Dr. Alan Mead steps in for Zach and Kevin in this "Very Clinical Takeover" episode to share his efficient and refined workflow for single-tooth crown preparations. Alan emphasizes that his streamlined, digital-centric process makes single crowns a quick, straightforward, and profitable procedure in his general practice. The core of the episode is his detailed Crown Prep Workflow and Hacks, which prioritizes patient comfort and efficiency. Profound Anesthesia: Mead's first step is always to achieve profound anesthesia, utilizing inferior alveolar nerve blocks for lower molars and buccal infiltrations (usually with Septocaine) for other teeth. He heavily relies on the  Septodont Paroject pdl syringe for supplemental tissue anesthesia around the entire tooth, often using it to avoid packing cord. Pre-Op Digital Scan: He scans the arch using his Medit i700 before prepping the tooth. This pre-op scan is used to register the final prep scan, ensuring a stable and accurate bite registration, which he praises as a massive efficiency improvement over traditional PVS impressions. Prep Sequence: His prep sequence is intentionally designed for efficiency: Break Interproximal Contacts First: He uses a Microcopy 0512C inverted pear bur for gross contact breaking, followed by a Microcopy 33140.10vf ("skinny") fine-tipped diamond for careful separation at the gingival. Occlusal Reduction: He uses the Microcopy 2137C which large, barrel-shaped coarse bur in an electric handpiece for quick, proper occlusal reduction. Core Build-up: After removing any old restoration or decay, he prefers Cosmecore from Cosmedent for build-ups, sometimes blocking out with Fuji IX from GC. He utilizes an old-school metal matrix band with Bioclear Diamond Wedges for isolation during the build-up. Margin Refinement and Tissue Management: He finishes margins under high magnification (microscope) and prefers a very fine diamond bur (Microcopy 3116.8vf) instead of traditional polishing cups for a smooth finish. Mead emphasizes smoothing the adjacent teeth with a Sof-Lex disc to ensure the lab can create an ideal, broad interproximal contact. Retraction: Mead overwhelmingly prefers Traxodent from Premier in the unit dose syringe tips for quick and clean tissue retraction, stating it generally works better for digital scanning than retraction cord. Temporization: While his assistant fabricates the temporary using a pre-made matrix, Mead remotes into his computer to mark the margins on the digital scan. He uses Clinician's Choice Template Ultra Quick Matrix Material and Mirror Image temporary material from Cosmedent, which he quickly refines with a Sof-Lex disc. He cements the temp with Dentotemp by Itena provisional cement for its strong retention. "You got to remember that you don't own time, it's time that owns you." --Stephen King, "My Pretty Pony" Alan dives into the elastic nature of time this week, a concept he got from a Stephen King short story. He argues that time is totally weird and feels different depending on a person's situation, age, and mood. The Bonus Day: The whole thing started when Alan woke up at 3:30 a.m., convinced it was a work day (Tuesday). Then it hit him: It's Monday. He doesn't work Mondays! It felt like he got an entire day given back to him—a huge, weird, little time-traveling gift. The "Freakout" Factor: He observes that things that are stressful right now—like when his office phones were down for three days—feel like they'll be a nightmare forever. He quickly realized, though, that those things are usually forgotten by next week, proving that "this too shall pass." Time's Sandpaper: He looks back at the big, scary things, like being terrified about becoming a dad or the stress of dental school, and notes that now those sharp edges are all smoothed out. Time has a funny way of making those tough memories almost fond. The "Old Person" Cliché: Now he's the one saying it: "just hold onto these moments." Seeing his oldest son's marching band career wrapping up hit him how fast it all goes. That "blink and you'll miss it" thing is totally real, he says. The Light at the End: On the financial side, time actually works out. Those massive commitments, like the mortgages on his house and office, that felt endless when they started? Mead is finally seeing the finish line. He finds it wild that they eventually just... end. He wraps up by encouraging listeners to think about their own perception of time and remember: whatever you're dealing with, it's going to work out and move on. He suspects much of the initial clin-check setup is AI-driven, with human checks afterward. Design & Manufacturing: AI is already used for the automated design and quick file return for appliances like night guards by companies like SprintRay, as well as for crown design in some printing workflows. Radiographic Diagnosis: The hosts discuss companies that use AI to read and assist in diagnosing findings on X-rays. Material Benefits: GI offers several advantages over traditional resin or amalgam: No Post-Operative Sensitivity and Minimal Shrinkage. Forgiving Technique in less-than-ideal isolation situations, sticking particularly well to dentin. Continued Hardening for months after the initial set, leading to excellent longevity and a surprisingly low failure rate. Efficiency and Cost: While the initial cost of GI is relatively high, the efficiency gained from eliminating the need for separate etching and bonding steps (using only a cavity conditioner provided in the kit) and the speed of placement makes it a cost-effective choice in terms of time and materials. Placement and Setting: Placement Technique: For occlusal restorations (like in grooves), Alan recommends over-squirting the material and using a moist, gloved finger to push it into the grooves, or using an anti-stick agent like Stick Free for metal instruments. Accelerating Set: Though not a light-cure material, applying the curing light can accelerate the chemical setting reaction via heat, cutting the two-and-a-half-minute set time down to about 90 seconds. He's a prolific author, lecturer, and practitioner who began his career after graduating from NYU in 1980 and acquiring a unique, century-old dental practice in New York City The main topic is the ABC's of Laser Dentistry, starting with a critical distinction between true lasers (like CO2​ and Erbium) and devices often called lasers but primarily used as hot glass tips (like diodes). Bob explains that diodes, which can operate at extremely high temperatures (750∘C to 1500∘C), work thermally, creating a burn, while CO2​ and Erbium lasers are absorbed by the water in soft tissue and vaporize it at 100∘C. He emphasizes that the CO2​ laser is the best option for general dentists doing soft tissue work, as it creates a much smaller zone of necrosis (thermal damage) than ElectroSurge or diode lasers, leading to better healing. He also offers a crucial financial perspective, arguing that while a CO2​ laser may have a higher initial purchase price (around $30,000), its lack of expensive disposable tips makes it a more cost-effective long-term investment than a cheaper diode laser (like a Picasso) with high operating expenses (potentially $18,000 in tips per year). For a multi-faceted approach, he recommends dentists invest in a CO2​ laser, a modern air abrasion unit, and a Photobiomodulation (PBM) laser (around $2,000 to $3,000). PBM lasers are purely palliative and can be used for pain management, like treating TMJ issues or for analgesia on deciduous teeth, and for promoting healing after invasive procedures. Finally, Bob stresses that the most crucial factor in laser dentistry is training, advising listeners that any course focusing on only one manufacturer or wavelength is likely a sales pitch, not a comprehensive educational seminar. Alan welcomes back Dr. Dawn Grimes to talk about burnout. Lately it seems like social media can't stop talking about it and they both have some things to say about it! They define burnout as emotional, physical, and mental exhaustion from prolonged stress, arguing it's not unique to dentistry. Dawn asserts that a poor relationship with stress, a lack of resilience, and unrealistic expectations—often fueled by social media—are the real causes. They suggest that instead of quitting or hiring a coach, dentists should reframe their perspective, manage expectations, and take practical steps to improve their lives both in and out of the office. Key Takeaways Burnout is real, but it's not unique to dentistry. It's the result of prolonged, unmanaged stress from any demanding part of life or work. Social media can worsen burnout. The algorithm feeds you more of what you look at, so lingering on videos about burnout can make you feel worse. The constant comparison to a curated, glamorous online presence creates unrealistic expectations for your own life and career. A "coach" may not be the answer. The hosts question the credibility of coaches who no longer practice clinical dentistry, suggesting that their advice may be disconnected from the day-to-day realities of the job. The only thing you can change is yourself. Rather than trying to change your job or circumstances, you should focus on changing your reactions to problems. This is a core part of developing resilience, which allows you to let go of negative experiences and start over. Rumination makes things worse. Dwelling on past mistakes or bad days (internal rumination) or consuming content about burnout (external rumination) reinforces negativity and prevents you from moving forward Meditation is a powerful tool. It helps you train your brain to let go of unhelpful thoughts and focus on the present moment, which is the key to building resilience and managing stress. Maintain perspective. Remember that dentistry is a well-paying profession and many other jobs are physically and emotionally harder. Accepting that life and work are often challenging is the first step toward finding peace. You have control. You can manage your social media algorithm by engaging with positive content like dog videos, mountain bike content or clips of cooking very tiny food! Alan shares his favorite verbal phrases that have improved his patient interactions over his 28-year career. He emphasizes the importance of building rapport and setting patient expectations to create a smoother, more enjoyable practice. Alan discusses how these simple word choices help him diagnose problems, manage treatment compromises, and handle post-op concerns, all while fostering trust and a human connection with his patients. Some of Al's Favorite Words: "Can you say more about that?" This phrase encourages patients to elaborate on their symptoms, often unlocking a more accurate diagnosis. It prompts them to think more deeply about their sensations, which is especially useful when a problem isn't visually apparent. "Are you going to be mad at me if...?" Used when a patient is considering a compromised or less-than-ideal treatment, this phrase invites them to contemplate a potential negative outcome. It shifts the responsibility for informed consent from a formal, authoritative one to a human, conversational one. "Julie is going to explain this to you in English." Dr. Mead uses this line when handing off a patient to a dental assistant or hygienist. It humorously acknowledges that dentists often use technical jargon and signals to the patient that a clearer, more accessible explanation is coming from the other team member. "We're here if you need us." This phrase is used for patients who have perfect checkups or don't require extensive treatment. It reassures them that they are still valued and that the practice is available for any future concerns, fostering a sense of long-term care and trust. "This is going to be sensitive for a couple days." When delivering a new crown or performing other procedures, setting the expectation of post-operative discomfort helps patients mentally prepare. It reduces the number of post-op calls and gives patients "grace" to experience normal sensitivity without alarm. "Are you okay if we wait to see what happens?" Used after cementing a crown while a patient is still numb, this asks for their consent to check the bite again once the anesthesia has worn off. It acknowledges that they can't accurately feel the bite while numb and gives them control over their final comfort. "You're going to be numb for a couple of hours." Dr. Mead uses this to manage a patient's expectations about how long the numbness will last. By saying "a couple of hours," he gives them a realistic timeframe, and if it lasts longer, they are less likely to be surprised or concerned. #174 - What does redemption look like at 72 years old? For Rand Timmerman, it meant lacing up his hiking boots and tackling the Appalachian Trail's 2,190 miles alongside his grieving brother—a journey that would test his newfound sobriety, physical limits, and spiritual foundation.Rand's path to that trailhead was anything but straight. Growing up in poverty after his father contracted polio, he discovered alcohol at 13 and was immediately transformed in his own mind from a skinny teenager to "Clint Eastwood." After impulsively enlisting during Vietnam, where both he and his brother served as helicopter gunners, Rand built a successful 40-year legal career while battling what many would call "functional alcoholism." Meanwhile, his brother Ronnie found faith, becoming a Mormon bishop and building a large blended family—until his wife's death left him shattered and seeking healing in nature.The brothers' epic hike across 600 mountains became much more than a physical challenge. For Rand, who had found sobriety just years earlier after hitting rock bottom at 65, it was the culmination of a spiritual awakening. "I was standing on a mountain in New Jersey," he recalls, "feeling spiritual power all around me, and I realized God made all of that and I never saw it before." Despite artificial knees, infections, and countless falls, Rand completed 1,863 miles before injuries forced him to stop.Today, Rand walks eight miles daily and dedicates hours to helping others overcome addiction. Dave emphasizes that a heart attack is one of the few life-threatening events that can be largely preventable if people are armed with the right knowledge. He shares key tests and prevention strategies he learned from his brother, a metabolic cardiologist, and warns against common misconceptions and tests that offer a false sense of security. A positive result would have led to an immediate diagnosis and intervention for Dr. Clark, preventing the heart attack and damage. Get a CAC (Coronary Artery Calcium) Score: This is a key test for "the healthy guy heart attack." A high CAC score indicates a genetic predisposition to having calcified plaque in your arteries. While a stress test only provides short-term security, a high CAC score is a significant warning sign that a rupture in the plaque could lead to a sudden, fatal heart attack. Be Cautious of Misleading Tests: Dr. Clark and his brother identified several tests that can give a false sense of security or are simply not effective in predicting a heart attack, including the standard stress test and most whole-body scans. Consider a GLP-1 Medication: Based on his brother's recommendation, Dr. Clark started taking a GLP-1 medication (like Ozempic or Wegovy) to reduce inflammation, which is a known contributor to cardiovascular problems. He notes that these medications are not just for weight loss but have significant, positive metabolic effects. Drawing from his own family history, Dr. Costes argues that an entrepreneurial spirit is often an innate trait (nature), but that the skills to succeed can absolutely be cultivated (nurture). They explore the massive shift in the dental profession from the "golden era"—when clinical skill alone could guarantee a great living—to today's reality, where staggering student debt loads make business acumen a non-negotiable key to survival and success. The conversation also contrasts the cottage industry of dentistry with the consolidated world of medicine, weighing the pros and cons of being a W2 employee versus taking on the risks and rewards of practice ownership. However, it requires a different kind of discipline focused on personal finance, smart investing, and tax strategy to build wealth without the final asset of a practice sale. If You Own, Do It Right: For those who do take the leap into practice ownership, it is crucial to commit to learning the business. Otherwise, you are taking on all the risk, liability, and stress for potentially no significant reward. The Problem with Medicine (and a Blessing for Dentistry): While medicine offers high-income W2 opportunities, it is a highly consolidated system that often leaves physicians with little control and massive tax burdens. Dentistry remains a "cottage industry" with immense entrepreneurial opportunity, despite its own challenges with insurance and patient payment discussions. Information vs. Willpower: Whether discussing personal health or business success, simply having the information is not enough. The key challenge is developing the discipline and willpower to act on that knowledge, a hurdle that many people struggle to overcome.
