engagement over download numbers, genuine conversation, and making the show about the listener. This episode features hosts Zach and Kevin, who welcome returning guest Dr. Frank Clayton to discuss the psychology and process of retiring from dentistry. Dr. Frank Clayton, who sold his practice and fully exited clinical work in July 2023, shares his insightful journey and the various phases of retirement he's experienced, including the "honeymoon phase." Kevin, who recently sold his practice in August and is now navigating the associate phase, offers his perspective on the initial mental shifts of stepping away from ownership. The hosts and Frank delve into the factors that drive the decision to sell—a combination of financial goals, mental fatigue, and a desire to "go out on top." They discuss the emotional weight of a "two-year breakup" with patients and staff, the challenges of detaching from the owner's mindset, and the importance of a well-structured ramp-down, which for Frank, included cutting back income and clinical work months before his final day. Alan reflects on his recent return to the Bioclear Learning Center, an experience that served as a powerful system reset for his restorative dentistry. He connects the challenge of maintaining new techniques in the dental office to the physics concept of entropy—the natural tendency of a system toward disorder. Alan explains that his initial failure to fully implement the prescriptive Bioclear system (including key steps like "bioblasting") was due to the energy required to fight against this entropy, often manifesting as personal laziness or team resistance to change. He stresses that consistent energy input—through regular audits, training, written protocols, and making the correct materials easily accessible—is necessary to maintain a low-entropy, high-functioning system that ensures excellent, predictable clinical outcomes. This episode features host Dr. Alan Mead sitting down with his friend, Dr. Lance Timmerman, in the artsy Hotel Murano lobby in Tacoma, Washington, while Dr. Mead is attending a four-day Bioclear core certification course. The conversation flows between the excellence of the Bioclear hands-on CE curriculum and the company's continuous innovation in dental matrices (like the redesigned twin ring). Dr. Mead and Dr. Timmerman also share humorous and insightful anecdotes from their dental school days, discussing the crushing weight of modern student debt, the challenges of implementing new CE concepts, and their personal preferences regarding in-person versus remote hands-on learning. Dr. Timmerman touches on the importance of having a CBCT and reflects on the extreme surgical procedure of zygoma implants as something he actively avoids. The episode closes with reflections on the transformative journey from dental school anxiety to current professional life, highlighting the value of connecting with peers outside of formal lectures. Hosts Zach and Kevin put the sports talk (mostly) aside to dive deep into a critical area of everyday dentistry: various scenarios involving soft and hard tissue grafting and the management of periodontal recession. They discuss how to handle common cases like recession in post-ortho teens (especially on the lower anterior teeth) and the different approaches for the challenging chewing tobacco patient. Zach and Kevin share their experience with generalized recession, debating when to refer for a graft—often noting that patient concern over esthetics or sensitivity drives the decision more than the presence of the recession itself. The conversation covers treatment techniques like free gingival grafts and the status of fads like the LANAP procedure and pinhole technique. They also explore other periodontal topics, including the management of gingival overgrowth (gummy smiles), peri-implantitis complications, and their skepticism regarding the widespread use of local antimicrobial delivery systems (like Arrestin) for routine scaling and root planing cases. Tune in for a candid discussion on what's working (and what's not) in the everyday management of periodontal health in a general practice. Join the Very Clinical Facebook group! Alan records from his hotel in Tacoma, Washington, after finishing day three of a four-day Bioclear anterior and posterior certification course. Despite the dark and rainy November weather in Tacoma, Alan shares his enthusiastically positive experience at the Bioclear Learning Center, highlighting the high quality of the instruction and hands-on training. He then offers a "recommendation engine" based on his trip, covering a specific Bioclear product, a movie he watched on the flight, an airport security service, a local restaurant, and a final endorsement of the learning center itself. Alan also announces the creation of a seventh password for the Very Dental Facebook page: Bioclear. Alan sits down once more with Dr. Tim Anderson to dive deep into the rapidly evolving landscape of digital dentistry. The episode begins with a hilarious, relatable discussion about the terrors of crashing recording software and the potential (and pitfalls) of AI, touching on everything from homework help to its role in revolutionizing clinical notes and reading X-rays. The conversation quickly pivots to Dr. Anderson's journey from engineering and architecture interests to his passion for the dental lab and in-house manufacturing, fueled by his residency and the purchase of a CEREC system. He reveals why he moved past early zirconia and EMAX to embrace new technologies. Dr. Anderson then shares his excitement for SprintRay's Midas, explaining how its new "Digital Press Stereo Lithography" technology is a game-changer, not just an evolution of 3D printing. He believes this new class of manufacturing, especially combined with AI, is poised to unlock limitless potential in restorative dentistry, particularly for procedures like onlays and eventually even complex bioprinting applications. He concludes by emphasizing that while technology is crucial, the biggest hurdle for dentists is often the software and the willingness to integrate and innovate. Zach and Kevin kick off this episode by catching up and discussing a shared, recent hobby: pickleball. They share their experiences, the learning curve, and their thoughts on why the sport has become so popular, acknowledging its accessibility compared to tennis. The conversation then shifts to a dental topic: periodontal disease. They reflect on the emphasis of perio in dental school and discuss their personal criteria for treating scaling and root planning cases in-office versus when to refer to a periodontist. Key referral points include cases with sudden, isolated deep pockets, unresolvable widespread disease, and severe issues in patients under 40. The discussion briefly touches on molar extractions and when to sacrifice a tooth to protect an adjacent one, but they leave the topic of tissue grafting for the next episode. Alan gets candid about the concept of intentional culture, reflecting on his own experience taking over a dental practice where the culture was largely unintentional. He explains how he inherited a laid-back, reparative-dentistry-focused office, a product of the previous owner's lax style and the robust GM insurance benefits of the time. Alan admits that while his practice's culture has been adequate, making changes now is incredibly difficult because the unintentional status quo is deeply ingrained. He argues that new owners or startups have a massive advantage in establishing an intentional culture from day one, which is key to implementing new systems and driving change. He challenges listeners to join the Very Dental Facebook group (passwords include Timmerman, Hornbrook, Lipscomb, Papa Randy, Gary, or McWethy) to share their stories of intentional culture or the struggles of changing an unintentional one. Alan welcomes his first-ever podcast guest, Dr. Lance Timmerman, back to the show for a candid look at the life of a "CE Junkie." Lance shares his extensive, and often scattershot, journey through continuing education—from early courses in sleep apnea and Botox to implants and orthodontics—and the key lesson learned: it's hard to dabble in specialty procedures. The discussion evolves into a broader reflection on how dentistry has changed, especially for new dentists facing massive student debt. Ultimately, Lance describes his successful transition to a semi-retired, three-day-a-week practice focused on implants and sleep apnea, proving that taking all that CE led to the perfect work-life balance for travel and life experiences. Alan shares his recent findings on dental supply purchasing, questioning the long-standing industry model where dentists pay a premium for the convenience and supposed "best price" offered by a dedicated sales rep. He argues that this relationship-based purchasing lacks transparency and costs practices significant money, especially as overhead increases and reimbursements remain flat. Alan endorses his sponsor, Net 32, as a modern solution, highlighting its efficiency and transparency as an online network of suppliers that instantly offers the lowest available price. Through personal invoice comparisons, he demonstrates that switching to this system results in substantial, immediate savings on common materials, making it a powerful way for dentists to reduce overhead without sacrificing quality or convenience. Back in August Alan talked with guests Julianne O'Connor and Michael Keeter, the founders of Influential Dental. They discussed the company's efforts to elevate the dental space through its Influential Dental Podcast and the glossy lifestyle magazine, Influence Eleve. Julianne and Michael emphasize that their focus is on the authentic human element and the non-clinical stories of dentists—highlighting their journeys, mentors, and the challenges they've overcome—rather than the negative connotations of the "influencer" label. Alan discusses his history with audio and the evolution of the Dental Hacks podcast, contrasting his audio-first approach with the increasing trend of video content. They also touch on the challenges of print media in the digital age, the creative process behind their respective content, and the surprising utility of AI in podcast production. Dr. Alan Mead steps in for Zach and Kevin in this "Very Clinical Takeover" episode to share his efficient and refined workflow for single-tooth crown preparations. Alan emphasizes that his streamlined, digital-centric process makes single crowns a quick, straightforward, and profitable procedure in his general practice. The core of the episode is his detailed Crown Prep Workflow and Hacks, which prioritizes patient comfort and efficiency. Profound Anesthesia: Mead's first step is always to achieve profound anesthesia, utilizing inferior alveolar nerve blocks for lower molars and buccal infiltrations (usually with Septocaine) for other teeth. He heavily relies on the Septodont Paroject pdl syringe for supplemental tissue anesthesia around the entire tooth, often using it to avoid packing cord. Pre-Op Digital Scan: He scans the arch using his Medit i700 before prepping the tooth. This pre-op scan is used to register the final prep scan, ensuring a stable and accurate bite registration, which he praises as a massive efficiency improvement over traditional PVS impressions. Prep Sequence: His prep sequence is intentionally designed for efficiency: Break Interproximal Contacts First: He uses a Microcopy 0512C inverted pear bur for gross contact breaking, followed by a Microcopy 33140.10vf ("skinny") fine-tipped diamond for careful separation at the gingival. Occlusal Reduction: He uses the Microcopy 2137C which large, barrel-shaped coarse bur in an electric handpiece for quick, proper occlusal reduction. Core Build-up: After removing any old restoration or decay, he prefers Cosmecore from Cosmedent for build-ups, sometimes blocking out with Fuji IX from GC. He utilizes an old-school metal matrix band with Bioclear Diamond Wedges for isolation during the build-up. Margin Refinement and Tissue Management: He finishes margins under high magnification (microscope) and prefers a very fine diamond bur (Microcopy 3116.8vf) instead of traditional polishing cups for a smooth finish. Mead emphasizes smoothing the adjacent teeth with a Sof-Lex disc to ensure the lab can create an ideal, broad interproximal contact. Retraction: Mead overwhelmingly prefers Traxodent from Premier in the unit dose syringe tips for quick and clean tissue retraction, stating it generally works better for digital scanning than retraction cord. Temporization: While his assistant fabricates the temporary using a pre-made matrix, Mead remotes into his computer to mark the margins on the digital scan. He uses Clinician's Choice Template Ultra Quick Matrix Material and Mirror Image temporary material from Cosmedent, which he quickly refines with a Sof-Lex disc. He cements the temp with Dentotemp by Itena provisional cement for its strong retention. Alan encourages listeners to critique his "bread and butter dentistry" workflow on the Very Clinical or Very Dental Facebook groups, as he believes an efficient crown prep is a key to a successful general practice. "You got to remember that you don't own time, it's time that owns you." --Stephen King, "My Pretty Pony" Alan dives into the elastic nature of time this week, a concept he got from a Stephen King short story. He argues that time is totally weird and feels different depending on a person's situation, age, and mood. The Bonus Day: The whole thing started when Alan woke up at 3:30 a.m., convinced it was a work day (Tuesday). Then it hit him: It's Monday. He doesn't work Mondays! It felt like he got an entire day given back to him—a huge, weird, little time-traveling gift. The "Freakout" Factor: He observes that things that are stressful right now—like when his office phones were down for three days—feel like they'll be a nightmare forever. He quickly realized, though, that those things are usually forgotten by next week, proving that "this too shall pass." Time's Sandpaper: He looks back at the big, scary things, like being terrified about becoming a dad or the stress of dental school, and notes that now those sharp edges are all smoothed out. Time has a funny way of making those tough memories almost fond. The "Old Person" Cliché: Now he's the one saying it: "just hold onto these moments." Seeing his oldest son's marching band career wrapping up hit him how fast it all goes. That "blink and you'll miss it" thing is totally real, he says. The Light at the End: On the financial side, time actually works out. Those massive commitments, like the mortgages on his house and office, that felt endless when they started? Mead is finally seeing the finish line. He finds it wild that they eventually just... end. He wraps up by encouraging listeners to think about their own perception of time and remember: whatever you're dealing with, it's going to work out and move on. Alan welcomes back Dr. Tim Anderson to discuss the transformative impact of digital technology on modern dentistry. The episode kicks off with a brief mention of Dr. Anderson's recent "Midas tour" focused on partial coverage restorations, which serves as a springboard into a deeper discussion about the dramatic technological shift since their dental school days at the University of Minnesota. Dr. Anderson reflects on what he'd tell his younger self, highlighting the near-unbelievable advancements in: Intraoral scanning and the power of AI in design. 3D printing and same-day ceramic restorations. The doctors agree that while technology is crucial, the patient interaction and communication it enables are the real game-changers, using large monitors and engaging patients in the co-discovery process. They delve into the exciting evolution of 3D-printed materials, moving beyond simple resins to explore high-translucency ceramics and "digital press stereolithography" (Midas), which opens the door for layered, customized restorations that mimic natural tooth anatomy. The conversation touches on the enduring debate between full-coverage crowns and partial coverage (onlays), noting the current culture's tendency toward black-and-white treatment planning. Dr. Anderson expresses excitement for new workflows and materials that bridge the gap between traditional artistry (like gold inlays) and digital precision, ultimately focusing on how technology empowers better patient care and education. In this throwback episode of Very Clinical, Zach and Kevin welcome back Charlie Fryer to talk about AI in dentistry. Charlie recently finished a master's thesis about class II caries and AI so he can see applications in dentistry from the perspective of a computer scientist. AI's Current Role in the Dental Industry The guys discuss existing and potential applications of Artificial Intelligence (AI) in dentistry, acknowledging that the future of the profession will be heavily influenced by this technology. Invisalign/Aligners: Kevin notes that companies like Align Technology (Invisalign) are essentially data companies that leverage millions of intraoral scans from iTero to perfect tooth-movement algorithms and quickly generate treatment plans. He suspects much of the initial clin-check setup is AI-driven, with human checks afterward. Design & Manufacturing: AI is already used for the automated design and quick file return for appliances like night guards by companies like SprintRay, as well as for crown design in some printing workflows. Radiographic Diagnosis: The hosts discuss companies that use AI to read and assist in diagnosing findings on X-rays. A key discussion point is how these companies, like DSOs (Dental Service Organizations) such as Heartland or Aspen, acquire the vast amounts of patient data needed to train their models. The hosts posit that users become the "product," providing data with every image analyzed. Future AI Applications and Benefits The conversation shifts to more future-facing applications and the overall impact of AI on dental practice. Net Positive Impact: Zach and Kevin agree that AI will be a net positive for dentistry, primarily as an aid rather than a replacement for clinical judgment, drawing an analogy to how a GPS system advises a driver but the driver still makes the final decisions. Combining Data Sets: Charlie suggests combining various patient data, such as cone beam CTs, intraoral scans, and radiographic images, to develop next-generation tools for complex procedures like implant placement guidance. Practice Optimization: A major area for AI development is practice management. They discuss using machine learning to: Optimize scheduling based on patient history (recurrent neural networks) and procedure mix to maximize profitability and account for unexpected needs. Forecast inventory needs to ensure the practice is always adequately stocked. Financial and Marketing Tools: Patient Financing: Kevin highlights Sun Bit, a payment program that uses an AI component to quickly determine a patient's payment risk and rate by analyzing non-traditional data like utility payments instead of a full credit report. Marketing: AI is already effective in predicting click-through rates for online advertisements, which could be leveraged by dental practices. Join the Very Clinical Facebook group! Patient churn—the natural cycle of patients choosing, staying, or leaving a dental practice—is a normal part of the business. Alan explores this by reflecting on his own 28 years in practice, concluding that dentists often struggle with churn because they view their office much differently than their patients do. Ultimately, a patient's decision to use a dental service, whether for comprehensive care or only for emergencies, reflects how that practice best fits their needs, and not necessarily a sign that the dental office itself is broken.
