This episode is a special interview where Alan is the guest! He's interviewed by his long-time friend and podcasting colleague, Andrew Johnston, the host of A Tale of Two Hygienists, as Andrew wraps up his run on that show. The conversation is a blend of dentistry, business, and the endurance of creative work. Dr. Mead shares his experience attending a Bioclear course and the challenge of changing established habits in dental practice. The core of the discussion centers on the evolution and future of dental podcasting, including the shift from audio-only to video/social content, and the role of AI in show production. They also dive into deeper topics about the future of the dental profession, the pressures of student debt on new dentists, the importance of longevity and permanence in practice, and the struggle to find social fulfillment outside of one's career. Dr. Mead offers sage advice for new podcasters, stressing the importance of engagement over download numbers, genuine conversation, and making the show about the listener. This episode features hosts Zach and Kevin, who welcome returning guest Dr. Frank Clayton to discuss the psychology and process of retiring from dentistry. Dr. Frank Clayton, who sold his practice and fully exited clinical work in July 2023, shares his insightful journey and the various phases of retirement he's experienced, including the "honeymoon phase." Kevin, who recently sold his practice in August and is now navigating the associate phase, offers his perspective on the initial mental shifts of stepping away from ownership. The hosts and Frank delve into the factors that drive the decision to sell—a combination of financial goals, mental fatigue, and a desire to "go out on top." They discuss the emotional weight of a "two-year breakup" with patients and staff, the challenges of detaching from the owner's mindset, and the importance of a well-structured ramp-down, which for Frank, included cutting back income and clinical work months before his final day. Alan reflects on his recent return to the Bioclear Learning Center, an experience that served as a powerful system reset for his restorative dentistry. He connects the challenge of maintaining new techniques in the dental office to the physics concept of entropy—the natural tendency of a system toward disorder. Alan explains that his initial failure to fully implement the prescriptive Bioclear system (including key steps like "bioblasting") was due to the energy required to fight against this entropy, often manifesting as personal laziness or team resistance to change. He stresses that consistent energy input—through regular audits, training, written protocols, and making the correct materials easily accessible—is necessary to maintain a low-entropy, high-functioning system that ensures excellent, predictable clinical outcomes. This episode features host Dr. Alan Mead sitting down with his friend, Dr. Lance Timmerman, in the artsy Hotel Murano lobby in Tacoma, Washington, while Dr. Mead is attending a four-day Bioclear core certification course. The conversation flows between the excellence of the Bioclear hands-on CE curriculum and the company's continuous innovation in dental matrices (like the redesigned twin ring). Dr. Mead and Dr. Timmerman also share humorous and insightful anecdotes from their dental school days, discussing the crushing weight of modern student debt, the challenges of implementing new CE concepts, and their personal preferences regarding in-person versus remote hands-on learning. Dr. Timmerman touches on the importance of having a CBCT and reflects on the extreme surgical procedure of zygoma implants as something he actively avoids. The episode closes with reflections on the transformative journey from dental school anxiety to current professional life, highlighting the value of connecting with peers outside of formal lectures. Hosts Zach and Kevin put the sports talk (mostly) aside to dive deep into a critical area of everyday dentistry: various scenarios involving soft and hard tissue grafting and the management of periodontal recession. They discuss how to handle common cases like recession in post-ortho teens (especially on the lower anterior teeth) and the different approaches for the challenging chewing tobacco patient. Zach and Kevin share their experience with generalized recession, debating when to refer for a graft—often noting that patient concern over esthetics or sensitivity drives the decision more than the presence of the recession itself. The conversation covers treatment techniques like free gingival grafts and the status of fads like the LANAP procedure and pinhole technique. They also explore other periodontal topics, including the management of gingival overgrowth (gummy smiles), peri-implantitis complications, and their skepticism regarding the widespread use of local antimicrobial delivery systems (like Arrestin) for routine scaling and root planing cases. Tune in for a candid discussion on what's working (and what's not) in the everyday management of periodontal health in a general practice. Join the Very Clinical Facebook group! Alan records from his hotel in Tacoma, Washington, after finishing day three of a four-day Bioclear anterior and posterior certification course. Despite the dark and rainy November weather in Tacoma, Alan shares his enthusiastically positive experience at the Bioclear Learning Center, highlighting the high quality of the instruction and hands-on training. He then offers a "recommendation engine" based on his trip, covering a specific Bioclear product, a movie he watched on the flight, an airport security service, a local restaurant, and a final endorsement of the learning center itself. Alan also announces the creation of a seventh password for the Very Dental Facebook page: Bioclear. Alan sits down once more with Dr. Tim Anderson to dive deep into the rapidly evolving landscape of digital dentistry. The episode begins with a hilarious, relatable discussion about the terrors of crashing recording software and the potential (and pitfalls) of AI, touching on everything from homework help to its role in revolutionizing clinical notes and reading X-rays. The conversation quickly pivots to Dr. Anderson's journey from engineering and architecture interests to his passion for the dental lab and in-house manufacturing, fueled by his residency and the purchase of a CEREC system. He reveals why he moved past early zirconia and EMAX to embrace new technologies. Dr. Anderson then shares his excitement for SprintRay's Midas, explaining how its new "Digital Press Stereo Lithography" technology is a game-changer, not just an evolution of 3D printing. He believes this new class of manufacturing, especially combined with AI, is poised to unlock limitless potential in restorative dentistry, particularly for procedures like onlays and eventually even complex bioprinting applications. He concludes by emphasizing that while technology is crucial, the biggest hurdle for dentists is often the software and the willingness to integrate and innovate. A Marine, lawyer, and recovering alcoholic, Rand set out with his 71-year-old brother to walk, reflect, and rediscover what really matters. This is a story about endurance, faith, and the long road home.Want to be a guest on Stories from Real Life? Send Melvin Edwards a message on PodMatch, here. Zach and Kevin kick off this episode by catching up and discussing a shared, recent hobby: pickleball. They share their experiences, the learning curve, and their thoughts on why the sport has become so popular, acknowledging its accessibility compared to tennis. The conversation then shifts to a dental topic: periodontal disease. They reflect on the emphasis of perio in dental school and discuss their personal criteria for treating scaling and root planning cases in-office versus when to refer to a periodontist. Key referral points include cases with sudden, isolated deep pockets, unresolvable widespread disease, and severe issues in patients under 40. The discussion briefly touches on molar extractions and when to sacrifice a tooth to protect an adjacent one, but they leave the topic of tissue grafting for the next episode. Alan gets candid about the concept of intentional culture, reflecting on his own experience taking over a dental practice where the culture was largely unintentional. He explains how he inherited a laid-back, reparative-dentistry-focused office, a product of the previous owner's lax style and the robust GM insurance benefits of the time. Alan admits that while his practice's culture has been adequate, making changes now is incredibly difficult because the unintentional status quo is deeply ingrained. He argues that new owners or startups have a massive advantage in establishing an intentional culture from day one, which is key to implementing new systems and driving change. He challenges listeners to join the Very Dental Facebook group (passwords include Timmerman, Hornbrook, Lipscomb, Papa Randy, Gary, or McWethy) to share their stories of intentional culture or the struggles of changing an unintentional one. Alan welcomes his first-ever podcast guest, Dr. Lance Timmerman, back to the show for a candid look at the life of a "CE Junkie." Lance shares his extensive, and often scattershot, journey through continuing education—from early courses in sleep apnea and Botox to implants and orthodontics—and the key lesson learned: it's hard to dabble in specialty procedures. The discussion evolves into a broader reflection on how dentistry has changed, especially for new dentists facing massive student debt. Ultimately, Lance describes his successful transition to a semi-retired, three-day-a-week practice focused on implants and sleep apnea, proving that taking all that CE led to the perfect work-life balance for travel and life experiences. Alan shares his recent findings on dental supply purchasing, questioning the long-standing industry model where dentists pay a premium for the convenience and supposed "best price" offered by a dedicated sales rep. He argues that this relationship-based purchasing lacks transparency and costs practices significant money, especially as overhead increases and reimbursements remain flat. Alan endorses his sponsor, Net 32, as a modern solution, highlighting its efficiency and transparency as an online network of suppliers that instantly offers the lowest available price. Through personal invoice comparisons, he demonstrates that switching to this system results in substantial, immediate savings on common materials, making it a powerful way for dentists to reduce overhead without sacrificing quality or convenience. Back in August Alan talked with guests Julianne O'Connor and Michael Keeter, the founders of Influential Dental. They discussed the company's efforts to elevate the dental space through its Influential Dental Podcast and the glossy lifestyle magazine, Influence Eleve. Julianne and Michael emphasize that their focus is on the authentic human element and the non-clinical stories of dentists—highlighting their journeys, mentors, and the challenges they've overcome—rather than the negative connotations of the "influencer" label. Alan discusses his history with audio and the evolution of the Dental Hacks podcast, contrasting his audio-first approach with the increasing trend of video content. They also touch on the challenges of print media in the digital age, the creative process behind their respective content, and the surprising utility of AI in podcast production. Dr. Alan Mead steps in for Zach and Kevin in this "Very Clinical Takeover" episode to share his efficient and refined workflow for single-tooth crown preparations. Alan emphasizes that his streamlined, digital-centric process makes single crowns a quick, straightforward, and profitable procedure in his general practice. The core of the episode is his detailed Crown Prep Workflow and Hacks, which prioritizes patient comfort and efficiency. Profound Anesthesia: Mead's first step is always to achieve profound anesthesia, utilizing inferior alveolar nerve blocks for lower molars and buccal infiltrations (usually with Septocaine) for other teeth. He heavily relies on the Septodont Paroject pdl syringe for supplemental tissue anesthesia around the entire tooth, often using it to avoid packing cord. Pre-Op Digital Scan: He scans the arch using his Medit i700 before prepping the tooth. This pre-op scan is used to register the final prep scan, ensuring a stable and accurate bite registration, which he praises as a massive efficiency improvement over traditional PVS impressions. Prep Sequence: His prep sequence is intentionally designed for efficiency: Break Interproximal Contacts First: He uses a Microcopy 0512C inverted pear bur for gross contact breaking, followed by a Microcopy 33140.10vf ("skinny") fine-tipped diamond for careful separation at the gingival. Occlusal Reduction: He uses the Microcopy 2137C which large, barrel-shaped coarse bur in an electric handpiece for quick, proper occlusal reduction. Core Build-up: After removing any old restoration or decay, he prefers Cosmecore from Cosmedent for build-ups, sometimes blocking out with Fuji IX from GC. He utilizes an old-school metal matrix band with Bioclear Diamond Wedges for isolation during the build-up. Margin Refinement and Tissue Management: He finishes margins under high magnification (microscope) and prefers a very fine diamond bur (Microcopy 3116.8vf) instead of traditional polishing cups for a smooth finish. Mead emphasizes smoothing the adjacent teeth with a Sof-Lex disc to ensure the lab can create an ideal, broad interproximal contact. Retraction: Mead overwhelmingly prefers Traxodent from Premier in the unit dose syringe tips for quick and clean tissue retraction, stating it generally works better for digital scanning than retraction cord. Temporization: While his assistant fabricates the temporary using a pre-made matrix, Mead remotes into his computer to mark the margins on the digital scan. He uses Clinician's Choice Template Ultra Quick Matrix Material and Mirror Image temporary material from Cosmedent, which he quickly refines with a Sof-Lex disc. He cements the temp with Dentotemp by Itena provisional cement for its strong retention. Alan encourages listeners to critique his "bread and butter dentistry" workflow on the Very Clinical or Very Dental Facebook groups, as he believes an efficient crown prep is a key to a successful general practice. "You got to remember that you don't own time, it's time that owns you." --Stephen King, "My Pretty Pony" Alan dives into the elastic nature of time this week, a concept he got from a Stephen King short story. He argues that time is totally weird and feels different depending on a person's situation, age, and mood. The Bonus Day: The whole thing started when Alan woke up at 3:30 a.m., convinced it was a work day (Tuesday). Then it hit him: It's Monday. He doesn't work Mondays! It felt like he got an entire day given back to him—a huge, weird, little time-traveling gift. The "Freakout" Factor: He observes that things that are stressful right now—like when his office phones were down for three days—feel like they'll be a nightmare forever. He quickly realized, though, that those things are usually forgotten by next week, proving that "this too shall pass." Time's Sandpaper: He looks back at the big, scary things, like being terrified about becoming a dad or the stress of dental school, and notes that now those sharp edges are all smoothed out. Time has a funny way of making those tough memories almost fond. The "Old Person" Cliché: Now he's the one saying it: "just hold onto these moments." Seeing his oldest son's marching band career wrapping up hit him how fast it all goes. That "blink and you'll miss it" thing is totally real, he says. The Light at the End: On the financial side, time actually works out. Those massive commitments, like the mortgages on his house and office, that felt endless when they started? Mead is finally seeing the finish line. He finds it wild that they eventually just... end. He wraps up by encouraging listeners to think about their own perception of time and remember: whatever you're dealing with, it's going to work out and move on. Alan welcomes back Dr. Tim Anderson to discuss the transformative impact of digital technology on modern dentistry. The episode kicks off with a brief mention of Dr. Anderson's recent "Midas tour" focused on partial coverage restorations, which serves as a springboard into a deeper discussion about the dramatic technological shift since their dental school days at the University of Minnesota. Dr. Anderson reflects on what he'd tell his younger self, highlighting the near-unbelievable advancements in: Intraoral scanning and the power of AI in design. 3D printing and same-day ceramic restorations. The doctors agree that while technology is crucial, the patient interaction and communication it enables are the real game-changers, using large monitors and engaging patients in the co-discovery process. They delve into the exciting evolution of 3D-printed materials, moving beyond simple resins to explore high-translucency ceramics and "digital press stereolithography" (Midas), which opens the door for layered, customized restorations that mimic natural tooth anatomy. The conversation touches on the enduring debate between full-coverage crowns and partial coverage (onlays), noting the current culture's tendency toward black-and-white treatment planning. Dr. Anderson expresses excitement for new workflows and materials that bridge the gap between traditional artistry (like gold inlays) and digital precision, ultimately focusing on how technology empowers better patient care and education. In this throwback episode of Very Clinical, Zach and Kevin welcome back Charlie Fryer to talk about AI in dentistry. Charlie recently finished a master's thesis about class II caries and AI so he can see applications in dentistry from the perspective of a computer scientist. AI's Current Role in the Dental Industry The guys discuss existing and potential applications of Artificial Intelligence (AI) in dentistry, acknowledging that the future of the profession will be heavily influenced by this technology. Invisalign/Aligners: Kevin notes that companies like Align Technology (Invisalign) are essentially data companies that leverage millions of intraoral scans from iTero to perfect tooth-movement algorithms and quickly generate treatment plans. He suspects much of the initial clin-check setup is AI-driven, with human checks afterward. Design & Manufacturing: AI is already used for the automated design and quick file return for appliances like night guards by companies like SprintRay, as well as for crown design in some printing workflows. Radiographic Diagnosis: The hosts discuss companies that use AI to read and assist in diagnosing findings on X-rays. A key discussion point is how these companies, like DSOs (Dental Service Organizations) such as Heartland or Aspen, acquire the vast amounts of patient data needed to train their models. The hosts posit that users become the "product," providing data with every image analyzed. Future AI Applications and Benefits The conversation shifts to more future-facing applications and the overall impact of AI on dental practice. Net Positive Impact: Zach and Kevin agree that AI will be a net positive for dentistry, primarily as an aid rather than a replacement for clinical judgment, drawing an analogy to how a GPS system advises a driver but the driver still makes the final decisions. Combining Data Sets: Charlie suggests combining various patient data, such as cone beam CTs, intraoral scans, and radiographic images, to develop next-generation tools for complex procedures like implant placement guidance. Practice Optimization: A major area for AI development is practice management. They discuss using machine learning to: Optimize scheduling based on patient history (recurrent neural networks) and procedure mix to maximize profitability and account for unexpected needs. Forecast inventory needs to ensure the practice is always adequately stocked. Financial and Marketing Tools: Patient Financing: Kevin highlights Sun Bit, a payment program that uses an AI component to quickly determine a patient's payment risk and rate by analyzing non-traditional data like utility payments instead of a full credit report. Marketing: AI is already effective in predicting click-through rates for online advertisements, which could be leveraged by dental practices. Join the Very Clinical Facebook group! Patient churn—the natural cycle of patients choosing, staying, or leaving a dental practice—is a normal part of the business. Alan explores this by reflecting on his own 28 years in practice, concluding that dentists often struggle with churn because they view their office much differently than their patients do. Ultimately, a patient's decision to use a dental service, whether for comprehensive care or only for emergencies, reflects how that practice best fits their needs, and not necessarily a sign that the dental office itself is broken. Alan is joined by the multi-talented Dr. Brady Smith, a dentist, standup comedian, and social media sensation. They dive into Dr. Smith's fascinating dual career, exploring whether he's a dentist who does comedy or a comedian who does dentistry, ultimately concluding he's a highly successful dual-profession guy. Dr. Smith shares his journey from running the noble but challenging Halo Dental Network charity—which led to his earlier podcast—to receiving a $15,000 piece of coaching advice that transformed his career: get on TikTok and be the face of his brand. He details his rapid rise to social media fame, the freedom it gives him to be authentic, and how he handles both the viral success and the inevitable "haters," including dealing with board complaints about his unique (and fully legal) "Laughing My Gas Off" podcast where he cleans celebrity and comic's teeth while they're on nitrous oxide. They also discuss the pressures and mental toll of dental practice, the difference between how dental professionals and the general public value dentistry, and Dr. Smith's successful experiment with a twice-weekly, 16-hour workday schedule. Alan discusses the significant differences between his intensive, abstinence-based path to recovery in 2002 and the current standards of addiction treatment. Prompted by renewing his DEA license and taking a required CE on opiate prescribing, Alan contrasts his successful, disruptive model (therapy and 12-step programs) with the modern focus on Medication-Assisted Treatment (MAT), such as buprenorphine (Suboxone), and harm reduction. He grapples with his past skepticism of these newer methods but shares a patient's story and discusses the Sinclair Method (using naltrexone for alcohol recovery) which have opened his mind to various effective ways of getting well. Alan invites listeners to discuss these evolving approaches in the Very Dental Facebook group and the smaller, recovery-focused group, Dentists That Don't. Alan welcomes Dr. Zach Meiners, co-host of the Very Clinical podcast and restorative dentist extraordinaire, back to the Very Dental podcast. They dive deep into their shared passion for glass ionomer (GI) restorative materials. Sparked by Alan's recent solo episode on using glass ionomer for groove restorations the guys talk GI in depth. Key Takeaways Glass Ionomer (GI) as a Primary Restorative: Both doctors advocate for GI (like Fuji 9 or Equia) as a legitimate, permanent restorative material for specific classes of restorations, moving past its traditional use as only a provisional or base. Ideal Clinical Applications: GI is highly effective for: Class I, III, and V restorations, particularly on posterior teeth where aesthetics are less critical. Geriatric and Pediatric patients due to its forgiving nature, minimal technique sensitivity, and fluoride release. Core Build-ups: Zach considers it his main build-up material, noting its reliable adhesion and stability against being pulled out by temporary crowns. Crown Margin Repairs. Material Benefits: GI offers several advantages over traditional resin or amalgam: No Post-Operative Sensitivity and Minimal Shrinkage. Forgiving Technique in less-than-ideal isolation situations, sticking particularly well to dentin. Continued Hardening for months after the initial set, leading to excellent longevity and a surprisingly low failure rate. Efficiency and Cost: While the initial cost of GI is relatively high, the efficiency gained from eliminating the need for separate etching and bonding steps (using only a cavity conditioner provided in the kit) and the speed of placement makes it a cost-effective choice in terms of time and materials. Placement and Setting: Placement Technique: For occlusal restorations (like in grooves), Alan recommends over-squirting the material and using a moist, gloved finger to push it into the grooves, or using an anti-stick agent like Stick Free for metal instruments. Accelerating Set: Though not a light-cure material, applying the curing light can accelerate the chemical setting reaction via heat, cutting the two-and-a-half-minute set time down to about 90 seconds. The GI "Dream": The doctors agree that a future GI material with the strength to reliably handle Class II restorations would revolutionize the entire direct restorative market, given GI's unique benefits. In this throwback episode Zach and Kevin feature Dr. Bob Convissar, a pioneer in laser dentistry with over 32 years of experience. He's a prolific author, lecturer, and practitioner who began his career after graduating from NYU in 1980 and acquiring a unique, century-old dental practice in New York City The main topic is the ABC's of Laser Dentistry, starting with a critical distinction between true lasers (like CO2 and Erbium) and devices often called lasers but primarily used as hot glass tips (like diodes). Bob explains that diodes, which can operate at extremely high temperatures (750∘C to 1500∘C), work thermally, creating a burn, while CO2 and Erbium lasers are absorbed by the water in soft tissue and vaporize it at 100∘C. He emphasizes that the CO2 laser is the best option for general dentists doing soft tissue work, as it creates a much smaller zone of necrosis (thermal damage) than ElectroSurge or diode lasers, leading to better healing. He also offers a crucial financial perspective, arguing that while a CO2 laser may have a higher initial purchase price (around $30,000), its lack of expensive disposable tips makes it a more cost-effective long-term investment than a cheaper diode laser (like a Picasso) with high operating expenses (potentially $18,000 in tips per year). For a multi-faceted approach, he recommends dentists invest in a CO2 laser, a modern air abrasion unit, and a Photobiomodulation (PBM) laser (around $2,000 to $3,000). PBM lasers are purely palliative and can be used for pain management, like treating TMJ issues or for analgesia on deciduous teeth, and for promoting healing after invasive procedures. Finally, Bob stresses that the most crucial factor in laser dentistry is training, advising listeners that any course focusing on only one manufacturer or wavelength is likely a sales pitch, not a comprehensive educational seminar. This episode features host Mohammed Abo-Basha's conversation with his classmate, Garrett Davis. They talked about his experience on a senior year dental externship at a Federally Qualified Health Center (FQHC). Key takeaways: Senior Externship Experience: Guest Garrett discusses his dental externship at a Federally Qualified Health Center (FQHC), contrasting it with the slow pace of dental school. Increased Patient Load: He was excited to move to a realistic, faster-paced schedule, seeing approximately eight patients a day (compared to two in school). Biggest Concern: His main apprehension was managing deep caries with less oversight from faculty. Workflow Differences: Garrett appreciated the realistic workflow and the support of competent dental assistants who took over many setup and cleanup tasks. Clinical Growth: He gained significant improvement in speed, confidence, and proficiency, particularly with Removable Partial Denture (RPD) procedures, which were completed in 30 minutes instead of the three hours allotted in school. Board Exam Advice: Garrett shared advice for the INBDE and OSCE, recommending using a study plan like Bootcamp, focusing on practice questions, and scheduling the two exams close together. It isn't very often that I get to interview a legend, but this episode is one of those. I had the chance to sit down with Dr. Joe Massad who has been educating dentists about removable prosthodontics for decades. Last weekend Joe lectured to the Francis B. Vedder Society at their fall meeting up in northwestern Michigan and he didn't disappoint. I had a chance to sit down and talk to Joe about the concepts he was teaching and how he learned them. Joe discussed techniques such as the neutral zone concept, cameograms and gothic arch tracing. He emphasizes the importance of accurate records, patient involvement in the process, and the need for lab precision. He also touched on his innovative approaches to digital dentistry and the benefits of offering patients secondary dentures. This insightful conversation offers valuable takeaways for any dental professional looking to excel in removable prosthodontics. Alan reflects on the value of specialization in dentistry after attending a meeting where he saw a presentation by Dr. Joe Massad, a renowned specialist in removable prosthodontics. Alan discusses how Dr. Massad's focused expertise allows for a streamlined, efficient practice and attracts a specific type of patient, leading to greater predictability and success in treatment. Alan contrasts this with the challenges of being a general dentist who performs a wide range of procedures, highlighting how a lack of specialization can make it difficult to gain mastery and efficiency in specific areas. A dentist, who hones in on a specific niche can gain significant advantages through focused expertise, efficient systems, and the ability to attract patients who are already committed to a particular type of treatment. In this powerful episode of Healthy Mind, Healthy Life, host Avik speaks with Rand Timmerman—retired attorney, recovered alcoholic, Vietnam veteran, and author—about his life-changing journey hiking the 2,200-mile Appalachian Trail at the age of 72. With an artificial knee, past trauma, and a backpack full of memories, Rand set out to support his grieving brother and ended up rediscovering himself. This episode is a deeply moving exploration of grief, resilience, addiction recovery, and the spirituality found in nature's wildest places. Whether you're struggling with age, addiction, or doubt—Rand's story proves it's never too late to begin again. About the Guest:Rand Timmerman is a former Marine Corps officer, attorney, and author of A Spiritual Passage, a photo-journal memoir of his 6-month hike through the Appalachian Trail. Now sober, he speaks openly about addiction, PTSD, and spiritual healing later in life. Rand's work offers hope to anyone feeling stuck, broken, or too old to chase transformation. Key Takeaways: At 72, Rand hiked 2,200 miles to help his grieving brother and discovered his own healing. Recovery is possible at any age—sobriety, service, and nature helped him reclaim purpose. The Appalachian Trail revealed emotional and physical limits—and how to push beyond them. Addiction is not a moral failure; understanding its biological basis changes the path to healing. Spiritual connection and surrender were critical in maintaining peace and sobriety. Alan welcomes back Dr. Dawn Grimes to talk about burnout. Lately it seems like social media can't stop talking about it and they both have some things to say about it! They define burnout as emotional, physical, and mental exhaustion from prolonged stress, arguing it's not unique to dentistry. Dawn asserts that a poor relationship with stress, a lack of resilience, and unrealistic expectations—often fueled by social media—are the real causes. They suggest that instead of quitting or hiring a coach, dentists should reframe their perspective, manage expectations, and take practical steps to improve their lives both in and out of the office. Key Takeaways Burnout is real, but it's not unique to dentistry. It's the result of prolonged, unmanaged stress from any demanding part of life or work. Social media can worsen burnout. The algorithm feeds you more of what you look at, so lingering on videos about burnout can make you feel worse. The constant comparison to a curated, glamorous online presence creates unrealistic expectations for your own life and career. A "coach" may not be the answer. The hosts question the credibility of coaches who no longer practice clinical dentistry, suggesting that their advice may be disconnected from the day-to-day realities of the job. The only thing you can change is yourself. Rather than trying to change your job or circumstances, you should focus on changing your reactions to problems. This is a core part of developing resilience, which allows you to let go of negative experiences and start over. Rumination makes things worse. Dwelling on past mistakes or bad days (internal rumination) or consuming content about burnout (external rumination) reinforces negativity and prevents you from moving forward Meditation is a powerful tool. It helps you train your brain to let go of unhelpful thoughts and focus on the present moment, which is the key to building resilience and managing stress. Maintain perspective. Remember that dentistry is a well-paying profession and many other jobs are physically and emotionally harder. Accepting that life and work are often challenging is the first step toward finding peace. You have control. You can manage your social media algorithm by engaging with positive content like dog videos, mountain bike content or clips of cooking very tiny food! Like and highlight the things that are joyful and light on social media and the algorithm will do the rest. This throwback episode of the Very Clinical podcast features hosts Zach and Kevin discussing the value of continuing education (CE) with special guests Dr. Erin Elliott and Dr. Tim Anderson! They share amusing anecdotes about their worst CE experiences, including a condescending lecturer and courses that didn't deliver on their promises. The conversation then shifts to a more positive outlook, focusing on modern dental practices. Dr. Elliott, a sleep apnea expert, and Dr. Anderson, a 3D printing specialist, highlight the importance of team-driven growth and how live, interactive courses are essential for successful implementation and patient care. They also touch on the growing public awareness of sleep issues and the political challenges dentists face in treating sleep apnea. The episode wraps up with a nod to their upcoming courses and the importance of professional development and collaboration. Regenerative agriculture isn't new. To quote today's guest, it's an “exciting old thing.” Yet in the modern world, this ancient, innate wisdom often feels forgotten, and the consequences show up in our food system, in our health, and in the climate. At its core, the challenge isn't just about farming practices, it's about how we see ourselves in relation to the land.In this episode, Monica sits down with journalist and author Kelsey Timmerman to explore what it means to belong to a place rather than own it. From Hawaiian teachings about belonging to a river to the idea that each of us carries with us a heart of the world, Kelsey invites us to rethink our connection to the Earth.Together, we dive into the paradox of hope in the face of devastation, why the people most connected to the land often carry the most optimism, and how agency and reverence can help us meet the urgent challenges of climate change.Show NotesAbout Kelsey TimmermanRegenerating Earth: Farmers Working with Nature to Feed Our Future by Kelsey TimmermanWhere Am I Eating? Alan reflects on his 27 years in dentistry, revealing how his past struggles with addiction and a conservative approach to treatment planning have impacted his practice. He admits he often defaults to a "watch-and-wait" approach in gray-area cases, feeling a need to save patients money or fearing they will say no to treatment. He worries this may lead to more complex and expensive procedures down the road, benefiting neither the patient nor his business. Alan challenges himself to be more assertive in presenting a full range of treatment options, allowing patients to make their own decisions. He believes this shift is crucial for both his professional growth and the long-term well-being of his patients. Alan is once again joined by Dr. Matt Standridge. The episode was recorded live at a Voices of Dentistry event following Matt's presentation on "Simplified Smile Design." Matt and Alan explore how a person becomes an expert, from their beginnings on forums like Dentaltown to Matt's current role as an industry educator. They discuss the difference between genuine experience and the illusion of expertise created by social media, and how both can impact the way dentists learn. Matt shares valuable lessons learned from his own journey, including the importance of setting boundaries and the value of detaching from the stress of a clinical practice. He also opens up about his recent decision to sell his practice and what it's like to transition from an owner to an associate. In this episode of the Very Clinical Podcast, hosts Zach and Kevin chat with guest Dr. Allie Frounfelter about digital dentistry and the process of creating "trial smiles." Allie, a self-proclaimed "trial smile queen," details the steps of designing, 3D printing, and fitting a temporary smile for patients so they can preview their results before committing to treatment. She explains the benefits of this modern approach, including improved patient confidence and better-fitting temporary restorations, which leads to healthier gums. Allie also discusses the shift in dentistry toward more fulfilling, life-changing procedures and shares that even if she won the lottery, she would continue doing this kind of work. The conversation concludes with a discussion about the essential tools needed to implement this process, emphasizing that a 3D printer is the most crucial piece of equipment for a dentist to own. Alan shares his frustrating experience with AT&T's poor customer service after his office's phones went down. The ordeal leads him to develop a theory about the inverse relationship between demand for a service and the need for excellent customer service. He explains that businesses like AT&T, which provide essential services with high demand and low-elasticity, can get away with poor service because it's difficult for customers to switch. On the other hand, professions like dentistry, which operate in a more competitive market, must provide great service to retain patients. Al's negative experience with AT&T highlighted the importance of treating his patients well, especially since they have other options. He emphasizes that providing excellent customer service isn't just a business strategy; it's the right thing to do. Alan brings together Dr. Lance Timmerman and Dr. Brady Smith for a lunch conversation at VoD Lite 2025. The trio discuss the unique, small-scale nature of the event, which was planned just three weeks beforehand and features a food truck instead of a hotel's expensive catering. Lance and Brady talk about their professional lives, including their pride in their respective hometowns' (Seattle and Portland) high poop-on-the-streets-per-capita and their mutual surprise at the high cost of a hard-boiled egg at hotels. They also delve into their unconventional dental practice models, with Lance discussing his use of the EMS Guided Biofilm machine and Brady explaining his two-day, 32-hour work week and his decision not to hire a hygienist. Their conversation also touches on Brady's side career as a stand-up comedian and the challenges he faces, from dealing with angry hygienists who file board complaints against him to navigating the complex and expensive world of venue rentals. Zach and Kevin welcome Dr. Allie Frounfelter to discuss her innovative approach to cosmetic dentistry using Trial Smiles. Dr. Frounfelter shares her journey from a high-volume "bread and butter" dental practice to a more balanced and fulfilling one, emphasizing the importance of slowing down to speed up. The episode details the step-by-step process of creating and using a 3D-printed resin shell, or Trial Smile, to help patients visualize their potential new smile. Dr. Frounfelter also highlights the importance of incorporating the entire dental team in the process and discusses how this patient-centered approach, along with the right technology and systems, has transformed her practice. Key Takeaways Invest in Digital Technology: Dr. Frounfelter and her husband significantly transformed their practice by investing in digital tools, including a CBCT, intraoral scanner, and 3D printer. They initially sought a CBCT but came back from a conference with a full suite of digital tools. The Trial Smile Process: A Trial Smile is a thin, 3D-printed resin shell that fits over a patient's natural teeth, allowing them to try on a new smile. This is a crucial step in cosmetic dentistry consultations as it offers a tangible "try before you buy" experience. Difference from Traditional Wax-ups: Unlike a traditional bis-acryl wax-up, a Trial Smile is easily placed and removed, making it repeatable and mess-free. This allows patients to get a clear and instant before-and-after comparison. Team Involvement is Crucial: Allie stresses the importance of training and involving the entire team in the new workflow. Her clinical director, Brandon, handles the initial patient conversation, photos, and records, while a dental assistant, Emma, designs the smiles using specialized software like Exocad. Slowing Down to Speed Up: By implementing new systems and focusing on comprehensive cases, Allie and Adam were able to move away from a high-volume model that led to burnout. They now see fewer patients but generate more revenue from cosmetic and full-mouth cases. Outsourcing Options: If a practice lacks the necessary design software or a 3D printer, they can partner with a dental lab like Roe Dental Lab, which offers a cosmetic design service to create and even print the Trial Smile for them. Alan argues that dental sealants should be treated more like restorations rather than simple preventive measures. He critiques the common practice of placing sealants without preparing the tooth, which he believes can lead to failure and hidden decay. Instead, he details his own protocol, which involves using a bur to open up and clean out the deep grooves of the tooth, often discovering stain, decalcification or even decay. He then fills these prepped grooves with a glass ionomer material. Alan asserts that this more involved approach results in a highly durable, decay-resistant solution that he feels far more confident in than traditional sealants. Alan welcomes his friend Adam McWethy back to the Very Dental podcast! Recorded in person at the 2025 Voices of Dentistry pop up event in Arizona, Adam drops in to talk about his new company, The Leadership Thread. The Leadership Thread works with specialist practices to achieve growth and life balance by taking control of their careers. Adam and Al explored how AI is poised to change the way dentists handle a variety of tasks, from documenting treatment to managing patient relationships. Adam discussed several ways that AI can dramatically reduce the time specialists spend writing referral letters and charting. He mentioned a new AI-powered platform that automates these tasks, giving providers hours of their week back. Alan discussed his experience that even with minimal training, AI can generate detailed, persuasive narratives to help justify treatment to insurance companies, a common pain point for general dentists. The conversation highlighted that AI in dentistry is quickly moving from a novel concept to a standard tool. Just as features like anti-lock brakes were once a luxury add-on in cars and are now standard, AI-driven solutions are becoming a core part of dental practice management, improving efficiency and communication across all dental specialties. In this "throwback" episode recorded live at Voices of Dentistry 2023, hosts Kevin and Zach are joined by guest Dr. Craig Harder to debate and discuss dental myths and tips. The main topic of conversation is the dental myth that Invisalign is a great value for dentists. Craig argues that Invisalign is not a good value because it eats up a significant portion of the fee, especially for dentists who don't do a high volume of cases and therefore don't qualify for discounts. Kevin, a high-volume provider, counters that the value of Invisalign is not just in the cost per case but in its powerful marketing engine. He explains that Invisalign's massive ad campaigns drive patients directly to dentists' offices, a level of promotion that individual practices could never achieve. The conversation also touches on the user experience of other clear aligner companies versus Invisalign, with Kevin stating he finds the Invisalign user interface to be far superior. Craig's tip: Always use Vivera retainers after aligner therapy! Join the Very Clinical Facebook group! Alan shares his favorite verbal phrases that have improved his patient interactions over his 28-year career. He emphasizes the importance of building rapport and setting patient expectations to create a smoother, more enjoyable practice. Alan discusses how these simple word choices help him diagnose problems, manage treatment compromises, and handle post-op concerns, all while fostering trust and a human connection with his patients. Some of Al's Favorite Words: "Can you say more about that?" This phrase encourages patients to elaborate on their symptoms, often unlocking a more accurate diagnosis. It prompts them to think more deeply about their sensations, which is especially useful when a problem isn't visually apparent. "Are you going to be mad at me if...?" Used when a patient is considering a compromised or less-than-ideal treatment, this phrase invites them to contemplate a potential negative outcome. It shifts the responsibility for informed consent from a formal, authoritative one to a human, conversational one. "Julie is going to explain this to you in English." Dr. Mead uses this line when handing off a patient to a dental assistant or hygienist. It humorously acknowledges that dentists often use technical jargon and signals to the patient that a clearer, more accessible explanation is coming from the other team member. "We're here if you need us." This phrase is used for patients who have perfect checkups or don't require extensive treatment. It reassures them that they are still valued and that the practice is available for any future concerns, fostering a sense of long-term care and trust. "This is going to be sensitive for a couple days." When delivering a new crown or performing other procedures, setting the expectation of post-operative discomfort helps patients mentally prepare. It reduces the number of post-op calls and gives patients "grace" to experience normal sensitivity without alarm. "Are you okay if we wait to see what happens?" Used after cementing a crown while a patient is still numb, this asks for their consent to check the bite again once the anesthesia has worn off. It acknowledges that they can't accurately feel the bite while numb and gives them control over their final comfort. "You're going to be numb for a couple of hours." Dr. Mead uses this to manage a patient's expectations about how long the numbness will last. By saying "a couple of hours," he gives them a realistic timeframe, and if it lasts longer, they are less likely to be surprised or concerned. Kevin and Zach welcome Alan back to discuss the world of business and entrepreneurship within dentistry. They begin by reminiscing about their childhood "businesses," which leads to a discussion sparked by a conversation Alan had with dental entrepreneur Dr. Mark Costes. The group explores the idea that while dentists of their generation could often succeed with a simple practice, the current landscape of high student debt and large dental organizations demands a more business-minded approach. They debate whether an entrepreneurial spirit is something you're born with or can be developed, and they consider the differences between simply owning a practice, running a dental business, and being a true entrepreneur. The conversation highlights the wide spectrum of career paths available in dentistry today, from being a salaried associate to building a multi-office empire. In this episode of the podcast, hosts Kevin and Zach, along with guest Alan, dive into the world of business and entrepreneurship within dentistry. They begin by reminiscing about their childhood "businesses," which leads to a discussion sparked by a conversation Alan had with dental entrepreneur Mark Costas. Key Takeaways Entrepreneurship is a Spectrum: Owning a dental office doesn't automatically make you an entrepreneur. The podcast outlines a spectrum from simply owning a job (a practice that only runs with you there) to having a true business that can function independently, to being a full-on entrepreneur who innovates and scales. The Changing Landscape of Dentistry: The hosts acknowledge that their generation of dentists came into the profession during a "golden age" with less debt and more opportunities for practice ownership. Today, with higher student loans and the rise of Dental Service Organizations (DSOs), younger dentists often need to be more business-savvy to succeed as owners. Technology and Business: The discussion touches on how technology, like digital dentistry and clear aligners, can be a business decision to add new services and level up a practice. This is presented as a way to evolve from a "drill, fill, bill" mindset into a more diverse and profitable business model. Dentistry as a Vehicle: The podcast concludes that dentistry can be a great field for entrepreneurs because it provides a high income that can be used to fund other business ventures, whether within dentistry (like teaching or consulting) or in completely different industries.
